Design and Development of an EHR Platform Based on Medical Informatics Standards (의료정보 표준에 기반한 EHR 플랫폼의 설계 및 개발)

  • As the ARRA enacted recently in the United States, the interest in EHR systems have been increased in the field of medical industry. The passage of the ARRA presents a program that provides incentives to office-based physicians and hospitals adapting the EHR systems to guarantee interoperability with various medical standards. Thanks to the incentive program, a great number of EHR systems have been developed and lots of office-based physicians and hospitals have adapted the EHR systems certified by CCHIT. Keeping pace with the rapid changes in the market of healthcare, some enterprises try to push in to the United States healthcare market based on the experience acquired by developing EHR systems for hospitals in Korea. However, the developed system must be customized because of the different medical environment between Korea and the United States. In this paper, therefore, we design and develop an integrated EHR platform to guarantee the interoperability between different medical information systems based on medical standard technologies. In the developed platform, an integrated system has been composed by integrating various basic techniques such as data transmission standards and its methods, medical standard terminologies and its usage, and knowledge management for medical decision-making support. Moreover, medical data can be processed electronically by adapting an HL7 interface engine and the terminologies for exchanging medical information and the standardization of medical information. We develop SeniCare, an EHR system for supporting ambulatory care of the office-based physicians, based on the platform, and we verify the usability of the platform by confirming whether SeniCare satisfies the criteria of "meaningful use" issued by CMS or not.

Clean Room Structure, Air Conditioning and Contamination Control Systems in the Semiconductor Fabrication Process (반도체 웨이퍼 제조공정 클린룸 구조, 공기조화 및 오염제어시스템)

  • Objectives: The purpose of this study was to examine clean room(C/R) structure, air conditioning and contamination control systems and to provide basic information for identifying a correlation between the semiconductor work environment and workers' disease. Methods: This study was conducted at 200 mm and 300 mm semiconductor wafer fabrication facilities. The C/R structure and air conditioning method were investigated using basic engineering data from documentation for C/R construction. Furthermore, contamination parameters such as airborne particles, temperature, humidity, acids, ammonia, organic compounds, and vibration in the C/R were based on the International Technology Roadmap for Semiconductors(ITRS). The properties of contamination control systems and the current status of monitoring of various contaminants in the C/R were investigated. Results: 200 mm and 300 mm wafer fabrication facilities were divided into fab(C/R) and sub fab(Plenum), and fab, clean sub fab and facility sub fab, respectively. Fresh air(FA) is supplied in the plenum or clean sub fab by the outdoor air handling unit system which purifies outdoor air. FA supply or contaminated indoor air ventilation rates in the 200 mm and 300 mm wafer fabrication facilities are approximately 10-25%. Furthermore, semiconductor clean rooms strictly controlled airborne particles(${\leq}1,000{\sharp}/ft^3$), temperature($23{\pm}0.5^{\circ}C$), humidity($45{\pm}5%$), air velocity(0.4 m/s), air change(60-80 cycles/hr), vibration(${\leq}1cm/s^2$), and differential pressure(atmospheric pressure$+1.0-2.5mmH_2O$) through air handling and contamination control systems. In addition, acids, alkali and ozone are managed at less than internal criteria by chemical filters. Conclusions: Semiconductor clean rooms can be a pleasant environment for workers as well as semiconductor devices. However, based on the precautionary principle, it may be necessary to continuously improve semiconductor processes and the work environment.

Effects of Load Center of Gravity and Feet Positions on Peak EMG Amplitude at Low Back Muscles While Lifting Heavy Materials (중량물 들기 작업시 물체 무게중심 및 발의 위치가 허리 근육의 최대 EMG 진폭에 미치는 영향)

