A Recognition for community Based Rehabilitation by Public Health Center Physical Therapists in Seoul Province (서울시 보건소 물리치료사들의 지역사회중심재활에 대한 인식)

  • To survey the recognition of Community Based Rehabilitation(CBR) by Public Health Center(PHC) Physical Therapists in Seoul Province. This study analysed general characteristic of Physical Therapists and cognition, participation, education, problems and requirements for CBR. The subjects of this study were 31 therapists who work at PHC in Seoul Province and Seoul City Childerns Municipal Hospital. Data was collected for 18 days from April 13 to 31, 1998 and results computed from 25 responses. First, Female therapists outnumber males. The age category 30 to 35 old represented 44.0% of the total number studied. 48.0% of therapists had worked more than 7 years and therapists usually 84.0% worked from 3 to 4 years at the PHC. Junior colleage graduates were 72.0%, and respondents with family responsibility were 52.0%. Second, Recently pain control and therapeutic exercises has become the major requirement of patients at PHC(56.0%). If the offered expanded services, a rehabilitation programe at PHC(36.0%) was highly. When asked about a working guide book, nobody had one. Third, 64.0% of therapists were aware of the present level of CBR, 28.0% of them had gotten an information from newspaper and mass media. Fourth, For CBR, 72.0% had positive ideas in which they could participate. 56.0% wished to compose a team of various rehabilitation specialists. 52.0% wished for the meeting day to be flexible and 64.0% desired to meet once or twice a week. Fifth, Almost all(92.0%) desired more educational opportunity about CBR. Example, once or twice a year(48.0%) and training period of 4 to 7 day(44.0%). They wanted to learn theory and parctice together(88.0%) and 48.0% wanted national mutirehabilitation center as a educational managing organization. Sixth, The most important requirement for accomplishing CBR by Physical Therapists was job security and adequate number of therapists(48.0%). Required number of physical therapists in the PHC was 3(52.0%). The cooperative agency should be the distict govement(32.0%) and community welfare center of the disabled persons(32.0%). Factors inhibiting the execution of CBR were lack of physical therapists(56.0%), and equipments for pain control and for therapeutic exercises(68.0%).

A Pilot Survey for the Community Based Rehabilitation In Uiwang (지역사회중심재활사업을 위한 기초 조사)

  • The purpose of this study nun to obtain basic information about the state of the physically disabled due to neurological impairments in order to provide precise data for planning and evaluating the Community Based Rehabilitation in Uiwang. The results were 1) the total number of registered disabled adults with cerebral vascular accident in this city consulted was 80 and the Prevalence rate was 1.0575 of total disabled Persons, 2) under 100 of the score of activity of daily living(ADL) with the subjects was 41.245 as Functional Independence Measure(FIM) and 3) ender 20 of the score of motor ability with the subjects was $45.1\%$ as Motor Assessment Scale(MAS). This results show that rehabilitation program for community based rehabilitation(CBR) should set up for each level of motor ability and ADL.

Exploring Recipients' Experience with the Home-based Rehabilitation Program Based on CBR Model through In-depth Interviews

  • Purpose: This study was conducted in order to explore self-perceived objectives, effects, determinant factors of satisfaction and demands on home-based rehabilitation service (HBRS) based on a community-based rehabilitation (CBR) model in community-dwelling disabilities. Methods: This research was conducted through in-depth interview. HBRS was conducted by four physical therapists for one hour a day, once a week, for eight weeks. After an eight-week intervention period, in-depth interviews were conducted using a semi-structured questionnaire for five recipients of HBRS and six care givers. Results: For the physical effect, some participants experienced positive effects, whereas others did not due to the short-term intervention period. For the social and emotional effects, 'occurrence of motivation for exercise', 'change of surroundings' and 'sorriness for the therapist' emerged as keywords. For the determinant factors of satisfaction, 'movement-inducing therapy', 'therapy from the specialist', 'development of friendship & social network', and 'learning the way of self-rehabilitation' emerged as keywords. For further demands on HBRS, participants stated that 'sufficient time for therapy', 'user opinion-reflected therapy', 'additional instructions for therapeutic exercise & activities of daily living', and 'active promotion for HBRS' were necessary. Conclusion: Participants were satisfied with the physical, social, emotional, and educational aspects of HBRS. In particular, the participants regarded educational aspects as the significant factor throughout self-perceived objectives, determinant factors of satisfaction and the demands. This result suggests that when providing HBRS to community-dwelling persons with disabilities, therapists should recognize the necessity and significance not only of the physical, but also the educational aspect of HBRS.

Necessity of Community-Based Rehabilitation System after Stroke (뇌졸중 환자에 대한 지역사회 재활시스템의 필요성)

  • Despite intensive therapy, a large proportion of stroke survivors are left with significant disabilities. To improve the disable condition, a major component of stroke management is aimed at facilitating functional independence and community reintegration. There are many evidences that community-based rehabilitation after stroke is most effective when provided by a well-organized multidisciplinary team. Key to the implementation of community-based rehabilitation(CBR) is that the disabled person, their family and their community decide on what the priorities of the service should be. The stroke patients in the community need a wide range of services, including the traditional rehabilitation services of physiotherapy, occupational therapy, speech therapy, social workers, orthotists, nurses and so on. The purpose of this research was to investigate necessity and methods of community-based rehabilitation system in Korea.

