CONSORT-CHM 2017 지침에 근거한 대한한의학회지 및 회원학회지에 투고된 경구투여 한약 중재를 활용한 무작위 배정 비교 임상 연구(RCT)의 양적, 질적 평가 연구 (Orally Administered Korean Herbal Medicine Medications of Randomized Controlled Trials Published in the Journal of Korean Medicine and Related Journals: A Narrative Analysis using CONSORT-CHM 2017)

  • Objectives: This study aims to explore the current usage status of orally administered Korean herbal medicine in randomized controlled trials (RCTs) published in the Journal of Korean Medicine and member journals using the CONSORT-Chinese Herbal Medicine Formulas 2017 (CONSORT-CHM 2017) checklist. Methods: We searched the OASIS, RISS, and KMBASE archives as well as the websites of the Journal of Korean Medicine and 45 member journals to identify RCTs that used herbal interventions. Two independent researchers searched and categorized the RCTs and performed a quantitative evaluation by journal, study design, and target disease, as well as qualitative evaluation of the literature using CONSORT-CHM 2017. Results: After the search, 66 articles were selected. The quantitative evaluation resulted in 13 articles (19.6%) that were published in the Journal of Korean Medicine and 12 articles (18.1%) in the Journal of Internal Korean Medicine. In terms of study design, 62 articles (93.9%) were parallel, 4 articles (6%) were crossover, and 2-arm parallel study designs were the most common in 45 articles (68.2%). In terms of the study participants, physiological characteristics and mechanisms in healthy individuals were the most common in 21 studies (31.8%) and obesity in 9 studies (13.6%). In terms of assessing completeness in the CONSORT-CHM 2017 items, 29 articles were rated high, 31 were rated moderate, and 6 were rated low. Items 4a, 6a, and 7a had low reporting rates (≤ 30%), while items 2a, 2b, and 12a were completely reported in all studies. Conclusion: Future RCTs using orally administered Korean herbal medicine need to be reported completely, and the CONSORT-CHM 2017 checklist can be a helpful tool for this purpose.

탕전실의 조제 관리 및 운영 기준에 대한 연구 (A Study on the Management and Dispensing Standards of Herbal Medicine in Herbal Dispensaries)

  • Objective : Herbal dispensaries, where medications are prepared for patients, require specific preparation and operations management for safe herbal medicines. This study aims to propose improvements in the herbal medicine preparation and operation of the herbal dispensaries as series study of the "A Survey on the Management Status of Extramural Herbal Dispensaries" Methods : We first checked current management regulations. In addition, we compared and analyzed related laws which are "State management norm of Traditional Chinese Medicine dispensaries", "Enforcement Rule of Medicinal Product Safety" and " Hazard Analysis and Critical Control Point(HACCP)", to introduce the assessment items of herbal dispenses evaluation and certification system. Results : In this study, we found that the current regulations, "Guideline on Installation and Utilization of Extremal Herbal Pharmaceutical Facility and Shared-use of Herbal Dispensary", are insufficient to ensure the quality and safety of herbal medicines. We suggested articles for the proper management of herbal dispensaries and these were introduced to the Herbal dispensary evaluation and certification system. Conclusion : We recommend evaluation articles in Herbal dispensary evaluation and certification systems need to be popular among herbal dispensaries, then Korean herbal medicine could restore credibility from the people.

해표방제간(解表方劑間)의 연관성(聯關性) 확립(確立)에 대(對)한 문헌적(文獻的) 연구(硏究) (A Study on the Interrelation Establishment of Herbal Formulas that Release the Exterior Syndrome)

  • Objectives : The process of disease treatment in traditional medicine is defined as differentiating patterns and determining treatment. As an important part of this process, the herbal formulas are specific combinations or configurations of medicinal herbs that serve as tools for treatment. As clinical experience increased, the number of herbal formulas also increased. For example, there are 61,739 herbal formulas in Pujifang, and 96,592 in Dictionary of traditional chinese medicine prescriptions. In chapter of Release the exterior syndrome (RES; 解表劑) of textbook, the 42 formulas were presented. For the understanding on the formula, most of books explained with description or figure about the formulas. But there is no study on the interrelationship of each herbal formulas. This study investigated the interrelationship of RES herbal formulas. Methods : The RES herbal formulas were analyzed with the ingredients, actions and clinical applications by literature study. Results : To improve cognition the interrelationship of RES herbal formulas, we analyzed the formulas and established the 19 interrelations of herbal formulas. Conclusions : Finally, this study established the interrelation of herbal formula as one figure. This result could contribute to improve cognition the interrelationship of RES.

