암환자의 예후인자로서 전신염증반응에 대한 고찰 (Systemic Inflammatory Response as a Prognostic Factor in Patients with Cancer)

  • Objective : The association of cancer survival and components of the systemic inflammatory response, combined to form inflammation-based prognostic scores (modified Glasgow Prognostic Score (GPS), Neutrophil Lymphocyte Ratio, Platelet Lymphocyte Ratio) is reviewed in this article. Methods and Results : With extensive research of papers in the PubMed, there is good evidence that preoperative measures of the systemic inflammatory response predict cancer survival, independent of tumor stage, in primary operable cancer. GPS also shows its prognostic value as a predictor of survival, independent of tumor stage, performance status and treatment in a variety of advanced cancer. GPS is associated with chemotherapy related toxicities as well as response to treatment and C-reactive protein shows its clinical value as a monitor of chemotherapy response. The systemic inflammatory response is closely related to cachexia and may be suitable measure for the clinical definition of cancer cachexia. Conclusion : Anticipated survival using the inflammation-based prognostic score is a major factor to be taken into consideration when deciding whether active intervention including surgery and chemotherapy or palliation therapy including acupuncture and herb medication is appropriate.

위암 및 간암환자의 치료에 있어 양.한방 협진의 효과에 관한 임상시험 (Clinical Study of the Efficacy of Combined Western-Oriental Medicine for Gastric Cancer and Hepatocellular Carcinoma)

  • Objectives: We compared the therapeutic response, the treatment-related toxicity, and the improvement of subjective symptoms between the chemotherapy alone group and the western-oriental combined treatment group and evaluated the role of oriental medicine for the improvement of chemotherapy-related toxicity in the advanced gastric cancer and hepatocellular carcinoma. Methods: We evaluated 36 gastric cancer or hepatocellular carcinoma patients(chemotherapy alone group 25 patients, combined treatment group 11 patients) who had been treated in Wonju Christian Hospital and Hana Hospital of Oriental Medicine between June 1999 and October 2000. Enrolled patients' general medical records, results of laboratory and imaging studies, treatment-related toxicities, and subjective symptoms were recorded regularly according to the planned protocol. Therapeutic responses were estimated according to the WHO response criteria and the changes of tumor marker value such as CEA, CA 72-4 and AFP. Results: 1. There was no significant difference of therapeutic response by the WHO response criteria between the two groups(p=.459). 2. There was a significant decrease of tumor marker value in the combined treatment group compared to the chemotherapy alone group(p=.023). 3. There was less comprehensive treatment-related toxicity in the combined treatment group compared to the chemotherapy alone group(p=.037), but there was not a significant difference of comprehensive improvement of subjective symptoms between the two groups(p=.091). Conclusions: Based on the above results, we could expect the possibility of improvements in therapeutic response and treatment-related toxicity with the western-oriental combined anticancer treatment.

Prediction of Treatment Outcome of Chemotherapy Using Perfusion Computed Tomography in Patients with Unresectable Advanced Gastric Cancer

  • Objective: To evaluate whether data acquired from perfusion computed tomography (PCT) parameters can aid in the prediction of treatment outcome after palliative chemotherapy in patients with unresectable advanced gastric cancer (AGC). Materials and Methods: Twenty-one patients with unresectable AGCs, who underwent both PCT and palliative chemotherapy, were prospectively included. Treatment response was assessed according to Response Evaluation Criteria in Solid Tumors version 1.1 (i.e., patients who achieved complete or partial response were classified as responders). The relationship between tumor response and PCT parameters was evaluated using the Mann-Whitney test and receiver operating characteristic analysis. One-year survival was estimated using the Kaplan-Meier method. Results: After chemotherapy, six patients exhibited partial response and were allocated to the responder group while the remaining 15 patients were allocated to the non-responder group. Permeability surface (PS) value was shown to be significantly different between the responder and non-responder groups (51.0 mL/100 g/min vs. 23.4 mL/100 g/min, respectively; p = 0.002), whereas other PCT parameters did not demonstrate a significant difference. The area under the curve for prediction in responders was 0.911 (p = 0.004) for PS value, with a sensitivity of 100% (6/6) and specificity of 80% (12/15) at a cut-off value of 29.7 mL/100 g/min. One-year survival in nine patients with PS value > 29.7 mL/100 g/min was 66.7%, which was significantly higher than that in the 12 patients (33.3%) with PS value ≤ 29.7 mL/100 g/min (p = 0.019). Conclusion: Perfusion parameter data acquired from PCT demonstrated predictive value for treatment outcome after palliative chemotherapy, reflected by the significantly higher PS value in the responder group compared with the non-responder group.

