Clinical usefulness of teleradiology in general dental practice

  • Purpose: This study was performed to investigate the clinical usefulness of teleradiology in general dental practice. Materials and Methods: Two hundred and seventy five cases were submitted for inquiry to the case presentation board of the website of The Korean Academy of Oral and Maxillofacial Radiology for a 5 year periods. The diagnosis results of those cases were analyzed according to the disease classification, the correlation with the patient's chief complaint, the necessity of additional examinations or treatments, the image modalities, and the number of dentists inquiring. Results: Differential diagnoses of normal anatomic structures were the most frequently submitted cases, covering 15.6% of all cases. Among 275 cases, 164 cases required no additional treatments or examinations. Panoramic radiographs were the most frequently submitted images, accounting for 248 inquiries. The 275 cases were submitted by 96 dentists. Fifty-two dentists wrote one inquiry, and 44 inquired 2 or more times. The average inquiry number of the latter group was 5.0 cases. Conclusion: A teleradiology system in general dental practice could be helpful in the differential diagnosis of common lesions and reduce unnecessary costs.

A Clinical Case Report of Soyangin Patient with Peripheral Coldness (양독백호탕으로 호전된 소양인 사지 말단부위 냉증 환자 1례 보고)

  • Objectives The purpose of this study was to report significant improvement of peripheral coldness in patient, who was diagnosed with 'Soyangin Chest-heat symptomatic pattern' and applied medication based on Sasang Constitutonal medicine. Methods The patient was treated with herbal medications, according to his own symptomatology. The objective outcome was measured by thermography and the subjective sensation was assessed by using Visual Analogue Scale (VAS). Results The symptoms of peripheral coldness improved remarkedly without any side effects. Accessory symptoms and original symptoms also changed for the better. Conclusions This results show that it is available that peripheral coldness is treated with herbal medications according to Sasang Constitutional medicine, especially ingenious in terms of the fact that some patient can be diagnosed with heat-based condition on the basis of original symptoms, although his/her chief complaint is classified as cold pattern.

Incidental finding of an extensive oropharyngeal mass in magnetic resonance imaging of a patient with temporomandibular disorder: A case report

  • In this report, we describe the incidental finding of an oropharyngeal mass in a patient who presented with a chief complaint of temporomandibular pain. The patient was initially evaluated by an otorhinolaryngologist for complaints of headaches, earache, and sinus congestion. Due to worsening headaches and trismus, he was further referred for the management of temporomandibular disorder. The clinical evaluation was uneventful except for limited mouth opening (trismus). An advanced radiological evaluation using magnetic resonance imaging revealed a mass in the nasopharyngeal/oropharyngeal region. The mass occupied the masticatory space and extended superioinferiorly from the skull base to the mandible. A diagnostic biopsy of the lesion revealed a long­standing human papilloma virus (HPV­16)­positive squamous cell carcinoma of the oropharynx. This case illustrates the need for the timely radiological evaluation of seemingly innocuous orofacial pain.

Clinical Report of One Case with Insomnia, Depression and Anxiety Disorder Improved by Traditional Korean Medical Treatment and Breathing Meditation (한방치료와 호흡명상으로 호전된 불면 및 우울증을 동반한 불안 장애 환자 치험 1례)

  • Objectives : Insomnia, depression and anxiety disorder are widespread in elderly patients. Mood disorder and insomnia share complex theoretical and clinical relationships. We investigated if traditional Korean medical treatment and breathing meditation are effective in patient with insomnia, depression and anxiety disorder. Methods : We treated one patient with traditional Korean medical treatment such as accupuncture, herb-medication, moxa and breathing meditation Results : After Treatment, the chief complaint and accompanying symptoms were improved, and the score of BAI, SADS, BDI, PSQI, IQ were decresed. Conclusions : According to this study traditional Korean medical treatment is effective for the cure of insomnia, depression and anxiety disorder and breathing meditation forifies the traditional Korean medical treatment.

