  • This study was conducted to provide the information for the promotion of oral health in nursing students. The subjects were 207 in female students for self-administered questionnaires. The percentage of subject to visit dental clinic was 64.3%, to Regular checkup was 25.9%, cure of caries was 23.5% for last one year. The percentage of reasons not treated 'less importance' were 12.6%, 'fear of the dental clinic' was 10.6%. The average number of brushing teeth were 3.1. 56.0% students brush their teeth for two ~ three minutes. But intention of visit to the dentist were 'only when treated' was 51.7%, 'only when there is pain' was 10.1%. The intention of oral education was 54.6%, the content of the desired education 'whitening' were 34.5%, 'prevention of bad breath' was 19.2%. Dental health information acquisition path 'internet' were 42.0%, 'family or friends' was 25.6%. Susceptibility was 2.47 points, severity was 2.00 points, benefit was 4.03 points in oral health belief. Compared to students who needed dental treatment but did not receive treatment, oral health beliefs were higher among those who were treated(p<.001), students with cavities showed a higher level of oral health belief than those without cavities(p<.001). There was significant corelation between 'number of caries' and susceptibility(r=.330, p=.002), severity(r=.25, p=.019). The result should be reflected in the development of effective program for nursing students' oral health care.

  • This study compared the utility of the serum uric acid/creatinine ratio with that of uric acid as a risk predictor of metabolic syndrome. From November 2016 to October 2018, 14,190 adult men under the age of 20 years, who underwent a comprehensive health checkup at a general hospital in their metropolitan area, were included. Metabolic syndrome was assessed according to the American Heart Association/National Heart Lung and Blood Institute (AHA/NHLBI) criteria. Abdominal obesity was based on the WHO criteria in the Western Pacific region. The serum uric acid/creatinine ratio was found to be higher in the fourth quartile than in the first quartile, with a high incidence of metabolic syndrome and metabolic syndrome components. On the other hand, ROC analysis revealed the serum uric acid/creatinine ratio to be a similar indicator of the metabolic syndrome risk to serum uric acid (AUC, 0.554 vs 0.566). The serum uric acid/creatinine ratio showed lower sensitivity and higher specificity than uric acid. In conclusion, the utility of the serum uric acid/creatinine ratio as an independent indicator to predict the risk of metabolic syndrome is limited, and should be used only as an auxiliary marker.

  • The purpose of this study was to compare the usefulness of the lipid ratio indicators for the diagnosis of metabolic syndrome in the elderly aged 65 years or older. From January 2018 to December 2020, 1,464 people aged 65 years or older who underwent a health checkup at a general hospital in Seoul were included. Lipid ratio indicators were measured through blood tests. The prevalence of metabolic syndrome according to the quartiles of the lipid ratio index was confirmed by logistic regression analysis. In addition, the metabolic syndrome predictive ability and cutoff value of the lipid ratio indices were estimated with the receiver operating characteristic(ROC) curve. The correlation between atherogenic index of plasma(AIP) and waist circumference was the highest in both men and women(r=0.278, p<0.001 vs r=0.252, p<0.001). As for the lipid ratio indices, the incidence of metabolic syndrome was higher in the fourth quartile than in the first quartile. The area under the ROC curve(AUC) value of AIP was higher at 0.826(95% CI=0.799-0.850) and 0.852(95% CI=0.820-0.881) for men and women, respectively, compared to other lipid ratio indicators, and the optimal cutoff values for both men and women was 0.44(p<0.001). Therefore, the AIP among the lipid ratio indicators was found to be the most useful index for diagnosing metabolic syndrome in the elderly aged 65 years or older.

  • The purpose of this study was to examine various factors influencing the needs of Seoul's newly implemented outreach community services for older adults, and to suggest the direction of the outreach community health services in Seoul. A multi-level regression was conducted using data collected by face-to-face interviews from 1,000 individuals aged 65 and 70 in 17 districts, where participated in the Seoul's outreach community services. The results demonstrated that socioeconomic status (higher income and living alone), health status (having multiple chronic conditions and depression, lower health literacy), limited experience of the outreach community services, and low government trust at the individual level were associated with higher unmet need for the community outreach services. In addition, shorter participation period of the outreach services and financial independency at the district level were associated with higher unmet need for the services. The findings from this study implies the need for improving the quality of services by focusing on vulnerable groups such as individuals with lower income and worse health status. In addition, the outreach community health services may need to target individuals aged 66 to increasing efficiency of the services through utilizing results of life-cycle health checkup by the National Health Insurance Corporation.

