RUG-III를 이용한 노인환자군분류의 타당성검증 (Application of RUG-m for Long-Term Care Elderly Patients)

  • The purpose of this study is to classify elderly patient in long-term care facilities using RUG(Resource Utilization Group)-III. It is designed by measuring patient medical characteristics and medical staff time. Elderly patients are classified into 7 categories by clinical(medical and behavioral) hierarchical typology of patients. Through the tertiary split, all 44 groups are formulated. This classification is explained by each patient resource(staff time) utilization level which is called CMI(Case-Mix Index). Major findings are as follows; 1. The objects in this study were classified into 35 groups out of 44 groups. The most frequent category is clinical complex category(CCC; 38.9%). And extensive service category(ESC; 18.8%), reduced physical function category(RPC; 13.1%), special rehabilitation category(SRC; 12.8%), and impaired cognitive category(ICC; 0.00%) are followed. 2. The mean of total CMI was $1.02{\pm}0.36$, ranging from 0.68 to 1.44(1 vs 2.12). The mean of CMI of SRC is only 1.17 which should be the highest. The means of ESC and see are equally 1.20. The means of CMI of CCI, ICC, BPC, and RPC were 0.90, 0.75, 0.83 and 0.96, respectively. 3. The validity of this classification was tested. Trend-test using Regression Analysis was done in the secondary split level. SCC, CCC, ICC, and RPC which covered 68.4% of this research objects showed linear trend of CMI in interim classification. This results were statistically significant. 4. In clinical hierarchy, the trend were showed linearity. But the multiple comparison of categories using Scheffe-test showed that SRC, ESC and see had same level of CMI means and CCC and ICC, too. This results were statistically significant. Classifying elderly patients with RUG-III, the results showed partly linear trend in clinical hierarchy and in interim classification in conclusion. But, in clinical hierarchy, it was failed to show the consistent order of CMI. It can be explained by two reasons. One is that this research subjects were overlapped in each clinical hierarchy group. And the other is that the some of the characteristics for clinical hierarchy is not appropriate for them. For the further study, it needs to have proper sample size and to modify RUG-III to K-RUG to consider our.. medical environment.

저소득가정 비만아동의 신체활동 증진 장애요인 : 지역아동센터 이용 아동을 중심으로 (Barriers to Promoting Physical Activity among Overweight and Obese Children from Low-income Families Using Community Child Care Centers)

  • Purpose: The aim of this study was to identify the barriers to promoting physical activity among obese children from low-income families using community child care (CCC) centers. Methods: This study employed an exploratory-descriptive qualitative research using focus group interviews. The participants were recruited from CCC centers, and 4 focus group interviews were conducted with 10 overweight and obese children and 14 primary caregivers. All interviews were recorded and transcribed verbatim. The data were analyzed using content analysis. Results: The barriers to promoting physical activity were classified into three areas: 1) individual, a lack of will for physical activity; 2) interpersonal, a change in communicative mean with the peer group, absence of parental monitoring due to work-family balance, and an absence of consistent rules; and 3) organizational, lack of physical activity programs and human resources, and confined space for physical activity. Conclusion: The physical activity of children in low-income families is influenced by a range of ecological barriers. These findings can be used to develop tailored intervention to prevent and manage childhood obesity in low-income children.

가스 크로마토그래피 분석과 세포독성에 의한 계피 정유의 품질평가 (Quality Evaluation of the Cinnamon Essential Oils Based on Gas Chromatographic Analysis and Cytotoxicity)

  • To evaluate the quality of the crude drugs using three kinds of Cinnamomum Cortex (CC), Vietnamese CC (VCC, the stem bark of Cinnamomum obtusifolium), periderm-peeled Chinese CC (PPCC, periderm-peeled stem bark of C. cassia), Chinese CC (CCC, stem bark of C. cassia) and a Cinnamomi Ramulus (CR, the twig of C. cassia), the four essential oils were prepared by steam distillation method. Cinnamaldehdye (CAN) and an unknown substance tentatively named hydroxy-cinnamaldehdye(HCNA) were detected in the four essential oils by gas chromatography-mass spectrometry, the contents of which are significantly different one another. Vietnamese CC had the highest content of HCNA whereas CR had the highest CAN content and the lowest HCNA. Vietnamese CC exhibited the greatest cytotoxic activity against the cancer cell lines, A549, HepG-2, HL-60, P-388, U-937, and KB and CR the lowest cytotoxicity. Contents of CAN and HCNA in CCC and PPCC are positioned between VCC and CR. These results suggest that measurement of HCNA and cytotoxicity may determine the quality of CC and CR.

