본태성 떨림의 한방 치료에 대한 임상 연구 동향 - China National Knowledge Infrastructure를 중심으로 (A Review of Studies for Treatment of Essential Tremor Using the China National Knowledge Infrastructure Database)

  • Objectives: The purpose of this research is to investigate clinical studies of essential tremor in China. Methods: We investigated clinical studies of Traditional Chinese Medicine therapies in traditional Chinese medical journals for essential tremor through the China National Knowledge Infrastructure (CNKI) database. In total, 15 clinical articles were analyzed. This study examined the authors; published years; types of study; treatment methods for the treatment and control groups; periods; outcome assessment; and results. Results: Nine articles used acupuncture, 6 articles used herbal therapy. The most commonly used acupoints were GV20, GB20, LR3, LI4, GB34, SP6, and EX-HN1. The most commonly used herbs were Paeoniae Radix Alba, Angelicae Gigantis Radix, Cnidii Rhizoma, Gastrodiae Rhizome and Uncariae Ramulus et Uncus. Total effective rate was 66.67-96.7%; the treatment group values were significantly higher than the control group values in all papers. Tremor score, HAMD, curative effect of TCM syndrome, and incidence of adverse effects were significantly more affected in the treatment group than in the control group. Conclusion: More active clinical studies are needed on essential tremor, and the results of this study can be used as a basis for future research.

"동의보감(東醫寶鑑)" "탕액편(湯液篇)"의 본초(本草) 분류에 대한 연구 (A Study on the classification of materia medica in medicinal part of Treasured Mirror of Eastern Medicine("東醫寶鑑" "湯液篇"))

  • The medicinal part of Treasured Mirror of Eastern Medicine("東醫寶鑑" "湯液編") is based on Classified Emergency Materia Medica("證類本草"), Compendium of Prescriptions from the Countryside("鄕藥集成方"). But it distinguished materia medica of countryside(鄕藥) from materia medica of China(唐藥) and properly selected the sentences of them by actual medical circumstances in Korean peninsula. Especially upon assortment of medical herbs, the medicinal part of Treasured Mirror of Eastern Medicine added the part of Water, Earth, Metal, and divided the part of Fish & Bug, Jade & Stone into Fish, Bug, Jade, Stone part. Moreover, it abolished the three grade classification adopted in Classified Emergency Materia Medica, Compendium of Prescriptions from the Countryside and attempted a new approach on itemization; 'representative herb' and 'secondary herb'. Hence, medicinal part of Treasured Mirror of Eastern Medicine should be evaluated as the 'settling in of Chinese medicine with autonomous interpretation, other than 'the fusion of prescriptions from countryside and Chinese medicine.

의부전록(醫部全錄)의 편집체제(編輯體制)와 주제분류(主題分類) (A Study on Composition and Content of Uibujeolok [醫部全錄])

  • $\ulcorner$Uibujeolok$\lrcorner$ is a part of ${\ulcorner}$gogeumdoseojibseong[古今圖書集成]${\lrcorner}$ edited in 1726 Chinese Ching[淸] Dynasty. It refers ${\ulcorner}$Uibu[醫部]${\lrcorner}$ included in ${\ulcorner}$Bakmulhwipyeon Yesuljeon-Ha[博物彙編 藝術典下], and is composed of 520 volumes with 141 chapters. Though the name of Eubujeolok is used in most printed editions, the proper name was ${\ulcorner}$Uibujibseong[醫部集成]${\lrcorner}$ according to the original text of the copper type printing. Each category consists of 13 parts including Commentary, Diagnosis, and Organs and Bowels etc. And each chapter follows identical structure with categories. This composition type of ${\ulcorner}$Uibujibseong${\lrcorner}$ is similar with that of ${\ulcorner}$Dong-uibogam[東醫寶鑑]${\lrcorner}$ which was published 130 years ahead, but different from those of clinical books which published in China until then. Unusually there is no chapter dealing with Chinese herbs and acupuncture.

