Rib Fixation for a Patient with Severely Displaced and Overlapped Costal Cartilage Fractures

  • Rib fixations for flail chest or displaced rib fractures are not a new technique. However, reports on rib fixations involving costal cartilage fractures are very few and surprisingly there are no reports of internal fixations involving only the costal cartilage in the English literature. The diagnosis is difficult and the necessity of the procedure may be quite controversial. Placing plates in screws into the costal cartilage alone may seem unstable and easily dislodged or stripped through the cartilage. We report a 31-year-old male scuba diver instructor who underwent rib fixations over his 7th and 8th costal cartilage ribs for severe pain. The procedure was done with conventional plates and screws. He had the plates and screws removed 2 months later due to lingering pain, but with them removed he is now quite happy with the results without pain. The procedure for fixation of painful overlapped costal cartilage is quite simple and can be done with the usual conventional methods, fixating plate and screws directly over the cartilage alone without fixation over the bony rib.

Analysis of Rainwater in Clean Air Area : Characteristics of Rainwater at 1100 Site of Mt. Halla and Cheju City in 1997-1998 (청정 강우의 분석:1997-1998년 한라산 1100 고지와 제주시 강우의 특성)

  • The rainwater samples were collected from the 1100 Site of Mt. Halla and Cheju city during the period of March in 1997 to August in 1998, and the major soluble ions were analyzed. The confidence of analytical data was confirmed by using the comparison methods such as ino-balance, electric conductivity and acid fraction, all of which correlation coefficients were above 0.94. The ionic strengths lower than $10^{-4}$ M, the basis for the pure rainwater, were found in 47% and 38% at 1100 Site and Cheju city, respectively. The precipitations in Cheju city were more influenced by the oceanic effect than those in 1100 Site. The acidity contribution was mainly by $SO_4^{2-}$ and $NO_3^-$ in both areas, and the organic acids have contributed to the acidity with only 5~7%. The neutralization factors by $NH_3$ were about 46% at both 1100 site and Cheju city, whereas those by $CaCO_3$ were 11% and 15% at 1100 site and Cheju city respectively, and the free acidity were both about 35% in average. From the MSA analysis, it was found that the air in Cheju island has been influenced by the pollution from the other areas. The sources of the rainwater components in 1100 Site and Cheju city were also studied with a factor analyzing way, and the most probable factors were found to be anthropotgenic, oceanic, and soil-sourced. The results of multiple regression analysis have shown that $SO_4^{2-}$ was dissolved mostly in the form of $H_2SO_4, CaSO_4 and (NH_4)_2SO_4$, and $NO_3^-$ was in the form of $HNO_3, Ca(NO_3)_2 and NH_4NO_3$.

The Effect of Litsea japonica on the Apoptosis Induction of HL-60 Leukemia Cells (까마귀쪽나무(Litsea japonica)의 HL-60 백혈병 세포 Apoptosis 유도효과)

  • This study investigated the antiproliferative effect of the EtOH extract from Litsea japonica. The extract markedly inhibited the growth of HL-60 cells. When treated with the extract, several apoptosis events like as DNA fragmentation, chromatin condensation and the increase of the population of sub-G1 hypodiploid cells were observed. The extract decreased the Bcl-2 expression, whereas the Bax expression was increased. Caspase-9 and -3 were activated and poly (ADP-ribose) polymerase was cleaved. The results suggest that the antiproliferative effect of L. japonica in HL-60 appears to arise from apoptosis induction via the down-regulation of Bcl-2 and the activation of caspases.

