Assessment Using the ICF-core set for Children and Youth with Cerebral Palsy: A Single Case Study

  • Purpose: This study evaluated the usefulness of International Classification of Functioning, disability and health core set for children and youths with cerebral palsy (ICF-core set for CP) by comparing the Gross Motor Function Measure-88 (GMFM-88), which is the most widely used outcome measure in children with cerebral palsy (ICF-core set for CP). Method: One subject (Female, 14) was evaluated by GMFM-88 and ICF-core set for CP. In addition, the concept of GMFM-88 was compared with the concept of the ICF-core set for CP in compliance with ICF linking rules. The numerical values both of the GMFM-88 categories and the ICF-core set for CP's items were compared. Results: The ICF b760 was linked to the GMFM-88 59~61. d415 was linked to 57~58 of GMFM-88. d450 was linked to GMFM-88's 65~72. d455 was linked to GMFM-88 at 77and 80~83. Conclusion: The association of the ICF-core set for CP and GMFM-88 was confirmed. In addition, the number of categories of ICF-core set for CP was smaller than the number of items of GMFM-88. In conclusion, the ICF-core set for CP is an evaluation tool that can identify the various features of children. In addition, GMFM-88 was linked to the ICF core set for CP according to the ICF linking rules.

Differences of Chest and Waist Circumferences in Spastic Diplegic and Hemiplegic Cerebral Palsy

  • Purpose: Circumference of the chest and waist can be one of clinical indicator to reflect respiratory function in children with cerebral palsy. In this study, we compared to differences in the chest/waist circumference and maximal phonation time between children with spastic diplegia and hemiplegia. Methods: Seventeen children with spastic diplegic and hemiplegic cerebral palsy were recruited, who were matched to gender, age, height, weight, and body mass index for control of the known factors affected to respiratory function. The chest/waist circumference and were measured in each group, when children took a breath at rest and at maximal voluntary inspiration/expiration. Results: No significant differences were found in the chest and waist circumference and expansion between the two groups. However, only in the waist expansion, children with diplegic CP were significantly lower extensibility of lung, compared to the other group. In comparison of the maximal phonation time, a significant lower score was shown in children with spastic diplegic CP, compared to children with hemiplegic CP. Conclusion: Our results indicated that children with spastic diplegic CP had smaller chest wall and waist, compared to children with spastic hemiplegic CP. In addition, they showed a shorter time for sustaining phonation than spastic hemiplegic CP did. Therefore, spastic diplegic CP will be required for careful monitor regarding respiratory function in rehabilitation settings.

  • Objectives: This study is to investigate the method for assesment of cerebral palsy(CP), especislly focusing on function assesment Methods: We searched the recent date of the publication and paper in Cerebral Palsy Results: Measuring the function of children with cerebral palsy is mobility, self-care and social ability. Early adequate evaluation of motor development and prognosis of degree of long-term motor disability is very important not only for parents, but also for correct management of goal oriented rehabilitation treatment and for planning of preventive measures. 1. Gross Motor Function Classification System(GMFCS) is valuable to prognostication about gross motor progress in children with CP, using longitudinal observation. 2. Gross Motor Function Measure(GMFM) is the instrument most commonly used to measure gross motor function in children with cerebral palsy(CP). 3. Pediatric Evaluation of Disability Inventory(PEDI) is one of the most commonly used assessments for children with a disability. Conclusions: The functional Assesment of children with CP are used GMFCS, GMFM and PEDI.

