Multiple Primary Cardiac Malignant Peripheral Nerve Sheath Tumors in the Left Atrium: Case Report

  • Malignant peripheral nerve sheath tumors are rare sarcomas of the heart. Herein, we report the case of a 24-year-old man who complained of dyspnea, cough, and upper left back pain. He was found to have multiple primary heart tumors obstructing the right superior pulmonary vein in the left atrium, which were diagnosed as malignant peripheral nerve sheath tumors. The patient underwent successful resection of the tumors and immunohistochemistry was utilized for diagnosis.

  • The anomalous connection of umbilical vessels to the heart is rare and has not yet been reported in the international scientific literature. Herein, we report the case of a newborn who was diagnosed with an anomalous connection of the umbilical vessels to the left ventricle. These anomalous vessels were functionally open for 2 weeks, and cellulitis was present in the area of the blood vessels connected to the skin. We performed division of these abnormal vessels and removal of the skin lesion.

  • Primary intrapulmonary thymomas (PITs) are defined as thymomas arising in intrapulmonary locations, without an associated mediastinal component. They are rare lesions, the diagnosis of which can be very difficult. We present a case of PIT in an asymptomatic 74-year-old woman in whom pulmonary nodules were found on pulmonary angiography performed for an episode of pulmonary embolism. She underwent wedge resection and the pathology report revealed a PIT. We also summarize this patient's clinicopathological features and discuss the diagnosis, pathogenesis, and treatment of PIT.

A Case Report of Hyperhidrosis treated by Oryeong-san based on Shanghanlun Provisions (『상한론(傷寒論)』변병진단체계(辨病診斷體系)에 근거하여 오령산(五苓散) 투여 후 호전된 다한증 증례 1례)

  • Objective : The purpose of this paper is to report the improvement of patient with hyperhidrosis treated by herb medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', the patient was diagnosed with Taeyang-byung Kyeolhyung number 156 provision, and took Oryeong-san herb medication 15days. The result of administration was evaluated by Hyperhidrosis Disease Severity Scale (HDSS) and Visual Analog Scale (VAS). Results : After administration of Oryeong-san 15days, based on 156 provision of Oryeong-san seclected according to Shanghanlun provisions, Hyperhidrosis Disease Severity Scale (HDSS) was decreased from 4 to 2 and Visual Analog Scale (VAS) was decreased from 10 to 2. Conclusions : The patient's hyperhidrosis improved. This case report suggests that the word '遂' on the 153-156 provisions of Shanghanlun be a direct emotional cause of hyperhidrosis in this case.

Subclinical Hypothyroidism Treated by Means of Modified Er-Xian Decoction (불현성 갑상선기능저하증에 대한 이선탕 가미 처방 투여 경과 : 증례보고)

  • Objectives: This study is the first report on a case of subclinical hypothyroidism treated with Modified Er-Xian Decoction, which is rarely used in Korea. Methods: Patients were treated for 3 months with Modified Er-Xian Decoction and their therapeutic effects were evaluated through thyroid function tests and Spleen-Kidney deficiency Questionnaire. Results: Initial improvement was apparent after the administration of Modified Er-Xian Decoction. After 3 months of treatment, the patient 's Spleen-Kidney deficiency index improved significantly, and thyroid hormone levels returned to normal. Conclusions: In this case report, symptoms and biochemical improvement were confirmed based on a single herbal medicine prescription for subclinical hypothyroidism. It is necessary to verify the effect of this prescription through well - established clinical trials.

  • Objectives The purpose of this case is to report the effectiveness of oriental medicine in Restless Legs Syndrome (RLS) in children who have sleep disorder. Methods A 6-year-old male patient, who was suffering from RLS, was treated by acupuncture, moxibustion and myofascial release. Results Treated with oriental medicine, the patient's RLS symptoms and sleep disorder were significantly improved. And the patient maintains his condition for 8 months. Conclusions This study shows that oriental medical treatments are effective on RLS, but further clinical studies are necessary.

Herbal Medicine Treatment for Induction and Maintenance of Remission in Ulcerative Colitis: A Case Report (한약치료를 통해 관해가 유도된 궤양성대장염 환자: 증례보고)

  • This case report details induction and maintenance of remission in a 34-year-old female patient with ulcerative colitis (UC) after Korean herbal medicine treatment. The patient diagnosed as UC after a series of examinations including endoscopy in a western medical hospital mainly presented bloody diarrhea and dyspepsia and symptoms were persistent even with medications. She was given individualized Korean herbal medicine prescriptions for 7 months and the symptoms were monitored during treatment and no adverse events were reported. After clinical remission, endoscopic remission was also confirmed by colonoscopy. Clinical remission was maintained for 30 months after treatment without Western medication for UC. This case report shows that Korean herbal medicine treatment may have the potential for complete remission of UC and further research is warranted.

  • Treatment of metastatic carcinoma of the oral cavity can be a challenge due to its rarity. The current case report details metastatic carcinoma of the mandible originating from follicular thyroid carcinoma, which is the second most prevalent tumor of the thyroid. As the mandibular lesion developed, the primary thyroid lesion of thyroid. Here, we report a case of metastatic carcinoma of the mandible in a 67-year-old female that was treated with partial mandibulectomy and fibular free flap.

Complex Korean Medicine Therapy for Tinnitus with Ear Fullness, Depression, and Anxiety: A Case Report (이충만감 및 불안, 우울 소견을 동반한 이명 환자의 복합 한의진료 경과 : 증례보고)

  • Background: The aim of this study was to determine the clinical effects of complex Korean medicine therapy for tinnitus associated with ear fullness, depression, and anxiety. Case Report: A 52-year-old female patient suffering from tinnitus with ear fullness, depression, and anxiety was treated with acupuncture, pharmacopuncture, and herbal medicine. We used the Tinnitus Handicap Inventory (THI), Beck Depression Inventory (BDI-II), Beck Anxiety Inventory (BAI), and a Verbal Numerical Rating Score (VNRS) to assess the patient's symptoms. The administration of the acupuncture, pharmacopuncture, and herbal medicine improved the ear fullness, depression and anxiety symptoms. No side effects were observed during the treatment. Conclusion: The study findings suggest that complex Korean medicine therapy, such as acupuncture, pharmacopuncture, and herbal medicine, may be effective for the treatment of tinnitus associated with ear fullness, depression, and anxiety.

A Case Report of Benign Paroxysmal Positional Vertigo treated by Baekho-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 백호탕(白虎湯) 투여 후 호전된 양성 돌발성 체위성 현훈 증례 1례)

  • Objective: The purpose of this paper is to report the improvement in a patient with benign paroxysmal positional vertigo (BPPV) treated with herbal medication based on the Shanghanlun disease pattern identification diagnostic system. Methods: According to the 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions,'the patient was diagnosed with Guorem-byung number 350 provision, and took Baekho-tang herbal medication for 15 days. The result of administration was evaluated using the Dizziness Handicap Inventory (DHI) and the Visual Analog Scale (VAS). Results: After administration of Baekho-tang for 15 days, based on the 350 provision of Baekho-tang diagnosed according to the Shanghanlun provisions, the DHI score decreased from 74 to 16 and the VAS score decreased from 10 to 1. Conclusions: The patient recovered from symptoms of BPPV. This case report suggests that the words '滑,''厥,''裏'on the 350 provision of Shanghanlun mean physical and psychological causes of BPPV in this case.
