Development of a virtual reality program in South Korea for the measurement of vital signs in children: a methodological study

  • Purpose: We developed a virtual reality (VR) program for use in pediatric nursing practicums to help nursing students learn to measure vital signs in children. Methods: The analysis, design, development, implementation, and evaluation model was employed between July 2021 and December 2021 at a university in South Korea. In the analysis phase, in-depth interviews were conducted with four nursing students, two nursing school graduates, and four experts. The topic and contents of the VR program were settled in the design phase. The VR program was developed and subsequently used and evaluated by 20 nursing students and four experts. Results: The contents of the VR program for pediatric nursing practicums included the measurement of vital signs in a newborn baby and a young child, as well as an evaluation system. The mean score for the nursing students' satisfaction with practice was 4.02 out of 5 points. The mean scores for overall satisfaction with the VR program were 4.15 and 4.79 out of 5 points for nursing students and experts, respectively. Conclusion: The VR program developed in this study allows nursing students to practice measuring vital signs in children, thus improving the students' clinical performance in pediatric nursing.

  • Purpose: The purpose of this study was to develop an instrument that evaluated QUality Of care Through patients' Eyes for hospitalized child (QUOTE-Child), and examine the validity and reliability. Methods: Preliminary items were reviewed through expert content validity and face validity. To test the validity and reliability of the instrument, the data were collected from 221 care givers of hospitalized children. Data were analyzed using exploratory factor analysis, Pearson correlation coefficient, Cronbach's alpha and Spearman-Brown coefficient. Results: Factor analysis yielded 19 items in four factors including 1) respect, 2) explanation, 3) kindness, and 4) skillfulness, with a cumulative explanatory variance of 70.68%. For criterion-related validity, a significant positive relationship was found between quality of care and pediatric family satisfaction. For internal consistency reliability, the Cronbach's ${\alpha}$ was .93 (importance) and .95 (performance) for the overall instrument. The half split reliabilities were .86, .95 (importance) and .92, .97 (performance). The Cronbach's ${\alpha}$ of 110 data was .92 (importance) and .94 (performance). Conclusion: Researchers and practitioners can use this instrument to systematically assess quality of care for hospitalized children and identify areas of support for hospitalized children and their family.

경련성 질환아 어머니의 스트레스, 대응 및 사회적 지지 (Stress, Coping and Social Support of Mothers of Children with Seizure Disorders)

  • Purpose: This study was done to investigate stress, coping and social support of mothers of children with seizure disorders, and to identify the relationship between these variables. Method: The participants were 105 mothers of children with seizure disorders. Data were collected through self-report questionnaires which were constructed to include stress, coping and social support. The data were analyzed using the SAS program. Result: Stress of mothers was significantly different according to number of times the child had been hospitalized and the mothers perception of the state of the child's health. Coping by mothers was significantly different according to religion, general health state and if mother knew the child's diagnosis. Social support for mothers was significantly different according to education level, monthly medical expenses and number of times the child had been hospitalized. Social support for mothers was correlated with stress and coping. Conclusion: The above findings indicate that coping in mothers of children with seizure disorder is related to social support. Therefore nursing interventions to reduce stress and improve coping should be provided for mothers of children with seizure disorder.

신종플루 확산(2009년) 이후 호흡기감염 아동 보호자의 감염관리 지식정도 및 수행정도 (Knowledge and Practice Level of Infection Management for Child Guardians with Respiratory Infections after H1N1 Diffusion (2009))

  • Purpose: The purpose of this study was to investigate the knowledge and practice level of infection managements in child guardians with respiratory infections after H1N1 diffusion of 2009. Methods: The data were collected during February and March 2010 using a self-report questionnaire. The participants were 228 child guardians. Data were analyzed using frequency, percentage, means, SD, t-test, F-test, Pearson correlation coefficients, and Cronbach's alpha with the SPSS 17.0 program. Results: Mean age of the guardians and children were 37.8 years, 4.1 years respectively. For knowledge level of respiratory infections, hand washing method had the highest score and snuffles prevention method, the lowest. For practice level for respiratory infections, environmental hygiene management had the highest score and symptom management, the lowest. Infection management knowledge and practice level had a positively significant correlation in every area. Conclusion: The results indicate the necessity of continual education on infection management, and that the most efficient timing for the education appears be for participants during their first pregnancy or after the woman has delivered her child.

모아상호작용의 측정에 대한 고찰 (Review of the Measurement of Mother Child Interaction)

  • Purpose: The purpose of this study was to review the concept of Mother-Child Interaction (MCI) as well as its theoretical model and measurement methods. Methods: A review of related theories and research was done to identify the concept and present status of MCI measuring techniques. Results: The concept of MCI contains the two aspect of attachment and child rearing. MCI is unique in that it includes a dyadic process. The concept of MCI was developed based on attachment theory, Barnard's theory, and the goodness of fit model, MCI involves sensitivity, responsiveness, reciprocity, contingency, mutual satisfaction, and child developmental stimulation including positive and negative affection. Most instruments involved observation, especially in structured situations, and most were developed for mother-infant interaction. Conclusion: MCI is a reciprocal, goal-oriented partnership process and is an ongoing process during childhood. Instruments for measuring MCI for preschoolers as well as appropriate self-reporting instruments need to be developed for wide use in practice and in research.