위.십이지장궤양의 식요방법(食療方法)에 관한 연구 (A study of dietetic on the gastric & duodenal ulcer)

  • Objectives : This research examined about dietary therapy on the gastric & duodenal ulcer based on the literary findings, for clinical treatment and prevention of the gastric & duodenal ulcer. Methods : This Research examined on histories, compositions, applications, and effects of dietetic on about 200 cases of dietary therapy of the gastric & duodenal ulcer from recent Chinese literatures Results : 1. Various vegetables, animals and mineral materials including herbs, grains, vegetables, fruits, food and drink were used for the dietary therapy. 2. Methods of the preparation for use as therapeutics were decoction, pulvis, gruel, medicinal wine, cake, tea, paste and gelatin and etc. 3. Frequently used materials were cuttle fish bone, Bletillae rhizoma, oyster shell, egg shell, sugar, aloe, licorice, lily, red jujube, and pig stomach. 4. Four properties of cold, warm, cool and hot were equal in frequencies. Most common tastes were sweet and bitter. Conclusion : Though dietary therapy for the gastric & duodenal ulcer is not based on clinical or experimental data, but through experience. It is mostly based on Yin-Yang and five elements, visceral manifestation, channels and their collateral channels and chinese herbal medicine theories. If we use them properly according to oriental medicine method, it will be effective on treating and preventing the gastric & duodenal ulcer.

  • Ginseng was the most revered of the herbs in ancient times in China, Korea, Japan, America. Ginseng was discovered over 5000 years ago in the mountains of Manchuria, China. References to ginseng are found in books dating back more than two millennia. It is revered by the Chinese people as it is considered a herb for everything use and therefore for a wide range of diseases (currently its Latin name derived from the Greek panacea, meanings, that is, for everything). So, it was used exclusively by the Chinese Emperor's, and they were willing to pay the price without problems. Increasing its fame, ginseng brought a flourishing international trade that allowed Korea to supply China with silk and medicines in exchange for wild ginseng and later along with what grows in America.

  • Objectives : Select and analyze the case record of dyspnea and wheezing asthma recorded in Xu Ming Yi Lei An that is the most abundant and wide in contents in existing case records that are systematic, comprehending relatively modern Traditional Chinese Medicine to secure more deep and objective basis of Traditional Chinese Medicine approach for dyspnea and wheezing asthma to analyze and review possibility for clinical application in this study. Methods: The study was conducted with the case records of dyspnea and wheezing asthma in whole Xu Ming Yi Lei An. Pattern identify and classify selected case records and again classified with deficiency syndrome and excess syndrome. Also analyzed prescriptions and herbs used in the case records. Nature of herbs and properties and flavors that were used in the case records were classified and frequency of each nature of herbs were analyzed. Applicable case records were interpreted and suggested prescriptions, pulse feelings, pattern classification were analyzed and described. Results : Among the 5254 case records stated on the complete collection, it is researched that there are 63 case records for the symptom complex of dyspnea as 1.2% of the whole case records, and the case records on the symptom complex of wheezing asthma are 14 as the 0.27% of the total examples. 63 case record examples related with symptom complex of dyspnea were pattern identified and classified. As a result, deficiency syndrome of the Kidney(33 %), deficiency syndrome of the Spleen(26.0%), Wind-Cold(12.3%), phlegm turbidity(12.3%), Heat in the Lung(8.2%), asthenia of the Lung(8.2%) were investigated as above order. 14 case record examples related with wheezing asthma were pattern identified and classified. As a result, phlegm-Heat(26.3%), upper excess and lower deficiency(26.3%), external affections Wind-Cold(15.8%), Dampness-phlegm(10.5%), Lung asthenia(10.5%), Cold phlegm(5.3%), mutual deficiency and detriment of Heart and Kidneys(5.3%) were investigated as above order. Symptom complex of dyspnea has 67.1% of deficiency syndrome, 32.9% of excess syndrome resulting more deficiency syndrome than excess syndrome. Symptom complex of wheezing asthma has 42.1 % of deficiency syndrome and 57.9% of excess syndrome resulting more excess syndrome than deficiency syndrome. In case of symptom complex of dyspnea prescription used in the case record, the order of frequency is as following. Palmijihwang-tang, Bojung-ikgitang, Yungmijihwang-tang, Ijintang, Sojaganggitang, Igongsan. In case of symptom complex of wheezing asthma prescription in the case record, Yungmijihwang-tang, Ohotang, Dodamtang were mostly used. Herbs used in case records of symptom complex of dyspnea are Ginseng Radix, Poria, Glycyrrhizae Radix, Aconiti Iateralis Preparata Radix, Atractylodis Macrocephalae Rhizoma, Dioscoreae Rhizoma, Angelicae Gigantis Radix, Rehmanniae Radix Preparat, Pinelliae Rhizoma, Zingiberis Rhizoma Recens are mostly used. Nature of herb properties used for symptom complex of dyspnea and symptom complex of wheezing asthma are herbs that are warm properties. When the symptom complex of dyspnea and the symptom complex of wheezing asthma were treated. if the patient felt tenderness at Pyesu, doctors conducted pricking blood around the opposite Pyesu or Sipseon acupoint. when the patient didn't have tenderness at Pyesu by soft press, pricking blood was performed both sidees, right and left Pyesu. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, when they got treatment, when the symptom complex of disease is severe, a doctor cauterized the opposite Pyesu while the other Pyesu felt tenderness, and decided how the above treatment is performed whether the degree of the symptom compolex of disease is severe or not. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, if the person felt tenderness at Pyesu and is caught by the Wind-Cold pathogen, slight acupuncture is treated at relevant Pyesu with Fire needling. When patient with symptom complex of dyspnea and symptom complex of wheezing asthma cannot hawk sputum up from the oral and laryngopharynx, suction method is treated. Conclusion : With this study, actual traditional and clinical pattern identification form and characteristics of symptom complex of dyspnea and symptom complex of wheezing asthma were recognized. Modern case report utilizing in clinical application need to be secured and an incurable disease asthma need to be diagnosed and improvement for treatments have to be searched through other case records.