수술이 불가능한 전이성 또는 국소 진행성 위암 환자에서 선행화학요법의 효과 (The Role of Preoperative Chemotherapy in Patients with Inoperable Metastatic or Locally Advanced Gastric Cancer)

  • Purpose: The purpose of this study was to evaluate the treatment result of surgical resection after preoperative chemotherapy in inoperable gastric cancer patients. Materials and Methods: We analyzed 18 gastric cancer patients who underwent gastric resection after preoperative chemotherapy because they showed some clinical response to chemotherapy (15 with distant metastasis and 3 with locally advanced lesions). The mean postoperative follow-up period was $15.3\pm15.5$ ($1\∼56$) months. Results: In 15 patients with distant metastasis, 2 ($13.3\%$) showed complete response (CR), 10 ($66.7\%$) partial response (PR), 2 ($13.3\%$) stable disease (SD), and 1 ($6.7\%$) progressive disease (PD). The clinical response rate was $80.0\%$ Five subtotal gastrectomies, 4 total gastrectomies, and 6 extended total gastrectomies were performed. Two cases of CR were alive without recurrence for 4 and 26 months, respectively. Mean survival period in PR case was 37.7 months, but 2 cases of SD and 1 case of PD died after 11.7, 17.9, and 0.9 months, respectively. Postoperative survival was significantly associated with the response to chemotherapy (P<0.01). The mean survival period of the 10 patients with a complete resection was 44.1 months, which was significantly better than that of the 5 patients with an incomplete resection (9.8 months, P=0.03). Among 3 patients with locally advanced gastric cancer, 2 cases showed PR to chemotherapy, and complete resection was possible only by gastrectomy for those patients. Conclusion: In some selected cases, surgical resection was achievable after preoperative chemotherapy for patients with inoperable metastatic or locally advanced gastric cancer.

제한병기 소세포폐암에서 흉부 방사선 치료의 도입 시기에 따른 치료 효과의 비교 분석 (The effects according to the timing of thoracic radiotherapy in limited stage small cell lung cancer)

  • Background: Combination chemotherapy is now considered to be the cornerstone of small cell lung cancer (SCLC). management but the optimal management of limited SCLC is not well defined. The role of thoracic radiotherapy (TRT) is less well established. Recent meta-analyses reports revealed that TRT combined with chemotherapy produce "good" local control and prolonged survival. But other reports that survival was not changed. The liming, dose, volume and fractionation for TRT with the combined chemotherapy of SCLC remains unsettled. In this study, we analyzed the effects according to the timing of thoracic radiotherapy in limited SCLC. Method: All fifty one patients received cytoxan, adriamycin and vincristine(CAV) alternating with etoposide and cisplatin(VPP) every 3 weeks for 6 cycles were randomized prospectively into two groups: concurrent and sequential. 27 patients received 4500cGy in 30 fractions(twice daily 150cGy fractional dose) over 3 weeks 10 the primary site concurrent with the first cycle of VPP(concurrent gorup). 24 patients received 4000 to 5000cGy over 5 or 6 weeks after completion of sixth cycles of chemotherapy(sequential group). Results: 1. Response rates and response duration : Response rates were not significantly different between two groups(p=0.13). But response duration was superior in the concurrent group(p=0.03). 2. Survival duration was nor different between two groups(p=0.33). 3. Local control rate was superior in the concurrent group(p=0.00). 4. Side effects and toxicities: Hematologic toxicities, especially leukopenia, infection and frequency of radiation esophagitis were higher in the concurrent group (p=0.00, 0.03, 0.03). Conclusion: The concurrent use of TRT with chemotherapy failed to improve the survival of limited stage SCLC patients compared with the sequential use of TRT but response duration and local control rate were superior in the concurrent group. Frequency of radiation esophagitis, life threatening hematologic toxicities and infection were more frequent in the concurrent group than sequential group. So, the selection of an optimal schedule of chemotherapy combined with TRT that would lead to a major increase in survival with minimal toxicity is remained to be validated in large scale study in the future.

