Diagnostic evaluation and educational intervention for learning disabilities (학습장애의 진단 평가와 교육학적 개입)

  • Learning disabilities (LD), also known as learning disorders, refers to cases in which an individual experiences lower academic ability as compared to the normal range of intelligence, visual or hearing impairment, or an inability to peform learning. Children and adolescents with learning disabilities often have emotional or behavioral problems or co-existing conditions, including depression, anxiety disorders, difficulties with peer relationships, family conflicts, and low self-esteem. In most cases, attention deficit and hyperactivity disorder coexists. As learning disabilities have the characteristics of a difficult heterogeneous disease group that cannot be attributed to a single root cause, they are diagnosed based on an interdisciplinary approach through medicine and education, such as mental health medicine, education, psychology, special education, and neurology. In addition, for the accurate diagnosis and treatment of learning disabilities, the diagnosis, prescription, treatment, and educational intervention should be conducted in cooperation with doctors, teachers, and psychologists. The treatment of learning disabilities requires a multimodal approach, including medical and educational intervention. It is suggested that educational interventions such as the Individualized Education Plan (IEP) and the Response to Invention (RTI) should be implemented.

A Study on the Comparison of Predictive Models of Cardiovascular Disease Incidence Based on Machine Learning

  • In this paper, a study was conducted to compare the prediction model of cardiovascular disease occurrence. It is the No.1 disease that accounts for 1/3 of the world's causes of death, and it is also the No. 2 cause of death in Korea. Primary prevention is the most important factor in preventing cardiovascular diseases before they occur. Early diagnosis and treatment are also more important, as they play a role in reducing mortality and morbidity. The Results of an experiment using Azure ML, Logistic Regression showed 88.6% accuracy, Decision Tree showed 86.4% accuracy, and Support Vector Machine (SVM) showed 83.7% accuracy. In addition to the accuracy of the ROC curve, AUC is 94.5%, 93%, and 92.4%, indicating that the performance of the machine learning algorithm model is suitable, and among them, the results of applying the logistic regression algorithm model are the most accurate. Through this paper, visualization by comparing the algorithms can serve as an objective assistant for diagnosis and guide the direction of diagnosis made by doctors in the actual medical field.

Facet joint disorders: from diagnosis to treatment

  • One of the most common sources of spinal pain syndromes is the facet joints. Cervical, thoracic, and lumbar facet joint pain syndromes comprise 55%, 42%, and 31% of chronic spinal pain syndromes, respectively. Common facet joint disorders are degenerative disorders, such as osteoarthritis, hypertrophied superior articular process, and facet joint cysts; septic arthritis; systemic and metabolic disorders, such as ankylosing spondylitis or gout; and traumatic dislocations. The facet pain syndrome from osteoarthritis is suspected from a patient's history (referred pain pattern) and physical examination (tenderness). Other facet joint disorders may cause radicular pain if mass effect from a facet joint cyst, hypertrophied superior articular process, or tumors compress the dorsal root ganglion. However, a high degree of morphological change does not always provoke pain. The superiority of innervating nerve block or direct joint injection for diagnosis and treatment is still a controversy. Treatment includes facet joint injection in facet joint osteoarthritis or whiplash injury provoking referred pain or decompression in mass effect in cases of hypertrophied superior articular process or facet joint cyst eliciting radicular pain. In addition, septic arthritis is treated using a proper antibiotic, based on infected tissue or blood culture. This review describes the diagnosis and treatment of common facet joint disorders.

Accuracy of the Registered Cause of Death in a County and its Related Factors (일개 군 사망신고자료에 기재된 사인의 정확성과 관련요인)

  • Objectives : To evaluate the accuracy of the registered cause of death in a county and its related factors. Methods : The data used in this study was based on 504 cases, in a county of Chonnam province, registered between January and December 1998. Study subjects consisted of 388 of the 504 cases, and their causes of death were established by an interview survey of the next of kin or neighbor and medical record surveys. We compared the registered cause of death with the confirmed cause of death, determined by surveys and medical records, and evaluated the factors associated with the accuracy of the registered cause of death. Results : 62.6% of the deaths were concordant with 19 Chapters classification of cause of death. external causes of mortality, endocrine, nutritional and metabolic diseases, neoplasms and diseases of the circulatory system showed the good agreement between the registered cause of death and the confirmed cause of death. The factors relating to the accuracy of the registered cause of death were the doctors' diagnosis for the cause of death (adjusted Odds Ratio: 2.67, 95% Confidence Interval: 1.21-5.89) and the grade of the public officials in charge of the death registry (adjusted Odds Ratio: 0.30, 95% CI=0.12-0.78). Conclusions : The accuracy of the registered cause of death was not high. It could be improved by using the doctors' diagnosis for death and improving the job specification for public officials who deal with death registration.

