독거노인의 노인돌봄서비스 만족도에 영향을 미치는 요인 (Factors Affecting the Satisfaction of Elderly Care Service in the Elderly Living Alone)

  • 본 연구의 목적은 독거노인의 노인돌봄서비스 만족도에 영향을 미치는 요인을 살펴보고, 노인돌봄서비스의 만족도 증진을 위한 기초 자료를 제공하고자 한다. 연구대상은 충북 J시의 노인돌봄기본서비스를 받고 있는 65세 이상 독거노인 301명을 대상으로 하였으며 영향요인을 살펴보기 위해 다중회귀분석을 실시하였다. 본 연구결과 독거노인의 노인돌봄서비스 만족도에 영향을 미치는 요인은 관리사 요인, 서비스 요인, 기관 요인, 소득수준이 유의미한 것으로 나타났다. 이는 생활관리사, 서비스 요인, 기관 요인이 높을수록, 소득수준이 낮은 독거노인 일수록 노인돌봄서비스 만족도가 높은 것으로 나타났다. 특히 생활관리사 요인이 노인돌봄서비스 만족도에 가장 큰 영향을 미치는 것으로 나타났으며, 다음은 기관요인, 서비스요인, 소득수준 순으로 나타났다. 이러한 연구결과를 근거로 정책제언을 하면 첫째, 생활관리사들의 전문성 증진과 처우개선, 둘째, 기관의 전문적이고 체계적인 서비스 지원과 사회안전망 강화, 셋째, 노인돌봄서비스의 품질향상을 위한 적절성·접근성·지속성 강화가 필요하다.

경남지역 일부 산업간호사의 보건관리 업무 및 관련요인에 관한 연구 (A Study on Health Care Activities of Some Industrial Nurses and their Related Factors in Kyungnam Area)

  • The purpose of this study was to assess the performance of the role and function of some industrial nurses and to characterize the factors affecting the performance of their activities. Thus the results could be used to suggest the direction in the performance of industrials nurses' activities effectively. During a period from January 10 to March 31, 1994, the data were collected from 87 industrial nurses, who were working as health managers in the plants, in Ulsan city and the vicinity in Kyungnam province, using a structured questionnaire. The results were as follows : 1. The general characteristics of industrial nurses in this study were 82.8% being 30 years old or less, 60.9%, being not married, and 93.1% having eduction levels above junior college. 2. With respect to general work conditions, 94.3% were working in a separate room provided for health care division, 40.2% working under the safety and health department, and 98.9% working as common-level staffs. And 60.9% were working less than 44 hours a week, 70.1% had work experiences less than 5 years, and 50.6% had annual incomes ranging 10 to 14 million wons. 3. As work conditions related to health care activities, 49.4% performed the activities not related to health care as always or occasionally, and 87.4% answered that occupational physicians were appointed in their plant and among them, however, only 6.9% worked on full-time basis and 52.8% perform little activities as occupational physicians. For a decision related to health care activity, 69.0% discussed the problems with the supervisors, and 19.5% made decisions by themselves. 4. As for attitude and perception to their activities as health managers, 66.7% moderately recognized the importance of health manager in the workplace, with 63.2% being satisfied their wages and treatment from the company, 57.5% being satisfied with their job positions and 51.7% having positive attitudes as being health managers. 5. The degree of performance at least in one of health related activities were very high in activities such as general medical care(100%), general health examination(98.0%) and specific health examination(100%), and relatively high in health education(72%), new employee health examination(60.9%), document handling(79.3%) and activity for work environment(70.1%). However, the performance rate was very low in preparing protective equipment (20.8%). 6. The levels of activities related to health care were significantly high when making decisions by themselves, when occupational physicians not being full-time, and when satisfying their job positions, and, on the other hand, significantly decreased as work hours increased. 7. In addition to some kinds of periodic education asked by all of the nurses, 89.7% wanted a specialized licensing system for industrial nurse, and 97.4% wanted to apply for the license test. As a conclusion, it is suggested that industrial nurses should be given more authority and placed in more self-controlled system to perform health care and other activities more efficiently, and the role and function of the occupational physician should be clearly distinguished from that of the industrial nurse as a health manager to avoid an unnecessary overlapping.

서비스 로봇용 결함 허용 미들웨어 (Fault-Tolerant Middleware for Service Robots)

  • Recently, robot technology is actively going on progress to the field of various services such as home care, medical care, entertainment, and etc. Because these service robots are in use nearby person, they need to be operated safely even though hardware and software faults occur. This paper proposes a Fault-Tolerant middleware for a robot system, which has following two characteristics: supporting of heterogeneous network interface and processing of software components and network faults. The Fault-Tolerant middleware consists of a Service Layer(SL), a Network Adaptation Layer(NAL), a Network Interface Layer(NIL), a Operating System ion Layer(OSAL), and a Fault-Tolerant Manager(FTM). Especially, the Fault-Tolerant Manager consists of 4 components: Monitor, Fault Detector, Fault Notifier, and Fault Recover to detect and recover the faults effectively. This paper implements and tests the proposed middleware. Some experiment results show that the proposed Fault-Tolerant middleware is working well.

