Establishment of Local Diagnostic Reference Levels of Pediatric Abdominopelvic and Chest CT Examinations Based on the Body Weight and Size in Korea

  • Objective: The purposes of this study were to analyze the radiation doses for pediatric abdominopelvic and chest CT examinations from university hospitals in Korea and to establish the local diagnostic reference levels (DRLs) based on the body weight and size. Materials and Methods: At seven university hospitals in Korea, 2494 CT examinations of patients aged 15 years or younger (1625 abdominopelvic and 869 chest CT examinations) between January and December 2017 were analyzed in this study. CT scans were transferred to commercial automated dose management software for the analysis after being de-identified. DRLs were calculated after grouping the patients according to the body weight and effective diameter. DRLs were set at the 75th percentile of the distribution of each institution's typical values. Results: For body weights of 5, 15, 30, 50, and 80 kg, DRLs (volume CT dose index [CTDIvol]) were 1.4, 2.2, 2.7, 4.0, and 4.7 mGy, respectively, for abdominopelvic CT and 1.2, 1.5, 2.3, 3.7, and 5.8 mGy, respectively, for chest CT. For effective diameters of < 13 cm, 14-16 cm, 17-20 cm, 21-24 cm, and > 24 cm, DRLs (size-specific dose estimates [SSDE]) were 4.1, 5.0, 5.7, 7.1, and 7.2 mGy, respectively, for abdominopelvic CT and 2.8, 4.6, 4.3, 5.3, and 7.5 mGy, respectively, for chest CT. SSDE was greater than CTDIvol in all age groups. Overall, the local DRL was lower than DRLs in previously conducted dose surveys and other countries. Conclusion: Our study set local DRLs in pediatric abdominopelvic and chest CT examinations for the body weight and size. Further research involving more facilities and CT examinations is required to develop national DRLs and update the current DRLs.

The reduction methods of operator's radiation dose for portable dental X-ray machines

  • Objectives: This study was aimed to investigate the methods to reduce operator's radiation dose when taking intraoral radiographs with portable dental X-ray machines. Materials and Methods: Two kinds of portable dental X-ray machines (DX3000, Dexcowin and Rextar, Posdion) were used. Operator's radiation dose was measured with an 1,800 cc ionization chamber (RadCal Corp.) at the hand level of X-ray tubehead and at the operator's chest and waist levels with and without the backscatter shield. The operator's radiation dose at the hand level was measured with and without lead gloves and with long and short cones. Results: The backscatter shield reduced operator's radiation dose at the hand level of X-ray tubehead to 23 - 32%, the lead gloves to 26 - 31%, and long cone to 48 - 52%. And the backscatter shield reduced operator's radiation dose at the operator's chest and waist levels to 0.1 - 37%. Conclusions: When portable dental X-ray systems are used, it is recommended to select X-ray machine attached with a backscatter shield and a long cone and to wear the lead gloves.

EFFECTIVE DOSE MEASUREMENT FOR CONE BEAM COMPUTED TOMOGRAPHY USING GLASS DOSIMETER

  • During image-guided radiation therapy, the patient is exposed to unwanted radiation from imaging devices built into the medical LINAC. In the present study, the effective dose delivered to a patient from a cone beam computed tomography (CBCT) machine was measured. Absorbed doses in specific organs listed in ICRP Publication 103 were measured with glass dosimeters calibrated with kilovolt (kV) X-rays using a whole body physical phantom for typical radiotherapy sites, including the head and neck, chest, and pelvis. The effective dose per scan for the head and neck, chest, and pelvis were $3.37{\pm}0.29$, $7.36{\pm}0.33$, and $4.09{\pm}0.29$ mSv, respectively. The results highlight the importance of the compensation of treatment dose by managing imaging dose.

CHEST WALL THICKNESS MEASUREMENTS AND THE DOSIMETRIC IMPLICATIONS FOR MALE RADIATION WORKERS AT THE KAERI

  • Using ultrasound techniques, the Korea Atomic Energy Research Institute has measured chest wall thicknesses of a group of male workers at the Korea Atomic Energy Research Institute. A site-specific biometric equation has been developed for these workers. Chest wall thickness is an important modifier on lung counting efficiency. These data have been put into the perspective of the ICRP recommended dose limits for occupationally exposed workers: 100 mSv in a 5-year period with a maximum of 50 mSv in anyone year. For measured chest wall thicknesses of 1.9 cm to 4.1 cm and a 30 min counting time, the achievable MDAs for natural uranium in the KAERI lung counter vary from 5.75 mg to 11.28 mg. These values are close to, or even exceed, the predicted amounts of natural uranium that will remain in the lung (absorption type M and S) after an intake equal to the Annual Limit on Intake corresponding to a committed dose of 20 mSv. This paper shows that the KAERI lung counter probably cannot detect an intake of Type S natural uranium in a worker with a chest wall thickness equal to the average value (2.7 cm) under routine counting conditions.