  • Objectives: This study's aims were to evaluate the effects of load center of gravity within an object lifted and feet placements on peak EMG amplitude acting on bilateral low back muscle groups, and to suggest adequate foot strategies with an aim to reducing low back pain incidence while lifting asymmetric load. Methods: The hypotheses that asymmetric load imposes more peak EMG amplitude on low back muscles contralateral to load center of gravity than symmetric load and maximum peak EMG amplitude out of bilateral ones can be relieved by locating one foot close to load center of gravity in front of the other were established based on biomechanics including safety margin model and previous researches. 11 male subjects were required to lift symmetrically a 15.8kg object during 2sec according to each conditions; symmetric load-parallel feet (SP), asymmetric load-parallel feet (AP), asymmetric load-one foot contralateral to load center of gravity in front of the other (AL), and asymmetric load-one foot ipsilateral to load center of gravity in front of the other (AR). Bilateral longissimus, iliocostalis, and multifidus on right and left low back area were selected as target muscles, and asymmetric load had load center of gravity 10cm deviated to the right from the center in the frontal plane. Results: Greater peak EMG amplitude in left muscle group than in right one was observed due to the effect of load center of gravity, and mean peak EMG amplitudes on both sides was not affected by load center of gravity because of EMG balancing effect. However, the difference of peak EMG amplitudes between both sides was significantly affected by it. Maximum peak EMG amplitude out of both sides and the difference of peak EMG amplitude between both sides could be reduced with keeping one foot ipsilateral to load center of gravity in front of the other while lifting asymmetric load. Conclusions: It was likely that asymmetric load lead to the elevated incidence of low back pain in comparison with symmetric load based on maximum peak EMG amplitude occurrence and greater imbalanced peak EMG amplitude between both sides. Changing feet positions according to the location of load center of gravity was suggested as one intervention able to reduce the low back pain incidence.

Evaluation of Effective Dose and Exposure Levels of Radon in Office and Plant Buildings (일부 제조업 사업장의 사무 및 공장동에서의 라돈농도 수준 및 유효선량 평가)

  • Objectives: Radon may be second only to smoking as a cause of lung cancer. Radon is a colorless, tasteless radioactive gas that is formed via the radioactive decay of radium. Therefore, radon levels can build up based on the amount of radium contained in construction materials such as phospho-gypsum board or when ventilation rates are low. This study provides our findings from evaluation of radon gas at facilities and offices in an industrial complex. Methods: We evaluated the office rooms and processes of 12 manufacturing factories from May 14, 2014 to September 23, 2014. Short-term data were measured by using real-time monitoring detectors(Model 1030, Sun Nuclear Co., USA) indoors in the office buildings. The radon measurements were recorded at 30-minute intervals over approximately 48 hours. The limit of detection of this instrument is $3.7Bq/m^3$. Also, long-term data were measured by using ${\alpha}-track$ radon detectors(${\alpha}-track$, Rn-tech Co., Korea) in the office and factory buildings. Our detectors were exposed for over 90 days, resulting in a minimum detectable concentration of $7.4Bq/m^3$. Detectors were placed 150-220 cm above the floor. Results: Radon concentrations averaged $20.6{\pm}17.0Bq/m^3$($3.7-115.8Bq/m^3$) in the overall area. The monthly mean concentration of radon by building materials were in the order of gypsum>concrete>cement. Radon concentrations were measured using ${\alpha}-track$ in parallel with direct-reading radon detectors and the two metric methods for radon monitoring were compared. A t-test for the two sampling methods showed that there is no difference between the average radon concentrations(p<0.05). Most of the office buildings did not have central air-conditioning, but several rooms had window- or ceiling-mounted units. Employees could also open windows. The first, second and third floors were used mainly for office work. Conclusions: Radon levels measured during this assessment in the office rooms of buildings and processes in factories were well below the ICRP reference level of $1,000Bq/m^3$ for workplaces and also below the lower USEPA residential guideline of $148Bq/m^3$. The range of indoor annual effective dose due to radon exposure for workers working in the office and factory buildings was 0.01 to 1.45 mSv/yr. Construction materials such as phospho-gypsum board, concrete and cement were the main emission sources for workers' exposure.

Performance Evaluation of Aprons according to Lead Equivalent and Form Types (방사선 방어용 앞치마의 납당량, 형태에 따른 성능 평가)

  • The apron is one of the essential protectors to reduce the exposure dose of radiological technologists. This study is to provide a guideline for purchasing the aprons with excellent performance and to help reducing the exposure dose by measuring the shielding ration and uniformity of aprons according to lead equivalent and form types. The shielding ratio of aprons were measured by using radiation generator and dosimeter. Exposure conditions were 81 kVp, 25 mAs, source to image receptor distance (SID) 100 cm and field of view (FOV) $17^{{\prime}{\prime}}{\times}17^{{\prime}{\prime}}$. Exposure areas for front type and around type aprons were divided into 9 areas and for 2 pieces type aprons were divided into 3 areas of top and 4 areas of skirt. The uniformity of aprons were measured by using fluoroscopy and Image J. The 4 regions of interest (ROI) were set into acquired images and measured uniformity by measuring the standard deviation of pixel intensity in ROIs. In continuous shielding ration measurement of aprons according to exposure area, there was not statistical significance (P>0.05). In ANOVA test of aprons, there was statistical significance (P<0.01). In the results of sheilding ratio, although the aprons had equal lead equivalent, there were difference in shielding ratio from 83.59% to 98.15%. In the results of uniformity, the front type aprons with equal lead equivalent indicated the similar uniformity. However, the around type and 2 pieces type apron with equal lead equivalent indicated the different uniformity each other, from 1.8 to 22.2. If the performance evaluation in this study were conducted regularly before and after purchase the aprons, the exposure does to patients and radiological technologists could be reduced.

