Microsurgical Reconstruction of Severe Radionecrotic Wounds Following Mastectomy (흉부의 심한 방사선 괴사 환부에 대한 미세 수술적 재건)

  • The purpose of this study is to investigate the appropriate management of severe radionecrotic wounds of the anterior chest wall associated with infection of the soft tissues and ribs and exposure of vital structures(heart and lung), and present our strategies for reconstruction of these complicated patients. 9 patients have undergone radical debridement and immediate microsurgical reconstruction for severe radionecrotic wounds of the anterior chest wall over last 7 years. All patients had extensive osteomyelitis of the ribs or sternum, and chronic infection or cutaneous fistulae. 2 patients had pericardial effusions due to longstanding inflammation, and 6 patients had pleural effusions. 2 patients had ipsilateral lung collapse. 10 free flaps were performed for coverage of the huge defects. One patient required 2 free flaps to control the inflammation. 8 free TRAM flaps were used for coverage of the defects and in addition, the rectus abdominis muscle was packed into any deep cavity. 1 patients underwent latissimus dorsi muscle free flap because of previous abdominal surgery. After extensive debridement of the infected, radionecrotic wounds, all 10 free flaps were successful. All the extensive radionecrotic defects of the anterior chest wall were completely healed. Free flaps successfully covered the exposed vital structures of the heart and lungs. Patients with severe radionecrotic defects of the anterior chest wall after ablative breast cancer surgery and radiotherapy were successfully treated by radical debridement and immediate free flap surgery. The TRAM flap together with the rectus muscle is the treatment of choice for these huge defects. The latissimus dorsi muscle flap was the second choice in patients with previous abdominal surgery. The recipient vessel should be carefully selected because of possible radiation damage and inflammation.

Neural Tube Defects with Abdominal Wall Defects in Sibling Dogs (복벽결손을 동반한 동복자견의 신경관결손)

  • This report describes Neural Tube Defects (NTDs) with Abdominal Wall Defects (AWDs) on the sibling of Yorkshire terriers. The NTDs and AWDs are rare serious congenital defects. The NTDs are neurulation abnormality that results from to failed transformation of the neurual tube by the incomplete closure of the embryonic neural plate. These dysraphic states range form mild to severe according to developmental malformation that include fusion defects of skull (crania bifida; CB) and fusion defects of vertebrae (spina bifida; SB). The AWDs are genetic defects that results from to failed formation of abdominal wall and cavity. These dysraphic states are omphalocele and gastroschisis. The 12-month dam was delivered by caesarian section and 4 littermate had obvious malformations. One male dead stillbirth fetus (L1) was revealed the extruded abdominal viscera, omphalocele. One female fetus (L2) was died within 1 hour after birth with defects of abdominal muscle upper umbilicus, gastroschisis. 3rd fetus (L3) was died within 36 hours after parturition and revealed a copious dermal and vertebral defects on the midline thorax, upper SB asperta. 4th fetus (L4) is still growing well now at 6 months but at the 2 week age, appears hairy nevus on the frontal cranium and dorsal thoracic portion. The radiograph of L1 and L2 are shown decrease bony density of calvarium and L3 was shown defect of spinose processes of the T9-T13. On our knowledge, this is first report of the SB and CB in Yorkshire terrier. And also sibling of NTDs with AWDs that has not previously been reported in the dog.

A STUDY OF CO2 LASER IRRADIATED DENTAL HARD TISSUES USING ELECTRON PROBE MICROANALYZER AND SCANNING ELECTRON MICROSCOPE (CO2레이저 조사가 치아경조직에 미치는 영향에 관한 EPMA분석 및 주사전자현미경적 연구)

  • The purpose of this study is to obtain the data of prestep in cavity preparation by observing changed conditions of laser irradiated dental hard tissues. The forty five extracted caries free human molars were divided into three groups and each group into five subgroups. A $CO_2$ laser irradiation was performed each subgroup of group I for one second with output power of 5 W, 10 Wand 20 W. On group II, laser irradiation was done ten times for 0.1 second duration using same power ratings as group I. On group III, laser irradiation was done 0.1 second, 0.2 second and 0.4 second with output power of 50 W. We investigated mineral contents and crater wall of obtained specimens, i.e., laser irradiated teeth, using computer controlled electron probe microanalyzer and scanning electron microscope. The following results were obtained: 1. Both calcium and phosphorus contents in laser irradiated enamel crater wall were increased, and magnesium content was decreased, but these trends were not statistically significant. 2. In laser irradiated dentin, change of mineral content was more significant. 3. In laser irradiated enamel and dentin, there were no significant differences on mineral content change due to irradiation condition and energy density. 4. In scanning electron microscopic study, enamel rods of the crater wall were fused and clefts were observed in parallel with the direction of enamel rod for all groups. 5. In laser irradiated dentin, irregular fusion and clefts were observed. In irradiated teeth with high power and short duration, the locally formed narrow crater wall was observed.