  • In the literature study on the cerebral palsy, the results were as follows : 1. Cerebral palsy is defined as a disorder of movement and posture due to a defect or lesion of the immature brain. For practical purposes it is useful to exclude from cerebral palsy those disorders of posture and movement which are of short duration, due to a progressive disease due solely to mental deficiency. 2. Cerebral Palsy is classified with quadriplegia, diplegia, hemi plegia, triplegia, and monoplegia or spastic CP, athetoid CP, ataxic CP, and combined classifications 3. Causes of Cerebral Palsy is any damage to the developing brain, whether caused by genetic or developmental disorders. And it is classified with prenatal.natal and postnatal causes. 4. Management consists of helping the child achieve maximum potential in growth and development. This should be started as early as possible with identification of the very young child who may have a developmental disorder. Certain medications, surgery, and braces may be used to improve nerve and muscle coordination and prevent dysfunction. 5. The aim of treatment is to encourage children and adults to learn to be as independent as possible. Some children and adults who have mild cerebral palsy will have no problems in achieving independence 6. Oji(五遲), Oyeon(五軟) and Okyeong(五硬) have the simmiar concepts with the cerebral palsy. 7. Oji(五遲) Oyeon(五軟) and Okyeong(五硬) are caused by seoncheon-pumbu-bujok(先天稟賦不足) and related with gan(肝), bi(脾) and sin(腎). 8. The treatment is achieved by the method of bogansin(補肝腎), ganggeungol(强筋骨) and boiungikki(補中益氣). And jihwanghwan(地黃丸) has been used most frequently.

  • This study evaluated the respiratory capacity of spastic cerebral palsy children who were grouped by GMFCS (Gross Motor Function Classification System) levels and identified the acoustic characteristics of three different types of Korean stops (stop consonants) which are needed for the temporal coordination of larynx and supra-larynx, in these children. Thirty-two children with dysarthria due to spastic cerebral palsy were divided into two subgroups: 14 children classified at GMFCS levels I~III were placed in Group 1 and 18 classified at GMFCS levels IV~V were placed in Group 11, and 18 children with normal speech were selected and placed in the control group. /a/ pronged phonation (sustained vowel /a/) and nine Korean VCV syllables were used. Examined acoustic characteristics were maximum phonation time (MPT) and closure duration and aspiration duration. The results were as follows: 1) The MPTs of the cerebral palsy (CP) groups, both Group I and Group II, were significantly shorter than those of the normal group. 2) The closure durations of the two CP groups were longer than those of the normal group for all 9 target syllables. 3) The aspiration durations of the two CP groups were longer than those of the normal group. 4) The closure duration of the normal and CP Group I was significantly different among tense, aspirated, and lax. However, the CP Group II was different from normal. 5) The aspiration duration of the normal and CP Group I was significantly different among aspirated, tense, and lax. However, the CP Group II was different from normal. 6) The place of articulation influenced less than the manner of articulation on closure and aspiration duration.

일부 뇌성마비 아동을 대상으로 한 아동균형척도의 검사 - 재검사, 측정자간 및 내의 신뢰도 (Test-retest, Inter-rater, and Intra-rater Reliability of a Pediatric Balance Scale in Children with Cerebral Palsy)

  • Purpose: This study was designed to determine the test-retest, inter-rater, and intra-rater reliability of the Pediatric Balance Scale (PBS) when applied to children with cerebral palsy (CP). Methods: Subjects were out-patient children with cerebral palsy at four CP clinics in Gyeonggyi-do and Chungcheong nam-do. For test-retest and inter-rater reliability studies, the PBS was used twice on 7 separate days by twenty-four children with CP. To assess intra-rater reliability, 10 CP subjects were selected by random sampling. Four pediatric-trained physical therapists with 2-13 years of clinical experience scored the children’s performance blindly, while replaying videotaped data. Results: There was no significant difference in total scores (ICC 3,1=0.89, 0.93, 0.90, and 0.91) measured by each of the four therapists on two occasions. The Inter-rater reliability assessed the 1st and 2nd time was high (1stICC 3,1=0.91, 2nd ICC 3,1=0.93). The intra-rater reliability measured by each of the four therapists using the 2nd scores was also high (ICC 3,1=0.98, 0.99, 0.97, and 0.98). Conclusion: The PBS is reliable. We believe that it can be used in characterizing children with CP.

