During the past several years, the maze operation has become the most effective method of treatment for chronic atrial fibrillation. When the maze procedure is done concomittantly with other cardiac operations, surgeons, in their initial experiences, may be concerned about the additional operative risks and uncertainty of the results. We performed the Cox-maze III procedure in six cases of chronic atrial fibrillation associated with mitral, mitral & aortic, or coronary arterial disease. Maze III procedure was done with open mitral commissurotomy(3 cases), mitral valve replacement(1 case), aortic and mitral valve replacement(1 case), and two-vessel coronary bypass graft(1 case). In spite of rather prolonged aortic cross clamp time, cardiac recovery was uneventful in all cases. No cases required reexploration for postoperative bleeding. All patients showed regular sinus rhythms immediate or between 2 and 20 days postoperateratively. Transient postoperative supraventricular arrhythmarias were easily controlled by various antiarrhythmic agents. In follow up evaluations, all cases showed regular sinus rhythm on ECG and the right and left atrial transport function was confirmed by Doppler echocardiography in all except one. Though our experience was limited in case number, the Cox-maze III procedure was effective in controlling the chronic atrial fibrillation without serious additional operative risks.
Epicardial adipose tissue(EAT) is metabolically active endocrine organ that secretes several hormones in fat thickness is a risk factor for cardiovascular disease and metabolic disorders. This study was to measure and then using ultrasound epicardial adipose tissue thickness, abdominal subcutaneous fat thickness in the target group correlates and general blood properties and characteristics, and presents a local thickness for prediction of metabolic disorders. Results epicardal adipose tissue of the average thickness measured in each of the subjects was 8.890mm, 4.783mm, 4.777, 6.147mm in each section. Showed the epicardial adipose tissue in correlation with the average thickness of the risk factors age, BMI, SBP, LDH, LDL, TC is a positive correlation relationship(p<0.05) in each section. In particular, the thickness of the metabolic disorders epicardial adipose tissue thickness, abdominal subcutaneous compared to subjects that do not have the risk subjects with a risk factor for fat significantly higher(p<0.05). It showed the most reliable that can be cut-off value of 8.950mm obtained with 66.7 % sensitivity and 80 % specificity for predicting the risk of metabolic disorders.

It is well known that soy isoflavones play beneficial roles in the prevention of chronic diseases such as breast cancer, cardiovascular disease and osteoporosis. However current data are not sufficient for estimating the level of isoflavone intake. To use the urinary isoflavone excretion as a maker of isoflavone intake,26 participants consumed the isoflavone extract capsule (90 mg soy isoplavones/capsule) daily for 3 months. The study compared effects of pre- and post-isoflavone supplement in the following parameters; physical examination, dietary recalls, and urinary isoflavon excretion. The average age, height and weight were 65.7 years, 149.4 cm, and 57.3 kg. Subjects maintained regular diet pattern, and average daily nutrient intakes between pre- and post- supplementation were not significantly different except vitamin A, carotene and vitamin C. There was no significant difference between pre- and post- supplementation in terms of daily isoflavone intake. The basal urinary isoflavone excretion was 8.37 mg/day (daidzein 6.23 mu genistein 2.14 mg), and average daily excretion rate was IS.8%. Urinary isoflavone excretion was significantly increased, after isoflavone supplementation for 3 months as compared the basal level (p<0.01). Our data suggest that urinary isoflavone level, especially daidzein and genistein, may be a useful maker to estimate isoflavone intake.

The purpose of this study was to investigate the effect of forest exercise on blood pressure, heart rate, MDA, and SOD. Twelve volunteers were placed in 2 groups: Forest Exercise Group (FEG, n=6; $age=44.2{\pm}2.68$) and Indoor Exercise Group (IEG, n=6; $age=45.6{\pm}2.97$). The subjects who joined this investigation were asked to participate in forest exercise and/or indoor exercise. Blood pressure, heart rate, MDA, and SOD were collected at rest, just after the submaximal exercise, and 30 minutes after each exercise. For data analysis, mean and standard deviation scores were calculated, and ANCOVA were used. The results were as followings : 1) There was no difference between FEG and IEG on systolic blood pressure after submaximal exercise and at 30 minutes after each exercise., however FEG showed better stability than IEG on diastolic blood pressure at 30 minutes after forest exercise. 2) Both groups showed similar change of heart rate and MDA after submaximal exercise and at 30 minutes after each exercise. 3) Both groups similarly activated on the SOD level after submaximal exercise. However, compared to IEG, FEG showed significant decrease on SOD level at 30 minutes after forest exercise. Therefore, this study suggests that forest exercise may have good effects on blood pressure and MDA, and cardiovascular disease prevention.
