The Characteristics of Symptoms of Depression and Anxiety Associated with Suicidal Ideation in Korean Employees (한국 직장인의 자살생각과 관련된 우울 및 불안증상의 특성)

  • Objective : We have explored the relationship between suicidal ideation and some very specific characteristics of the symptoms of depression and anxiety in Korean employees. Methods : During the period 2013 through 2014, 100,793 employees who visited the Health Screening Center were asked to complete questionnaires, including the Center for Epidemiologic Study-Depression scale (CSE-D), the Beck Anxiety Inventory (BAI) and the presence of suicidal ideation. We attempted to determine which specific subscale, of each the CES-D and BAI would be more closely related to suicidal ideation by different gender and age by logistic regression. Results : Depression has been found to be a more prevalent and predominant factor in suicidal ideation, when compared to the presence of the condition of anxiety. Of four subscales of CES-D, issues arising from interpersonal relationships were most often associated with suicidal ideation, regardless of the chronologic age or gender of the subject. In BAI, cognitive anxiety showed a more significant association with suicidal ideation than did somatic anxiety in our study participants, showing same pattern in different gender and age groups. Conclusions : Among subscales of CES-D and BAI, the interpersonal relationship in CES-D and cognitive anxiety in BAI showed a higher and more significant association with suicidal ideation in Korean employees.

Contributing Factors for the Registration Rates within Emergency Department Based Post-suicidal Care Program (자살시도 후 생존 퇴원한 환자에서 지역정신건강센터 등록 요인 분석 - 응급실 기반 자살시도자 관리시스템 자료 중심으로 -)

  • Purpose: This study was conducted to investigate the independent factors associated with the registration rate for the community-based post suicidal care program in the emergency department (ED). Methods: This prospective observational study was conducted between January and September 2015 at the academic ED in the tertiary urban hospital. The variables examined included gender, age, address, type of insurance, history of previous psychiatric disease, suicide methods, number of previous attempts, CES-D (The Center for Epidemiologic Studies-Depression Scale), and disposition at ED. Univariate and multivariate logistic regression analysis were conducted to identify factors affecting the registration rate for the community-based post suicidal care program. Results: Overall, 331 suicides were investigated, 61 (18.4%) of which were registered in the post-suicide care program. Factors such as a intervention by psychiatric physician (OR: 3.287, 95%; CI: 1.207-9.624) and levels of depression by CES-D score of 16-24 (OR: 3.635; CI: 1.055-12.526) were significantly correlated with registration for the program. Conclusion: The registration rate for the community-based post suicidal care program was influenced by frequent intervention by a psychiatric physician and levels of depression by CES-D score of 16-24.

Effects of Smartphone Use on Muscle Fatigue and Pain and, Cervical Range of Motion Among Subjects With and Without Neck Muscle Pain (목 주위 근육 통증 여부에 따른 스마트폰 사용이 근 피로도와 통증, 목뼈운동범위에 미치는 영향)

  • The purpose of this study was to investigate the effects of smartphone use on muscle fatigue and tenderness in the cervical erector spinae (CES) and the upper trapezius (UT) and on the cervical range of motion among subjects with and without neck muscle pain. The subjects were 30 smartphone users in their 20 s who -were assigned to either an experimental group with neck muscle pain or a control group without neck muscle pain. Muscle fatigue and tenderness in the CES and the UT as well as the subjects' cervical range of motion were measured before and after 20-min smartphone sessions in a sitting position. In a between-group comparison of muscle fatigue, the experimental group showed a significantly greater decrease in median frequency in the CES and the right UT after smartphone use (p<.05). Regarding the assessment of muscle tenderness after smartphone use, the experimental group showed a statistically significant decrease in the pressure-pain threshold (PPT) in all muscles (p<.05), whereas the control group showed a significantly decreased PPT in the right CES and the UT (p<.05). The assessment of the cervical range of motion revealed a statistically significant reduction in the cervical flexion-extension and left lateral flexion in the experimental group (p<.05) after smartphone use. However, there was no significant change in the cervical range of motion in the control group (p>.05) after smartphone use. When compared with the control group, the experimental group demonstrated greater changes in cervical extension, lateral flexion, and rotation, except for cervical flexion (p<.05). In conclusion, when smartphone users have pre-existing neck muscle pain, the use of a smartphone further increased muscle fatigue and tenderness in the neck and reduced PPT and the cervical range of motion.