한약의 국소 외용 마취제 활용 현황에 대한 문헌 고찰 (Literature Review on the Current Status of Herbal Medicine Use as Topical Anesthetics)

  • Objectives : The aim of this study is to review the topical anesthetic effect of herbal medicine. Methods : Using domestic database(OASIS, RISS) and foreign database(Pubmed, Embase, Cochrane library, CNKI), clinical studies about efficacy of herbal medicine as topical anesthetics were searched. Only randomized controlled trials(RCT) were selected and analyzed. Results : Total 647 studies were searched. After screening process, 5 articles were selected. 2 were from Chinese database and 3 were from English database. Articles were about clinical use of herbal medicine as topical anesthetics and compare their efficacy with conventional topical anesthetics. Out of 5 articles, 4 articles used pain rating scales recorded by subjects as outcome measurement. All studies showed lower pain score in herbal medicine using groups. 4 studies showed that there was no statistically significant difference in pain score between herbal medicine using group and conventional topical anesthetics using group. Conclusions : The results suggest that topical anesthetics using herbal medicine is as effective as or maybe even more effective than conventional topical anesthetics. However, experimental design and composition of intervention were all different, which provides inaccuracy when comparing the experimental results. Also, sample sizes were small and the number of RCTs were insufficient to identify full list of herbs that have potentials to be used topical anesthetics. Further studies need to be conducted to find out potentials of herbal medicine as topical anesthetics.

간암 한약제제 임상시험 가이드라인 개발을 위한 한약제제 무작위배정 대조군 임상시험 고찰 (Analysis of Existing Guidelines and Randomized, Controlled, Clinical Trials for Development of [Guideline of Clinical Trial on Herbal Medicinal Product for Liver Cancer])

  • Objective: This study aimed to ascertain what should be considered in the "Guideline for Clinical Trials with Herbal Medicinal Products for Liver Cancer," by analyzing existing guidelines and clinical trials. Methods: Committee for the development of a guideline, consisting of 6 Korean medicine doctors, reviewed guidelines and clinical trials on using herbal medicine for treating liver cancer. The trials were analyzed in terms of inclusion and exclusion of participants, intervention, comparators, outcomes, and trial design. We then compared the results of our analysis with the guidelines to identify issues we must to consider when following the "Guideline for Clinical Trials with Herbal Medicinal Products for Liver Cancer." Several guidelines for antitumor agents and clinical trials on herbal medicine were obtained from the Ministry of Food and Drug Safety homepage, etc. The search terms were as follows: "liver neoplasms"; "herbal medicine"; "medicine, Korean traditional"; and "medicine, Chinese Traditional.". Results: Ten articles were obtained from pubmed and Embase. There was no guideline for clinical trials on using herbal medicine for treating liver cancer. All the participants in the reviewed articles had primary liver cancer, and the type of intervention varied (e.g., decoction, patches, and capsules. The comparators included placebos and conventional treatments such as chemotherapy. The outcome assessment methods were tumor response, quality of life, survival, and liver function tests. Adverse events occuring during the trial were also evaluated. Conclusion: Findings were derived by reviewing existing guidelines and comparing them with clinical trials on liver cancer and herbal medicinal products. These results will be utilized in the development of the "Guideline for Clinical Trials with Herbal Medicinal Products for Liver Cancer."