보건관리자의 역할수행이 유병근로자의 출근으로 인한 업무손실에 미치는 영향 (Effect of Workplace Health Manager's Role Performance on Presenteeism in the Workers)

  • Purpose: This study was conducted to determine the effect of the performance of Workplace Health Manager at the workplace on presenteeism in the workers. Methods: Three months before this study, between June 2010 and September 2010, a self-reporting survey of 316 employees in 136 workplaces in each of each hired a Workplace Health Manager was performed at their workplace with explanations of the purpose and methods of this study after their consent was obtained. Results: The average performance score of the Workplace Health Manager as graded by the employees was 3.8 out of 5 points. The duties of the Workplace Health Manager which received higher grades were posting of the Material Safety Data Sheets (MSDS), health education, and future management with respect to the results and procedure of health care-related work. According to the employees' survey, the score for presenteeism was evaluated as 14.3 out of 30 points. Based on the presenteeism analysis results, when (1) the employee was a male, (2) the workplace was established and managed by Industry Safety and Health Committee, (3) the employees were aware of the role of Workplace Health Managers, and (4) Workplace Health Manager fulfilled his/her role actively and successfully, presenteeism was observed to a lesser extent. Conclusion: According to the results of the study, presenteeism was observed to a lesser extent when the Workplace Health Manager actively performed his/her role.

사례관리수행요소가 정신질환자의 삶의 질에 미치는 영향 -정신건강 토탈케어 바우처 기관을 중심으로- (The influence of Case Management Implementation Factors on Quality of Life for the mentally Ill -Based on Mental Health Total Care voucher agencies-)

  • 정신건강 토탈케어 바우처 기관의 사례관리자와 이용자를 대상으로 사례관리 수행요소가 정신질환자의 삶의 질에 미치는 영향을 분석하였다. 클라이언트 요인, 사례관리자 요인, 서비스 전달 요인과 기관요인을 위계적 선형모형(HLM: Hierarchical Lineral Model)을 적용하여 검증하였다. 본 연구는 국내최초로 사례관리자의 사회복지경력과 사례관리 개입범위가 정신질환자의 삶의 질에 유의미한 영향력이 있다는 것을 검증하였고, 담당 사례수가 1인당 20케이스 이하일 경우 삶의 질에 영향을 미치지 않는 것을 확인하였다. 연구를 통해 사례관리자의 사회복지경력, 담당 사례수, 지역사회환경체계 개입범위의 유용성에 대한 실천적 함의를 제안하였다.

좋은 돌봄의 필요조건과 저해요인에 관한 연구 - 노인돌봄을 중심으로 - (The Necessary Conditions and Deterring Factors of Good Care)

  • 노인장기요양보험 도입으로 돌봄서비스는 크게 성장하였지만, '우리가 과연 충분히 좋은 돌봄을 하고 있는가'라는 질문에 대해서는 긍정적 대답을 하기 어렵다. 이 연구는 우리 사회가 좋은 돌봄을 실현하기 위해서는 어떠한 조건이 필요한가를 파악하는 데 목적이 있다. 이 연구에서는 좋은 돌봄을 창출하는 필요조건과 저해하는 상황이 무엇인지에 대하여 현실적인 지식을 습득하기 위한 방안으로 포커스그룹 인터뷰(FGI)를 활용하였다. 포커스그룹 인터뷰는 요양보호사와 기관장을 대상으로 2014년 1월부터 3월까지 총 3회에 걸쳐서 진행하였으며, 인터뷰마다 3시간 정도 소요되었다. 분석 결과, 좋은 돌봄의 필요조건은 적극적 소통을 통한 상호 이해와 인정, 상호신뢰하고 존중하는 좋은 관계 맺기, 요양보호사의 전문성, 센터 관리자의 전문성, 가족-노인-요양보호사의 3각 파트너십으로 분석되었다. 반면, 좋은 돌봄의 저해요인은 사회적 인정을 받지 못하는 돌봄노동, 돌봄노동의 무정형성과 남용, 요양보호사 및 관리자의 비전문성, 가족의 부적절한 간섭과 무관심, 장기요양환경의 구조적 제약으로 분석되었다.