  • In Korea, the collapse of traditional Patriarchal family system has caused the weakening the patriarchal authority which represents the decrease of traditional attitude of respect for old age, influenced by the strong tendency of Materialism in ethics. The urgent problem is how to solve the conflicts of the alienation almost every old people has felt from the society and family which they belong to and how to harmonize the respect for old age who used to have had in the past. From 21st to 30th of July 1979, I explored the small village in Cheju Island with eight students which I selected on purpose that village had preserved the traditionalism. I collected the interview data form a sample of 221 persons of the village in Cheju Island. I studied this small village in Cheju Island for the intention of suggesting the ideal model of regional society which both traditional and non-formal, although the phenomena of nuclear family system and materialism and industrialism are overwhelming in ur present society. Generally people in Cheju Island are economically poor and major occupation is farming, since the industrialization proceeds very slowly and there still strongly remains the traditionalism. Alienation and dissatisfaction of old people from their family and society in Cheju island are less common in comparison to the main land. Old people in Cheju Island usually value the importance of labour. Divison of labour has been well observed in farming, and men and women in Cheju island are equally regarded in comparison to the main land. Old people are independent economically and spiritually from their offsprings and they organize the nuclear family system firmly. Also the old generation is ready to compromise with the new generation and they defy the authoritariansim.

Induction of the apoptosis of HL -60 leukemia cells by Scytosiphon lomentaria

  • The present study was taken to examine the inhibitory effect of extracts of Scytosiphon lomentaria, a marine alga growing in Jeju Island, on the growth of cancer cells and to develop an anti-cancer agent using components of S. lomemtaria. The effect was observed by the measurement of metabolic activity using colorimetric 3-(4,5-dimethylthiazol)-2,5-diphenyltetrazolium bromide (MTT) assay. In results, crude extract of this alga markedly inhibited the growth of leukemia cell lines such as HL-60 and KG-1, but could scarcely inhibit the growth of normal cells (HEL299) and adenocarcinoma cells (SNU-16 and HCT-I5). When HL-60 cells were treated with the extract, DNA fragmentation and the increase of proportion of sub-G1 hypodiploid cells were observed. Therefore, the inhibitory effect of S. lomemtaria on the growth of HL-60 cells seems to arise from the induction of apoptosis. In order to understand the mechanism of apoptosis inducton by S. lomemtaria, we examined the changes of Bcl-2 and Bax expression. The extract reduced Bcl-2, an anti-apoptotic protein, but increased Bax, a pro-apoptotic protein in a dose-dependent manner. When we examined the activation of caspase-3, an effector of apoptosis, the expression of active form(19 kDa) of caspase-3 was increased and the increase of their activities was demonstrated by the cleavage of poly(ADP-ribose)polymerase, a substrate of caspase-3, to 85 kDa. The results indicate that extract of S. lomentaria induces the apoptosis of HL-60 cells via the down-regulation of Bc1-2 and the activation of caspases.

Effects of Dietary Hizikia fusiformis on Growth and Immune Responses in Juvenile Olive Flounder (Paralichthys olivaceus)

  • An eight week feeding trial was conducted to investigate the effects of dietary supplementation of hizikia (Hizikia fusiformis) on growth performance, immune responses and resistance of juvenile olive flounder (Paralichthys olivaceus) to Streptococcus iniae. Four experimental diets (designated as Hiz 0, Hiz 2, Hiz 4 and Hiz 6) were formulated to be isonitrogenous (50% crude protein) and isocaloric (17.2 MJ/kg DM). Hizikia powder was added at 0%, 2%, 4% and 6% in diets Hiz 0, Hiz 2, Hiz 4 and Hiz 6, respectively. Three replicates of fish groups (15 fish/tank) were fed one of the experimental diets. At the end of feeding trial, no significant differences were observed in final body weight, specific growth rate, protein efficiency ratio, feed utilization and feed intake among fish groups fed the experimental diets. However, there was clear trend that the growth performances of fish were improved by the increment of dietary hizikia showing a positive growth effects. Mean phagocytes activated with nitro-blue-tetrazolium were significantly increased with the increment of dietary hizikia. The cumulative mortality was significantly (p<0.05) lower in the fish groups fed Hiz 6 diet (no mortality) than that in the other fish groups for 15 days of S. iniae challenge test. The findings of this study suggest that a dietary supplementation of hizikia could enhance the nonspecific immune response and improve the resistance of juvenile olive flounder to S. iniae.