  • Dental clinicians commonly encounter extrusion of the anterior teeth due to localized alveolar bone loss in adult patients with chronic periodontitis. This pathologic migration of the anterior not only may cause loss of functional anterior guidance and continuous periodontal destruction, but also can lead to esthetic disaster. In fact, most frequently reported chief complaint of the patients with extruded anterior teeth is the 'loss of esthetics', not the periodontal problem itself. The utilization of the expertise and skills in the various dental disciplines is thought to be the best treatment modality for the ultimate functional and esthetic attainment. Interdisciplinary therapy, involving periodontics, orthodontics, and restorative dentistry, was performed for the highest levels of patient and provider satisfaction in this case.

Microsurgical Approach for Root Coverage of Receding Gingiva in the Esthetic Zone

  • Facial esthetics and smiling are key components in nonverbal communication and have an important role in determination of the first impression of a person. The various components of the smile in dental esthetics include Gingival scaffold, lip framework, and Teeth. The periodontist creates a smile by performing various periodontal plastic microsurgery procedures for management of mucogingival problems. A 25-year-old patient reported to the Department of Periodontology at Teerthanker Mahaveer Dental College and Research Center, Moradabad, Northern India, with the chief complaint of long looking teeth in the upper jaw, making him conscious while smiling. Miller class I gingival recession with Maxillary left canine (23) was diagnosed. Periodontal plastic microsurgery employing double papilla grafting with connective tissue graft harvested from the palate in order to cover denuded root was performed using microsurgical instruments and microsuturing with 6-0 suturing material under magnification. Healing was uneventful, with achievement of 100% root coverage of denuded root after three months. The patient was highly impressed and satisfied with his enhanced smile.

  • 1. Objectives: This study aims to report symptomatic improvement of a Soyangin persistent hiccup patient with Hyeongbangdojeoksan. 2. Methods: The patient was treated with Hyeongbangdojeok-san and acupunctural therapy. The patient's chief complaint and ordinaty symptoms were observed during the outpatient care period. 3. Results and Conclusions: In results, Soyangin patients's hiccup and ordinary symptom as well as improved. The management of the constitutional pathological syndromes by constitutional perspective than focusing on the symptoms of hiccups will provide more comfortable life and better health levels.

  • For the treatment of bronchial stenosis due to trauma, inflammatory and neoplastic lesion, bronchoplastic procedure in the interest of preservation of lung tissue are relatively new developments in the field of thoracic surgery. We reported on case of bronchoplasty using to pericardial patch for the treatment of bronchial stenosis due to chronic inflammation. The patient was 26 years old female and chief complaint was respiratory difficulty. Bronchogram revealed diffuse stenosis of left main bronchus about 4cm and especially, at just below the carina marked narrowing of lumen and fine serration in the wall. At the time of operation, longitudinal incision was made at left main bronchus about 5cm and reconstructed bronchus using to pericadial patch at membranous compartment of bronchus. The postoperative course was uneventful and post-operative follow up bronchography showed that improvement of bronchoplastic segmented region.

  • The malignant germ cell tumor found in the gonad can originate in the anterior mediastinum. Endodermal sinus tumor[Yolk sac tumor] is a kind of malignant germ cell tumor and is derived from extra-embryonic mesoderm. We experienced a case of primary mediastinal endodermal sinus tumor occurred in 22 year old male patient. His chief complaint was anterior chest pain for 2 days. The tumor located in the anterior mediastinum and invaded upper lobe of the deft lung and pericardium. A left upper lobe resection including phrenic nerve and pericardium was performed and the tumor in the anterior mediastinum was excised. The patient has been treated with combination chemotherapy[Cis-platinum, Vincristine, Actinomycin-D, & Cyclophosphamide} and followed up for 4 months with partial remission.

  • Authors carried out a retrospective study in 22 infants with Bochdaleck`s hernia who were operated upon, the results are; 1] the male to female ratio was 2:1 and the incidence of left side was threefold prevalent than right, 2] the chief complaint of those who were operated upon in neonatal period was dyspnea and diagnosis was possible by chest A-P, 3] primary repair was possible except one patient in whom a prosthesis was used. 4] complication occurred in 41% and pulmonary complication was most common including 5 pneumothorax, 5] mortality rate was 14%, the deaths occurred in infants who were operated upon within 48 hours of life and the lesion was left without sac, the defect was larger than average and all died within 72 hours after operation.
