Objective: To explore the incidence, clinical characteristics, diagnosis and treatment strategies, prognosis of patients with malignancy-associated hypercalcemia (MAH). Methods: The data of 115 patients with MAH who were treated at the Medical Oncology Department of Chinese PLA General Hospital from Jan., 2001 to Dec., 2010 was retrospectively reviewed. Survival analysis was performed using the Kaplan-Meier method and the Cox proportional hazard model with statistic software SPSS 18.0. Results: The patients had blood calcium levels ranging from 2.77 to 4.87 mmol/L. Except for 9 cases who died or were discharged within 5 days after admission, all other patients recovered to normal blood calcium level after treatment with bisphosphonates or intravenous hydration and diuretics; their survival after occurrence of MAH was from 1 day to 4,051 days, and the median survival time was only 50 days. In the log-rank test, the male, renal metastasis, central nervous system symptoms and hypercalcemia occurring over 140 days after cancer diagnosis were predictors of poor survival (P=0.002, P=0.046, P=0.000, P=0.009). In the COX analysis, being male, central nervous system symptoms and hypercalcemia lasting over 140 days after cancer diagnosis were independent prognostic factors for survival time (RR=2.131, P=0.027; RR=3.054, P=0.002; RR=2.403, P=0.001). According to these factors, a score system was established to predict the patient prognosis and adjust the treatment. Conclusion: Cancer patients with MAH have an extremely poor median survival. Some independent factors indicate poor prognosis, including male gender, central nervous system symptoms and hypercalcemia lasting over 140 days after cancer diagnosis. The prognostic score can serve as a reference for MAH prognosis and treatment, worthy of further investigation.

This study investigated compensatory mechanisms in the brain during a verbal working memory task among people with Alcohol Use Disorders (AUD). A total of 21 college male students participated in the study: eleven AUD participants and 10 normal controls. Study participants were asked to complete the Korean version of the Wechsler Adult Intelligence Scale-III (K-WAIS-III) prior to the fMRI experiment. Verbal 0-back and 2-back tasks were used to assess brain activities of the participants' verbal working memory. Brain scanning was performed on Siemens SONATA 1.5T Scanner while participants were performing the 0-back and 2-back tasks. Within the AUD group, participants with greater dependency to alcohol (based on DSM-IV criteria) in the past 1 year showed lower mean score on the 'Similarities' of the K-WAIS-III (r=-0.63, p<0.05, N=11). The more participants experienced alcohol withdrawal symptoms in the past 1 year, the lower the score they received on the K-WAIS-III 'Picture Arrangement' (r=-0.69, p<0.05, n=11). The fMRI regression results showed that individuals who present greater degree of alcohol dependency symptoms are likely to show greater brain activation in the bilateral middle frontal gyri (BA 9) during the verbal working memory task. The degree of alcohol withdrawal symptoms were associated with increased brain activation in the left superior and middle frontal gyri (BA8), left precentral gyrus (BA 6), and left inferior parietal lobule (BA 40). The study findings showed that the degree of alcohol abuse/dependence and withdrawal symptoms were associated with decreased cognitive function and increased activations in brain regions particularly important for abstract reasoning (BA 9), central executive (BA 9), or spatial storage (BA 40) during a working memory task. Therefore, these results could support previous studies suggesting that the neural system of people with ADD may adopt a brain compensatory mechanism to maintain normal level of cognitive functions.

The People's Republic of China's influence in the international community is growing in political, economic, military, and diplomatic spheres. The "reform and opening-up" policy proposed and implemented at the 11th Plenary Session of the CPC Central Committee in December 1978 under Deng Xiaoping led to the rapid growth of China's economic and military power. The establishment of the National Defense Mobilization Commission in 1994 during Jiang Zemin's presidency also promoted defense mobilization, and the Standing Committees of the 9th, 10th, and 11th National People's Congresses, held since December 1998, formulated plans for defense mobilization legislation, and the first draft of the Defense Mobilization Law was approved in August 2008. In November 2005, under the leadership of President Hu Jintao, the draft Defense Mobilization Law passed the Standing Committee of the State Council, and in February 2010, the Standing Committee of the National People's Congress passed it after several rounds of deliberation and amendment, and the Defense Mobilization Law has been promulgated and implemented since July 1, 2010. The People's Republic of China is ruled by the one-party dictatorship of the Communist Party of China and the People's Liberation Army, the armed forces of the Communist Party of China. In this paper, it reviews the contents of the Defense Mobilization Law of China, a totalitarian state, analyzes and evaluates the issues.