Objectives The purpose of this study was to investigate the effectiveness of the Gongjin-dan (Gongchen-dan, here in after GJD) in order to obtain the evidence for clinical application. Methods The GJD-related articles published from 1990 to 2013 were searched using "Korean Traditional Knowledge Portal", "Oriental Medicine Advanced Searching Integrated System (OASIS)", "Korean Association of Medical Journal Edition (Koreamed)", "Research Information Services (RISS4U)", "Korean Medicine Database (KMbase)", "National Discovery for Science Leader (NDSL)", "PubMed", "China National Knowledge Infrastructure (CNKI)". The search keywords were "Gongjin-dan", "Gongchen-dan". Thirty-nine articles were obtained. After excluding the eighteen article which did not meet inclusion criteria, finally twenty-one articles were included; five clinical articles and sixteen experimental articles. Results In clinical studies, GJD has the various effectiveness in cardiovascular diseases, alcoholic hepatitis, mild dementia, anemia. Also experimental studies related to the GJD show a variety of effects, such as anti-oxidative activity, neuroprotective activity, hepatoprotective activity, anti-inflammatory activity, immunological activity, reproductive recovery activity with fewer side-effects. Conclusions It has been suggested that there are various effects of GJD in treating a wide-range disease. However, in order to put GJD to use for many kinds of diseases in more reasonable ways, it is needed to publish well-design clinical trial based on the variety of results of experimental studies.
This study was performed to investigate the effects of dry powders, ethanol extracts and juices of radish and onion on lipid metabolism, lipid peroxidation and antioxidative enzyme activity in rats. Forty-nine male Sprague-Dawley rats weighing 157$\pm$6g were blocked into seven groups according to body weight and raised for four weeks with diets containing 5%(w/w) dry powders of two different vegetables consumed frequently by Korean-radish(Raphanus sativus L.) and onion(Allium cepa L.), ethanol extracts and juices from equal amount of each dry powder. All the powders, ethanol extracts and juices of radish and onion decreased total lipids, triglycerides and total cholesterol concentrations in plasma and liver. Above all, onion ethanol extract decreased them most remarkably. It was thought that organosulfur compounds and flavonoids extracted from onion by ethanol inhibited biosynthesis and absorption of lipid and promoted degradation of lipid. Radish powder also decreased them by increasing fecal excretions of total lipids, triglycerides and total cholesterol most effectively. Catalase and glutathine peroxidase(GSH-px) activities in red blood cell(RBC) were most remarkably increased by radish powder and onion powder respectively. Superoxide dismutase(SOD), catalase and GSH-px activities in liver were most remarkably increased by onion ethanol extract, radish powder and onion ethanol extract respectively. Xanthine oxidase(XOD) activities in liver were most effectively decreased by ethanol extracts of radish and onion. Thiobarbituric acid reactive substance (TBARS) levels in plasma and liver of experimental groups were significantly lower than those of controls. Above all, onion powder decreased them most effectively. It was thought that vitamin E and high flavonoids in onion powder inhibited lipid peroxidation, promoting liver and RBC SOD, catalase and GSH-px activities and inhibiting XOD activities effectively. Flavonoids in onion ethanol extract inhibited lipid peroxidation, promoting three antioxident enzyme activities and inhibiting XOD activities most remarkably. Also flavonoids and high vitamin C in radish powder inhibited lipid peroxidation, promoting liver and RBC catalase most remarkably and inhibiting XOD activities. In conclusion, radish and onion were effective in lowering lipid levels and inhibiting of lipid peroxidation in animal tissue. From these data, radish and onion can be recommended in the treatment and prevention of diseases such as cardiovascular disease and cancer and in delaying aging. As ethanol from onion were most effective in lowering lipid level and promoting three antioxident enzymes, and inhibited lipid peroxidation as did we should try to utilize onion skin which is discarded though reported to have abundant flavonoids. Cudrania tricuspidata has been reported to have many biological activities, including anti-inflammatory, anti-cancer, and antioxidant properties. However, the effects of C. tricuspidata root extract (CTE) on human platelet aggregation induced by collagen as well as the signaling pathways involved remain unknown. In the present study, we investigated the effect of CTE on human platelets. CTE inhibited platelet aggregation via down-regulation of thromboxane A2 (TXA2) by blocking cyclooxygenase-1 (COX-1) activity and intracellular Ca2+ mobilization in collagen-induced platelets. CTE also reduced the phosphorylation of phospholipase C (PLC) γ2 and syk. CTE regulated platelet aggregation via cyclic guanosine monophosphate (cGMP)-dependent phosphorylation of vasodilator-stimulated phosphoprotein (VASP) Ser239. In addition, administration of CTE (50 and 100 mg/kg) significantly reduced hyper-aggregated platelet aggregation by collagen (5 ㎍/mL) without hepatotoxicity in HFD (high fat diet)-fed rats. Taken together, these results suggest that CTE has anti-platelet effects both in vitro and in vivo. Therefore, CTE may be an effective therapeutic and preventive agent for cardiovascular disease, and is a safe and natural product.