흉부 CT 스캔에서 재구성 알고리즘 변화적용 시 화질과 스캔 선량 분석 (Analysis of Image Quality and Scan Dose when Applying Reconstruction Algorithm Changes to Chest CT Scans)

  • 흉부 CT 검사 조건 중 관전압을 100, 80 kVp, 재구성 알고리즘을 FBP, ASIR-V, DLIR로 변화 적용하여 검사 선량과 화질 변화를 비교 분석해 보았다. 그 결과 기존 관전압 보다 낮은 관전압인 100 kVp 에서 ASIR-V, DLIR 적용 시 선량을 낮추면서 120 kVp, FBP 적용 시와 가장 유사한 화질로 구현. 특히, DLIR 재구성 영상은 모든 관전압에서 SNR, CNR이 우수하였다. 또한, SSIM 지수가 1에 가장 근사하게 분석되어 원본 영상과 가장 유사도가 높은 것을 알 수 있었다. 따라서 반복적으로 흉부 CT 검사를 시행하는 경우 DLIR 적용은 검사 선량을 약 29.7% 감소시킬 수 있어 CT 검사의 가장 큰 문제점인 검사로 인한 피폭을 일부 해결하는 데 도움을 줄 수 있을 것으로 사료된다.

인체등가형 흉부팬텀과 유리선량계를 이용한 고해상력 및 저선량 CT의 선량측정 (Measurement of Radiation Dose of HR CT and Low Dose CT by using Anthropomorphic Chest Phantom and Glass Dosimetry)

  • MDCT에서 인체등가형 흉부팬텀과 유리선량계를 이용하여 고해상력 및 저선량 CT로 검사하여 영상의 평가 및 흡수선량 및 유효선량을 측정하여 임상 기초자료를 제공하는데 목표를 두고자한다. 인체등가형 흉부팬텀내부에 유리선량계를 삽입하여 조직선량을 측정하였다. 64-slice CT system (SOMATOM Sensation 64, Siemens AG, Forchheim, Germany)과 CARE Dose 4D를 이용하였고, 고해상력 CT에서의 파라메터는 관전압 120 kVp, Eff. mAs 104, scan time 7.93 sec, slice 1.0 mm (Acq. 64×0.6 mm), convolution kernel (B60f sharp)의 스캔 파라메터가 사용되었고, 저선량 CT는 120 kVp, Eff. mAs 15, scan time 7.41 sec, slice 3.0 mm (Acq. 64×0.6 mm), convolution kernel B50f medium sharp의 스캔하였다. 인체등가형 흉부팬텀을 이용하여 스캔에 따른 CTDIvol은 고해상력 CT에서 8.01 mGy, 저선량 CT는 1.18 mGy로 측정되었다. 저선량 CT 검사는 고해상력 CT 검사에 비해 흡수선량이 85.49%가 감소하였고 영상의 차이는 없어 임상에서 유용하게 적용할 수 있다.

흉부 디지털 단층영상합성 시스템의 영상 재구성 알고리즘 개발 및 선량과 화질 평가 (Development of Image Reconstruction Algorithm for Chest Digital Tomosynthesis System (CDT) and Evaluation of Dose and Image Quality)

  • 최근 디지털 단층영상합성 시스템 (digital tomosynthesis system, DTS)은 일반 X-ray의 영상 중첩현상과 전산화단층촬영장치 (computed tomography, CT)의 높은 선량의 문제점을 해결하기 위하여 개발되었다. 본 연구의 목적은 흉부 촬영용 디지털 단층영상합성 시스템 (chest digital tomosynthesis, CDT)의 재구성 알고리즘 개발 및 화질과 선량 평가를 수행하는 것이다. 영상의 재구성은 필터 후 역투영 (filtered back-projection, FBP)을 모델링하였고, 팬텀 영상을 획득하기 위한 X-선과 검출기 사이의 각도를 ${\pm}10^{\circ}$, ${\pm}15^{\circ}$, ${\pm}20^{\circ}$, 그리고 ${\pm}30^{\circ}$로 구성하였다. 영상의 화질 평가는 평균 제곱근 편차 (root mean square error, RMSE)와 신호대 잡음 변화율 (signal difference-to-noise ratio, SDNR)로 수행하였고, 선량 평가는 ${\pm}20^{\circ}$의 범위에서 유효선량으로 수행하였다. 결과적으로, 모든 각도에서 Slice thickness 필터를 적용한 팬텀영상이 가장 우수한 RMSE와 SDNR 결과를 나타내었고, 최종 유효 선량은 0.166 mSv로 측정되었다. 결론적으로, 개발한 CDT 재구성 알고리즘의 유용성을 증명하였고, 최종 유효 선량을 측정하여 CDT의 기초 실험 데이터를 구축할 수 있었다.