불면장애에 사용된 귀비탕의 동질성 연구 -CNKI에서 검색한 무작위배정 임상연구를 중심으로- (A Study on Homogeneity Analysis of the Guipi Decoction for Treatment of Insomnia Disorder -Focusing on Randomized Controlled Clinical Trials in CNKI)

  • Objectives: The study was designed to provide clinical evidence of homogeneity in analysis of the herbal medicine, Guipi decoction in treating insomnia, using a t-test of randomized controlled trials (RCTs). Methods: This study included RCTs that verified homogeneity of Guipi decoction in treating insomnia. Literature studies of CNKI (Chinese National Knowledge Infrastructure Database) were performed. The basic components group was made to the Guipi decoction of the Yixuerumen?s text. The excluded group was created with the number of herbs excluded from the basic components group. T-test performed that the effective rate in the basic components group and excluded groups. Results: The basic components of Guipi decoction contain Atractylodis Rhizoma Alba, Poria Sclerotium (Poria Sclerotium Cum Pini Radix), Polygalae Radix, Astragali Radix, Zizyphi Semen, Angelicae Gigantis Radix, Aucklandiae Radix, Glycyrrhiza uralensis Fischer, Ginseng Radix (Codonopsis Pilosulae Radix), Longanae Arillus. Herbs excluded from the basic components group were Longanae Arillus, Ginseng Radix (Codonopsis Pilosulae Radix), Glycyrrhiza uralensis Fischer and herbs added to the basic components group were Polygonum multiflorum Thunberg, Schisandrae Fructus, Salviae Militiorrhizae Radix, Paeoniae Radix Alba (in order from the most used to the least). T-test revealed the effective rate in the basic components group was not statistically significant compared to the excluded groups (p=0.161/p=0.162/p=0.103). Conclusions: Effectiveness in treatment of basic components of Guipi decoction and excluded groups were revealed to be homogeneous in treating insomnia. As the number of herbs in the basic components group decreased, mean value in effectiveness of treatment also decreased consequentially. However, there were no statistical significances.

Clinical Significance of Upregulation of mir-196a-5p in Gastric Cancer and Enriched KEGG Pathway Analysis of Target Genes

  • Background: miRNAs are relatively recently discovered cancer biomarkers which have important implications for cancer early diagnosis, treatment and estimation of prognosis. Here we focussed on expression of mir-196a-5p in gastric cancer tissues and cell lines so as to analyse its significance for clinicopathologic characteristics and generate enriched KEGG pathways clustered by target genes for exploring its potential roles as a biomarker in gastric cancer. Materials and Methods: The expression of mir-196a-5p in poorly, moderate and well differentiated gastric cancer cell lines compared with GES-1 was detected by RT-qPCR, and the expression of mir-196a-5p in gastric cancer tissues comparing with adjacent non cancer tissues of 58 cases were also assessed by RT-qPCR. Subsequently, an analysis of clinical significance of mir-196a-5p in gastric cancer and enriched KEGG pathways was executed based on the miRWalk prediction database combined with bioinformatics tools DAVID 6.7 and Mirfocus 3.0. Results: RT-qPCR showed that mir-196a-5p was up-regulated in 6 poorly and moderate differentiated gastric cancer cell lines SGC-7901, MKN-45, MKN-28, MGC-803, BGC-823, HGC-27 compared with GES-1, but down-regulated in the highly differentiated gastric cancer cell line AGS. Clinical data indicated mir-196a-5p to beup-regulated in gastric cancer tissues (47/58). Overexpression of mir-196a-5p was associated with more extensive degree of lymph node metastasis and clinical stage (P < 0.05; x2 test). Enriched KEGG pathway analyses of predicted and validated targets in miRWalk combined with DAVID 6.7 and Mirfocus 3.0 showed that the targeted genes regulated by mir-196a-5p were involved in malignancy associated biology. Conclusions: Overexpression of mir-196a-5p is associated with lymph node metastasis and clinical stage, and enriched KEGG pathway analyses showed that targeted genes regulated by mir-196a-5p may contribute to tumorgenesis, suggesting roles as an oncogenic miRNA biomarker in gastric cancer.