고추냉이 종자(種子)의 휴면타파(休眠打破), 저장중(貯藏中) 유아출현억제(幼芽出現抑制) 및 파종기(擺種期)에 관(關)한 연구(硏究) (Studies on Dormancy Breaking, Sowing Time and Inhibition of Germination during Storage of Seed in Wasabia japonica MATSUM)

  • 고추냉이는 전형적으로 종자(種子)의 저장(貯藏) 및 발아(發芽)가 까다로운 작물(作物)로 알려져 있으며 본 시험(試驗)에서 종자저장(種子貯藏) 방법(方法)에 따른 발아(發芽), 파종적기(播種適期) 시험(試驗)을 수행(遂行)한 결과(結果)를 요약(要約) 하면 다음과 같다. 1. 모래습윤층적법에 의해 저장중인 종자의 $GA_3$ 100ppm+BA 10ppm 처리에서 발아율(發芽率) 95%, $GA_3$ 100ppm 처리에서 발아율(發芽率) 91%를 보이는 효과(效果)가 있었다. 2. 저장중 종자의 발아억제(發芽抑制)를 위한 CCC, 유니코나졸 약제는 각각 저장전 3시간 침지처리시(浸漬處理時) 발아억제(發芽抑制) 효과(效果)가 있었다. 3. 종자저장(種子貯藏)은 모래습윤저장 후 9월하순에서 10월중순 파종(播種)이 출현률(出現率) 93%로 가장 양호하여 효과적(效果的)인 방법인 것으로 나타났다. 4. 자연(自然) 조건(條件)하에서 월동중에 종자의 휴면(休眠)이 타파(打破)되기 때문에 노천매장도 효과적(效果的)인 방법(方法)인 것으로 나타남.

코레옵시스(Coreopsis spp.)와 석죽(Dianthus spp.)의 삽목시 마디수, 적심 횟수 및 식물생장억제제 처리가 생장에 미치는 영향 (Effect of Node size, Pinching and Growth Retardants on Growth of Coreopsis spp. and Dianthus spp.)

  • 본 연구는 Dianthus spp. 및 Coreopsis spp.을 분화용으로 개발하기 위하여 줄기 마디수, 적심 횟수 및 왜화제 처리가 생장에 미치는 영향을 조사하였다. Coreopsis spp. 삽목시 마디 수와 육묘일수에 관한 실험 결과 정식 후 한 달 이후에는 도장의 우려가 있어 육묘 기간은 4주를 넘기지 않는 것이 좋다고 판단되었다. 또한, Coreopsis spp. 분화용으로는 2마디 처리에서 초장, 분지수 등의 및 관상가치가 적당하고 분화 상품으로서의 가치가 가장 좋았다. 적심횟수는 Coreopsis spp.는 2회 적심하는 것이 초장 신장 억제 및 측지 분화에 효과적인 것으로 나타났다. Coreopsis spp.의 식물생장억제제 처리결과에 있어서, B-9 처리는 식물 생육이나 분화 및 꽃수 형성에 효과적인 것으로 관찰되었으며, 특히 4,000 ppm 2회 처리한 것이 가장 좋았다. 반면에 CCC 처리는 큰 차이를 보이지 않아 Coreopsis spp.의 분화 상품 생산에는 B-9이 효과적인 것으로 판단되었다. Dianthus spp.는 적심 및 식물생장억제제를 처리하지 않아도 생장이 양호하며 초장신장이 적당하여 분화용으로 적합한 품종으로 판단되었다. 그러나 B-9 4,000ppm 2회 처리한 것이 엽수가 월등히 증가하였다.

지역아동센터 급식을 위한 적정 배식량 제안 (Recommendation of Serving Size of the Meal Service of Community Child Centers in Korea)

  • Objectives: This study was performed to set easily applicable portion sizes by sex and age for children at the Community Child Centers (CCC) in Korea. Methods: Considering the age and gender specific energy level at Target Patterns for children aged 6-18 years, which were suggested as a part of the 2010 Korean Food Guidance System (KFGS), we set three meal sizes. We reclassified the recommended daily servings of Grains, Meat fish eggs beans and Vegetables group at Target Patterns into three meal sizes, and then calculated the recommended serving per meal. Each proposed amount of food per meal was calculated based on serving size of foods commonly eaten at KFGS, which was then allocated to five meal components; rice, soup stew, protein and vegetable side-dishes and Kimchi. Each proposed amount of food per meal was applied to 173 menus' recipes from CANpro 3.0 as main ingredient's amounts. We cooked the 173 menus at the medium size and measured their weights after cooking. Results: Each recommended serving per meal was 0.75, 0.9 and 1.2 for Grains; 1.2, 1.6 and 2.4 for Meat fish eggs beans; 2, 2.4 and 2.8 for Vegetables by meal sizes. Among five meal components, the ratio of small and large to medium size was 1/5 less and 1/3 more for rice and 1/3 less and 1/3 more for soup stew, protein side-dish and Kimchi, respectively. We suggested the same amount for a vegetable side-dish to encourage vegetable intake. Proper portion sizes per meal of medium were rice 190 g, soup stew 210 g (solid ingredients 60 g), protein side-dish 100 g (meat eggs beans) and 70 g (fish), vegetable side-dish 80g and Kimchi 30 g. Conclusions: Proper portion size per meal suggested in this study may be useful at the CCC where dietitians are not available and the approach could be applicable to the other types of meal services.