Applying Fire Risk Analysis to Develop Fire-safe Modular Walls: Guidance to Material Selection, Design Approach and Construction Method

  • For the past decade, South Korea had experienced catastrophic building fires, which resulted in consider-ably high number of casualties. This motivated research to develop fire-safe wall assemblies. In this study Fire Risk Analysis (FRA) is conducted as part of the project designing phase to ensure fire safety of the final product. Traditional approach was to consider fire performance at the end of the designing stage, when PASS/FAIL fire test results are required to be submitted to the Authority Having Jurisdiction (AHJ). By applying a fire risk analysis to guide the designing phase, overall fire safety of a wall assembly can be achieved more systematically as conducting FRA allows designers to clearly identify elements that are more vulnerable to fire and simply replace them with other practical options. Severity of fire risk is determined by considering the fire hazards of a wall assembly such as the exterior layer, insulation, vertical connectivity, and external ignition sources (e.g., photovoltaic panels). Frequency of fire risk is assessed based on the factors affecting fire likelihood, which are air cavity and fire-stopping applied in the design, and random design changes occurring during on-site construction. Fire risk matrix is proposed based on these fire risk factors and efforts to reduce the fire risk level associated with the wall assembly are given by systematically assessing the fire risk factors identified from fire risk analysis. Current study demonstrates how fire risk analysis can be applied to develop fire-safe walls by reducing the relevant fire risks- both severity and frequency.

Clinical Application for Video-Thoracoscopy in Lung Cancer Surgery Patients (폐암 수술환자에서 흉강경수술의 적용)

  • Background: Video-thoracoscopy is known to be an useful method to provide accurate pre-resectional staging in patients with lung cancer in addition to the conventional radiologic studies and mediastinoscopy, for the pleural cavity is inspected directly and biopsy specimens call be obtained. This study is undertaken to evaluate how video-thoracoscopy can be used in deciding pre-resectional stage Material and Method: Video-thoracoscopy was performed in patients with lung cancer who were scheduled for surgical resection based on the radiologic staging and mediastinoscopic biopsy. 37 patients were included in this study. Pre-thoracoscopically 18 cases were in TNM stage 1, 7 in stage 2, and 12 in stage 3. Result: In 15 of 37 cases, video-thoracoscopy could not be performed effectively due to heavy adhesions in the pleural cavity, diaphragmatic and chest wall invasion of tumor and bulky tumor mass es. Mediastinal lymph nodes were positive postresectionally in 6 of these 15 cases. In 22 cases, video-thoracoscopy was performed as usual. Positive mediastinal lymph nodes were identified in 2 cases and exploratory thoracotomy was prevented. Surgical resection were carried out in remaining 20 cases and 5 cases among them had positive mediastinal lymph nodes. Conclusion: We believe that it is difficult to perform pre-thoracotorny video-thoracoscopy for all lung cancer patients for there were many cases that thoracoscory could not be undertaken doe to heavy adhesions in the pleural cavity, tumor involvement of the chest wall and/or diaphragm and bulky tumor mass. However we think it is helpful in preventing unnecessary exploratory thoracotomy for some patients with lung cancer whom pre-thoracotomy video-thoracoscopy was carried out.

Fluid Injection Simulation Considering Distinct Element Behavior and Fluid Flow into the Ground (지반내 입자거동 및 흐름을 고려한 수압작용 모델링)

  • It is interesting to note that distinct element method has been used extensively to model the response of micro and discontinuous behavior in geomechanics. Impressive advances related to response of distinct particles have been conducted and there were difficulties in considering fluid effect simultaneously. Current distinct element methods are progressively developed to solve particle-fluid coupling focused on fluid flow through soil, rock or porous medium. In this research, numerical simulations of fluid injection into particulate materials were conducted to observe cavity initiation and propagation using distinct element method. After generation of initial particles and wall elements, confining stress was applied by servo-control method. The fluid scheme solves the continuity and Navior-Stokes equations numerically, then derives pressure and velocity vectors for fixed grid by considering the existence of particles within the fluid cell. Fluid was injected as 7-step into the assembly in the x-direction from the inlet located at the center of the left boundary under confining stress condition, $0.1MP{\alpha}\;and\;0.5MP{\alpha}$, respectively. For each simulation, movement of particles, flow rate, fluid velocity, pressure history, wall stress including cavity initiation and propagation by interaction of flulid-paricles were analyzed.

Effects of Electrohydrodynamic Flow and Turbulent Diffusion on Collection Efficiency of an Electrostatic Precipitator with Cavity Walls

  • The effects of the electrohydrodynamic (EHD) flow and turbulent diffusion on the collection efficiency of a model ESP composed of the plates with a cavity were studied through numerical computation. The electric field and ion space charge density were calculated by the Poisson equation of the electrical potential and the current continuity equation. The EHD flow field was solved by the continuity and momentum equations of the gas phase including the electrical body force induced by the movement of ions under the electric field. The RNG $k-{\varepsilon}$ model was used to analyze the turbulent flow. The particle concentration distribution was calculated from the convective diffusion equation of the particle phase. As the ion space charge increased, the particulate collection efficiency increased because the electrical potential increased over the entire domain in the ESP. The collection efficiency decreased and then increased, i.e. had a minimum value, as the EHD circulating flow became stronger when the electrical migration velocity of the charged particle was low. However, the collection efficiency decreased with the stronger EHD flow when the electrical migration of the particle was higher relatively. The collection efficiency of the model ESP increased as the turbulent diffusivity of the particle increased when the electrical migration velocity of the particle was low. However, the collection efficiency decreased for increasing the turbulent diffusivity when the electrical migration of the particle was higher relatively.

