위암 진단에 있어서의 CT 위장 조영술과 상부위장관 조영술과의 비교 (Usefullness of CT Gastrography and Vurtual Gastroscopy using Computed Tomography in Detection of Gastric Cancer)

  • Purpose: Advancement of computed tomography (CT) hardware and software has allowed thin section scanning and reconstruction of fascinating 2-dimentional (2D) and 3- dimentional (3D) images. Especially, the reconstruction of 3D images of gastrointestinal tract has been used in the detection and diagnosis of pre-malignant and malignant diseases. To compare the efficacy of CT gastrography with conventional upper gastrointenstinal series (UGIs) in gastric cancer patients. Materials and Methods: During Nov. 2002 and Mar. 2003, twenty-seven patients who had gastric cancer received both double contrast upper GI series and CT gastrography prior to radical surgery. Among these patients, nineteen had early gastric cancer (EGC) and 8 had advanced gastric cancer (AGC). Fifteen patients were male and 12 were female. The mean age was 54 yrs (range, $27\∼75$ yrs). The patients were placed on NPO and Stomach was distended with gas in fasting state prior to CT scanning. Double contrast upper GI series were performed as routine manual. CT scan was conducted in all patients using 8 or 16-channel multidetector CT in this study. The collimation and reconstruction for CT scanning were set at 2.5 mm and 1.25 mm, respectively. CT scanning was performed in the supine position. For image processing, CT gastrography, in which raysum and surface rendering images were constructed, virtual and 2D image in coronal and sagittal images were performed. The detectability of gastric cancer was assessed between UGIs and CT gastrography. Results: In AGCs, the detection rate of cancer using CT gastrography and virtual gastroscopy was higher than EGC cases. However, CT gastrography and virtual gastroscopy showed less favorable results than UGIs. Even though only a small number of cases had been studied, we might conclude that CT gastrography and virtual gastroscopy could replace UGIs in the detection of AGC cases. Conclusion: The detection rate used with CT gastrography and Virtual gastroscopy is not better than that of UGIs in early gastric cancer, however, in advanced gastric cancer cases, it is nearly equal to that of UGIs.

성인과 소아 CT 촬영시 IR 적용에 따른 영상화질 및 선량에 미치는 영향 (Effect of Image quality and Radiation Dose using Iterative Reconstruction in Adult and Pediatric CT: A Phantom Study)

  • The main issue of CT is radiation dose reduction to patient. The purpose of this study was to estimate the image quality and dose by iterative reconstruction (IR) for adults and pediatrics. Adult and pediatric images of phantom were obtained with 120 and 140 kV, respectively, in accordance with radiation dose in terms of volume CT dose index ($CTDI_{vol}$): 10, 15, 20, 25, 30, 35 mGy. Then, the adult and the pediatric images are reconstructed by filtered-backprojection (FBP) and iterative reconstruction (IR). The images were analyzed by signal-to-noise ratio (SNR). SNR is improved when IR and 140 kV are applied to acquire adult and pediatric images. In the adult abdomen, according to diagnostic reference level, the SNR values of bone were increased about 27.84 % and 27.77 % at 120 kV and 140 kV, and the tissue's SNR values of the IR were increased about 29.84 % and 33.46 % 120 and 140 kV, respectively. Dose is reduced to 40% in adults abdomen images when using IR reconstruction. In pediatric images, the bone's SNR were also increased about 17.70% and 18.17 % at 120 kV and 140 kV. The tissue's SNR were increased about 26.73 % and 26.15 % at 120 kV and 140 kV. Radiation dose is reduced from 30% to 50% for bone and tissue images. In the case of examinations for adult and pediatric CT, IR technique reduces radiation dose to patient, and it could be applied to adult and pediatric imaging.

CT 영상에서 뼈의 불균질 모델 생성을 위한 B-스플라인 볼륨의 빠른 보간 방법 (Method of Fast Interpolation of B-Spline Volumes for Reconstructing the Heterogeneous Model of Bones from CT Images)

  • 복잡한 불균질성을 가지는 뼈의 경우 물성치 분포를 B-스플라인 볼륨 함수를 이용해 표현하는 것이 적합하다고 알려져 있다. 물성치 분포에 대한 B-스플라인 볼륨 함수는 CT 영상의 각 화소값들을 보간하여 얻을 수 있다. 그러나 뼈의 CT 영상 데이터는 3차원이며 크기가 매우 크기 때문에 보간 시간이 오래 걸리며, 많은 컴퓨터 메모리가 필요하다. 본 연구에서는 이를 해결하기 위한 방법을 제안한다. 제안하는 방법에서는 영상이 가지는 화소 간격의 균일성 및 B-스플라인 기저 함수의 특징을 이용해 B-스플라인 볼륨 보간 문제를 단순화 시킨다. 이는 결과적으로 계산 시간 및 메모리 사용량을 줄여준다. 검증을 위해, 제안한 방법을 컴퓨터 프로그램으로 구현하였으며, 실험을 통해 계산 시간이 줄어든 것을 확인하였다.