Prediction of Chemotherapeutic Response in Unresectable Non-small-cell Lung Cancer (NSCLC) Patients by 3-(4,5-Dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium (MTS) Assay

  • Background: Selecting chemotherapy regimens guided by chemosensitivity tests can provide individualized therapies for cancer patients. The 3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2Htetrazolium, inner salt (MTS) assay is one in vitro assay which has become widely used to evaluate the sensitivity to anticancer agents. The aim of this study was to evaluate the clinical applicability and accuracy of MTS assay for predicting chemotherapeutic response in unresectable NSCLC patients. Methods: Cancer cells were isolated from malignant pleural effusions of patients by density gradient centrifugation, and their sensitivity to eight chemotherapeutic agents was examined by MTS assay and compared with clinical response. Results: A total of 37 patients participated in this study, and MTS assay produced results successfully in 34 patients (91.9%). The sensitivity rates ranged from 8.8% to 88.2%. Twenty-four of 34 patients who received chemotherapy were evaluated for in vitro-in vivo response analysis. The correlation between in vitro chemosensitivity result and in vivo response was highly significant (P=0.003), and the total predictive accuracy, sensitivity, specificity, positive predictive value, and negative predictive value for MTS assay were 87.5%, 94.1%, 71.4%, 88.9%, and 83.3%, respectively. The in vitro sensitivity for CDDP also showed a significant correlation with in vivo response (P=0.018, r=0.522). Conclusion: MTS assay is a preferable in vitro chemosensitivity assay that could be use to predict the response to chemotherapy and select the appropriate chemotherapy regimens for unresectable NSCLC patients, which could greatly improve therapeutic efficacy and reduce unnecessary adverse effects.

Biphasic Tumor Oxygenation during Respiratory Challenge may Predict Tumor Response during Chemotherapy

  • Our previous study showed that switching the inhaled gas from hypoxic gas to hyperoxic gas for 10 minutes increased tumor oxygenation and that the magnitude of oxyhemoglobin increase responded earlier than tumor volume change after chemotherapy. During 10 minutes of inhaled-oxygen modulation, oxyhemoglobin concentration first shows a rapid increase and then a slow but gradual increase, which has been fitted with a double-exponential equation in this study. Two amplitude values, amplitudes 1 and 2, respectively represent the magnitudes of rapid and slow increase of oxyhemoglobin. The trends of changes in amplitudes 1 and 2 were different, depending on tumor volume when chemotherapy started. However, both amplitudes 1 and 2 changed earlier than tumor volume, regardless of when chemotherapy was initiated. These results imply that by observing amplitude 1 changes post chemotherapy, we can reduce the time of a respiratory challenge from 10 minutes to less than 2 minutes, to see the chemotherapy response. We believe that by reducing the time of the respiratory challenge, we have taken a step forward to translating our previous study into clinical application.

Expression of IER3 in Primary Hepatocarcinoma: Correlation with Clinicopathological Parameters

  • Background: Studies indicate the immediate early response gene 3 (IER3) is involved in many biological processes. Recently, it was discovered that IER3 plays an important role in tumorigenesis and tumor progression. Thus it may be a valuable biomarker in tumor. This study was designed to investigate the expression status of IER3 in primary hepatocarcinoma (PHC) and correlation with clinicopathological parameters. Materials and Methods: Real-time PCR was performed to evaluate the expression levels of IER3 in 62 pathologically diagnosed human PHC specimens. Results: A statistically significant association was disclosed between the expression of IER3 and P53 mutant protein (short for P53), Ki-67, EGFR and the biggest diameter, differentiation grade of tumor. Conclusions: This work is the first to shed light on the potential clinical usefulness of IER3, as an efficient tumor biomarker in PHC.