수박의 유묘 정단 배양시 유전적 차이가 기관 형성에 미치는 영향 (Genotypic Effect of Watermelon (Citrullus lanatus Thunb.) on Organogenesis from Shoot Tip Culture of Seedlings)

  • 배수성이 상이한 수박(Citrullus lanatus Thunb.) 2배체, 3배체 및 4배체의 유묘 정단을 배양해 본 결과 배수성 차이가 기관 형성에 큰 영향을 미치지 않았으나 시토키닌의 종류와 농도에 따라 기관 형성에는 차이가 심했다. BA 처리시에는 0.3 mg/L의 저농도에서, 2iP 나 kinetin 처리시에서 5∼10.0 mg/L 첨가 배지에서 신초형성이 양호하였으나 생장은 심히 억제되었다. 재생된 신초의 정단을 배양해 본 결과 kinetin은 신초 형성에 거의 효과가 없었고 BA 0.3∼0.5mg/L 첨가구에서 신초증식이 양호하였으며 IAA 0.3 mg/L와 혼합처리는 신초의 증식과 생장 및 마디수 증가에 효과적이었다. 기내에서 형성되는 신초는 부정아 및 액아 유래였으나 대부분은 액아가 발달하여 신초로 생장하였다. 광도와 아가 농도를 달리하여 배양해 본 결과 4배체는 광도와 아가 농도에 관계 없이 신초형성수가 거의 비슷하였으나 생장은 억제되었으며, 3배체는 광도와 아가의 농도가 증가할수록 신초형성수 및 생장이 감소하였다. 또한 광도가 높아질수록 측지의 발생이 양호하였으나 잎의 노화현상을 나타냈다. 생장억제제의 처리는 신초형성수 및 생장을 현저하게 감소시켜 계대배양 기간을 연장할 수 있었다. Ancymidol ; TIBA ; CCC ; PP333은 처리농도간 신초형성수에 차이를 나타내지 않았으며 억제제 중 ABA 처리구가 신초형성과 생장에 가장 억제적으로 작용하였다. 모든 억제제 처리구에서 잎의 전개와 발달이 불량하였다.

가솔린 엔진의 배기 열교환기가 촉매 온도에 미치는 영향에 관한 연구 (Effect of Exhaust Heat Exchanger on Catalytic Converter Temperature in an SI Engine)

  • Close-coupled catalyst (CCC) can reduce the engine cold-start emissions by utilizing the energy in the exhaust gas. However, in case the engine is operated at high engine speed and load condition, the catalytic converter may be damaged and eventually deactivated by thermal aging. Excess fuel is sometimes supplied intentionally to lower the exhaust gas temperature avoiding the thermal aging. This sacrifices the fuel economy and exhaust emissions. This paper describes the results of an exhaust heat exchanger to lower the exhaust gas temperature mainly under high load conditions. The heat exchanger was installed between the exhaust manifold and the inlet of close-coupled catalytic converter. The exhaust heat exchanger successfully decreased the exhaust gas temperature, which eliminated the requirement of fuel enrichment under high load conditions. However, the cooling of the exhaust gas through the heat exchanger may cause the deterioration of exhaust emissions at cold start due to the increment of catalyst light-off time.

  • Kinik Company pioneered diamond pad conditioners protected by DLC barrier ($DiaShield^{(R)}$ Coating) back in 1999 (Sung & Lin, US Patent 6,368,198). Kink also evaluated Cermet Composite Coating (CCC or $C^3$, patent pending) with a composition that grades from a metallic (e.g. stainless steel) interlayer to a ceramic (e.g. $Al_2O_3$ or SiC) exterior. The metallic interlayer can form metallurgical bond with metallic matrix on the diamond pad conditioner. The ceramic exterior is both wear and corrosion resistant. The gradational design of $C^3$ coating will assure its strong adherence to the substrate because there is no weak boundary between coating and substrate.

고정밀 운동제어를 위한 2축 서보메커니즘의 최적튜닝 (Optimal Tuning of Bi-axial Servomechanisms for High-Precision Motion Control)

  • In this paper, the optimal tuning of a cross-coupled controller linked with the feedforward controller is studied to reduce contouring and tracking errors of a bi-axial servomechanisms by using the previously developed integrated tuning method. The CCC system for an arbitrary curve, which is combined with the feedforward controller, is formulated by a state-space based on a series of linear motion trajectories. An optimal tuning problem is formulated as a nonlinear constrained optimization problem including relevant controller parameters of the servo. To verify the effectiveness of the proposed optimal tuning procedure, linear and circular motion experiments are performed on the xy-table. Experimental results confirm that both tracking and contouring errors are significantly reduced by applying the proposed control and tuning system.