지역적 가중치 거리맵을 이용한 3차원 영상 정합 (Three-Dimensional Image Registration using a Locally Weighted-3D Distance Map)

  • 본 논문에서는 동일 환자에 대해 시간차를 두고 촬영한 뇌 CT-CT 혈관조영영상간 움직임을 보정하기 위한 강인하고 고속의 정합방법을 제안한다. 먼저, 두 영상에서 3차원 경계검출 기법을 이용하여 특징점을 추출하고, 기준영상에서는 이를 지역적 가중치 3차원 거리맵으로 변환한다. 부유영상을 기준영상으로 강체변환하면서 두 경계간의 상관관계가 최대인 위치를 탐색한다. 이 때, 최대위치가 더 이상 변화하지 않고 일정 이상 반복되면 해당위치를 최적위치로 하여 부유영상을 최적위치로 변환시켜 두 영상을 정합한다. 실험을 위하여 인공영상을 사용하여 정화성과 강인성을 평가하였고, 육안평가를 위하여 뇌 CT-CT 혈관조영영상을 사용하였다. 본 제안방법은 지역적 가중치 3차원 거리맵을 이용함으로써 적은 샘플링 개수에도 국부최대인 위치에 수렴하지 않고 최적위치로 강인하면서 고속으로 영상이 정합되었다

  • Positron emission tomography-computed tomography (PET/CT) provides both functional and anatomical images of high quality non-invasively with better precision in localization than PET alone. Increase in the use of PET/CT, coupled with increasing concerns about the quality of medical services accrued the demands for accurate evaluation of system performance and quality assurance. Thus, well designed programs for performance evaluation and quality assurance are needed. Widely used protocols for performance evaluation of PET are the methods proposed by National Electrical Manufacturers Association (NEMA) in 1994 and 2001. In addition, in order to maintain high quality of PET/CT images, quality assurance programs including periodic (daily, monthly, and yearly). Therefore, in this article, the methods and present state of performance evaluation and quality assurance of PET/CT are reviewed.

금속 인공물 감소를 위한 CT 알고리즘 적용에 따른 영상 화질 비교 (Comparison of Image Quality among Different Computed Tomography Algorithms for Metal Artifact Reduction)

  • 본 연구는 CT 촬영 시 금속으로 인해 발생한 금속 인공물 감소를 위한 알고리즘 적용에 따른 영상 화질에 대한 정량적 비교를 하고자 한다. Spectral detected-based CT와 CT ACR 464 팬톰을 이용하여 일반적인 필터보정역투영 알고리즘을 적용한 기준 영상을 10장 획득하고, 동일 팬톰에 금속 인공물을 발생시켜 일반적인 필터보정역투영 알고리즘을 적용한 영상을 10장 획득하였다. 금속 인공물을 발생시켜 획득한 영상의 원시 데이터에 metal artifact reduction 알고리즘, 가상 단일 에너지 알고리즘, metal artifact reduction 알고리즘 적용 후 추가로 가상 단일 에너지 알고리즘을 적용한 영상을 각각 10장씩 획득하였다. 알고리즘 적용에 따른 hounsfield unit 비교를 위해 CT ACR 464 팬톰 module 1에 위치한 폴리에틸렌, 뼈, 아크릴, 공기, 물에 관심영역을 설정하고, 전체 영상 화질 평가를 위해 평균 제곱근 오차, 평균 절대 오차, 신호 대 잡음비, 최대 신호 대 잡음비, 구조적 유사도 지수 지표를 통해 알고리즘 별 비교하였다. 알고리즘 적용 영상 별 hounsfield unit 비교 결과 알고리즘 적용 영상 간 유의한 차이를 보였으며(p < .05), 아크릴을 제외한 관심영역에서 가상 단일 에너지 알고리즘 적용 영상에서 큰 변화를 나타냈다. 영상 화질 평가 지표 결과 metal artifact reduction 알고리즘 적용 영상 화질이 가장 높았으나, 구조적 유사도 지수는 metal artifact reduction 알고리즘 적용 후 추가로 가상 단일 에너지 알고리즘이 동시에 적용된 영상이 가장 높았다. CT 촬영 시 금속 인공물 감소에 metal artifact reduction 알고리즘이 가상 단일 에너지 알고리즘에 비해 효과적이었지만, 양질의 CT 영상 획득을 위해 알고리즘 적용에 따른 이점과 영상 화질 변화를 파악하고 효율적인 활용이 필요하다고 사료된다.