Body Measurements for Designing Hip Dislocation Prevention Garment in Children with Cerebral Palsy

  • This study aims to provide basic size data for the development of a hip dislocation prevention garment for cerebral palsy (CP) children and useful information for the design of garment products for CP children through identifying differences in body shape between CP and non-CP children and reviewing the tibial-stature prediction formulas of previous studies. Forty-seven Korean children with CP aged 2 to 14 years were measured for body size from October 2019 to August 2020. Body measurements of 18 sites, including greater trochanter length, which is an important site for a hip dislocation prevention garment, were collected and analyzed. Data of non-CP children were taken from same age of Size Korea and compared. Tibial-stature prediction formulas suggested in four previous studies were also reviewed. CP children had significantly lower stature as well as circumferential dimensions when compared to non-CP children. Greater trochanter length is difficult to predict through other body dimensions. Thus, direct measurement is required. Of the general key dimensions used in the clothing industry, only hip circumference could explain the body shape of CP children. Tibial-stature prediction formulas cannot always but tend to largely predict the actual stature of CP children.

뇌성마비 환아 어머니의 사회적 지지와 부담감 (Burden and Social Support of Mothers with Cerebral Palsy Children)

  • Purpose: The purpose of this study was to identify the level of burden and social support of mothers with Cerebral Palsy (CP) children, and to determine whether providing social support is an effective strategy to relieve the burden of those mothers. Method: From October to November 2007, the data were collected from 63 mothers with CP children who were admitted and treated for CP at Y rehabilitation center in Seoul. The data were analyzed with t-test, ANOVA, Scheffe's test, Pearson's Correlation Coefficient, and Stepwise Multiple Regression using SPSS 12.0. Results: Burden of mothers was related to the Gross Motor Function Classification System (GMFCS) of the CP children. Social support was negatively correlated and GMFCS was positively correlated with burden of mothers. Social support and GMFCS was statistically significantly related with burden of mothers with CP children by multiple regression analysis. Conclusion: There was a significantly negative correlation between social support and burden. It demonstrated that social support, an important factor relating to burden imposed on mothers of CP children, can be considered as an intervening variable which can contribute to decrease of burden. In addition, applying developmental screening measures are needed to decrease disability severity.

뇌성마비 아동의 한방 및 보완대체의료 이용 실태 (Review of the Use of Korean Medicine or Complementary and Alternative Medicine among Children with Cerebral palsy)

  • Objective : The aim of this study was to review the use of Korean medicine (KM) or Complementary and Alternative Medicine (CAM) therapies among children with cerebral palsy (CP). Method : Literature searches were performed using Pudmed, CNKI, J-STAGE and four Korean databases. We investigated the frequency, types, predictors of the use of KM or CAM therapies used in children with CP. Results : Thirteen survey studies including use of KM or CAM among children with CP were selected - six Korean studies and seven overseas studies. The utilization rate of CAM among children with CP varied 26.8~56.0% according to the studies. CP children used KM or CAM more than other children with chronic disease. Acupuncture and Korean herbal medicine was the major treatment used in Korea, however physical manipulations such as massage, osteopathy and chiropractic were in high demand in western country. Disease severity of CP, use of CAM in the past by the parent or family member and high education of parent are the factors that significantly affect CAM utilization. Conclusion : Further studies on effectiveness and safety of KM therapies combined with conventional rehabilitation treatment would be required to expand the implementation of KM treatment for CP children.

Identification of pathways and genes associated with cerebral palsy

  • Cerebral palsy (CP) is a non-progressive neurological disease, of which susceptibility is linked to genetic and environmental risk factors. More and more studies have shown that CP might be caused by multiple genetic factors, similar to other neurodevelopmental disorders. Due to the high genetic heterogeneity of CP, we focused on investigating related molecular pathways. Ten children with CP were collected for whole-exome sequencing by next-generation sequencing (NGS) technology. Customized processes were used to identify potential pathogenic pathways and variants. Three pathways (axon guidance, transmission across chemical synapses, protein-protein interactions at synapses) with twenty-three genes were identified to be highly correlated with CP. This study showed that the three pathways associated with CP might be the molecular mechanism of pathogenesis. These findings could provide useful clues for developing pathway-based pharmacotherapies. Further studies are required to confirm potential roles for these pathways in the pathogenesis of CP.
