This study explored the effects of maternal ambivalence over emotional expressiveness (AEE) and mother-adolescent communication on adolescent depression (depressed affect, positive affect, somatic symptoms and activity inhibition, interpersonal difficulties). Data were taken from 233 middle school students (128 boys, 105 girls) and their mothers. Regression analyses showed varying gender differences in the relationships among these variables. Boys with high maternal AEE had lower positive affect, and higher depressive affect, somatic symptoms and activity inhibition, and interpersonal difficulties, whereas girls' moods were not influenced by maternal AEE. In addition, boys with problems in mother-dolescent communication exhibited lower positive affect, higher depressed affect and interpersonal difficulties, but open communication was not related to any depressive symptoms. The mother-adolescent communication type did not appear to be associated with depression in girls either. Both boys and girls both had less open communication and more problem communication with mothers experiencing high AEE. Problem communication with mothers partially mediated the relationship between maternal AEE and interpersonal difficulties in boys only. Examination of maternal ambivalence over emotional expressiveness provides a deeper context for our understanding of negative family communication patterns and the psychological consequences, especially in mother-adolescent boy dyads.

Objectives: This study was conducted to investigate the reliability and validity of the Korean version of the DSM-5 Level 2 Cross-Cutting Symptom Measure-inattention [Swanson, Nolan and Pelham, version IV (SNAP-IV)] and anger [Patient-Reported Outcome Measurement Information System (PROMIS) Anger] for parents and guardians of children aged 6-17 years. Methods: We included 104 children and adolescents diagnosed with attention-deficit/hyperactivity disorder (ADHD), ADHD with anxiety and depression, depressive disorder, anxiety disorder, and tic disorder with somatic symptoms (ADHD=41, depression=9, anxiety=14, ADHD+anxious depression=11, tic+somatic symptoms=29). Their ages ranged from 8 years to 15 years. The participants' mothers completed the SNAP-IV, PROMIS Anger scale, Korean version of the IOWA Conners Rating Scale (K-IOWA), and Korean ADHD Rating Scale (K-ARS) so that the reliability and validity of the SNAP-IV and PROMIS Anger scales, which are DSM-5 scales for assessing inattention and anger of children and adolescents, could be examined. Results: The reliability coefficient of SNAP-IV (Cronbach's α) was 0.94. The correlation coefficients between SNAP-IV, K-IOWA inattention, and K-ARS inattention scores ranged from 0.73 to 0.86. The mean SNAP-IV scores of the ADHD and the ADHD+anxious depression groups were significantly higher than those of the anxiety and the tic+somatic symptoms groups. The reliability coefficient of the PROMIS Anger was 0.91. The correlation coefficient between PROMIS Anger and K-IOWA oppositional/defiant scores was 0.75. The PROMIS Anger mean score of the ADHD+anxious depression group tended to be higher than that of the other groups. Conclusion: These results suggest that the Korean version of the DSM-5 Level 2 Cross-Cutting Symptom Measure-inattention and anger for parent and guardian of child age 6-17 might be a reliable and valid test and may be useful for screening children and adolescents with ADHD.

Objectives : This study was conducted to examine whether there are qualitative differences in attention problem among children with various psychiatric disorders, including attention-deficit hyperactivity disorder (ADHD), depressive disorder, anxiety disorder, and tic disorder using clinical ADHD diagnostic system (ADS) T-scores. Methods : The subjects were 794 outpatient children aged from 5 to 15 years, including 540 children with ADHD, 95 children with depressive disorder, 86 children with anxiety disorder, and 73 children with tic disorder. Clinical T-scores on the ADS were calculated using the mean and standard deviations of four ADS variables for the ADHD group. Results : All four groups had T-scores on the ADS in the abnormal range. However, when comparing the clinical T-scores, the children with depressive and anxiety disorders performed better than the children with ADHD. We also found that although the four groups seemed to be similar in terms of clinical T-scores for omission and commission errors, there were significant differences in clinical T-scores for reaction time and the standard deviation of response time (RT) between the ADHD and other groups. Conclusions : We concluded that inattention and impulsivity might not be specific only to ADHD and that the clinical T-scores of RT and standard deviation of RT on the ADS could be used to discriminate between ADHD and other clinical groups.

