Factors Affecting Communication Satisfaction of Geriatric Caregiver : Focusing on the case of an urban-rural complex community (노인 돌봄서비스 제공자의 의사소통 만족도에 영향을 미치는 요인 : 도·농 복합 지역사회의 사례를 중심으로)

  • Objectives: The aim of this study was to identify factors influencing communication satisfaction between geriatric caregivers and older adults in urban-rural complex communities. The ultimate goal was to design local community educational programs and policies to enhance communication satisfaction among geriatric caregivers and improve the quality of care services for older adults. Methods: To identify factors influencing communication satisfaction between elderly caregivers and older adults, a survey titled "CCEP: Assessment of Communication Status between Elderly Care Service Providers and Recipients" was conducted from February to July 2020, focusing on rural-urban complex areas. The survey was administered based on providers of elderly healthcare services. The survey targeted 131 respondents involved in providing care services for older adults. The dependent variable of this study was the communication satisfaction reported by elderly caregivers in their interactions with the elderly. The independent variables included perceptions of older adults, factors associated with communication difficulties, and communication efforts. Additionally, gender, working environment, working experience, and the proportion of face-to-face interactions with older adults during caregiving were controlled for the hierarchical multiple regression analysis. Results: The analysis revealed that communication efforts with older adults significantly influenced communication satisfaction (β=.09, p<0.01). However, perceptions of the elderly and communication hindrance factors did not have a significant impact on communication satisfaction among geriatric caregivers. Conclusion: Effective communication between geriatric caregivers and older adults is crucial for identifying and meeting the needs and demands of caregiving services, and it plays a vital role in overall caregiving service satisfaction. To enhance communication skills and satisfaction among geriatric caregivers and ensure the appropriate fulfillment of elderly care needs in the local community, the development of community-centered, specialized health communication programs and other initiatives will be necessary in the future.

Empathy In Informal Caregiving: Extension of A Concept from Professional Practice

  • The concept of empathy was examined In the professional caregiving relationship and its application was extended to the context of informal caregiving. Using the Lazarus and Folkman model, the influence of empathy on the caregiver's experience in the caregiving relationship was illustrated. The effects of the caregiver's empathy on his/her own caregiving outcomes were investigated by examining the relationship between empathy and burnout experience and life satisfaction. Empathy increased emotional exhaustion while increased personal accomplishment and personalization, indicating conflicting relationship between empathy and burnout. This conflict relationship between empathy and burnout can be explained by suggesting the distinct roles of two dimensions of empathy emotional and cognitive. The needs for more research to support the critical roles of empathy in informal caregiving context and to examine the definite roles of two dimensions of empathy were suggested.

  • The purpose of this study was to explore the relationship between caregivers' characteristic variables and quality of child care and to investigate the magnitude of the association between variables and quality of child care according to children's age. The following factors were suggested as the caregivers' characteristic variables: Total years of caregiving experience, educational level, job satisfaction, amounts of turnover, time for commuting, salary, pay step, years of work in charge of current children, perception on the recommended caregiver/child ratio. The results of the study were as follows : First, it was found that caregivers' salary, total years of caregiving experience, pay step, perception on the recommended caregiver/child ratio, age, and amounts of in-service program were significant among caregivers' variables. Second, when the caregivers' characteristic variables were examined in terms of children's age respectively, the effects of teacher-related variables to influence the child-care quality were significantly different according to children's age group.

  • Purposes: The purpose of this study is to analyze the factor of affecting on job satisfaction by mediating attitude of care service. Methodology: (1)Identify socio-demographic characteristics, work characteristics, the elderly support period, the presence of education related to the elderly, the presence of service associated with the elderly, and significant factors of the research subjects. (2)Identify changes in the number of education and services before and after obtaining a caregiver qualification. (3)Identify the attitude of providing care services and job satisfaction according to the characteristics of the research subjects. (4)Identify the direct and indirect factors influencing job satisfaction by mediating the service attitudes of the research subjects. Findings: First, age factors have positive effects and the volunteering experience have negative effects on job satisfaction by mediating attitude of care service. Although the number of education class has increased after qualification, educational factors had no significant effect. Second, the longer elderly support period had a positive effect on job satisfaction. Practical Implications: This study is meaningful in that it identifies the direct and indirect impacts on job satisfaction through care attitudes. In the future, it will be necessary to pay attention to improving the quality of long-term care services by analyzing the influence factors more.

