요양보호사의 보상체계가 직무만족도에 미치는 영향 - 자기효능감 매개 효과 중심으로 - (Effect of Compensation System on Job Satisfaction for Care-Workers - Focused on the Mediating Effect of Self-efficacy -)

  • 본 연구는 요양보호사의 보상체계가 직무만족도에 미치는 영향에서 자기효능감에 대한 매개 효과를 규명하는 것을 목적으로 한다. 연구방법은 수도권 소재 노인요양시설과 이용시설 재가복지에 근무하는 요양보호사를 대상으로 설문 조사한 367명의 자료를 SPSS V22.0으로 분석한 결과는 다음과 같다. 첫째, 요양보호사의 보상체계에서 월평균급여, 보상 만족, 보상 공정성이 직무 만족에 정적인 영향을 미치는 것으로 나타났다. 둘째, 요양보호사의 보상체계에서 월평균급여, 보상 만족, 보상공정은 자기효능감에 정적인 영향을 미치는 것으로 나타났다. 셋째, 요양보호사의 자기효능감이 직무만족도에 미치는 영향은 긍정적으로 나타났다. 넷째, 요양보호사의 보상체계에서 월평균급여, 보상 만족, 보상공정은 직무만족도에 미치는 영향에서 자기효능감은 긍정적인 매개 영향을 미치는 것으로 나타났다. 이는 요양보호사의 보상체계에서 월평균급여, 보상 만족, 보상공정이 높을수록 자기효능감에 정적인 영향을 미치고, 이런 자기효능감은 요양보호사의 직무만족도에 정적인 영향을 미치는 것을 밝혔다.

  • 본 연구의 목적은 재가방문요양시설장의 변혁적 및 거래적 리더십이 조직유효성에 미치는 영향과 리더 십유형과 조직유효성간 관계에서 조절변수로서의 조직신뢰의 역할을 탐구하는데 있었다. 본 연구에서 조직유효성 변수로는 직무만족과 이직의도의 두 변수를 사용하였다. 연구를 위해서 구조화된 질문지를 이용하여 서울시 소재 50개 재가방문요양시설에 근무하고 있는 요양보호사 290명으로부터 자료를 수집하였다. 수집된 자료는 단계적 회귀분석방법을 사용하여 분석하였다. 연구 결과 시설장의 변혁적 리더십은 요양보호사들의 직무만족에 유의미한 긍정적인 영향을 미치고, 이직의도에 부정적인 영향을 미치는 것으로 나타났다. 반면에 거래적 리더십은 직무만족에 이직의도에 유의미한 영향을 미치지 않는 것으로 나타났다. 또한 조직신뢰는 변혁적 리더십과 직무만족간의 관계를 조절하는 것으로 나타났다.

요양보호사의 소명의식, 의사소통능력, 직무만족이 직업의식에 미치는 영향 (The Effects of professional Consciousness on Calling, Communication Competence, Job Satisfaction of Caregivers)

  • 본 연구의 목적은 요양보호사의 직업의식에 영향을 주는 영향요인을 파악하여 직업의식 향상을 위한 기초자료를 제공하는데 있다. 본 연구는 서술적 연구로서 G시에서 요양보호사로 근무하고 있는 110명을 대상으로 2019년 9월부터 6주 동안 설문지를 이용하여 자료수집 하였다. 분석을 위해 서술적 통계, t-test, ANOVA, Pearson's correlation coefficients, Multiple regression으로 분석하였다. 연구결과 요양보호사의 소명의식 3.41점, 의사소통능력 3.23점, 직무만족은 4.12점이었다. 직업의식과 직무만족은 정 상관관계(r=.636, p<.001)), 직업의식과 직무만족(r=.378 p<.001), 직업의식과 소명의식(r=.356, p<.001)로 정 상관관계를 보였다. 요양보호사의 직업의식에 영향을 미치는 요인으로는 직무만족(B=.329, p<.001)이 가장 높은 영향을 주는 것으로 나타났으며, 의사소통능력(B=.216, p<.001), 소명 의식(B=.165, p=.002)을 포함한 모델의 설명력은 57.5%로 나타났다. 추후 요양보호사에게 직무만족 강화훈련을 지속적으로 유지해야 할 것이다.