A Study on the Infection Statue of Tadpoles and Frogs by the Metacercariae of Fibricola seoulensis in Korea (국내 일부 지역산 올챙이와 개구리의 Fibricola seoulensis 유충에 의한 감염상)

  • A total of 220 tadpoles was captured in 6 areas and total 117 frogs, Rana nigromacuzata, were collected in 11 areas in Korea. They were examined for their infection status by the metacercariae of Fibricola seoulensis by peptic digestion technique and by histological observation with hematoBylineosin staining. This study was carried out from August, 1983 to September, 1984. Followings are the results. 1. The tadpoles of R. nigromaculata were positive for the metacercariae from 3.3% to 100% by area. The number of metacercariae per infected tadpole ranged from 1 to 584, and the mean number Per tadpole ranged from 7.6 to 221 by area. 2. The metacercariae from 16 tadpoles were counted by the body portion. A great majority of the metacercariae was collected from abdominal cavity, 98.3% of 484 counted larvae. And 6(1.2%) larvae were from proximal tail and 2(0.4%) from trunk. 3. Histological sections of tadpoles showed many metacercariae in abdominal cavity but none in other parts. The larvae were free in the spaces among intestinal loops or around primitive liver. A few larvae were in duct-like tissues near trunk wall. There was little infiltration of inaammatory cells. 4. The metacercarial infection rates of frogs ranged from 0% to 100% by area. The larval burden was 1 to 470 by infected frogs, and mean number ranged from 1 to 175.6 by area. By above results, it is suggested that the cercariae of F. seoulensis may infect R. nigromaculata already in the stage of tadpole. Almost all of the metacercariae were concentrated in abdominal cavity of tadpoles. According to the infection status of frogs, this nuke is prevalent almost nationwidely in rice paddies in Korea.

A Case of Nasal T/NK-cell Lymphoma (비강 T/NK 세포형 림프종 1례)

  • The advance of the immunobiology clarifies the nature of non-Hodgkin's lymphoma(NHL). In addition the proceed in the immunophenotyping renders the classification of NHL. According to the Revised European American Lymphoma(REAL) classification, classified by the etiologic factors, molecular biological characteristics, immunophenotype, cytogenetics and histologic feature, nasal T/NK-cell lymphoma(=angiocentric lymphoma) belongs to the category of peripheral T-cell and natural killer cell lymphoma. Nasal T/NK-cell lymphoma is a distinct clinicopathologic entity characterized by progressive necrotic lesions in the nasal cavity, nasopharynx, and palate. The cellular origin of this tumor has been controversial. Although most nasal T/NK-cell lymphomas are of NK-cell lineage, being CD56+, negative for surface CD3(Leu4), and unassociated with rearrangements of the T-cell receptor genes, other minor variants have been reported. This lymphoma is a rare disease and usually experienced in adult. Recently, we experienced a rare type lymphoma, nasal T/NK-cell lymphoma, in 14 years old boy. His soft mass occupied the right nasal cavity including the nasal septum and turbinate. Pathologically this nasal mass showed the infiltration into the vascular wall, illustrating angiodestructive lesion. The cellular origin was NK-cell lineage, being CD56+ and negative to CD3. Now, we report the case with a brief review of related literatures.

The Sensitivity Analysis of Thermal Expansion Breakage of Multi-layer Glazing in Building Envelope (건물 외피에 적용된 복층창의 열팽창 파손에 대한 민감도 분석 연구)

  • Curtain wall system of office buildings has recently become very common in Korea. As the multi-layer curtain glazing is exposed to outdoor environment, it is very subjected to direct environmental impact. Consequently, breakage and cracks of glazing due to heat expansion is frequently observed. This study explores various causes and aspects for destruction of multi-layer glazing. A sensitivity analysis was performed on the basis that thermal changes causes damage to the multi-layer glazing. Air temperature in air cavity within the multi-layer glazing was examined to find its effect on multi-layer glazing breakage. Analysis showed high deflection to depth ratio of 1:1.8 and that higher the aspect ratio, smaller is the deflection. Allowable pressure showed that the weakest value is for aspect ratio of 1:2.9. Sensitivity analysis by the area of the glazing showed that as area of glazing becomes higher, allowable pressure and deflection-depth ratio becomes smaller. For allowable pressure and allowable deflection-depth within air cavity, the glazing breakage occurred at least $107^{\circ}C$. The results from glazing breakage by thermal factor shows that it is hard to break the glazing with only an increase in air cavity temperature in multi-layer glazing applied in buildings.