  • Objectives : This study was done to determine the effect of Korean medical treatment to patient diagnosed as cauda equina syndrome(CES) suffering from both lower limb dysesthesia, saddle dysesthesia, buttock dysesthesia, dysuria, dyschezia and gait disturbance. Methods : A patient with CES was treated with acupuncture, sweet bee venom pharmacopuncture, herbal medicine from February 2 to April 9 at Department of Acupuncture & Moxibustion, Daejeon Korean Hospital of Daejeon University. Patient received acupuncture treatment($CV_1$, $CV_3$, $CV_4$, $CV_6$, $ST_{36}$, $BL_{31}$, $BL_{32}$, $BL_{33}$, $BL_{34}$) 12 times a week, bee-venom pharmacopuncture($BL_{23}$, $BL_{28}$) was conducted twice a week, herbal medicine(Samilsingihwan decoction) was applied to patient thrice a day for nine weeks. The following symptoms were observed saddle, and lower limb dysesthesia, dysuria, dyschezia and gait disturbance. Results : After treatment, dysesthesia, dysuria, dyschezia and gaiting developed good outcome. But anal reflex remained still. Conclusions : Acupuncture, bee-venom pharmacopuncture, herbal medicine combined treatment might be effective to patients with CES.

A Study of Psychological Distress, Anxiety and Depression on Motor Recovery of Acute Bell's Palsy Patients' Facial Muscle (불안 및 우울이 급성기 벨마비 환자의 안면근 운동기능 회복에 미치는 영향)

  • Objectives : The aim of this study is to investigate the influence of anxiety and depression on motor recovery of acute Bell's palsy to estimate how much psychological factors affect the clinical prognosis. Methods : A total of 20 inpatients with acute unilateral Bell's palsy within 1 week of onset participated in this study. The severity of participants' facial palsy was measured by Yanagihara(Y-system) score, FDI and House-Brackmann scale at the time of 1 week and 3 weeks from the onset. The motor recovery of acute Bell's palsy is defined as ${\Delta}Y$-system during 2 weeks. Beck anxiety scale(BAI) and the center for epidermiologic studies depression scale(CES-D) were adopted to assess anxiety and depression, respectively. Correlation analysis and linear regression analysis were conducted between ${\Delta}Y$-system and prognostic factors including anxiety and depression. Results : Significant associations were found between ${\Delta}Y$-system and depression(CES-D) but no significant associations were found between ${\Delta}Y$-system and other prognostic factors, hypertension, diabetes, postauricular pain, disgeusia, age, degree of initial palsy and anxiety(BAI). And a regression equation with 0.295 for coefficient of determination was obtained. Through this analysis, the ${\Delta}Y$-system can be predicted using regression equation which cover 29.5 % of depression index(CES-D). Conclusion : Depression is a significant clinical prognostic factor on motor recovery of acute Bell's palsy. So, Bell's palsy treatment should be combined with psychological care and support.