  • This study investigated the effect of cash benefit and in-kind benefit policy supported by disabled children on the satisfaction of service care of main caregiver. The results of this study are as follows. First, the parental stress, parenting burden, and family members' difficulties were investigated. However, the research on the salary policy supported by the handicapped children has been scarce, and it has been found that the research on the service satisfaction of the main caregiver is also insufficient by the type of salary. The purpose of this study is to examine the effect of variables (parents gender, child gender, parents age, child age, disability grade, average income) on service satisfaction. As a result, parents age, child age, and child gender showed statistically significant effects on service satisfaction. This resulted in statistically significant overall effects on the application process, economic burden, support and selection criteria, service volume, and overall satisfaction. Based on the results of the study, the implications for cash benefits and in-kind benefits could be improved by increasing the amount and scope of benefits, and customized services considering the age of the handicapped children, ultimately improving the service satisfaction of the main caregiver.

A Study on the Effect of Caregiver Burden on Suicidal Ideation among Caregiver for the Elderly with Dementia (치매노인의 증상정도가 부양자의 자살생각에 미치는 영향에 대한 연구: 부양부담의 매개효과를 중심으로)

  • The purpose of the study is examining the mediation effect of caregiver burden's segmentalized sub factors between dementia caregivers on the relationship between Symptom extent of dementia patients and Suicidal Ideation of dementia caregiver, and suggesting social welfare intervention methods for dementia caregiver The survey is targeted to demented elderly people and caregivers, and currently using medical care institution and day care center in Seoul, Gyeonggi Province and Pusan city. As a result of the survey, 415 cases were collected for the final analysis. In data analysis process, we used SPSS 21.0 for the mediation effect of conversational satisfaction and its significance, and the results are following. First, 21% of the caregivers responded that they had thoughts of suicide in the past year. Second, Symptom extent of dementia patients was positively related to caregiver burden. Third, worse in family relationships, which is sub factors of mediate variable, has partial mediate effect on the model. Based on these outcomes, we suggest the importance and necessity of improved approach about dementia elderly and caregiver between elderly couple as way to reduce caregiver burden and proposed social work-based intervention program for enhancing this.

The Comparison of the Perceived Needs between Patients with Cancer, their Caregivers, and Nurses According to the Cancer Patient,s Phases of Illness (질병단계에 따라 암환자, 돌보는 가족원, 간호사의 간호요구 지각 비교)

  • This descriptive study was conducted to ascertain whether the needs of patients with cancer, their caregivers and their nurses changed according to the illness phases and if the perceived needs of the three groups were different for three categories of nursing needs. At two hospitals in Seoul and Choongnam, three groups of subjects, -patients with cancer(79), caregivers(92), and nurses(72) - responded to a questionnaire consisting of items on educational need(11 items), physical need(8 items), emotional need(9 items) using a 4-point Likert scale. The patients and caregivers were selected according to the phase of the cancer (initial, intermediate or recurred, terminal phases). Finding revealed that the level of perception and degree of satisfaction of the needs were low, just around two points in patients and caregivers. Of the three categories of needs, physical needs were received the highest score and the degree of satisfaction of physical needs was also the highest. There was no significant difference between the level of perception and satisfaction of needs in patients and caregivers according to the phases of the illness and the degree of per reception and the satisfaction of the patients were not significantly different and caregivers showed the same result. There was a significant difference in the level of importance of the needs of nurses according to the phases of the cancer. They perceived emotional needs were the most important in first phase and second phase, physical needs in third phase and the educational needs were more important in the first phase than in any other phase. The degree of importance of needs was significantly lower than the degree to which needs were addressed, according to the nurses response. In a comparison of patient and caregiver's perceived degrees of need, and need satisfaction, and nurse's perceived degree of need provision, patient and caregiver scores were lower than the nurses.