Isolation of 20(S)-Ginsenoside Rg3 and Rg5 from the Puffed Red Ginseng (팽화 홍삼으로부터 20(S)-Ginsenoside Rg3와 Rg5의 분리 및 구조동정)

  • Red ginseng tail roots (9.8 g water/100 g sample) were puffed at 7, 8, 9, and 10 $kg_{f}/cm^{2}$ using a rotational puffing gun. Puffed red ginseng was extracted with 70% ethanol, and the concentrated extract was successively partitioned with diethyl ether, n-butanol and $H_{2}O$. Two unknown ginsenosides from puffed red ginseng were found at 63 and 65 min of retention time in HPLC chromatogram suggesting that chemical structure of some ginsenosides might be altered during the puffing process. Identification of two unknown compounds was carried out using TLC, HPLC and NMR. Two major compounds were isolated from TLC. According to TLC result, compound I was expected to be the mixture of ginsenosides Rk1 and Rg5, and compound II was expected to be a 20(S)-ginsenoside $Rg_{3}$. Three compounds were isolated from n-butanol fraction through repeated silica gel and octadecyl silica gel column chromatographies. From the result of $^{1}H$- and $^{13}C$-NMR data, the chemical structures of unknown compounds were determined as ginsenoside $Rg_{5}$ and 20(S)-ginsenoside $Rg_{3}$. Unfortunately, ginsenoside $Rk_{1}$ could not be separated from ginsenoside-$Rg_{5}$ in the compound I. It was carefully reexamined using HPLC and confirmed that the last unknown compound was ginsenoside-$Rk_{1}$.

Validity of a Simulated Practical Performance Test to Evaluate the Mobility and Physiological Burden of COVID-19 Healthcare Workers Wearing Personal Protective Equipment (COVID-19 감염병 대응 의료진용 개인보호복의 동작성 및 생리적 부담 평가를 위해 개발된 모의 작업 프로토콜의 타당도)

  • This study evaluated the validity of a newly developed mobility protocol examining the comfort functions and requirements of personal protective equipment (PPE) for COVID-19 healthcare workers. Eight males (age: 24.7 ± 3.0 y, height: 173.4 ± 2.3 cm, and body weight 69.9 ± 3.7 kg) participated in the following three PPE conditions: (1) Plastic gown ensemble, (2) Level D ensemble, and (3) Powered air purifying respirator (PAPR) ensemble. The mobility protocol consisted of 10 different tasks in addition to donning and doffing. The 10 tasks were repeated twice at an air temperature of 25oC with 74% RH. The results showed significant differences among the three PPE conditions in mean skin temperature, local skin temperatures (the forehead, thigh, calf, and foot), clothing microclimate (the chest and back), thermal sensation, thermal comfort, and humidity sensation, while there were no significant differences in heart rate or total sweat rate. At rest, the subjects felt less warm and more comfortable in the PAPR than in the Level D condition (P<0.05). However, subjective perceptions in the PAPR and Level D conditions became similar as the tasks progressed and mean skin and leg temperature became greater for the PAPR than the Level D condition (P<0.05). An interview was conducted just after completing the mobility test protocol, and suggestions for improving each PPE item were obtained. To sum up, the mobility test protocol was valid for evaluating the comfort functions of PPE for healthcare workers and obtaining requirements for improving the mobility of each PPE item.

