Endodontic treatment of a C-shaped mandibular second premolar with four root canals and three apical foramina: a case report

  • This case report describes a unique C-shaped mandibular second premolar with four canals and three apical foramina and its endodontic management with the aid of cone-beam computer tomography (CBCT). C-shaped root canal morphology with four canals was identified under a dental operating microscope. A CBCT scan was taken to evaluate the aberrant root canal anatomy and devise a better instrumentation strategy based on the anatomy. All canals were instrumented to have a 0.05 taper using 1.0 mm step-back filing with appropriate apical sizes determined from the CBCT scan images and filled using a warm vertical compaction technique. A C-shaped mandibular second premolar with multiple canals is an anatomically rare case for clinicians, yet its endodontic treatment may require a careful instrumentation strategy due to the difficulty in disinfecting the canals in the thin root area without compromising the root structure.

  • Measuring of Water Losses in irrigation canals have been done in 4 irrigation area. Four concrete canals and nine earth canals have been measured by the inflow-outflow method. The results obtained in this experiment were that the loss rates in the main irrigation canal and the secondary irrigation canal were 11.54%, 19.29% respectively. And the loss rates in concrete irrigation canals were 4.77%.

호제근충재(糊劑根充材) Vitapex의 근관폐쇄성(根管閉鎖性)에 관(關)한 연구(硏究) (A STUDY ON THE USE OF VITAPEX WITH GUTTA-PERCHA CONES AS A ROOT CANAL FILLER)

  • The purpose of this study was to examine the sealing ability of the vitapex, when used with gutta-percha cone, as a root canal filling material. Fourty five canals from extracted human maxillary and mandibular teeth were randomly selected and instrumented in a conventional method with k-file. After instrumentation and dry the canal with paper points, the canals were divided into three groups and fifteen canals in each group were filled with the following materials; Vitapex, Vitapex in combination with gutta-percha cone, and Gutta-percha cone and Zinc-oxide Eugenol Cement. All the specimens were immersed in 2% methylenblue dye solution and the depth of dye penetration into the canals were evaluated by macroscope at the intervals of 1 day, 2days and 7days. The following results were obtained; 1. All the materials experimented showed varying degrees of dye penetration. 2. The canals filled with Vitapex and Vitapex in combination with gutta perch a cone revealed sudden increase of dye penetration with time passage compared to the canals obturated with Gutta-percha cone and Zinc-oxide eugenol cement. 3. In the canals filled with Vitapex, the mean dye penetration was 1.6mm at 1day, but the specimen exposed to the dye for 7days showed mean dye penetration of 9.2mm. 4. In the canals obturated with Vitapex and gutta-percha cone, the mean dye penetration was 2mm at 1day, 2.2mm at 2days, and 8mm at 7days.

The effect of root canal preparation on the surface roughness of WaveOne and WaveOne Gold files: atomic force microscopy study

  • Objectives: To examine the surface topography of intact WaveOne (WO; Dentsply Sirona Endodontics) and WaveOne Gold (WOG; Dentsply Sirona Endodontics) nickel-titanium rotary files and to evaluate the presence of alterations to the surface topography after root canal preparations of severely curved root canals in molar teeth. Materials and Methods: Forty-eight severely curved canals of extracted molar teeth were divided into 2 groups (n = 24/each group). In group 1, the canals were prepared using WO and in group 2, the canals were prepared using WOG files. After the preparation of 3 root canals, instruments were subjected to atomic force microscopy analysis. Average roughness and root mean square values were chosen to investigate the surface features of endodontic files. The data was analyzed using one-way analysis of variance and post hoc Tamhane's tests at 5% significant level. Results: The surface roughness values of WO and WOG files significantly changed after use in root canals (p < 0.05). The used WOG files exhibited higher surface roughness change when compared with the used WO files (p < 0.05). Conclusions: Using WO and WOG Primary files in 3 root canals affected the surface topography of the files. After being used in root canals, the WOG files showed a higher level of surface porosity value than the WO files.