소아 천식의 한약 치료에 대한 임상연구 동향 - 중의학 무작위 대조군 임상시험을 중심으로 (Trends in Clinical Research on Herbal Medicine Treatment in Pediatric Asthma - Focusing on Randomized Controlled Clinical Trials in Traditional Chinese Medicine)

  • Objective This study analyzed randomized clinical trials (RCTs) on pediatric asthma using traditional Chinese medicine (TCM) to determine its efficacy and safety. Methods We searched electronic databases in English, Chinese, Japanese, and Korean and evaluated 35 articles published up to December 28, 2022. Based on data from the literature, we analyzed treatments, results, composition of herbal medicine, frequency of medicinal herbs, and decoction. Results TCM for pediatric asthma was compared to supportive therapy, inhaled corticosteroids, and standardized treatments. Of the 35 studies, 32 showed that TCM treatments were effective in alleviating asthma symptoms. Furthermore, TCM treatment was considered safe compared to common asthma treatments. Among the TCM treatments for pediatric asthma, the most frequent decoctions were Gamisaganmahwangtang (加味射干麻黃湯) and Saganmahwangtang (射干麻黃湯). Ephedra Herba (麻黄), Armeniacae Semen (杏仁), Asiasari Radix et Rhizoma (細辛), Belamcandae Rhizoma (射干) were herbs that were frequently used for asthma in TCM decoctions. Conclusion This review showed that TCM is effective in treating pediatric asthma. More clinical RCTs are needed to confirm the efficacy and safety of TCM treatment.

Integrative applications of network pharmacology and molecular docking: An herbal formula ameliorates H9c2 cells injury through pyroptosis

  • Background: QiShen YiQi pills (QSYQ) is a Traditional Chinese Medicine (TCM) formula, which has a significant effect on the treatment of patients with myocardial infarction (MI) in clinical practice. However, the molecular mechanism of QSYQ regulation pyroptosis after MI is still not fully known. Hence, this study was designed to reveal the mechanism of the active ingredient in QSYQ. Methods: Integrated approach of network pharmacology and molecular docking, were conducted to screen active components and corresponding common target genes of QSYQ in intervening pyroptosis after MI. Subsequently, STRING and Cytoscape were applied to construct a PPI network, and obtain candidate active compounds. Molecular docking was performed to verify the binding ability of candidate components to pyroptosis proteins and oxygen-glucose deprivation (OGD) induced cardiomyocytes injuries were applied to explore the protective effect and mechanism of the candidate drug. Results: Two drug-likeness compounds were preliminarily selected, and the binding capacity between Ginsenoside Rh2 (Rh2) and key target High Mobility Group Box 1 (HMGB1)was validated in the form of hydrogen bonding. 2 μM Rh2 prevented OGD-induced H9c2 death and reduced IL-18 and IL-1β levels, possibly by decreasing the activation of the NLRP3 inflammasome, inhibiting the expression of p12-caspase1, and attenuating the level of pyroptosis executive protein GSDMD-N. Conclusions: We propose that Rh2 of QSYQ can protect myocardial cells partially by ameliorating pyroptosis, which seems to have a new insight regarding the therapeutic potential for MI.

위축성 질염의 한약 치료 임상 연구에 관한 체계적 문헌 고찰 (Review on Clinical Studies of Traditional Korean Medicine Treatment for Atrophic Vaginitis)

  • Objectives: The purpose of this study is to review clinical studies of traditional Korean medicine treatment (herb medicine) on vaginal atrophy. Methods: Key words "Vaginal atrophy", "Vulvovaginal atrophy", "Vaginal dryness", "Atrophic vaginitis", "Herb medicine", "Acupuncture", "Moxibustion", "Catgut embedding" were searched on 11 database systems (PMC, CENTRAL, EMBASE, CNKI, KTKP, KISS, NDSL, OASIS, DBpia, KMBASE, RISS) on May 31th 2019. In all searches, limits were set for MeSH (human) and language (English, Chinese or Korean). Results: 18 controlled clinical trials were collected in accordance with the selection and exclusion criteria. The types of intervention were herbal medicine, and types of herbal medicine were decoction, cream, suppository, oil, ect. The study results and methods of intervention were analyzed. Conclusions: Among the 18 trials, 5 studies showed that herbal medicine with hormone or antiprotozoal therapy was more effective than hormone or antiprotozoal therapy alone. And 2 studies showed that herbal medicine treatment was more effective than oil treatment of topical use. There was no abnormal vaginal bleeding, breast swelling and pain in TCM group.