Background: Breast cancer is the second most common malignancy among Nepalese women, accounting for 60% of the total cancer cases in females. Women diagnosed with germline mutations in BRCA1 like 185delAG, 1294del40 develop breast and/or ovarian cancer with a lifelong likelihood of up to 85% whereas presence of a mutation increases the risk for mutations to occur in other genes. The major objective of this study was to find the prevalence of these mutations in Nepalese cancer patients. Materials and Methods: This prospective study was carried out at two cancer hospitals in the Kathmandu valley over a period of 11 months. Irrespective of age group and stage of canceran appropriate amount of blood was withdrawn from 50 breast cancer patients and 20 controls. DNA was extracted manually and subjected to PCR using primers for 185delAG and 1294del40 mutations. PCR products were then digested with restriction enzyme (DdeII) followed by electrophoresis. Results: Prevalence of 185delAG in reference breast cancer patients was found to be 4/50 (8%) but no 1294del40 was apparent. Conclusions: Several mutations occurring in different exons of BRCA1 as well as mutations in other genes like BRCA2, for example, should also be taken in account.

The great estate system of the Old World crossed the Atlantic Ocean in the 1500s along with the Spanish Royal Army, mission, merchants, crops and domestic arrivals, landing at the end of the journey in the Middle and South Americas. The latifundio of Spain's Middle Age combined with the environment of the New World to be regenerated in the name of hacienda which bad became tightly roared in the countryside landscape of New Spain by fin-de-colonial period of 1820s. The haciendas were distributed mainly over the central part of the present-dey Mexico, and the presence of water and towns determined the specific location of the large landed estates. Depending on the activities performed, the hacienda can be divided into several types such as grain hacienda, livestock hacienda, mining hacienda, henequen hacienda, and so forth. Consisting of landlords, estate managers and waged labor called peons, the hacienda as a semi-autarkic settlement played various roles as the home of church, the agrarian center and the hearth of cultural diffusion, as well as dwelling. Toward the end of the colonial period the hacienda experienced internal transformations driven by capitalism.
Supply units in each command are multi-tiered and each supply unit keeps a supply level independently, which can cause excess stock, leading false reports to the Logistics Command and increasing difficulties in managing user needs. This causes excess assets and the excess then causes deformation in demand. therefore, the supply support systems of our armed forces have become high-cost/low-efficiency and are insufficient to meet the needs of users in combat units. Civilian corporations and the US Department of Defense are downsizing the aforementioned multi-tiered supply systems thus revolutionizing efficient and effective logistics by adopting Supply Chain Management(SCM), and Prime Vendor policies. Prime Vendor policy is a logistics support method that allows users to directly request and receive supply items from suppliers, based on supply contracts between suppliers and central maintenance organizations like KDA. In other words, it is a system that allows for users to make orders to suppliers directly and suppliers to deliver goods to the users directly, cutting out the middle stage, thus allowing an efficient supply. This is a way forward in finance that cuts costs in net supplies and allows an efficient utilization of civilian assets. which is also known to fasten the speed of logistical support and stripping down the logistical structure. therefore, this report will explore Prime Vendor policies adopted in certain number of units for medical supplies that were taken in consideration of improvements in stock management in civilian organizations and the US Army, and aims to apply such policies for repair parts.
The main theme of this study is about North Korea's contemporary military strategy which remained a blind spot in the 21st century. Indeed, Pyongyang's contemporary military strategy is evolved from the logic of War. On the basis of this logic, this study examined North Korea's contemporary military strategy with three analytical frameworks. The first is the discovery of Clausewitz's dictums and application of the Trinitarian analysis on the Korean cases. During the course of applying Clausewitzian main dictum--War as continuation of politics, the methodological analysis on war played a important role generalizing the pattern and matrix of North Korea's strategic thinking and military strategy. In particular, Clausewitz's Trinitarian framework on war -Government (reason), Army (chance), People (passion)- was a universal framework to scrutinize the North Korean missile and nuclear strategy. The second is about the matrix of North Korea's military strategy, The study suggested the genealogical feature of Pyongyang's military strategy. In principle, the dictum of 'Military-First Politics' 1S the combination of the political philosophy, Chuch'e (self-reliance) of Kim Il Sung and Kin Jong Il reflected in the military readiness. As a result this analysis was able to equate Clausewitz's dictums to explain Pyongyang's idea of the nature of war in that North Korea's military strategy is the central instrument of delivery to achieve political objectives. The third is about the theoretical encounter of 'Clausewitz's Wonderful Trinity' and 'Remarkable Trinity on North Korea's contemporary strategy'. On the basis, three elements are connected to one of three groups in society; the people, the military, and the government. In order to apply the Clausewitz's Trinitarian analysis into Kim Jong Il's 'Military-First' strategy, two case studies (Missile and Nuclear strategy) were examined. The finding of this study is that Clausewitzian dictum in the 19th century is still valid in the 21st century as it provided plausible theoretical framework to explain the North Korean contemporary military strategy with a reminder that the nature and logic of war are fixed in the socially constructed state.