경남지역 일부 보육시설 원아의 영양섭취에 관한 연구 (Foodservice and Nutrition Survey of Children of Day Care Centers in Gyeongnam Area)

  • Investigation of foodservices was carried out in 7 different day care centers in Gyeongnam area and no trition survey of their 481 children were undertaken between October 5 and November 3, 1994. The results were summarized as follows: Conditions of most equipments and facilities used in the kitchen were very defective especially in dishwashing and sterilizing step. Number of cooks and assistant cooks in foodservices was 1 or 2. Purchasing of foods and planning of menu were mainly done by the manager or the secretary. Menu was cycled weekly. None of the day care centers has empolyed a dietitian. Mean values of height and weight of children showed slightly over the standard of Korean children. Mean intakes of energy and all the nutrients from the lunch, which was served by day care centers, were below 30% of the Korean RDAs. Mean value of protein, thiamine and niacin were above 25% of the RDAs. Intakes of Ca, Fe and Riboflavin were lower than the ones of other nutrients. It is suggested that dietitians should be empolyed to take care of meal services and nutrition education for the children.

병원 위탁급식 품질관리를 위한 품질평가도구 개발 (The Development of a Quality Measurement Tool for a Contract-Managed Hospital Foodservice)

  • The purposes of this study were: a) to develop the a quality measurement tool for the contract-managed hospital foodservice, and b) to evaluate their performance with the developed quality measurement tool, and c) to verify the reliability and validity of the quality measurement tool. The developed quality measurement tool comprised two parts, which were foodservice management and medical nutrition care service. The foodservice management part was classified into six functional categories which were Menu, Procurement and Storage, Production and Distribution, Facility and Utility, Sanitation and Safety, and Management and Evaluation. The medical nutrition care service part indicated the medical nutrition care provided. Quality measurement tool had 91 standards and 324 indicators. The quality measurement tools were distributed to the hospital foodservice manager employed by the foodservice company. The 324 indicators were measured by foodservice manager on the 5-Likert-type scales, and then adapted to a 100 point scale. The SPSS Ver. 11.0 was used for statistical analysis. The categories whose scores were evaluated as being high were Procurement', General Sanitation', Personal sanitation' and Waste' and the categories whose scores were evaluated as being low were Diet Order Manual', Standard Recipe', Appropriateness (Facility and Utility)', Check (Facility and Utility)'and Information Management'. All the categories of medical nutrition service were evaluated as having seriously low scores. Therefore, it was necessary for the contract-managed hospital foodservice to improve its performance in the area of medical nutrition care service. For the verification of the developed quality measurement tool, the reliability obtained by calculating Cronbach's α was 0.8747, and the content validity was also proved by scrutiny of the modification of the Professional group's techniques. 병원중심 가정간호 기관의 기록체계개발 - 미국 일개 종합병원을 대상으로 - (Development of Documentation System in Hospital-based Home Health - in one general hospital in the U.S.A. -)

  • The purposes of this study were 1) to assess the currunt documentation system 2) to identify the problems in communication regarding to documentation 3) to develop new documentation system 4) to suggest effective communication channel using new documentation system Research was conducted by direct observation, chart review, staffs interview and servey. Results were as follows: 1) nursing care plans were not used in ongoing care 2) documentation format was primarily narrative and charting was time consuming 3) documentation did not reflect the nursing process 4) patient records were not used as effective communication tool between case manager and part time nurse 5) difficult access to patient record for nurse manager created inefficiency in coordinating 6) documentation of patient education did not describe the precise contents of education, and the responses of the patients and evaluation To solve these problems, new documentation format was developed. With new formats nurses : 1) use standardized care plan which contains nursing diagnosis, ecpected outcome, time frame for evaluation, flow sheet for updating the plans 2) leave one copy of care plan at patient home for mutual agreement with patent and communication among nursing staffs 3) carry one copy of care plan for updating 4) document and evaluate the patient education using education check list keeping in patient's home 5) document nursing process in focus charting visit report 6) carry one copy of visit report 7) have one copy of visit report which was deligated to part time nurses 8) use documentation in direct communication with part time nurse 9) use beeper and memo to promote communication

재활전문간호사에게 기대되는 역할과 기능 (Roles and Functions of the Rehabilitational Nurse Practitioner Expected by Nurses and Doctors in Rehabilitation Hospital)

  • Purpose: This study aims to identify the role and function of the RNP(rehabilitational nurse practitioner) expected by nurses and doctors. Method: This study was a survey. The data were collected 188 nurses and 21 doctors who worked for disabled patients in the rehabilitation hospital during months of June, 2004 and August, 2005. Results: 98.4% of nurse and 61.9% of doctors agreed at opening of RNP course. The major role of RNP expected by nurses were educator, counsellor and case manager. The major role of RNP expected by doctors were direct care, self care promoter & exercise and emotional care. There was a significant difference about the need for opening of RNP course and major role and function of RNP between the group of nurses and doctors. Conclusion: The results of this study showed that the need for opening of RNP was identified and the major role of RNP was educator, counsellor, case manager and direct care. So there is a need for further research about major role of RNP related to various setting including rehabilitation hospital, nursing home, home care etc.
