요양병원 간호사의 임종간호 스트레스가 이직의도에 미치는 영향 (Effects on Turnover Intention due to Terminal Care Stress of Nurses Working in Long-term Care Hospitals)

  • Purpose: This study was done to examine the effect on turnover intention (TI) of terminal care stress (TCS) on nurses working in long-term care hospitals (LCH). Methods: Participants were 182 nurses from 6 Seoul LCH. Data were collected from October to December, 2017. Self-report questionnaires were used to collect data on general characteristics, TCS, and TI. Results: Subjective satisfaction on the job (r=.52, p<.001), number of monthly terminal care elders (r=.16, p=.043), TCS (r=.16, p=.027), and sub-categories of TCS, 'difficulty for assigning timetable to care for terminally ill patients' (r=.17, p=.025), 'feeling a burden of caring for terminally ill patients' (r=.23, p=.002), and 'conflict with terminally patients' (r=.16, p=.034) showed statistically significant correlation with TI. Multiple regression analysis showed significant influence of subjective satisfaction with job (${\beta}=.52$, p<.001) and TCS (${\beta}=.23$, p=.001) with a 30.3% explanatory power. When sub-categories of TCS were entered, subjective satisfaction with the job (${\beta}=.50$, p<.001) and 'feeling burden of terminally ill patients' (${\beta}=.28$, p<.001) were factors significantly influencing TI with explanatory power of 32.8%. Conclusion: Findings of this study suggest that it is needed to develop standardized practice guidelines and educational programs for terminal care in LCH as well as stress healing programs for nurses.

여성 베이비부머의 부양지원 제공과 우울 (Providing family support and depression: Focussing on babyboom women)

  • Objectives: This study observes how providing family support - financial, emotional, and instrumental - is related to depression in babyboom women. Method: I analysed data from the Korean Longitudinal Survey of Women and Families 5th wave, collected by the Korean Women's Development Institute. The analysis was conducted using regression, t-test, Chi-square test, frequency analysis, and Pearson's correlation analysis. Results: Financial support for grown-up children, either married or unmarried, increases depression in babyboom women. In the dimension of instrumental support, caring for grandchildren reduces babyboom women's depression. It implies that contact with grandchildren is perceived as an emotional support rather than a physical burden. The results of this study also show that babyboom women are comforted by living with their own parents, but not by living with parents-in-law. The most impressive observation, however, is in the dimension of emotional support, which showed that the conversation with elderly parents reduces baby-boom women's depression, but that the conversation with married children increases it. The opposite directions in the effects seem to be related to the subjects of the conversations. The subjects of the conversation with children are mostly about problems and agenda of the children, while those with elderly parents are mainly about parents' health and care for each other. Conclusions: The results of the analyses suggest that babyboom women should become less involved with their grown-up children's lives. It also calls for the societal efforts to be put into building up the support system, especially for eco-boom generation, to reduce babyboomer's burden of supporting the children.

손자녀 양육지원을 거부한 조모의 경험에 관한 연구 (A Study on the Experience of the Grandmothers Who Refused to Support Childcare)

  • 본 연구는 손자녀 양육지원을 거부한 조모의 경험을 살펴봄으로써 손자녀 양육을 지원하고 있는 가족 및 노년기 여성에 대한 이해를 풍부하게 하는데 목적이 있다. 이를 위해 가족 내에서돌봄 역할을 주로 맡아온 조모를 대상으로 이들이 손자녀 양육을 수용하지 않은 계기와 맥락, 이를 둘러싼 가족관계 경험을 탐색해 보고자 하였다. 이를 위해 질적 연구방법 중 현상학적연구방법을 활용하였으며 손자녀 양육지원을 '거부한' 경험이 있는 노년기 여성 7명이 참여하였다. 연구결과, 첫째 이들은 손자녀 양육에서 오는 부담감과 연장된 어머니 역할에 대한 거부감, 가족 간 갈등발생 우려로 손자녀를 돌보지 않기로 하였으나, 성인자녀의 요청을 거절한 것에 대해 미안함을 느끼고 있었다. 둘째, 이들의 돌봄지원 거부 결정은 가족 구성원의 다양한역동을 야기했으나 이들은 심리적 어려움을 경험하면서도 나름의 방식으로 성인자녀들의 요구에 대응하며 자신의 결정을 관철하고 있었다. 셋째, 노년기 여성들은 손자녀 양육을 성인자녀들의 과업 혹은 사회가 책임져야 할 문제로 인식하고 있었으며, 가족과 사회가 여성 노인에게돌봄 책임을 전가한 것으로 인식하고 있었다. 이러한 연구결과는 돌봄 및 가족관계에 대해 과거 노인세대와 다른 관점을 가진 여성 노인들이 등장했다는 점, 전통적인 젠더 인식과 가족돌봄 규범에 균열과 변화가 발생했다는 점을 확인해준다. 이는 돌봄지원 정책에 수립 시 이들의 관점을 보다 적극적으로 반영해야 할 필요성이 있음을 시사한다. 이를 바탕으로 손자녀 양육을 지원하고 있는 가족 및 노년기 여성을 위한 실천적 정책적 개입 방안을 제시하였다.