Reactive Oxygen Species(ROS) are continuously produced at a high rate as a by-product of aerobic metabolism. Since tissue damage by free radical increases with age, the reactive oxygen species(ROS) such as hydrogen peroxide($H_2O_2$), nitric oxide(NO). Several lines of evidence provided that ROS appears to cause to develop aging-related various diseases such as cancer, arthritis, cardiovascular disease. In this study, we have conducted to investigate the biological activities of Hominis Placenta Herbal Acupuncture by measuring total polyphenol content, DPPH radical scavenging, ABTS radical scavenging, Superoxide dismutase(SOD)-like activity, Nitrite scavenging ability in vitro. The total polyphenol contents of Hominis Placenta Herbal Acupuncture was $24.6m{\ell}/m{\ell}$. Elctron donation ability on DPPH was 49.4%. The 2,2'-azinobis-3-ehtlbezothiazoline-6-sulfonic acid radical decolorization (ABTS) was 50.01%, similar 10 the DPPH free radical scavenging. The superoxide dismutase (SOD)-like activities of hominis placenta herbal acupuncture was 50.876%. The nitrite scavenging abilities at pH 1.5, pH 3.0, pH 6.0 were 52.8%, 29.4%, 15.4%, respectively; these abilities decreased as pH increased. We conclude that Hominis Placenta Herbal Acupuncture may be useful as potential sources of antioxidant.
There is some evidence that soy isoflavone has beneficial effects on the concentration of blood lipids. In this study, we investigated habitual isoflavone intake levels and the relation among dietary isoflavone intake, blood pressure, and blood lipids of adult men (n = 149) and women (n = 205). Anthropometric measurements including blood pressure, dietary in-take assessment using 24-hour recall method, and biochemical assessment using blood were conducted. The average age, height, weight, and BMI were 54.7 years, 168.5 cm, 67.3 kg, and 24.5 $kg/m^2$ for men and 53.9 years, 153.8 cm, 59.2 kg, and 25.0 $kg/m^2$ for women, respectively. The mean daily intakes of total food and energy were 1219.1 g and 1740.9 kcal for men and 1071.3 g and 1432.6 kcal for women, respectively. The mean daily isoflavones (daidzein + genistein) intake of men and women were 20.0 mg and 14.2 mg, respectively. Blood pressure of the subjects was 128.3/75.5 mmHg for men and 124.1/73.7 mmHg for women. Serum lipids of men and women were 180.2 and 184.9 mg/dL for total cholesterol, 160.8 and 137.6 mg/dL for triglyceride, 41.5 and 44.7 mg/dL for HDL-cholesterol, 106.6 and 112.7 mg/dL for LDL-cholesterol, and 3.5 and 3.3 for atherogenic index, respectively. Serum triglyceride and atherogenic index of men were significantly higher than those of women. In men, isoflavone intake and the level of total serum cholesterol were negatively (p < 0.05) correlated, after adjusted age. In women, there were significant correlations between isoflavone intake and blood pressure (systolic and diastolic), after adjusted age (p < 0.05). Based on these results, we concluded that higher isoflavone intake seemed to be related to a better lipid profile in men and lower blood pressure in women. But more epidemiological studies and controlled clinical trials would help to confirm the optimal amount required for the prevention and treatment of cardiovascular disease.
Purpose: Long-term survivors of childhood cancer appear to have an increased risk for the metabolic syndrome, subsequent type 2 diabetes and cardiovascular disease in adulthood compared to healthy children. The purpose of this study was to investigate the frequency of the metabolic syndrome and associated factors in childhood cancer survivors at a single center in Korea. Methods: We performed a retrospective review of medical records of 98 childhood cancer survivors who were diagnosed and completed anticancer treatment at Samsung Medical Center, Seoul, Korea between Jan. 1996 and Dec. 2007. Parameters of metabolic syndrome were evaluated between Jan. 2008 and Dec. 2009. Clinical and biochemical findings including body fat percentage were analyzed. Results: A total of 19 (19.4%) patients had the metabolic syndrome. The median body fat percentage was 31.5%. The body mass index and waist circumference were positively correlated with the cranial irradiation dose (r=0.38, P<0.001 and r=0.44, P<0.00, respectively). Sixty-one (62.2%) patients had at least one abnormal lipid value. The triglyceride showed significant positive correlation with the body fat percentage (r=0.26, P=0.03). The high density lipoprotein cholesterol showed significant negative correlation with the percent body fat (r=- 0.26, P=0.03). Conclusion: Childhood cancer survivors should have thorough metabolic evaluation including measurement of body fat percentage even if they are not obese. A better understanding of the determinants of the metabolic syndrome during adolescence might provide preventive interventions for improving health outcomes in adulthood.