Serum Talin-1 is a Potential Novel Biomarker for Diagnosis of Hepatocellular Carcinoma in Egyptian Patients

  • Background: Hepatocellular carcinoma (HCC) is a major cause of cancer mortality worldwide. The outcome of HCC depends mainly on its early diagnosis. To date, the performance of traditional biomarkers is unsatisfactory. Talins were firstly identified as cytoplasmic protein partners of integrins but Talin-1 appears to play a crucial role in cancer formation and progression. Our study was conducted to assess the diagnostic value of serum Talin-1 (TLN1) compared to the most feasible traditional biomarker alpha-fetoprotein (AFP) for the diagnosis of HCC. Methods: TLN1 was detected using enzyme linked immunosorbent assay (ELISA) in serum samples from 120 Egyptian subjects including 40 with HCC, 40 with liver cirrhosis (LC) and 40 healthy controls (HC). Results: ROC curve analysis was used to create a predictive model for TLN1 relative to AFP in HCC diagnosis. Serum levels of TLN1 in hepatocellular carcinoma patients were significantly higher compared to the other groups (p<0.0001). The diagnostic accuracy of TLN1 was higher than that of AFP regarding sensitivity, specificity, positive predictive value and negative predictive value in diagnosis of HCC. Conclusions: The present study showed for the first time that Talin-1 (TLN1) is a potential diagnostic marker for HCC, with a higher sensitivity and specificity compared to the traditional biomarker AFP.

  • Recently, due to the increase of power conversion devices and nonlinear loads with the development of information, communication and control technologies, the instantaneous minute interruption factors such as voltage & current harmonics, surge occurring frequency, instantaneous voltage variation, voltage unbalance, flicker etc. have greatly threatened the power quality, and the deterioration of electric power facilities and the functional error of controllers are increasing. As such an instantaneous minute interruption appears to be small and local, accurate evaluation with measurement is difficult and total analysis system is required through a wide range of power quality effect analysis such as the simultaneous measurement on various power supply phenomena and the analysis on the interrelation with system loads. Most of conventional power quality diagnosis equipments have beer developed and applied, which were able to measure the stability rate of frequency, the stability rate of voltage, the electricity-failure duration etc, However, they were insufficient to analyze the system present situation, understand the cause of the failure occurred by the problem of power quality and analyze out the phenomena. Accordingly, this study will address the development of the system for a wide range of power quality diagnosis over the present level, the system for supporting the determination such as the analysis on risk factors, failure mode and impact, the system for harmonic evaluation based on international standards(IEC 61000 Series) and the total power quality diagnosis network & system with the extension and openness as a local and national-scale broadband power quality diagnosis system.

A Study on the Infrared Thermographic Imaging in Diagnosis of the Central Type of Herniated Disc (중앙형 추간판탈출증의 진단에서 체열촬영의 의의)

  • Infrared thermographic imaging visualizes noninvasively various abnormal condition by detecting the skin temperature. As the imaging represents the objective condition by the changes in blood flow under the control of autonomic nervous system, it is used to diagnosis and monitor the lumboscral radiculopathy. And asymmetry is important in the diagnosis of disc herniation. The most common type of disc herniation occurs psoterolaterally. This frequently causes nerve root compression leading to a radiculopathy in the distribution of the involved nerve root, most of which also provoke the asymmetric changes in thermography. Central disc herniation, which accounts for 5% to 35% of disc herniation, is typically associated with low back pain. But radiculopathy is usually abscent unless central disc heriniaton is large enough to cause compression of the cauda equina. To evalute the diagnostic value of the thermographic imaging in the diagnosis of central disc herniation, the imaging of 15 normal subjects and 48 patients with central disc herniation documented by CT scan were analyzed. The patients had either bilateral radiculophathy or no radiculopathy. The imaging of patient group with non rediculopathy did not show any significant thermal difference to control. While bilateral radiculopathy group reveled hypothermic pattern compared twith control. Thermal difference between left and right side did not present any significance in non radiculopathy group but hypothermia in bilateral radiculopathy group. Large herniation group demonstrated hyperthemic pattern while the others showed no significant change. Cranial caudal thermal difference did not show any difference between experiment groups. These results shows that infrared thermographic imaging can be used central disc herniation with bilateral radiculopathy, while it seems to little useful on the diagnosis of non radiculopathic disc herniation.