A Reliability and Validity Study of the Korean Version of PANSI(Positive and Negative Suicide Ideation) Scale (한국어판 PANSI(Positive and Negative Suicide Ideation)척도의 신뢰성 및 타당성 연구)

  • The purpose of the present study was to examine the validity and reliability of the Korean version of PANSI(Positive and Negative Suicide Ideation: K-PANSI) scale which was developed to measure adolescents' suicide ideation. The study sample consisted of 552 middle school students using a questionnaire survey including the K-PANSI. A reliability analysis, a correlation analysis, and a confirmatory factor analysis were used to test internal consistency(Cronbach alpha), criterion validity (with respect to CES-D, BDI, RES, SWLS), and construct validity. Major study findings are as follows. First, Cronbach' ${\alpha}$ of the K-PANSI scale is supported to some degree by significant correlations with CES-D, BDI, RES, SWLS. Lastly, the confirmatory factor analysis of the K-PANSI scale showed that a two factor model was appropriate with 66.0 percent of explanatory power. These findings suggest that the K-PANSI scale could be used as a reliable and valid measure for adolescent' suicide ideation.

  • Cat eye syndrome (CES) is a very rare chromosomal syndrome characterized by various malformations such as anal atresia, preauricular malformation, coloboma of the iris, and congenial heart and renal defects. This genetic disorder is caused by partial duplication of chromosome 22, mostly as a result of a supernumerary isodicentric marker chromosome idic(22)(q11.2). Various congenital abnormalities and extreme phenotypic variability in CES patients have been reported, which have made prenatal diagnosis of CES difficult. We report the first case diagnosed with CES prenatally by multiplex ligation-dependent probe amplification in a woman who was referred to our hospital, for a fetus presenting with heart anomaly.

Comparison of Muscle Activation on Cervical and Lumbar Erector Spinae, and Upper Trapezius according to Sitting Postures while using a Smartphone in a Bathroom (화장실에서의 스마트폰 사용 시 앉은 자세에 따른 목, 어깨, 허리의 근활성도에 미치는 영향)

  • Purpose: The purpose of this study was to compare the level of muscle activation on the cervical erector spinae (CES), lumbar erector spinae (LES), and upper trapezius (UT) according to the sitting postures while using a smartphone in the bathroom. Method: Thirty-two healthy subjects were recruited for this study. The CES, LES, and UT were evaluated by surface electromyography according to two sitting postures while using a smartphone on the toilet seat. A paired t test was performed for the root mean square of reference voluntary contraction (%RVC) comparisons between two sitting postures, and one-way ANOVA was used to compare the three muscle activations within each posture. Results: A comparison between the two sitting postures revealed the muscle activation of both CESs in sitting posture 2 and both LESs in sitting posture 1 to be significantly higher than those of the others. In sitting posture 1, the muscle activation of both LESs was significantly higher than those of the CES and UT. In sitting posture 2, the muscle activation of both CESs was significantly higher than those of the LES and UT. Conclusion: High muscle activation of the CES and LES was observed according to the sitting postures when using smartphone in the restroom. Therefore, long time use of smartphones on a toilet seat should be avoided.

An evolutionary approach for predicting the axial load-bearing capacity of concrete-encased steel (CES) columns

  • In this research, the gene expression programming (GEP) technique was employed to provide a new model for predicting the maximum loading capacity of concrete-encased steel (CES) columns. This model was developed based on 96 CES column specimens available in the literature. The six main parameters used in the model were the compressive strength of concrete (fc), yield stress of structural steel (fys), yield stress of steel rebar (fyr), and cross-sectional areas of concrete, structural steel, and steel rebar (Ac, As and Ar respectively). The performance of the prediction model for the ultimate load-carrying capacity was investigated using different statistical indicators such as root mean square error (RMSE), correlation coefficient (R), mean absolute error (MAE), and relative square error (RSE), the corresponding values of which for the proposed model were 620.28, 0.99, 411.8, and 0.01, respectively. Here, the predictions of the model and those of available codes including ACI ITG, AS 3600, CSA-A23, EN 1994, JGJ 138, and NZS 3101 were compared for further model assessment. The obtained results showed that the proposed model had the highest correlation with the experimental data and the lowest error. In addition, to see if the developed model matched engineering realities and corresponded to the previously developed models, a parametric study and sensitivity analysis were carried out. The sensitivity analysis results indicated that the concrete cross-sectional area (Ac) has the greatest effect on the model, while parameter (fyr) has a negligible effect.