Several numerical models for predicting beach profile changes have been developed by many researchers. Many of the earlier models are known to simulate the erosional profiles with the formation of offshore bar. However, most of the models don't have proper mechanism to incorporate the recovery process of the eroded profiles after a storm and can not simulate the beach accretion with acceptable accuracy. In order to overcome these shortcomings, we propose a new numerical model which has new features to simulate the accretional phase of beach recovery process after storm including such as redistribution of suspended sand particles near the breaking point. The simulation results of the proposed model were compared with LWT (Large Wave Tank) experiments performed at CRIEPI (Central Research Institute of Electric Power Industry in Japan) and CE (the Us Army Corps of Engineers) and it was shown to have performed better compared to SBEACH (Storm-induced BEAch CHange).
In the early days of the Goryeo Dynasty, the royal guard was comprised of the central regular army in the 2-gun-6-wi system, and Nasungeomgun was in charge of the security inside the palace. However, the military system became disorganized during the period of military rule and the royal guard had to rely on military subjects. The military officials suppressed the civil ministers centering on Jungbang, the guarding organization close to the King to incapacitate the royal authority and control the state affairs. When the rule of the three leaders of the military rule became short-lived and Dae-seung Gyeong is raised to the ruler, he organized a do-or-die squad comprised of a hundred and more people for his personal safety, and this became the first dobang. Dobang was disassembled after Dae-seung Gyeong died of disease, but under the rule of Chung-heon Choe, Dae-seung Gyeong's dobang was revived and reinforced into 'Yukbeon Dobang' to provide the ruler with personal protection and intensify the ruling system, and it was quite a large organization with more refined system. Yukbeon Dobang was expanded and reinforced into Naeoedobang under the rule of Woo Choe, the son of Chung-heon Choe, and it was enhanced even more into Dobang Samsipyukbeon System under the rule of the grandson, Hang Choe. Dobang can be considered as the guard organization in modern sense, and it collected information and surveyed the area where guarding is required and house troops that belonged to Naedobang eliminated the risks that may follow afterwards to make assurance doubly sure for guarding. The Choe's regime established Mabyeolcho as a private guard organization in addition to dobang, and this formed the cavalry and infantry units with dobang. Yabyeolcho organized by Woo Choe in the reign of King Gojong was divided into Joabyeolcho and Ubyeolcho, and later Sineuigun was integrated with them to form Sambyeolcho. Originally, Yabyeolcho was established under the rule of Woo Choe to prevent crime in the evening, but after Sineuigun was organized with the ones who were captured by Mongolian army but escaped, in other words when Sambyeolcho was organized, the organization displayed much broader influence by covering military and police affairs as well as punishment and imprisonment. The guarding organization during the Period of Military Rule in the Goryeo Dynasty did not have strict distinction between official guard and personal guard. The private guard in modern days which is the equivalent of personal guard is characterized by its commerciality, however, house troops and the members of dobang did not seem to pursued profit. The guard organization during the period of military rule started from dobang which was organized for personal safety but gradually developed publicness through the participation of civil ministers and expansion, and later it played the pivotal role for social security serving official purpose up to the level where the distinction between official and private activities was blurred during the period of Sambyeolcho.