디지털흉부X선촬영에서 검출기 방식에 따른 최적의 노출조건 (Optimal Exposure Conditions according to Detector Type in Chest Digital Radiography)

  • The aim of this study was to set up the optimal exposure condition according to detector type considering image quality (IQ) with radiation dose in chest digital radiography. We used three detector type such as flat-panel detector (FP) and computed radiography (CR), and charge-coupled device (CCD). Entrance surface dose (ESD) was measured at each exposure condition combined tube voltage with tube current using dosimeter, after attaching on human phantom, it was repeated 3 times. Phantom images were evaluated independently by three chest radiologists after blinding image informations. Standard exposure condition using each institution was 117 kVp-AEC at FP and 117 kVp-8 mAs at CR, and 117 kVp-8 mAs at CCD. Statistical analysis was performed by One way ANOVA (Dunnett T3 test) using SPSS ver. 19.0. In FP, IQ scores were not significant difference between 102 kVp-4 mAs and 117 kVp-AEC (28.4 vs. 31.1, p=1.000), even though ESD was decreased up to 50% ($62.3{\mu}Gy$ vs. $125.1{\mu}Gy$). In CR, ESD was greatly decreased from 117 kVp-8 mAs to 90 kVp-8 mAs without significant difference of IQ score (p=1.000, 24.6 vs. 19.5). In CCD, IQ score of 117 kVp-8 mAs was similar with 109 kVp-8 mAs (29.6 vs. 29.0), with decreasing from $320.8{\mu}Gy$ to $284.7{\mu}Gy$ (about 11%). We conclude that optimal x-ray exposure condition for chest digital radiography is 102 kVp-4 mAs in FP and 90 kVp-8 mAs in CR, and 109 kVp-8 mAs in CCD.

인체 흉부 모형 팬텀을 이용한 컴퓨터방사선영상에서 노출지수의 적용 사례 연구 (A Case Study of Application of Exposure Index in Computed Radiography by Using Human Chest Phantom)

  • As the use of digital radiographic system has been expanded, there are some concerns an increase about in patient of radiation dose. Therefore, International Electro-technical Commission (IEC) has been proposed a standard foe exposure index (EI). In this study, the EI was measured on human chest model using computed radiography (CR). Radiation quality used RQA5 of IEC62494-1. After acquiring the chest anterior posterior image (Chest AP) by using the phantom, the EI was obtained by applying the system response. In this study, we have analyzed the images with the detector size (Full filed ROI) and the optimized image (Fit filed ROI). The EI increased proportionally with radiation dose increase. Due to the discrete increase in pixel value, the EI showed an exponential increase. The discrete increase in noise equivalent quanta (NEQ) resulted in a discrete increase in the EI. The EI of the two images used in this study increased with increasing NEQ but showed different increments. For the measurement of the EI, IEC standards must be followed. The EI should be used as an index to evaluate the image quality for quality control of X-ray image rather than as an indicator of exposure dose. When calculating the EI, the system response should be applied depending on whether or not the grid is used. The size of the field should be obtained by including only the necessary parts.

임산부 흉부촬영 시 복부차폐의 적정성 평가 (Adequacy Assessment to Abdomen Shield of Pregnant X-ray Chest PA)

  • When performing Chest x-ray examination to pregnant woman, normally we shield back side of abdomen. In this situation, scattered rays made by equipment and surrounding structure can enter front side of abdomen. Therefore, in this study, we evaluate suitability of abdomen shield especially to pregnant woman. In case of One shielding material placed back of abdomen, the measured value is $0.676{\pm}0.19uSv/hr$. Two shielding material is $0.764{\pm}0.04uSv/hr$. Three is $0.685{\pm}0.16uSv/hr$. The exposure dose inferred in this study does not excess annual effective dose limit. But It is not mean absolute safety. So we have to minimize occurrence of stochastic effect of radiosensitivity by shielding front side of abdomen of pregnant woman in clinic.