Analysis of Metacommunicative Episodes between Nurses and Children

  • Purpose: The purpose of this study was to analyze metacommunicative episodes comprised of nurses' metacommunicative behavior and children's responses occurring in the dyads of nurse-child within the context of an inpatient pediatric unit. Methods: Twelve dyads of nurses and children were videotaped for four hours each day over a two-day period as they interacted with each other on the inpatient unit. The metacommunicative episodes were recorded from the videotapes. The metacommunicative behavior was categorized within Shin's metacommunicative behaviors. Results: The total number of episodes between the nurses and children included in this study was 242. The most frequently used metacommunicative behavior was 'reflection', followed by tagging, baby talk, approaching, mediating eye level, friendly demand, encouraging, description of acts, symbolization, turnabouts, touching, and mimic voice, respectively. The most common response to the nurses' metacommunicative behavior was agreeing, followed by tension release. Conclusion: Young children responded positively to metacommunicative behavior by the nurses. Understanding metacommunicative behavior may help nurses approach their young patients more easily and with confidence.

입원 아동 어머니의 피로 영향 요인 (Factors Influencing Fatigue in Mothers of Hospitalized Children)

  • Purpose: Fatigue in mothers of hospitalized children is an important component for her child's health. This study was done to identify factors influencing fatigue in mothers of hospitalized children. Methods: A descriptive correlation study design was used. Participants were 157 mothers of hospitalized children in a university hospital located in one city. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients and stepwise multiple regression. Results: Mean score for fatigue in mothers of hospitalized children was $2.11{\pm}0.53$. There were significant differences in scores for fatigue in these mothers according to education, sleep satisfaction, and family support. Multiple regression analysis showed that the important factors related to fatigue in mothers of hospitalized children were parenting stress (${\beta}=.44$), hours of sleep (${\beta}=-.25$), and age (${\beta}=-.21$). These factors explained 38% of the total variance. Conclusion: Research results suggest that nursing interventions for mothers of hospitalized children are needed in order to improve the comfort and health of the mothers and facilitate the recovery of their child.

  • Purpose: The purpose of this study was to examine the frequency, perceived importance and competence of pediatric emergency nursing practice (PENP) in nurses who cared for pediatric patients in the emergency department. Methods: This cross-sectional descriptive study analyzed 175 emergency department nurses caring for pediatric patients at 7 university hospitals with more than 500 beds, located in Seoul and Gyeonggi Province. The measurement tool was modified from the Classification of Standard Nursing Activities to measure the frequency, importance of PENP, and nursing competence. It comprised 143 items in 16 domains of PENP. Results: The most frequent nursing practice was the domain of 'nursing records and environmental management' and the least frequent practice was the 'research and consulting' domain. The nursing care domain perceived to be most important by nurses was 'specialized intensive nursing care'. The nursing care domain with the highest level of nursing competence was 'hygiene care', and the domain with the least level of nursing competence was the 'research and consulting'. Conclusion: These results will be utilized as basic data for future pediatric emergency nursing education initiatives and for establishing priorities of nursing policy to improve health care for children admitted to the emergency department.

Effects of Massage and Attachment Promotion Program on Social Maturity, Child Autism and Attachment of Children with Autism and Their Mothers

  • Purpose: The aims of this study were to evaluate effects of massage therapy and the attachment promotion program with autistic children. Method: A quasi-experimental design was conducted with a convenience sample of 44 autistic children. The experimental group (n= 23) attended the massage therapy and the attachment promotion program for 4 months. The control group (n=21) only had the attachment promotion program. Measurements were made twice for both groups using the social maturity scale and child autism rating scale. Recording of mother-child attachment was done for 15 minutes in only the experimental group twice using video equipment. Results: After 4 months of interventions, there was significant difference on social maturity (F=9.01, p=.005) between the groups. However, there was no significant difference on CARS ((F=2.47, p=.124). The total scores of mother-child attachment between pre- and post-interventions showed a significant difference (Z=-3.42, p=.001). Conclusion: The results showed that massage therapy and the attachment promotion program might be an effective way for providing a chance to increase social maturation and to increase attachment between mother and autistic child.

  • The purposes of this study were to identify the relationship between parent and child with chronic illness and developmental disability, and to review the strategy for improvement of their relationship. The effects of chronic illness and developmental disability is that the child has delayed growth and development, and his(her) parent has sadness, guilty feeling, anxiety, grief, disappointment, low self-esteem, anger and resentment. Chronic illness and developmental disability also have a negative effect to the parental marital relationship. The reaction of parent and child varies in age of onset, a developmental transition period, crisis and parent-child relationship. Through this study, I propose that parent-child relationship could be improved by touch, communication between them and education for parents.