선행화학요법을 시행한 식도암 환자의 외과적 고찰 (Surgery of Advanced Esophageal Cancer after Chemotherapy)

  • Between June 1988 and June 1994, twenty five patients with locAlly advanced esophageal carcinoma received preoperative chemotherapy (Cisplatln, 5-Fluorouracil with or without Etoposide) and followed by resection. All patients had clinical evidence of airway involvement or distant Iymphnode involve- ment (M 1 Iymphnode) on bronchoscopy or computed tomographic scans. The major response rate to chemotherapy decided by the postoperative stage was 48% (12125). The resection rate was 92% (23/25) with overall complete resection rate of 72% (18125). Two patients had exploratory laparotomy (thorn- cotomy) only. Thirteen patients had esophagogastrostomy with a combined abdominl and Rt. thoracic approach (Ivor Lewis operation), slx pAtients had transhiatal esophagectomy, four patients had esophagogastrostomy with a combined Rt. thoracotonly & abdominal, cervical approach. There were three postoperative deaths (12%). Follow-up duration was between 3.3 months to 65 months. Median survival ime of resected patients except hospital death was 14.8 months. Actuarial survival at 12, 24 months was 72.9%, 26.2%. Signifi- cant better survival was associated with responder group (postoperative stage less than lIB) (P=0.029). These results demonstrate that 1) Preoperative Cisplatin based combined chemotherapy Produce high response rate, 2) High complete resection rate with acceptable mortality rate occur after preoperative chemotherapy, 3) Better surviL dl can be anticipated if complete resection performed after major re- sponse to preoperative chemotherapy.

국소 진행된 하인두암의 선행 항암화학요법 후 방사선치료 (Neoadjuvant Chemotherapy and Radiotherapy in Locally Advanced Hypopharyngeal Cancer)

  • 목적 : 국소 진행된 하인두암에서 선행 항암화학요법과 방사선치료를 시행했을 때 결과와 각 치료법에 대한 반응이 생존율에 미치는 영향을 알아보고자 본 연구를 시행하였다. 대상 및 방법 :국소 진행된 하인두암으로 진단받고 선행 항암화학요법과 방사선치료를 받은 32명의 환자를 대상으로 후향적 분석을 실시하였다. 방사선치료는 일일 선량 1.75$\~$2.0 Gy를 하루 1회, 주 5회 조사하였다. 총 방사선량은 60.8$\~$73.8 Gy (평균선량 68.6 Gy) 이었다. 선행 항암화학요법은 29명의 환자에서 5-FU와 cisplatin을 병용했고 나머지 환자들에서는 cisplatin을 기본으로 하여 bleomycin또는 vinblastin을 병용 투여했다. 24명(75$\%$)의 환자에서 3주 간격으로 3회의 선행 항암화학요법을 모두 시행하였고, 6명에서 2회, 2명에서 1회의 항암화학요법을 시행하였다. 결과 : 추적관찰기간은 7개월에서 134개월이었고 중앙값은 28개월이었다. 전체 대상환자들의 2년 및 5년 생존율은 각각 66$\%$, 43$\%$였고, 5년 국소제어생존율은 34$\%$였다. 12명(38$\%$)의 환자에서 5년 이상 후두가 보존되었다. 전체32명의 환자 중 선행 항암화학요법 후에 5명(16$\%$)에서 완전관해를 보였고, 19명(59$\%$)에서 부분관해, 8명(25$\%$)의 환자에서 무반응을 보여 반응률은 75$\%$였다. 부분관해를 보인 19명 가운데 8명은 방사선치료 후에 완전관해를 보였다. 선행 항암화학요법에 대해 무반응이었던 8명 중 2명은 방사선치료 후에 완전관해를 보였고, 나머지 6명은 부분관해를 보였다. 모든 치료가 종료한 후 무반응인 환자는 한 명도 없었다. 선행 항암화학요법에 대해서 완전관해, 부분 관해, 무반응을 보인 환자군별로 5년 생존율은 각각 60$\%$, 35.1$\%$, 50$\%$였다. 세 군간에 5년 생존율을 비교했을 때 통계학적으로 유의한 차이는 없었다(p=0.93). 모든 치료가 끝난 후에 완전관해를 보인 환자군과 부분관해를 보인 환자군의 5년 생존율은 각각 73.3$\%$, 14.7$\%$로 통계학적으로 의미있는 차이를 보였다(p<0,01). 생존율에 영향을 미치는 예후인자에 대한 분석을 실시했을 때, 통계학적으로 유의성이 있었던 요인은 방사선 치료 후 최종 치료반응이었다(CR vs. PR, p<0.01). 결론 : 국소로 진행된 하인두암의 치료에 있어서 본 연구에서는 선행 항암화학요법에 대한 반응과 장기 생존율간에 관계가 없는 것으로 나타났고, 방사선치료 후 최종 치료 반응이 장기 생존율에 가장 중요한 예후 인자였다.