일 병원의 환자중심 지능형 병상 지원(Smart Bedside Station) 시스템의 이용현황 및 사용 만족도 (Utilization of and Satisfaction with Smart Bedside Station System as a Patient-centered Healthcare System)

  • Purpose: The aim of this study was to examine the utilization of and satisfaction with the smart bedside station (SBS) system among users in a hospital. Methods: A cross-sectional descriptive design was used. The participants were 190 patients, 186 family caregivers, and 154 nurses in a hospital. Results: Around 78.1% of patients or family caregivers used the SBS system at least once during their hospital stay. The commonly used items on the SBS system menu were "lab findings", "hospital cost", "today's medication", and the "alarm message". Satisfaction with the SBS system of patients and family caregivers were significantly higher than those of nurses (F=39.88, p<.001). Conclusion: A patient-centered SBS system was a useful system that could increase patient satisfaction and comfort. More specific and technical service contents reflecting the current healthcare system should be added.

노인요양병원 간병인의 소진에 대한 영향요인 (Factors Affecting Burnout of Caregivers in Geriatric Hospitals)

  • Purpose: This study was conducted to identify the factors affecting burnout of caregivers in geriatric hospitals. Methods: Data were collected from 266 formal caregivers working at Korean geriatric hospitals using self-report questionnaires between May 16 and June 30, 2016. The measured variables were burnout, emotional labor, job stress, job satisfaction, and self-efficacy experienced by caregivers. T-tests, ANOVA, Pearson correlation coefficients, and multiple regression were employed for data analysis. Results: The results indicated that emotional labor and job stress significantly influenced burnout of caregivers in geriatric hospitals; the explanatory power of the model was 27.3%. Conclusion: Based on the study results, we suggest further investigation of factors influencing burnout, and the development of strategies to prevent burnout by reducing emotional labor and job stress experienced by caregivers.

Burdens, Needs and Satisfaction of Terminal Cancer Patients and their Caregivers

  • Objectives: Terminal cancer patients and their caregivers often experience traumatic stress and need many types of assistance. In the present study we interviewed terminally ill cancer patients and caregivers to determine how much burden they experienced and to find out what factors are most important for satisfaction. Design: We constructed a questionnaire including overall care burden and needs experienced, and administered it to 659 terminal cancer patients and 659 important caregivers at 11 university hospitals and 1 national cancer center in Korea. Results: Finally, 481 terminal cancer patients and 381 caregivers completed the questionnaire. Care burden was not insubstantial in both and the caregiver group felt more burden than the patient group (P<0.001). While the patient group needed financial support most (39.0%), the caregiver group placed greatest emphasis on discussion about further treatment plans (44.8%). Stepwise multiple logistic regression analyses showed that in the patient group, patient's health status (OR, 2.03; 95%CI, 1.16-3.56) and burden (OR, 2.82; 95%CI, 1.76-4.50) influenced satisfaction about overall care, while in the caregiver group, high education level (OR, 1.84; 95%CI, 1.76-4.50), burden (OR, 2.94; 95%CI, 1.75-4.93) and good family function (OR, 1.94; 95%CI, 1.24-3.04) were important. Conclusions: Our study showed that burden was great in both terminal cancer patients and their caregivers and was perceived to be more severe by caregivers. Our study also showed that burden was the factor most predicting satisfaction about overall care in both groups.

노년기 수발 경험에 따른 건강 변화 : 사회적 관계망과 만족도의 매개효과 분석 (Elderly Caregiving and Health : Analysis of the mediate effect of social network and Satisfaction)

  • 본 연구는 수발 경험에 따른 수발자의 건강상태 변화를 살펴보고, 수발 경험이 수발자의 건강에 미치는 영향에 대해 사회적 관계망과 만족도가 매개변수로서 어떻게 작용하는지를 검증하는데 목적이 있다. 소기의 목적을 달성하기 위해 고령화연구패널자료를 활용하여 구조방정식모형을 활용하여 분석을 시도하였다. 분석 결과 수발 이후 남녀 모두 우울감이 증가하고 주관적 건강상태나 낮아졌다. 사회적 관계망과 만족도 변화 양상에는 성별에 따른 차이가 있는 것으로 나타났다. 여성의 경우 수발이 주관적 건강상태와 우울감에 영향을 미쳤으며, 매개효과는 발견되지 않았다. 남성의 경우 수발 경험이 우울감에 미치는 총 효과는 1.087(p<.05), 간접효과는 0.546(p<.05)이었으며, 직접효과는 유의미하지 않은 것으로 나타났다. 남성의 수발 경험은 기능적 가족망의 감소(0.42, p<.01)를 완전매개로 주관적 건강상태에 영향을 미쳤다. 이러한 연구 결과를 통해 수발 이후 수발자들의 건강 취약성이 확인되었다. 또한, 수발자의 건강과 이를 매개하는 요인을 탐색하는 데 있어 성별에 따라 그 양상이 따르게 나타나는 것으로 나타났다. 특히 남녀 모두 기능적 가족망이 유의미하게 감소하였음에도 남성에게서만 기능적 가족망의 매개효과가 확인되어 남성의 기능적 가족망 강화의 필요성을 제기하였다.