Antiaging Activity of Mixed Extracts from Korean Medicinal Herbs on HS68 Skin Fibroblast (한약재복합 추출물의 인간피부섬유아세포 HS68에 대한 항노화 효과)

  • Objectives : The purpose of this study was to investigate antiaging and antioxidant effects on cultured human skin fibroblast with 80% ethanol extracts of plants including of stem of Dendropanax morbifera, Corni fructus and Lycii Fructus. Methods : An ethanol extract of three medicinal plants including stem of Dendropanax morbifera, Corni fructus and Lycii Fructus. Extracts were assessed to determine the mechanism of antioxidant and antiaging activities. Antioxidant activity of extract was evaluated by two different assays as 2,2-diphenyl-2-picrylhydrazyl (DPPH) radical scavenging and super oxide dismutase (SOD) like activities. These extracts were tested for cell viability on HS68 skin fibroblast by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-tetrazolium bromide (MTT) assay. We investigated the effects of Ultraviolet-B irradiation on cytotoxicity, type 1 collagen, elastin level and oxidative damage in cultured human skin fibroblast (HS68). Recently, many studies have reported that elastin is also involved in inhibiting or repairing wrinkle formation, although collagen is a major factor in the skin wrinkle formation. Results : The extracts obtained dose-dependently increased the scavenging activity on DPPH radical scavenging activity and SOD like activity. The extracts of complex herbal medicine showed low cytotoxicity as more than 100% cell viability in 100ppm/ml concentration. HS68 fibroblasts were survived 70% at 120 $mJ/cm^2$ UVB irradiation and treated tumor necrosis factor (TNF)-alpha. The levels of aging factors and cytotoxicity were decreased by ethanol extract of complex herbal medicine. Conclusions : These results suggest that ethanol extracts of complex medicinal plants of including of stem of Dendropanax morbifera, Corni fructus and Lycii Fructus may have value as the potential antioxidant and antiaging medicinal plant.

Effect of Irrigation Methods on the Growth and Yield of Rice in Desert Climates (사막토양 환경에서 벼 재배시 관개방법에 따른 생육 및 수량 특성)

  • This study was conducted by directly sowing Asemi in late April at 30 × 10 cm intervals to determine the optimal irrigation method and irrigation amount to maximize the use of limited agricultural water and to increase the yield when growing rice in a desert climate. Conventional irrigation (Conv.), surface drip irrigation (Sur), subsurface drip irrigation (Sub.), and sprinkler irrigation (Spr.) methods were used. The following amounts of irrigation were tested based on field capacity (0.33 bar): 80% (V/V, FC80), 100% (FC100), and 120% (FC120), and data for 2 years were averaged. The total amount of irrigation by irrigation method was the lowest, at 627 ton/10 a, for Sub. irrigation with the FC80 treatment, which was 60.4% less than the amount of irrigation with the FC120 treatment (1,584 ton/10a). Sub. irrigation with the FC120 treatment gave the greatest amount of rice, at 665 kg/10 a, and this condition obtained a yield of 88.1% (754 kg/10 a) of the yield obtained with the conventional treatment. Therefore, when planting rice in a desert climate, subsurface drip irrigation at 120% of field capacity is considered advantageous to increase water use efficiency and crop yield.

Effect of Individualized Exercise Program for Preventing Metabolic Syndrome among IT Company Office Workers (IT 기업 사무직 근로자의 대사증후군 예방을 위한 맞춤형 운동프로그램의 효과)

  • Objectives: Interventions promoting physical exercise and healthy habits in workplaces have been shown to be effective in reducing risk factors for metabolic syndrome. This study was conducted to examine the effects of an individualized conditioning exercise program of IT company office workers with or at higher risk of metabolic syndrome. Methods: A total of 444 IT company office workers with or at higher risk of metabolic syndrome participated in a 3-month conditioning exercise program. Body composition data using bioelectrical impedance analysis and cardiopulmonary data using cardiopulmonary exercise testing from 53 individuals (mean age: 34.8 ± 7.1 years, sex : 21% female, height : 170.4 ± 6.8 cm, weight : 75.2±12.2 kg, body mass index : 25.8±3.3 kg/m2) who have successfully completed pre-test, intervention, and post-test were analyzed. The 12 weeks intervention encompassed: (1) health counseling (2) supervised exercise(endurance-based, aerobic exercise, or circuit training once a week for 50 minutes at heart rate reserve(HRR) of 77-95%) (3) self-directed exercise and biweekly health screening checks. Results: The results indicated a significant decrease in body weight, body fat mass and body mass index, respectively. Moreover, VO2peak, AT VO2 and AT Time significantly improved, respectively. Resting blood pressure(SBP/DBP) showed positive changes but were not statistically significant. We observed the correlation between characteristics of participants and rate of changes in cardiopulmonary outcomes of participants, there are no significant correlation. These results indicate positive changes in body composition and cardiorespiratory fitness parameters following individualized conditioning exercise program. Conclusions: Individualized workplace exercise program for preventing metabolic syndrome can lead to improvements in body composition and cardiorespiratory fitness.