A case of idiopathic thrombocytopenia in a Yorkshire terrier dog (개 특발성 혈소판감소증 1례)

  • A 4-year-old spayed female, Yorkshire terrier dog with a history of petechial and ecchymotic hemorrhages on the face, trunk and hind limb was referred to Veterinary Medical Teaching Hospital, Cheju National University. The complete blood count revealed a marked thrombocytopenia ($96{\times}10^3/{\mu}l$). The biochemical profile showed only slightly increased glucose. The coagulation profile such as prothrombin time and activated partial thromboplastin time, was within the reference range. In Giemsa staining, there were no endoparasites like as Babesia spp. on the RBC. This case was diagnosed as a idiopathic thrombocytopenia in a Yorkshire terrier dog. She was treated with prednisone (1 mg/kg BW, IM q 12 h) and cephalosporin (10 mg/kg BW, IM, q 12 h). When rechecked on day 6, the platelet was within reference range ($507{\times}10^3/{\mu}l$) and also petechial and ecchymotic hemorrhages on the body were gradually improved without any complications. The dose of prednisone was decreased to 0.5 mg/kg BW, q 24 h. On day 17, we finished treatment because all the clinical signs, blood and serum chemistry were reference range and platelet count was dramatically increased.

  • Acute mediastinitis is a life threatening disorder that causes severe morbidity in affected patient. We experienced a case of acute mediastinitis in a 56 year-old man as a result of swallowing a fish bone (order Perciformes, Family Pomacentridae). Conservative treatment, including starvation therapy and the injection of antibiotic drugs, were successful in treating this patient. Although the optimal treatment for acute mediastinitis remains controversial, it is important to choose the appropriate strategies when encountering cases of acute mediastinitis.

Effects of Callophyllis japonica powder on carbon tetrachloride-induced liver injury in rats

  • A limited number of studies have been conducted on the bioactivity of Callophyllis japonica (C. japonica), which is a red seaweed that is traditional in the oriental diet. In this report, the hepatoprotective effect of C. japonica was studied in a carbon tetrachloride $(CCl_4)-induced$ hepatotoxicity model in rats. A single intraperitoneal injection of 1.25 ml/kg of 20% $CCl_4$ in olive oil produced an elevated level of serum glutamic pyruvic transaminase (SGPT) and increased enzyme activity of serum glutamic oxaloacetic transaminase (SGOT). Pre-treatment with C. japonica (150 mg/kg/d) for 3 days in $CCl_4-injected$ rats reduced the levels of SGPT and SGOT compared with control levels (P < 0.05), while higher doses (300 and 600 mg/kg) were ineffective. The administration of C. japonica (150 mg/kg/d) for 3 days after the $CCl_4$ injection in rats was ineffective at reducing SGPT and SGOT. The histopathological findings in each group largely agreed with the biochemical data. The results of this study support the suggestion that C. japonica has a hepatoprotective effect on chemical-induced liver injury.

Effects of Polycyclic Aromatic Hydrocarbons (PAHs) on Early Development of Sea Urchin Anthocidaris crassispina (Polycyclic aromatic hydrocarbons (PAHs)가 보라성게 (Anthocidaris crassispina)의 초기발생에 미치는 영향)

  • Effects of five polycyclic aromatic hydrocarbon (PAHs) constituents (naphthalene, fluorine, fluoranthene, benzo(a)pyrene, pyrene) on fertilization and early development of sea urchin egg, sperm and fertilized egg were investigated. The eggs, sperm and fertilized eggs were exposed to several concentrations of PAHs (1, 10, 100, 1000 and 10000㎍/L). The rate of fertilization and hatching decreased when the eggs and sperm were exposed to aqueous solution of PAHs. Also, Exposure of fertilized eggs with each PAHs did decrease survival and hatching rate. Concentration-dependent toxic effects on the rate of fertilization, hatching, survival and abnormality in A. crassispina were observed following exposure to PAHs (1, 10, 100, 1000 and 10000㎍/L). These data show that PAHs exposure decreased in fertilization success of sea urchin egg and sperm and producted abnormal embryo. It is plausible to suggest that PAHs had the potential to significantly reduce coastal recruitment of sea urchin.