『의방고(醫方考)』중 한의학 원전의 인용 (Citation of Traditional Chinese Medical Classics in 『Yifangkao』)

  • Objectives : The purpose of this paper is to describe how Wu Kun(吳昆) understood Huangdineijing 『黃帝內經』 and implemented his understanding when writting Yifangkao 『醫方考』. Methods : From Yifangkao 『醫方考』, we collected all paragraphs including "經曰", "內經曰" while excluding the ones with "論曰" to filter out Sanghanlun 『傷寒論』's perspective. The contents of Huangdineijing·Suwen 『黃帝內經·素問』 and Huangdineijing·Lingshu 『黃帝內經·靈樞』, cited in Wu Kun's Yifangkao 『醫方考』 were extracted, identified from which part it comes and sorted into headings under RyuPyeon Huangjenaegyung 『類編黃帝內經』. Results : The most cited traditional chinese medical classic in Yifangkao 『醫方考』 was Huangdineijing·Suwen 『黃帝內經·素問』 Yinyangyingxiangdalun 「陰陽應象大論篇」 which was cited 41 times. Zhizhenyaodalun 「至眞要大論篇」 had a total of 39 citations followed by Shengqitongtianlun 「生氣通天論篇」 and Liuyuanzhengjidalun 「六元正紀大論篇」 which were cited 13 times and 11 times each. In addition, it was confirmed that other 28 chapters from Huangdineijing·Suwen 『黃帝內經· 素問』, 3 chapters from Huangdineijing·Lingshu 『黃帝內經·靈樞』, 3 chapters from Nanjing 『難經』 and one chapter from Zhenjiujiayijing 『鍼灸甲乙經』 were also cited. Also the analysis of the classification of the Huangdineijing·Suwen 『黃帝內 經·素問』 showed that 57 citations were classified as 'treatment', 41 citations were classified as 'Yin and Yang and Five phases', 39 citations were calssified as 'Cause and Mechanism of Disease'. Conclusion : 1. Yifangkao 『醫方考』 have explained prescriptions by implementing principles from 5 kinds of traditional chinese medical classic including Huangdineijing·Suwen 『黃帝內經·素問』, Huangdineijing·Lingshu 『黃帝內經·靈樞』 etc. 2. According to table 3, Yifangkao 『醫方考』 have cited Huangdineijing·Suwen 『』 to explain how each compounds were used and how the Korean medical principle of Yin and Yang was applied in the prescriptions. 3. It can be suggested in formula study there is utility to understand principles of traditional chinese medical classic to comprehend prescriptions.

A Review of Current Clinical Research on Herbal Monotherapy for Coronavirus Disease-19 (COVID-19)

  • Objectives: The purpose of this study was to evaluate the effectiveness and safety of traditional herbal medicine as a stand-alone treatment group through major English databases due to the lack of RCTs in Korea, and to provide a review of the herbal interventions used. Methods: Using four databases (Pubmed, EMBASE, OASIS, RISS), combination of words such as "Coronavirus" "RCT" "Herb" "Decoction" "TCM" were used. RCTs using herbal medicines to treat coronavirus were searched. Final 4 studies were selected by two authors according to inclusion and exclusion criteria. Results: A total of 1,435 patients were studied. The Chinese herbs used in the treatment group were Shengmai Yin, JingYinGuBiao granules, Jinhua Qinggan granules, and Bufei Huoxue capsules. The intervention group showed greater attenuation of pneumonia lesions on CT. Also, improvement in 6-min walk distance (6MWD), and negative conversion rate in treatment group were reported. Furthermore, scores on the Fatigue Assessment Inventory (FAI) were lower in the herbal group than in the placebo group. The median time to recovery of COVID-19 related symptoms was shorter in TCM group compared to the control group. Reported adverse effects were diarrhea, liver dysfunction, and excessive menstruation, and two papers did not mention side effects in detail. Conclusion: Herbal medicine alone can increase the conversion rate of viral negativity and relieve COVID-19 related symptoms without significant adverse effects.