Objectives : The aim of this study is to assess the career paths and practice patterns of child and adolescent psychiatrists in South Korea. Methods : A survey on demographic data, work environment, opinion on fellowship training program, and current practice patterns was conducted in 76/223 (34.0%) of all child and adolescent psychiatrists (N=223) were from 1980 to 2008. The survey data from the responders were analyzed. Results : More than half of those surveyed were women. The mean age of survey participants was 40.8 years; most were married, and lived in the Seoul National Capital Area. In general, the responders were very satisfied with their work. They worked in private practice, public sector, group practice, and as consultants. Up to 87% of the patients treated were children and adolescents. The most common treatment modalities were medication and parent psychoeducation. Social skills training and play therapy were applied mainly for children aged 4 to 12 years old. Cognitive behavioral therapy and counseling were preferred in adolescents. Conclusion : The current study used a database approach in order to define current practice patterns. These data may be helpful in consideration of future training programs, workforce issues in child and adolescent psychiatry in South Korea, and also in collaboration with other mental health providers.

Objectives: The aim of the current study is to test for the association of DRD4 exon III VNTR polymorphism with attention-deficit hyperactivity disorder(ADHD) in Korean children population. Methods: 113 Korean children with ADHD, 102 parents, and 133 control subjects participated with the current study. The distribution of genotypes and alleles of DRD4 exon III VNTR in children with ADHD was compared with that in control subjects. In addition, 69 children with ADHD and their parents were analyzed using the transmission disequilibrium test(TDT). Results: We could not find any significant differences in the distribution of genotypes and alleles at DRD4 exon III VNTR polymorphism between children with ADHD and control subjects. In addition, there was no preferential transmission of long allele of DRD4 exon III VNTR polymorphism. Conclusion: These results suggest that DRD4 is not associated with ADHD in Korean children population.

Electroencephalography (EEG) measures neural activation due to various cognitive processes. EEG and event-related potentials (ERPs) are widely used in studies investigating psychopathology and neural substrates of psychiatric diseases in children and adolescents. The present study aimed to review recent ERP studies in child and adolescent psychiatry. ERPs are non-invasive methods for studying synaptic functions in the brain. ERP might be a candidate biomarker in child-adolescent psychiatry, considering its ability to reflect cognitive and behavioral functions in humans. For the EEG study of psychiatric diseases in children and adolescents, several ERP components have been used, such as mismatch negativity, P300, error-related negativity (ERN), and reward positivity (RewP). Regarding executive functions and inhibition in patients with attention-deficit/hyperactivity disorder (ADHD), P300 latency, and ERN were significantly different in patients with ADHD compared to those in the healthy population. ERN showed meaningful changes in patients with anxiety disorders, such as generalized anxiety disorder, separation anxiety disorder, and obsessive-compulsive disorder. Patients with depression showed significantly attenuated RewP compared to the healthy population, which was related to the symptoms of anhedonia.

소아청소년 정신과에서는 정신분열증과 기타 정신증상에 기존 항정신병 약물을 사용하고 있다. 이들 약물은 졸리움, 기립성 저혈압, 추체외로 증상 등의 부작용이 문제가 된다. 성인에서는 최근 비정형 항정신병 약물사용이 기존 약물을 대체해가고 있다. 소아정신과에서도 기존 항정신병 약물과 비슷한 효과를 보이면서도 부작용이 훨씬 적은 비정형 항정신병 약물의 사용을 시도하고 있다. 하지만 소아환자에 대한 장기 사용 자료는 물론 단기 사용 자료도 매우 빈약한 실정이다. 본 논문에서는 소아청소년 환자에서의 비정형 항정신병 약물 사용에 대한 자료를 검토하였다. 그렇게 하므로 써 소아청소년 정신과에서의 비정형 항정신병 약물 사용에 대한 적절한 접근 방법을 제시하고자 한다.
A sample of 578 adolescents responded to self-report questionnaires. Results showed that parental education levels and occupation and adolescent perceptions of their socioeconomic status were positively related to maternal expectations. Adolescent perceptions of their socioeconomic status were related to maternal emotional support and adolescent self-esteem. Adolescent achievement motives were positively related to parental education levels and adolescent perceptions of their socioeconomic status. Maternal expectations/emotional support and adolescent perceptions of their socioeconomic status were positively related to adolescent achievement motivation. Maternal expectations played a mediating role between parental educational levels and adolescent perceptions of their socioeconomic status and adolescent achievement motives. Maternal emotional support and adolescent self-esteem played mediating roles between adolescent perceptions of their socioeconomic status and achievement motives.