Detection of Deep Vein Thrombosis by Follow-up Indirect Computed Tomography Venography after Pulmonary Embolism

  • Background: Information regarding the incidence and risk factors for deep vein thrombosis (DVT) detected by follow-up computed tomographic (CT) venography after pulmonary embolism (PE) is sparse. The aim of the present study was to identify the predictors of DVT in follow-up CT images, and to elucidate their clinical significance. Methods: Patients with PE were classified into the following three cohorts based on the time of indirect CT venography follow-up: within 1 month, 1 to 3 months, and 3 to 9 months after the initial CT scan. Each cohort was subdivided into patients with or without DVT detected by follow-up CT. Clinical variables were compared between the two groups. Results: Follow-up CT revealed DVT in 61% of patients with PE within 1 month, in 15% of patients with PE at 1 to 3 months, and in 9% of patients with PE at 3 to 9 months after the initial CT scan. Right ventricular (RV) dilation on the initial CT (odds ratio [OR], 8.30; 95% confidence interval [CI], 1.89-36.40; p=0.005) and proximal DVT at the initial presentation (OR, 6.93; 95% CI, 1.90-25.20; p=0.003) were found to independently predict DVT in follow-up CT images within 1 month, proximal DVT at the initial presentation was found to independently predict DVT in follow-up CT images at 1 to 3 months (OR, 6.69; 95% CI, 1.53-29.23; p=0.012), and central PE was found to independently predict DVT in follow-up CT images at 3 to 9 months (OR, 4.25; 95% CI, 1.22-4.83; p=0.023) after the initial CT scan. Furthermore, the detection of DVT by follow-up CT independently predicted the recurrence of venous thromboembolism (VTE) (OR, 4.67; 95% CI, 2.24-9.74; p<0.001). Conclusion: Three months after PE, DVT was not detected by follow-up CT in most patients with PE. RV dilation on the initial CT, central PE, and proximal DVT at the initial presentation were found to predict DVT on follow-up CT, which might predict VTE recurrence.

3차원 다중 치과 CT 영상의 고화질 스티칭 기법 (High-quality Stitching Method of 3D Multiple Dental CT Images)

  • In this paper, we propose a high-quality stitching method of 3D multiple dental CT images. First, a weighted function is generated using the difference of two distance functions that calculate a distance from the nearest edge of an overlapped region to each position. And a blending ratio propagation function for two gradient vectors is parameterized by the difference and magnitude of gradient vectors that is also applied by the weighted function. When the blending ratio is propagated, an improved region growing scheme is proposed to decide the next position and calculate the blending intensity. The proposed method produces a high-quality stitching image. Our method removes the seam artifact caused by the mean intensity difference between images and vignetting effect. And it removes double edges caused by local misalignment. Experimental results showed that the proposed method produced high-quality stitching images for ten patients. Our stitching method could be usefully applied into the stitching of 3D or 2D multiple images.

특징 추출을 이용한 다중 영상 정합 및 융합 연구 (Multimodality Image Registration and Fusion using Feature Extraction)

  • 본 논문에서는 소동물 생체내 실험시 서로 다른 장비에서 획득된 영상의 융합 및 정합을 위한 방법을 제안한다. 마우스의 꼬리 정맥에 $[[^{18}F]FDG$를 주사하여 60분 섭취후 서로 다른 장비에서 동일한 위치의 영상을 획득하기 위하여 아크릴 재질의 소동물 가이드에 기준마크를 설정하고 microPET과 CT 영상을 획득하였다. MicroPET으로 획득된 리스트모드(list-mode) 데이터는 Fourier Rebinning(FRB) 방법을 사용하여 사이노그램(Sinogram)으로 변환 후 4 번의 반복횟수를 가지는 Ordered Subset Expectation Maximization(OSEM) 알고리즘으로 재구성하였다. MicroPET 영상획득후 PET/CT의 CT를 이용하여 CT영상을 획득하였다. MicroPET 영상에서 폐영역을 정확히 찾아내는 어려움이 있어. 해부학적 정보를 제공하는 CT 영상을 이용하여 폐 영역을 구분하였다. 영상 융합을 위한 불일치 부분을 해결하기 위하여 기준마크의 정보와 폐 영역의 정보를 이용하여 회전과 이동정보를 가지는 어파인 (affine) 변환 행렬 구하여 영상 정합에 사용하였다. 이 방법은 정량적 정확성과 영상 해석의 정확성을 개선할 것으로 기대된다.

PET/CT 영상 움직임 보정 (Motion Correction in PET/CT Images)

  • PET/CT fused image with anatomical and functional information have improved medical diagnosis and interpretation. This fusion has resulted in more precise localization and characterization of sites of radio-tracer uptake. However, a motion during whole-body imaging has been recognized as a source of image quality degradation and reduced the quantitative accuracy of PET/CT study. The respiratory motion problem is more challenging in combined PET/CT imaging. In combined PET/CT, CT is used to localize tumors and to correct for attenuation in the PET images. An accurate spatial registration of PET and CT image sets is a prerequisite for accurate diagnosis and SUV measurement. Correcting for the spatial mismatch caused by motion represents a particular challenge for the requisite registration accuracy as a result of differences in PET/CT image. This paper provides a brief summary of the materials and methods involved in multiple investigations of the correction for respiratory motion in PET/CT imaging, with the goal of improving image quality and quantitative accuracy.