  • The purpose of this study is to compare the shape of the apical regions of root canals after instrumentation by various enlarging instruments. 120 extracted, single-rooted human teeth were seperated into 4 experimental groups. Each group provided 30 teeth for experimental use. Group 1 root canals instrumented by the hand-operated Reamer. Group 2 root canals instrumented by the hand-operated H-file. Group 3 root canals instrumented by the hand-operated K-file. Group 4 root canals instrumented by the automated Giromatic file. The results were as follows: 1. The degree of the roundness of the apical regions of root canal walls: 1) Hand-operated instruments were superior to the automated Giromatic file. 2) The Reamer was the most effective instrument among hand-operated instruments 3) There was little difference between the H-file and the K-file. 2. The degree of the smoothness of the apical regions of root canal walls: 1) Root canal walls instrumented by the hand-operated Reamer, H-file and, K-file were generally smooth, and there was little difference among them. 2) Root canal walls instrumented by the automated Giromatic file showed many irregular canal walls. 3. The existance of organic debris in the apical regions of root canals: All organic debris is not removed from root canals, and there was little difference between hand-operated instruments and the automated Giromatic instrument in removing organic debris.

Isthmuses, accessory canals, and the direction of root curvature in permanent mandibular first molars: an in vivo computed tomography study

  • Objectives: This study was performed to assess the anatomy of mandibular first molars. Materials and Methods: In this in vivo study, cone-beam computed tomography (CBCT) volumes of 312 bilateral intact first mandibular molars from 156 patients (79 men and 77 women; average age, 35.6 ± 11.2 years) were investigated in terms of the direction of each canal's curvature in the buccolingual and mesiodistal dimensions (direction of the position of the apex in relation to the longitudinal axis of the root), the presence of an isthmus (a narrow, ribbon-shaped communication between 2 root canals) in 3 segments (0-2, 2-4, and 4-6 mm) from the apex), and the presence and number of accessory canals (smaller canals besides the main root canals, connecting the pulp to the periodontium). Data were analyzed statistically (α = 0.05). Results: Mesiolingual canals were mostly buccally and distally inclined, while mesiobuccal and distolingual canals were mostly distally curved. Isthmuses were more common in younger patients (χ2 test, p < 0.05). The average numbers of accessory canals in the apical, middle, and coronal segments were 9.9 ± 4.2, 6.9 ± 2.9, and 9.3 ± 3.0 canals per segment, respectively (analysis of variance, p < 0.001). Age and sex were not associated with the number of accessory canals (p > 0.05). Conclusions: The complex anatomy of these teeth deserves attention during non-surgical or surgical endodontic treatment. Around the apex, isthmuses might be more prevalent in younger and female individuals.

  • The author observed thy actual position of apical foramen and the radiographic appearance of files when the files were filled through canals to the external surface of apical foramen in 280 canals of extracted teeth. All the teeth were radiographed by bisecting technic and once again Walton's projection was employed on mandibular molars. The results were as followings. 1. Sixty five percents of 280 canals were actually classed as having foramina deviant from true apex of root. 2. 160 of 280 canals(Fifty seven percents) appeared to be filled short of apex on the radiograph. 3. When Walton's projection was employed to open up two mesial canals of mandibular molars and compared to straight-on projection, twelve of 120 canals Ten percent appeared to be different in radiographic appearence.