만성질환자 가족의 삶의 질 예측모형 구축에 관한 연구 (A Model for Quality of Life of Family Caregivers with a Chronically Ill Patient)

  • This study was designed to construct a model that predicts the quality of life of family caregivers with a chronically ill patient. The hypothetical model was developed based on the findings from past studies on quality of life and on the family with a chronically ill patient. Data were collected by self-reported questionnaires from 200 family caregivers in Seoul & Kyung Gi-Do, from May 1 to July 21, 1997. Data were analyzed using descriptive statistics and correlation analysis. The Linear Structural Relationship(LISREL) modeling process was used to find the best fit model which predicts causal relationships among variables. The results are as follows : 1. The overall fit of the hypothetical model to the data was moderate [X$^2$=31.54(df=23, p=.11), GFI=.96, AGFI=.91, RMR=.04]. 2. Paths of the model were modified by considering both its theoretical implication and the statistical significance of the parameter estimates. Compared to the hypothetical model, the revised model has become parsimonious and had a better fit to the data expect chi-square value(GFI=.95, AGFI=.91, RMR=.04). 3. Some of predictive factors, especially economic status, physical ability to perform daily-life activity, period after disease-onset, social support and fatigue revealed indirect effects on the quality of life of family caregivers with a chronically ill patient. 4. The factors, burden and role satisfaction revealed significant direct effects on the quality of life of family caregivers with a chronically ill patient. 5. All predictive variables of quality of life of family caregivers with a chronically ill patient, especially economic status, physical ability to perform daily-life activity, period after disease-onset, social support, fatigue, burden and role satisfaction explained 38.0% of the total variance in the model. In conclusion, the derived model in this study is considered appropriate in explaining and predicting quality of life of family caregivers with a chronically ill patient. Therefore it can effectively be used as a reference model for further studies and suggests direction in nursing practice.

영유아를 돌보는 양육자의 방임 및 학대에 영향을 미치는 심리 사회적 요인 탐색 (Exploring Psycho-social Determinants to Child Neglect and Abuse among Caregivers with Young Children)

  • Purpose: The objective of this study was to investigate the relations among psycho-social factors regarding child neglect and abuse using the data from the 2013 Korea National Survey on Children and Youth. Methods: Data from a sample of 1,062 primary caregivers with young children were analyzed with the SPSS and AMOS programs to examine the interrelationships among depression, parenting stress, marital satisfaction, parent-child attachment, and child neglect or maltreatment. Results: Depression, stress, and attachment had a direct influence on child neglect and abuse. Satisfaction with marital relationship, parenting stress, and attachment were found to play mediating roles in accounting for child neglect and abuse, explaining 12% of the variance. The results of multi group path analysis showed that some coefficients were different according to the age group of the children. For caregivers with infants, parenting stress did not predict either attachment or child maltreatment, whereas for caregivers with toddlers, depression did not have a significant effect on attachment. Conclusion: In order to prevent child maltreatment, efforts should be made to develop community-based psycho-social support interventions focused on marital relationship as well as parent-child dyads and to provide practical child care support.

Effects of Interprofessional Nurse-led Rehabilitation Intervention Program for Subacute Stroke Patients and their Caregivers: a non-randomized study

  • Purpose: Rehabilitation nursing requires an interdisciplinary comprehensive team approach that allows for enough time to promote patients' functional recovery. We identified the effects of a nurse-led rehabilitation intervention program on subacute stroke patients' activities of daily living, anger, and muscle strength, as well as their chief caregivers' satisfaction. Methods: Intervention group participants (n = 20) completed a rehabilitation intervention program, which integrated physical activities with psychological and social approaches. Results: The program did not significantly improve patients' activities of daily living or anger management; however, it significantly affected both anger-in (t = 3.384, p = .002) and anger-out (t = 2.167, p = .037) attitudes. Caregivers' satisfaction among the intervention group (t = 6.935, p < .001) decreased more significantly than that of the comparison group (t = 0.224, p = .825). Conclusion: Rehabilitation intervention program should enlist nurses' participation and promote a team-based approach during the rehabilitation program. Further, patients and their caregivers should be encouraged to express their emotions during counseling.