A Study on Changes of Primary Caregivers' Fatigue, Depression and Life Satisfaction by Using Dementia Day Care Service (치매노인의 주간보호서비스 이용에 따른 주부양자의 피로, 우울, 생활 만족도 변화)

  • Purpose: This study is to examine how dementia day care service affects fatigue, depression and life satisfaction of caregiver. Methods: The study was conducted using a convenient sampling method from 6 Dementia Day Care Center in Incheon. Thirty nine primary caregivers answered the questionnaires. Pre-test was done before demented elders start using the dementia day care center and post-test was done five months after. Data were analyzed by using descriptive statistics and paired t-test. Results: There was a significant difference in caregivers' fatigue level after using dementia day care service(t=2.188, p=.035). Results of subcategories were as follows; There was a significant difference in caregivers' physical fatigue level(t=2.270, p=.029) and psychological fatigue level(t=2.277, p=.029) after using dementia day care service. However, there was not significant difference in caregivers' neurological fatigue level(t=1.312, p=.197). There was a significant difference in caregivers' depression level(t=3.066, p=.004) and life satisfaction(t=-2.131, p=.040) after using dementia day care service. Conclusion: The results indicated that dementia day care service is helpful for family with demented elders. Therefore it is necessary to expand dementia day care center in terms of its size and numbers in order to support increasing number of demented elders and their families.

Burdens, Needs and Satisfaction of Terminal Cancer Patients and their Caregivers

  • Objectives: Terminal cancer patients and their caregivers often experience traumatic stress and need many types of assistance. In the present study we interviewed terminally ill cancer patients and caregivers to determine how much burden they experienced and to find out what factors are most important for satisfaction. Design: We constructed a questionnaire including overall care burden and needs experienced, and administered it to 659 terminal cancer patients and 659 important caregivers at 11 university hospitals and 1 national cancer center in Korea. Results: Finally, 481 terminal cancer patients and 381 caregivers completed the questionnaire. Care burden was not insubstantial in both and the caregiver group felt more burden than the patient group (P<0.001). While the patient group needed financial support most (39.0%), the caregiver group placed greatest emphasis on discussion about further treatment plans (44.8%). Stepwise multiple logistic regression analyses showed that in the patient group, patient's health status (OR, 2.03; 95%CI, 1.16-3.56) and burden (OR, 2.82; 95%CI, 1.76-4.50) influenced satisfaction about overall care, while in the caregiver group, high education level (OR, 1.84; 95%CI, 1.76-4.50), burden (OR, 2.94; 95%CI, 1.75-4.93) and good family function (OR, 1.94; 95%CI, 1.24-3.04) were important. Conclusions: Our study showed that burden was great in both terminal cancer patients and their caregivers and was perceived to be more severe by caregivers. Our study also showed that burden was the factor most predicting satisfaction about overall care in both groups.

A Study on the Usage Behavior of Elderly Welfare Facilities for the Elderly with Moderate Dementia: Focusing on Satisfaction with Memory School Users (경증치매노인 노인복지시설의 이용행태에 관한 융합연구: 기억학교 이용자만족도를 중심으로)

  • The purpose of this study is to provide basic data to confirm the necessity and effectiveness of the memory school by investigating the perception of users and caregivers about the services and programs provided by the memory school, a social welfare institution for the elderly with minor dementia. For this purpose, 363 students and students in Daegu City were surveyed about the satisfaction and attitude of using memory school. As a result of the analysis, it was found that the use of the memory school is very helpful to the users, and at the same time, it is very meaningful for the decrease of the caregiver burden of the caregiver. Thus, it can be seen that the memory school has the effect of appropriately matching the installation purpose of the facility. In particular, it was found that the degree of satisfaction with the program level, type of service, and future sustainability of the memory school exceeded 90%. Therefore, this study aims to provide policy implications for the social welfare response for the elderly with mild dementia and for the basic data on the memory school for this purpose.