  • The primary aim of this dissertation is to contribute to the improvement of methods in physical examination by providing quality information for the current school system and advice for improving status. Present status, controversial points and possible remedies in physical examination were analyzed on a frequency and percentage basis. An $x^2$-test was used to verify the statistics between the results from the examination and each variable. In case of multiple categories of variables, an $x^2$ cs was adopted. Chronological data as well as both total and sampling physical examination data verified the statistics using an $x^2$-test. This thesis is based both on the data from middle school health care specialists in Gyunggi Province and on the analysis of physical examinations reported from local schools to the municipal education agency from 1997 to 1999. The results of the study are as follows: First, according to the survey, only 29.0% of the total schools had their school doctors examine all the students while most of the educational institutions failed to implement the whole process of physical examination on the list. It also turned out that the more students the schools have, the lower the rate of implementation of physical examination by school doctors(p=0.014). Second, the average time a school doctor spends for checkup turned out to be approximately 1.7 minutes per student This means that the quality of the physical examination is not guaranteed in the process. Third, 47.7% of those surveryed say that a dental examination was performed, each taking 21.24 seconds on average. In addition, it shows that some 31.5% wanted to have a task force team for dental checkups at the local health center. Given the fact that dental caries among students is progressively on the rise, the dental health centers that are now set up in some elementary schools should be expanded to cover the whole educational institution in order to raise awareness of the importance of dental care. Fourth, 48.5% of those surveyed say that a comprehensive physical examination should be adopted to promote the health of high schoolers. Since it takes a lot of public funds to implement a comprehensive method, it is essential to make sure that in-depth studies should be based on the frequency and methods of physical examination. Fifth, regarding such diseases among 3rd year middle school students in 1999, statistics shows that there was a slight difference in the prevalence rate of color blindness, and allergic diseases for male students ; and color blindness, hearing disturbance and allergic disease for female students. For those items, however, it is too little to say that there is a significant difference and accordingly it is assumed to be a problem of the measuring process. Sixth, the result of analysis on the sample physical examination and the total physical examination of the year 1999 shows as follows: For male students in the 3rd year of middle school, a slight difference appeared to those students in 11 items including eye problems and eye disease, otitis media, tonsillar hypertrophy, spinal shape, respiratory urinary allergic disease and other abnormal diseases(p<0.05). Particularly, the prevalence rate between students with and without disease was shown to be two times more in the following: eye problems, otitis media, tonsill hypertrophy, allergic diseases, etc. For female students in the 3rd year, prevalence rate showed little difference in 14 items(p<0.05). For items including eye problem, otitis media, tonsill hypertrophy, allergic disease, etc. it was shown that the rate was two times more between students with and without diseases. Physical examinations under the current school system are not producing any fundamental results for the health of the students. Methods and results are not trustworthy. Accordingly, a drastic overhaul of the current practices is needed in frequency, methods and items on the list in order to promote the health of the students. Cost-benefit studies as well as political considerations to ensure the development of efficient methods for physical examination are urgently needed at this moment.

  • Purpose : The purpose of this study was to evaluate the timing of sexual maturation and secular trends of menarchial age in Korean adolescents. Methods : Nine hundred eighty two school students and 119 university female students were included. Tanner's pubertal assessment method was applied using a self-assessment sheet with pictures for their developmental stages. A medical checkup database from 1,156,022 women was also used to analyze the age of menarche from the year 1900 to 1980. Results : Onset of puberty was at $11.3{\pm}1.3$ years(y) in girls and $12.1{\pm}1.5$ y in boys and total pubertal duration was 3.6 y in girls and 3.3 y in boys. Height at the onset of puberty was $146.1{\pm}7.9cm$ in girls and $152.7{\pm}9.8cm$ in boys. Weight at the onset of puberty was $39.3{\pm}6.9kg$ in girls and $47.7{\pm}14.4kg$ in boys. The mean age of menarche was $12.0{\pm}1.0y$ and mean age at nocturnal emission was $12.3{\pm}1.8y$. By age ten, 3.0 percent of girls experienced menarche, which increased to 41.1 percent by age 12, and 98.0 percent by age over 15. By SMR 2 breast stage, 18.2 percent of girls experienced menarche, which increased to 66.1 percent and 91.7 percent by SMR stages 3 and 4, respectively. By SMR 2 pubic hair stage, 52.0 percent and by SMR 3, 93.8 percent of girls experienced menarche. While the menarchial age of women born around 1980 was $12.4{\pm}1.1y$, the menarchial age of women born around 1990 was $12.0{\pm}1.1y$. Age at menarche has been advanced approximately by 2 years between 1900 and 1980 birth cohorts. Conclusion : There was a downward secular trend in pubertal timing. Age at menarche has been advanced by 2 years during the last 80 years. Further large scale, prospective studies, including inspection by specialists, are needed to provide reference data for pubertal timing in Korean adolescents.