영남권역에서 가정용 인공호흡기를 사용하는 환자 가족간병인의 간병 부담과 삶의 질 (Survey on the Care Burden and Quality of Life in Family Caregivers of Patients Using Home Mechanical Ventilator in Yeongnam Region, Korea)

  • 목적 : 본 연구는 영남권역 내 가정에서 인공호흡기 사용 환자를 돌보는 가족간병인이 느끼는 간병 부담과 삶의 질을 알아보고자 하였다. 연구방법 : 영남권역 내에 거주하며 가정에서 인공호흡기를 사용하는 환자를 돌보는 주간병인을 대상으로 설문조사를 시행하였고 가족간병인의 응답 내용을 분석하였다. 설문지는 환자의 정보, 간병 실태 및 추가 지원 요구 사항 등으로 구성하였다. 가족간병인의 간병 부담과 삶의 질을 평가하기 위해 각각 Korean version of Short Form Zarit Burden Interview(K-ZBI-12)과 한국형 가중치 모형에 따라 교정한 3-Level version of EuroQol-5 Dimension(KEQ-5D-3L)을 사용하였다. 통계적 유의수준은 p<0.05로 하였다. 결과 : 총 150명 중 주간병인이 가족간병인인 98명을 분석하였다. 환자의 연령은 54.10±17.94세, 성별은 남자 74명(75.5%), 여자 24명(24.5%)이었고 주 진단은 신경근육계 질환이 83명(84.7%)으로 가장 많았다. 가족간병인의 K-ZBI-12 점수는 33.08±10.34로 간병에 대한 고부담을 보였고 EQ-5D-3L 점수는 0.71±0.25로 두 항목은 음의 상관관계를 보였다(p=.038). 환자의 연령, 인공호흡기 사용기간, 의료비에 대한 가족간병인의 경제적 부담, 간병비에 대한 가족간병인의 경제적 부담 및 전문간병인의 참여 시간은 K-ZBI-12 점수와 각각 통계적으로 유의한 상관관계를 보였다. KEQ-5D-3L 점수는 가정용 인공호흡기의 사용기간과 음의 상관관계를 나타냈다(p=.017). 환자가 침습적 인공호흡기를 사용하는 경우 비침습적 인공호흡기를 사용하는 경우보다 가족간병인의 KEQ-5D-3L 점수가 낮았다(p=.008). 간병에 참여하는 인원이 두 명 이상인 경우가 한 명인 경우보다 K-ZBI-12 점수가 낮았다(p=.001). 가족간병인이 추가지원을 바라는 요구사항 중 경제적 지원에 대한 요구도가 가장 높게 조사되었다. 결론 : 영남권역에서 가정용 인공호흡기를 사용하는 환자들의 보호자가 느끼는 간병 부담이 크고 삶의 질이 낮다는 것을 알 수 있다. 가족간병인의 간병 부담과 삶의 질을 개선하기 위해서 지역에서 실질적으로 필요한 사회경제적 지원 및 환자와 가족들의 요구 사항을 확인하고 추가적인 지원을 위한 노력이 필요하다.

입원환아 어머니의 간호요구에 대한 영향요인 분석 (Study on Determinants of Nursing Needs of Mothers with Hospitalized Children)

  • Purpose: The purpose of this study was to analyze the factors influencing nursing needs of mothers caring for children suffering from acute diseases and being treated in hospital. Method: The data were collected, using a nursing needs, social support, burden, and stress questionnaire. Data collection was done from July 10, to October 20, 2001 in the pediatric department of a general hospital. The data were analyzed by using descriptive statistics. Result: The results of this study are as follows. There were statistically significant differences in nursing needs according to differences in age, education level, marital status, religion, family income and relationship with spouse. The relationship between the nursing needs of the mother and anxiety (r=.758, p <.01), social support (r=-.659, p<.01), stress (r=.324, p <.01) were the most significant variables. The stepwise multiple regression analysis showed that anxiety, social support and stress define nursing needs at 57.5%, 4.3% and 1.1% respectively. Conclusion: Consideration needs to be given to anxiety, social support and stress when developing nursing intervention programs for mothers whose children are hospitalized with acute diseases.

중증 만성폐쇄성 폐질환 환자로 살아가기 -살얼음판 위를 걸어가는 삶- (Living as Severe COPD Patient - Life of Stepping on the Thin Ice)

  • Purpose: The purpose of this study was to explore the life experiences of patients with a severe Chronic Obstructive Pulmonary Disease (COPD). Methods: The data were collected through in-depth interviews of six patients suffering from severe COPD. The interviewed data were audio-recorded and transcribed verbatim and checked for accuracy. The Giorgi method of phenomenology was used for analyzing data. Results: Eight themes forming the, units of meaning, were: Repeated and Unpredictable Suffering of Dyspnea, Confidence Loss/Exhaustion Life due to non-efficient breathing, Gradually Deprived Liberty, Absolute Being to Sustaining my life, Source of Burden but Significant Person I am in the Family, Endless Tug-of-War-Capability/Endeavor to Breath, Longing for my Life, and Dead-end of breathing. Conclusion: The study results provide an in-depth understanding of life experiences of patients suffering from severe COPD. The findings will be useful to nurses caring for this population.