중의학 데이터베이스 (CAJ)를 이용한 소아 비만의 한약치료에 대한 임상 연구 동향 (Review of Clinical Studies for Herbal Medicine Treatment on Childhood Obesity - Focusing on Studies from the China Academic Journal (CAJ) -)

  • Objectives The purpose of this study was to analyze recent traditional Chinese medicine clinical studies for treatment of childhood obesity. Methods We analyzed 11 clinical studies from January, 2000 to March, 2018 about childhood obesity from the China Academic Journal (CAJ) and China National Knowledge Infrastructure (CNKI). All selected studies were Randomized Controlled Trials and analyzed in order of publication year, demographic information, treatment method, evaluation methods, results, adverse reactions and relapse after treatment discontinuation. Results The main Traditional Chinese Medicine (TCM) treatment was the herbal decoctions, granules and pills. In most studies, the total efficacy of the treatment group was reported to be higher than that of the control group. Adverse reactions were found in small number of studies. Frequently used medical herbs were Crataegi Fructus (山?), Atractylodis Rhizoma (蒼朮), Citri Pericarpium (陳皮), Poria (茯?), and Nelumbinis Folium (荷葉). Also, Water-draining and swelling-dispersing medicinal (利水退腫藥), Qi-regulating medicinal (理氣藥), Resolving dampness with aroma medicinal (芳香化濕藥), Blood-activating and stasis-dispelling medicinal (活血祛瘀藥), Qi-tonifying medicinal (補氣藥), and Digestant medicinal (消食藥) were commonly used. Conclusions This study showed that traditional Chinese medicine, especially herbal medicine treatment can be effective and safe option for treating childhood obesity. These research results can be utilized in other clinical studies as well as in treatment of childhood obesity. Additional well-designed randomized controlled trials and systematic reviews are need to confirm the conclusion.

Immunoglobulin productivity assay를 이용(利用)한 홍삼투여(紅蔘投與) 실험동물(實驗動物)의 IgG, IgM, IgA 비교(比較) 연구(硏究) (The Comparative Study of IgG, IgM, and IgA in Laboratory Animal Administrated Red-ginseng, Using Immunoglobulin Productivity Assay)

  • Objective : The immune system is a complex of systems, all of which work together to clear infection from the body. In Korea, red ginsenghas been one of the herbs most widely used to enhance the immune system for thousand of years. More recently, red ginseng has been reported to have many positive effects on the immune system. The purpose of this study was evaluate the effects of Korean red ginseng and Chinese red ginseng on IgG, IgM, and IgA, using immunoglobulin productivity assay. Methods : Male SD rats were separated into 3 groups. We administered Korean red ginseng (KRG) to one group and Chinese red ginseng (CRG) to another, with normal saline for the Control group consecutively and orally for 3 months. The dose of red ginseng was 500mg per day, as a powder with soluble water. Immunoglobulin levels from spleen cell were estimated by ELISA kit. Results : In immunoglobulin productivity assay (cell), the IgG level of the KRG group significantly increased but there was no significant difference in the IgG of the CRG group. The IgM level of the KRG group significantly increased stimulated with PWM. When it was unstimulated, the level of IgM in KRG and CRG increased together. The IgA level of the KRG group significantly increased when it was stimulated with PWM and unstimulated. Conclusion : According to the above results, oral administration of red ginseng for 3 months is considered useful for immunomodulatory effect, and Korean red ginseng may be superior to Chinese red ginseng in that effect.
