The aim of this study is examine the protection effect for artery and antithrombotic effect of Sokmyong-tang(SMT; xuming-tang) on carotid artery thrombosis in a rat model. Thirty minutes before $FeCl_3$ treatment, SD rats were intraperitoneal injected with SMT. We tested the effects of SMT on time to occlusion (TTO) in thrombosis model by induced $FeCl_3$ using the laser Doppler flow meter and accessed thrombus weight (TW) inhibition and measured collagen fibers in the vessel after injury using Masson's trichrome stain. SMT(100 mg/kg, i.p.) showed significantly delayed TTO ($13.17{\pm}1.33$ min, P < 0.001) compared to vehicle control group ($8.63{\pm}0.92$ min) and inhibiting effect on TW with $0.72{\pm}0.02$ mg/mm (P < 0.05). In addition, SMT prevented collagen fibres damage in injured vessel ($22.24{\pm}4.48%$, P < 0.001). These results provide experimental evidence for SMT can be used to prevent vascular injury and thrombosis such as hypertension, arteriosclerosis, and so on.

A carotid-cavernous sinus fistula is a rare condition in which an abnormal communication exists between the internal or external carotid artery and the cavernous sinus. It typically occurs within a few weeks after craniomaxillofacial trauma. In most cases, the carotid-cavernous sinus fistula occurs on the same side as the craniomaxillofacial fracture. We report a case of delayed carotid-cavernous sinus fistula that developed symptoms 7 months after the craniomaxillofacial fracture. The fistula developed on the side opposite to that of the craniomaxillofacial fracture. Based on our experience with this case, we recommend a long follow-up period of 7-8 months after the occurrence of a craniomaxillofacial fracture. We also recommend that the follow-up should include consideration of the side contralateral to the injury.

We have reported that oral administration of Saiko-Ka-Ryukotsu-Borei-To (SRB), a traditional Chinese formulation, inhibited the intimal thickening in carotid artery after balloon injury in cholesterol-fed rats. To elucidate its mechanism, the effects of SRB on migration and proliferation of vascular smooth muscle cell (VSMC) were examined in vivo and in vitro. Carotid artery-internal jugular vein arteriovenous fistula is very rare, but it should be suspected in case of vascular injury by neck trauma because the diagnosis may be missed due to anatomical complexity of neck, We report a 57-year old male who had the carotid artery-jugular vein arteriovenous fistula caused by gunshot injury in the neck 44 years ago.

Objectives : This study was conducted to evaluate the effects of Corni Fructus, the dried fruits of Cornus officinalis Sieb., on unilateral common carotid artery occlusion (UCCAO) in mouse model. Methods : The Corni Fructus used in the experiment was extracted with anhydrous methanol, then filtered and freeze-dried. C57BL/6 mice used in the experiments were conducted left UCCAO surgery to set up UCCAO rodent model for mice. The mice were divided into five groups for evaluate the effect of methanol extract of Corni Fructus (COM) on UCCAO induced ischemic brain injury. The expression levels of nitric oxide in cerebrum and serum, body weight change were measured. To determine the effect of UCCAO and COM administration on brain neurons, morphological changes of the cerebrum through a microscope was conducted. And western blot was performed to confirm the underlying mechanism of neuroprotective effect of COM administration. Results : COM administered UCCAO groups (CO50, CO150, and CO500) had no significant effects on nitric oxide production in ipsilateral hemisphere proteins and sera. The CO500, 500 mg/kg COM administration, attenuated UCCAO-induced p38 inflammatory signaling pathway and inflammatory mediators such as iNOS and COX-2. The CO500 group showed resilient morphological changes of hippocampus neuronal cells about brain damage caused by decreased flow of blood. These group also showed decreased inflammation and cellular stress response in neuronal cells. Conclusions : From these results, COM has a neuroprotective property via moderating inflammatory factors and cellular stress inducing factors in brain cells.
Vertebral artery (VA) occlusion is frequently encountered, usually without acute ischemic injury of the brain. However, when it is accompanied by hypoplasia or stenosis of the opposite VA, brain ischemia may develop due to insufficient collateral supply. Both hemodynamic instability and embolic infarction can occur in VA occlusion, which may cause severe symptoms in a patient. Extracranial carotid-VA bypass should be considered for symptomatic VA occlusion patients, especially when the patient has repeated ischemic brain injuries. In this report, the cases of three extracranial carotid-VA bypass patients are introduced, along with a brief description of the surgical techniques. All three cases were treated with different bypass methods according to their disease location.
A 27-year-old man was admitted with a penetrating injury at the mid-manubrium. Computed tomographic (CT) angiography showed a f illing def ect in the aortic arch. This was evaluated as a sign of injury and the patient underwent an emergency operation. No active bleeding or clot was f ound in the mediastinum during the operation. The laceration point was between the innominate and the left carotid artery posteriorly. The injury was approached using hypothermic circulatory arrest. Aortotomy and exploration showed a 2-cm-long full-thickness aortic injury with an overlying clot. A filling defect on angiography as a sign of a penetrating arch injury has never been reported previously, but was the main pathological finding on CT angiography in our case. The aorta is a high-pressure system and injuries to it should be treated aggressively.
Objectives : Clear and detailed analysis on Pulse-taking on both Carotid and Wrist Pulses is an absolute prerequisite if it is to be applied in real practice or its practical value proven. Methods : The original notes found in Suwen and Lingshu, their translations, and conventional studies regarding Suwen LiuJieCangXiangLun's Pulse-taking are compared and analyzed to find cases within medical texts where this method was used and to get an idea of the direction taken by the conventional study. Results : The ilsung, esung, samsung, and sasung of Carotid (or wrist pulses) refers that the pulse is one-, two-, three-, four-fold in differences. Refrainment should be practiced while comparing carotid and wrist pulses. Rather, they need to be compared with their normal states. Used by Luo Tianyi in Ming Dunasty, this method of pulse-taking was used for (radial artery) when diagnosing and treating the degree of seriousness of food injury. The measurement of maximum blood flow velocity using TCD done recently proved the validity of this pulse-taking, and it's been used for alleviating hypertension or tinnitus through acupuncture, or abating intractable diseases (around carotid). Conclusions : The obscurity of the measuring method of this pulse-taking can be resolved, and the problem which occurs while comparing carotid and wrist pulses can be solved. Even though there are differences in opinion regarding the positions when comparing the two pulses, their practical values are acknowledged since their usages in diagnosis, treatments, experiments, and researches have yielded positive results to a degree. They may not be used that often, but they are nonetheless under utilization.
As carotid-cavernous fistula is the major complication that can be occurred uncommonly after maxillofacial injuries, it is abnormal arteriovenous communication between cavernous sinus and internal carotid artery. Such an arteriovenous communication is most often the result of injuries, but need not be associated with bony fracture. It usually begins soon after an injury, but it may be delayed for as long as several months. It begins undramatically with eye pain, headache and slow protrusion of eye ball. A bruit may be heard above the eye with stethoscope. Close examination will reveal dilatation of superficial veins of the eyelid and forehead and periorbital edema. There will be complete or partial ophthalmoplesia of the affected eye. Compression of the common carotid artery on the ipsilateral side will reduce or obliterate the bruit. The lesion in the cavernous sinus is them demonstrable by angiography.