여성농업인 생산활동 지원정책으로서의 농가도우미제도 평가 (A Study on the Farm Helper System for Rural Women)

  • The objectives of this study were 1) to evaluate level of satisfaction of the farm helper system which has been executed by Ministry of Agriculture & Forestry from the year 2000 as one of the welfare policies for assisting production activities of woman farmers. and 2) to suggest some measures for improving farm helper system. The data were collected from selected 729 women farmers who utilized the farm helper system. The major findings and suggestions from this study were as follows: 1) Women farmers responded that they had positive attitudes toward utilizing farm helper system by answering 'obtaining mental stability'(34.6%). 'reducing a burden of child birth'(29.2%), 'continuing to work on farm activities'(24.4%) and 'trust in national policies'(10.7%). 2) In addition, women farmers wanted to use the helpers for two months, before and after having a child birth. 3) They suggested that farm helpers system should be extended to other areas such as farming(21.5%), nursing(18.6%), and child-caring(16.1%).

요양시설노인의 자립적 식사를 위한 중재 효과와 경험 (An Intervention Study of Self-feeding for the Elderly in Nursing Homes)

  • Purpose: This study aims to investigate the effects of an intervention of self-feeding for elderly residents who were eating with assistance or eating by himself/herself with spilling food. Methods: The Participants were 11 elderly people and 6 formal caregivers from 7 nursing homes in Korea. The intervention was to use the spoon and chopstick sets designed for compensating the weakened eating function. Both quantitative and qualitative data were collected through observation, structured questions, and in-depth interviews. Results: The mealtime was significantly increased by 3.2 minutes (p=.011) after the intervention. Three themes were extracted for the meaning of self-feeding expressed by the elders; fighting alone for self-feeding, pride of participating in the study, and burden for self-feeding and research participation. Caregivers expressed the meaning of the elder's self-feeding such as the regret of missed chances, facilitating rehabilitation, the increase of the eating pleasure and quality of care, and ambivalence. Conclusion: Self-feeding has become an opportunity to recognize life values for the elders in nursing homes; for the caregivers, to reconsider caring of the elderly. Posture and eating utensils were also important to improve self-feeding skills.

아동의 고통경험에 관한 연구 (The Meaning of Suffering to Teeanger)

  • Suffering is a human burden that may not be truly avoidable. In order to put that view in perspective we must examine suffering in a form as isolated from self-inflicted behavior as possible. The suffering of a child is one such example. The purpose of this study is to understand the meaning of suffering in the teenager and is to analyze difference of suffering in children and adult. The subjects of this study were 6 childrens (12year-17year) including in-patients and out-patients of a general hospital who were diagnosed as having cancer. The data was collected from October 10, 1996 to April 15, 1997. Qualitative research methods of in-depth interview and participant observation were used for data collection. Data analysis progressed according to the fieldwork phases suggested by the Hybrid Model. According to the results of the study, the meaning of suffering in the teenager can be described as follows : Suffering is an inevitable experience of all human beings. When each child experiences pain and destroying child-adult relationship, suffering in which threaten one's personal integrity is perceived differently among each child depending on their personal inner factors, one's significant others, exterior circumstances and stimuli. Suffering brings severe and unendurable distress which accompany anguish, depression, anxiety and fear. This findings provide data for new insights of suffering. When caring for teenager who experience suffering, nurses need to consider the influence of suffering. Moreover, appropriate nursing interventions aimed at relieving suffering need to be developed.

말기 암 환자 가족이 경험하는 어려움에 관한 내용분석 (Content Analysis of Difficulties in Families with Terminal Cancer Patients)

  • Purpose: The objective of this study was to understand the caring experience of families with terminal cancer patients. Method: This was designed to be an inductive and descriptive study. Forty-seven families with terminal cancer patients were interviewed in depth and collected data were examined through content analysis. Result: The main categories of difficulties found in this study were 'suffering of patient', 'emotional suffering of family', 'bereavement of patient', 'difficulties in coping', 'problems in treatment', 'incurable situation', 'family problems', 'relationship with other people', 'economic problems', 'spiritual problems', 'problems in the future', 'informing patients of their condition', 'preparing death', 'emotional unstability', 'meaninglessness', 'unkindness of medical teams', 'poor environment for treatment', 'difficulties in hospital environment' and 'economic burden'. Conclusion: The main point found from this result was that families taking care of terminal cancer patients are suffering emotionally from watching the patients' pains and had difficulties in coping with the patients' situation and treatment. In addition, they had negative experiences in medical teams' attitude and hospital environment. This result can be used as an important guide for nurses to assess families' needs in the terminal care setting.
