Pulmonary Hydatid Cyst -Two Cases Report- (폐포충낭종 -2예 보고-)

  • Echinococcal or hydatid cyst is one of the most important zoonoses and commonly seen throughout the world, especially sheep and cattle raising areas, but rare in Korea. The causative organism, Echinococcus granulosus, is a small tapeworm and dogs are the usual source of infestation. Lung is the second most common focus for this disease, after liver. Recently, with increasing numbers of Koreans visiting to those endemic countries especially Middle East, the reports of hydatidosis are increasing. Authors experienced two cases of pulmonary hydatid cyst in the 31 year old male and 44 year old male who had a history of spending 2 * 3 years in Middle East. we found them in incidental routine chest X-ray.

Intralobar Puhnonary Sequestration - Report of one case - (폐 내엽형 격절부:1례 보고)

  • We experienced a case of intralobar pulmonary sequestration preoperatively confirmed. The 10 years old male patient was admitted beacuse of recurrent episode of coughing and production of purulent sputum. the chest X-ray showed a dense mass containing a large cyst with air-fiuid level in right lower lung field. An aortogram was performed and revealed that the sequestrated portion of the RLL was supplied by an aberrant large artery arising directly from the lower thoracic aorta. After division and ligation of the aberrant artery in pulmonary ligament, the right lower lobectomy was performed and the patient`s postoperative course was uneventful.

Localized pleural mesothelioma -A case report- (Localized pleural mesothelioma -수술치험 1예-)

  • Pleural mesothelioma is the tumor of the cell of mesodermal origin lining the pleura. It is relatively uncommon tumor and its localized form is much rarer than diffuse form. Authors experienced a localized mesothelioma in a patient who was 44 year old male worker at copper plumbing field for 20 years, and admitted due to incidentally found abnormal chest X-ray. Exploratory thoracotomy was done and a 23 x 16 x 8 cm sized solitary mass was resected with adherent right middle lobe. Low grade malignancy of the pleural mesothelioma was confirmed by the pathology. We report the case with a literature review.

Solitary fibrous tumor of Pleura - 1 case report - (흉막에 발생한 고립성 섬유종 - 외과적 치험 1례 -)

  • Solitary fibous tumor of pleura is a rare and slow vowing neoplasm. The histogenesis of this tumor is thought as mesenchymal origin by immunohistochemical and ultrastructural bases. This case report is of a 69-year-old female who presented severe dyspnea and orthopnea with long-term history. Preoperative chest X-ray an chest CT revealed huge and hemithorax-occupying heterogenous mass and cardiomegaly. She underwent surgical resection and a 22 X 18 X 10 cm sized visceral pleural based mass was evacuated. Histologic findings of tumor showed patternless proliferation in a fibrocollagenous and well vasculized stroma. Immnohistochemical staining revealed strong positive in CD34, that are compatible with solitary fibrous tumor of pleura. The patient was discharged without any complications and no evidence of recurrence was found after 2 years.

Comparison of Image Quality and Dose According to the Arm Positioning in the Chest CT (흉부 CT 검사에서 환자 팔의 위치에 따른 영상의 화질과 선량 비교)

  • The aim of this retrospective research was to investigate the influence of the patient's arm position on radiation dose and scanning during CT. Chest CT image created image degradation, artifact and overdose to the patient due to the difference of the chest thickness. Therefore, the patient's arm should up position during the CT chest examination. In 2012, 1,642 patients underwent chest CT examination in Seoul K hospital. 118 chest CT examination performed hands down position. The average DLP value of the CT chest arm up examination was 275 $mGy{\cdot}cm$. The average DLP value of the CT chest arm down examination was 312.46 $mGy{\cdot}cm$. In the retrospective study with same patient, The average DLP value of the CT chest examination arm up vigorously was 267.5 $mGy{\cdot}cm$. The average DLP value of the CT chest arm down examination was 307.5 $mGy{\cdot}cm$. Chest CT scan without raising arm created linear artifact due to the lack of X-ray photons which is the thickest part of the human body of shoulder area. In conclusion, arm positioning patients' arms above the shoulders at CT of the chest increases image quality and substantially reduces effective radiation dose.