  • The relationship of cartilage canals to initial osteogenesis of primary ossification center of developing vertebrae in human fetuses ranging from 50mm to 260mm in crown rump length was studied by light and electron microscopy. The cartiage canals of the thoracic vertebrae were first observed at 60mm fetus. Cartilage canals were identified as vascular channels arising from perichondrium surfaces. A number of cartilage canals were observed around the primary center of ossification at 80mm fetus. At 120mm fetus, cartilage canals of the bodies of vertebra were increased. Eventually the canals were eroded from the main medullary cavity and remained at only peripheral regions of growth cartilage. Superficial, intermediate, and deep canals were identified by the characteristics of cartilage cells. Fibroblasts, undifferentiated mesenchymal cells, and vacuolated macrophages were observed adjacent to the matrix of resting cartilage cells in the superficial canal. Fibroblasts and mesenchymal cells were densely packed at the tip of canal, giving an epithelial appearance to the clustered cell in the intermediate canal. Vacuolated macrophages were in contact with matrix of hypertrophied cartilage. The thick-walled vessels in the intermediate and deep canals consisted of typical endothelial cells, but in the newly formed vessels contained mesenchymal cells and fibroblasts incorporated into the vessel wall. During lengthening of cartilage canal, the matrix of cartilage cells were invaded by newly formed capillaries and vacuolated macrophages. At the deep canal, the lateral wall of the canal terminated in matrix containing calcified cartilage. The mesenchymal cells began to differentiate into osteoblasts adjacent to the calcified matrix. The results indicate that the connective tissue cells within the cartilage canals proliferate and differentiate into osteoblasts at the site of primary ossification center.

미노반규관(迷路半規管)과 외안사근(外眼斜筋)의 기능적(機能的) 관계(關係)에 관(關)하여 (Functional Relationship between the Vestibular Canals and the Extraocular Oblique Muscles)

  • This experiment was designed to explore specific functional relationship between the vestibular canals and the extraocular oblique muscles by observing the isometric tension responeses of the muscles to the selected vestibular canal excitation. The vestibular excitation was simulated by either stimulation of the individual canal nerve or endolymphatic fluid displacement in each canal. Each canal nerve was subjected to square wave pulses with a monopolar wire electrode placed closely to the ampullary nerve endings for electrical stimulation, and a fine stainless cannula was introduced into the each canal toward the ampulla and a minute amount $(0.5{\sim}3.5\;microliter)$ of fluid was injected in or ejected out by means of a microsyringe connected to the cannula to produce ampullopetal or ampullofugal displacement of endolymphatic fluid. The superior oblique muscle was contracted by the excitation of homolateral canals and was relaxed by contralateral canals. On the contrary, the inferior oblique was contracted by the contralateral canals and was relaxed by the homolateral canals. Summation of excitatory and inhibitory canal effects from the bilateral vestibular system was demonstrable on the tension changes of the oblique muscles. Excitation of either dual or triple canals of the unilateral vestibular system also caused summation effect on the tension response of the oblique pair; thus multiple signals from the different ampullary receptors seems to be converged into the relevant ocular motor muclei. Since the superior and inferior obliques are known to receive their motor fibers from the contralateral trochlear nuclei and intermediate nuclei of the homolateral oculomotor complex respectively, the above experimental evidences indicate that the ocular motor nuclei for oblique muscles receive excitatory signals from the contralateral vestibular canals and inhibitory signals from the homolateral canals.

Influence of operator's experience level on lifespan of the WaveOne Primary file in extracted teeth

  • Objectives: The aim of this study was to assess the influence of operator experience level on the lifespan of the WaveOne Primary file (Dentsply Maillefer, Ballaigues, Switzerland) in extracted teeth. Materials and Methods: Moderately curved canals of extracted maxillary and mandibular molars were randomly distributed into 2 groups: experienced and inexperienced operators. Ten files were allocated to each group (n = 10). Each canal was prepared until the working length was reached, and the same file was used to prepare additional canals until it separated. The number of canals prepared before file separation was recorded. The fragment length of each file was measured, and the location of the fragment in the canal was determined. Data were statistically analysed using the independent 2-sample t-test. Results: The 2 operators prepared a total of 324 moderately curved canals of maxillary and mandibular molars. There was no significant intergroup difference in the mean number of canals prepared (p = 0.27). The average lifespan of the WaveOne Primary file was 17.1 and 15.3 canals, and the longest lifespan was 25 and 20 canals, when used by experienced and inexperienced operators, respectively. There were no statistically significant intergroup differences in separated fragment length and location. Conclusions: Within the limitations of this study, operator experience level appears to have no effect on the lifespan of the WaveOne Primary file in preparation of moderately curved canals. Single teeth with multiple canals can be prepared safely even by a novice operator by using a single file.