Considerations on an Oriental Medical Doctor like Indentation System (한의사 맥진 가압력 재현에 대한 고찰)

  • In pulse diagnosis, the indentation pressure is one of the most important factors as well as the change of pulse shape and the distribution of pressure via time. But, on the oriental medical doctor's indentation pressure control, the understandings of the neurophysiological meanings and mechanisms have been lacked. So, in this paper, we considered on these issues and then proposed a proper system which can imitate the OMD's indentation pressure control mechanisms. As a result, both tactile information and kinesthetic information were found to be essential to the indentation pressure control so that a system, which can measure both the physical indent pressure and the displacement of an indentation arm, has been proposed. With this proposed system, while the indentation was being controlled through the moving step number of the step motor, the physical indentation pres sure and displacement of the indentation arm were measured. From these measured data, the relationships between the moving step number and both physical indentation pressure and displacement were revealed to have linear characteristics in early phase and to have nonlinear characteristics in latter phase. Additionally, three types of graph were generated whose X axis means the moving step number, the physical indentation pressure and the displacement respectively and Y axis means the pulse pressure. By comparing these graphs, we come to conclude that different concepts on indentation pressure control cause different diagnostic results on floating/sinking degrees for the same subject. Consequently, an indentation system for the pulse diagnosis should be able to provide both the tactile information and kinesthetic information, that is, the physical indentation pressure and the displacement of the indentation arm. In future, the proposed system should be optimized to the pulse diagnosis environment and how to combine the both information for more reliable diagnosis should be studied.

Determination of a Change Point in the Age at Diagnosis of Breast Cancer Using a Survival Model

  • Breast cancer, the second cause of cancer-related death after lung cancer and the most common cancer in women after skin cancer, is curable if detected in early stages of clinical presentation. Knowledge as to any age cut-off points which might have significance for prognostic groups is important in screening and treatment planning. Therefore, determining a change-point could improve resource allocation. This study aimed to determine if a change point for survival might exist in the age of breast cancer diagnosis. This study included 568 cases of breast cancer that were registered in Breast Cancer Research Center, Tehran, Iran, during the period 1986-2006 and were followed up to 2012. In the presence of curable cases of breast cancer, a change point in the age of breast cancer diagnosis was estimated using a mixture survival cure model. The data were analyzed using SPSS (versions 20) and R (version 2.15.0) software. The results revealed that a change point in the age of breast cancer diagnosis was at 50 years age. Based on our estimation, 35% of the patients diagnosed with breast cancer at age less than or equal to 50 years of age were cured while the figure was 57% for those diagnosed after 50 years of age. Those in the older age group had better survival compared to their younger counterparts during 12 years of follow up. Our results suggest that it is better to estimate change points in age for cancers which are curable in early stages using survival cure models, and that the cure rate would increase with timely screening for breast cancer.

Survival of Patients with Stomach Cancer and its Determinants in Kurdistan

  • Background: Stomach cancer is the fourth most common cancer and the second leading cause of death from cancer in the world. In Iran, this type of cancer has high rates of incidence and mortality. This study aimed to assess the survival rate of patients with stomach cancer and its determinants in Kurdistan, a province with one of the highest incidence rates of stomach cancer in the country. Materials and Methods: We studied a total of 202 patients with stomach cancer who were admitted to Tohid Hospital in Sanandaj from 2009 to 2013. Using Kaplan-Meier nonparametric methods the survival rate of patients was calculated in terms of different levels of age at diagnosis, gender, education, residential area, occupation, underweight, and clinical variables including tumor histology, site of tumor, disease stage, and type of treatment. In addition, we compared the survival rates using the log-rank test. Finally, Cox proportional hazards regression was applied using Stata 12 and R 3.1.0 software. The significance level was set at 0.05. Results: The mean age at diagnosis was $64.7{\pm}12.0$ years. The survival rate of patients with stomach cancer was 43.9% and 7% at the first and the fifth year after diagnosis, respectively. The results of log-rank test showed significant relationships between survival and age at diagnosis, education, disease stage, type of treatment, and degree of being underweight (P<0.05). Moreover, according to the results of Cox proportional hazards regression model, the variables of education, disease stage, and type of treatment were associated with patient survival (P<0.05). Conclusions: The survival rate of patients with stomach cancer is low and the prognosis is very poor. Given the poor prognosis of the patients, it is critical to find ways for early diagnosis and facilitating timely access to effective treatment methods.