The Significances of Chest X-ray Findings and Serum IFN-γ, IL-5, ECP as Predictors of Prognosis of Bronchiolitis and Bronchopneumonia in Infants (영아의 세기관지염과 기관지 폐렴의 예후 지표로서 혈청 IFN-γ, IL-5 및 ECP치와 흉부 방사선 소견의 의의)

  • Purpose : Lower respiratory tract infections in infant and young children are often due to a virus, especially the Respiratory syncytial(RS) virus. Chest X-ray findings in bronchiolitis and bronchopneumonia are different. The radiographic hallmark of bronchiolitis is pulmonary hyperinflation and similar to that of bronchial asthma. Bronchiolitis is predisposed to later development of bronchial asthma. To evaluate the difference of immuno-pathophysiology between bronchiolitis and bronchopneumonia, we measured $IFN-{\gamma}$(Th1 cytokine), IL-5(Th2 cytokine) and ECP. We also investigated whether X-ray findings in infants with viral infected respiratory disease are useful in predicting the development of asthma. Methods : We measured IL-5, ECP, $IFN-{\gamma}$ levels in serum from 21 infants with bronchiolitis and 21 infants with bronchopneumonia and 16 infants without pulmonary viral diseases. Results : IL-5 levels of bronchiolitis and bronchopneumonia were significantly higher than those of the control(P=0.02, P=0.042). IL-5 levels of bronchiolitis were higher than those of bronchopneumonia but there was no significant difference. $IFN-{\gamma}$ levels of bronchopneumonia were higher than those of bronchiolitis but there was no significant difference. ECP levels of bronchiolitis and bronchopneumonia were higher than those of the control but only those of bronchiolitis were significantly higher than those of the control(P=0.045). IL-5 and ECP levels did not show any significant correlation in bronchiolitis, bronchopneumonia and control groups. Conclusion : We cannot prove the distinct differences in serum Th1/Th2 cytokine profiles between bronchiolitis and bronchopneumonia in infants. These results suggest that the different findings on chest X-ray between bronchiolitis and bronchopneumonia could not be a predictor of later development of asthma.

Hemangiopericytoma, Originating from the Posterior Mediastinum: Report of A Case (후종격동에서 발생한 혈관 외피 세포종: 1례 보고)

  • Hemangiopericytoma is a rare tumor of vascular origin, first described by Stout and Murray in 1942. It is characterized by a proliferation of capillaries, surrounding by a mass of spindle shaped or round cells. A 55 year-old man was admitted with a 2 years history of dull pain on the right upper posterior chest and mild dyspnea on Feb. 1978. On admission, chest PA and right lateral x-ray showed a large well defined homogenous increased density in the right upper posterior chest. Yellowish to brownish colored huge firm mass, which occupied entirely the right superior posterior mediastinum, was removed. The tumor was dense adhesive with right upper & lower lobe and Rt. upper posterior chest wall. The origin of tumor was not obvious. The tumor was confirmed as hemangiopericytoma, locating at the right posterior mediastinum by histopathologic examination. The postoperative course was uneventful, and he was made irradiation therapy after discharge.

Traumatic Tracheal Rupture by Blunt Chest Injury -Report of a Case- (비관통성 흉부 손상에 의한 기도 파열 -1례 보고-)

  • Tracheal rupture by a blunt trauma is an uncommon injury, and its clinical presentations are variable. It is a kind of the modern hazard. Herewith, we report a successful management of the tracheal rupture. A 22 year-old female was transferred from other hospital 4 hours after a car crash. Physical examination, simple chest X-ray, Chest CT and fiberoptic bronchoscopy revealed rupture of the membranous portion of the trachea about 5cm in length extending to the right main bronchus. Ruptured membraous portion of the trachea was sutured directly with absorbable suture. Her postoperative course was uneventful, and follow-up fiberoptic bronchoscopy revealed intact membranous portion of the trachea.

A Study on the Chest Radiography by Simultaneous Double Radiation Quality (동시(同時) 이선질(二線質)에 의한 Chest Radiography의 연구(硏究))

  • We have developed of the exposure technique for the chest radiography to get different contrast in left and right lung fields simultaneously for the effective diagnosis, I.E., get high contrast to detect tuberculosis, dispersal shadow or exudative lesions of early tuberculosis and get better sharpness and wider latitude to detect growing lesions of lung cancer or lesions overlapped with bones. As a result, by using an additional filter Cu 1.2mm with BX-III screen side and sheet of yellow cellophane on BF-III screen side at the conventional KV ($80KV{\sim}100KV$) for the chest X-ray, we can get similar densities in both left and right lung fields and, thus, this method is considered to be very effective for the quality diagnosis for the routine chest radiography.

Malignant Hemangiopericytoma of the Chest Wall (흉벽에 발생한 원발성 악성 혈관주위세포증)

  • Primary hemangiopericytoma of chest wall is very rate and only a few cases have ever been reported. The tumor aries from the capillary pericyte of Zimmerman. It is a highly vascular slow growing tumor which can be calssified as both malignant and benign varieties. We report a case of a 66 year-old man in whom recurrent hemangiopericytoma was treated by complete surgical excision. In October 1993 he had received surgical excision of hemangiopericytoma on posterior chest wall. For more than 6 years after the operation he was in good condition until a recurrent mass was found on the chest X-ray. The patient was discharged 9 days after the operation and is receiving radiotherapy.