Effect of Community-based Rehabilitation on Patients with Brain Injury: Meta-analysis Based on the ICF Model (뇌손상 환자의 지역사회재활(CBR) 프로그램의 효과: ICF 모델에 근거한 메타분석)

  • In this study, a meta-analysis was conducted to investigate the effect of community-based rehabilitation on patients with brain injury by ICF factor in previous studies and examine the application of recent community-based rehabilitation program. Total 15 articles that satisfied the selection criteria were divided by ICF factors to obtain the effect size. Homogeneity and publication bias were tested for those selected papers. As a result, the overall effect size of community-based rehabilitation was 0.22; the effect size by physical function and structure among the ICF factors came to be 0.21; and the effect size of activity and participation was 0.20, indicating a small effect size. The studies involved in a meta-analysis were considered as the absence of heterogeneity and publication bias, which indicates that the results of this study are subjectively reliable. These results well explained the level of change on ICF factors, especially physical function and structure, activity and participation, in other words, the intervention effect of community-based rehabilitation on patients with brain injury.

  • The purpose of this study was to investigate the systematic review and meta analysis the effect of community based rehabilitation on elderly people with mild cognitive impairment by ICF factors. This study used PubMed MEDLINE, Cochrane CENTRAL database from January 2009 to January 2019. As a result, a total of 5 studies were selected. The ICF factor effect size of the community based rehabilitation was 4.77 for physical function and structure, and 6.17 for activity and participation. The results of this study showed that the effect of community based rehabilitation of the elderly with mild cognitive impairment is effective on physical function, structure, activities and participation.

Proposed Application Design for Community-Based Rehabilitation Services Access in Community Care System: Occupation and Activity Based (커뮤니티케어 제도 내 지역사회중심재활 서비스 접근을 위한 애플리케이션 디자인의 제안 : 작업과 활동 중심으로)

  • Chronic diseases have been increasing recently as the average life expectancy of humans has been extended, and this trend has caused problems such as the widespread demand for health and rehabilitation services and rising medical costs. In order to solve this problem, the community-based rehabilitation has been developed and strengthened in Korea and gradually promoted since 2019. It is important to secure access to clients who want to use services to revitalize community-based rehabilitation. So in this study, as part of the community-based rehabilitation, intends to devise smartphone applications designs and develop a prototype to secure access to community-based occupational therapy services based on occupation and activities. For Occupational Therapy Practice Framework (OTPF), International Classification of Functioning, Disability and Health (ICF), and Allen Diagnostic Module-2 (ADM-2) were used to devise and categorize occupation and activity based application content, and link OTPF, ICF, and ADM-2 through prior research analysis and expert meetings. The derived content was visualized through literature review and activity analysis, and was implemented to enable direct playback within the application using the YouTube API, and finally developed a prototype application. The Android Studio 3.5.2 for Windows 64-bit was used to build the application prototype. In further research, converging various digital technologies for user convenience and additionally researching community-based occupational therapy service providers opinions and service user satisfaction will improve accessibility to community-based occupational therapy services for clients who have difficulty occupational performance in the community.

The Effects on Home Visiting Nursing Service for Community Based Disabled Persons (지역사회 지체장애인을 위한 방문간호사업 효과 평가 - 재가 뇌병변장애인을 중심으로 -)

  • To evaluate the effects of planned visiting nursing services for homebound disabled persons in the community who have had a cerebrovascular accident Methods: One group pre-test and post-test research deign was applied to 61 disabled persons who were undergoing rehabilitation with visiting nursing services to evaluate the effects on the health status (SF-36) and activity daily living (ADL/IADL) from March to August, 2001. Results: The health status score by SF-36 of homebound disabled people was significantly improved; the average score was 28.56$\pm$21.24 before service, 34.29$\pm$22.30 at 3 months after, and 40.84$\pm$=27.41 at 6 months after. The activity daily living score by OASIS II was also significantly increased (z= -6.09, p=.000; z= -6.04, p= .000) at 3 months and 6 months after home visiting nursing services. Conclusion: The strategy plan for developing a visiting nursing service in health centers should be prepared to develop community based rehabilitation (CBR) programs as well as to improve the level of health status and ADL/IADL for homebound disabled people in the community.

The Service of Community-Based Rehabilitation in Stroke Patient: A Case Study (뇌졸중 환자의 지역사회중심재활서비스: 사례연구)

  • Objective : The purpose of this case study was to proposed a introduction and result of the disabled with stroke in community using services of Community-Based Rehabilitation. Methods : The subject was a 63-years old female patient with stroke who first visit to Public health center in July. She was evaluated on activity daily living action level by using the Modified Barthel Index(MBI) and depression level by using Beck Depression Inventory(BDI). The intervention that was suggested by WHO were Community-Based Rehabilitation(CBR) and general occupational therapy. We were applied from April, 27th 2011, to November 30th, 2011 by occupational therapist. Results : The patient's total MBI score increased from 35 to 55, it means that activity daily living performance capacity was improved. And BDI score decreased from 26 to 14, it means that depressed symptoms were in great decline. She could stand until 5-10 minutes at first, but later stand over 30minutes now. It means her muscle strength and endurance level was increased. The patient's mood condition also was stabilized and then she often showed her smiling face during the therapy. And her protector feels the client's change too. Conclusion : We could know that function level and quality of life of the patient with stroke who return to the community was improved through service of community-based rehabilitation.

A Study on the Effect that Rehabilitation Education Influence on the Knowledge Attitude and Practice of Public Health Nurse (재활교육이 보건간호사의 재활에 대한 지식, 태도, 실천에 미치는 효과 연구)

  • The home visiting health nurses are important man-power who can serve various and persistent rehabilitation care to disabled person in community. The Community Based Rehabilition project(CBR) of national rehabilition center have been carried out from 1995. As a part of that project national health center performed rehabilition education program for home visiting health nurses. The purpose of this study is to analysis the effect of this education. In the first stage all of those groups were educated for two weeks in national rehabilitation center. But only two group nurses, one is in a urban and the other in a rural community, have been educated continually in the field through discussing rehabilitation care case study. The data in this study were gathered from three group healh nurses and analysed by SAS computer program. The results about knowledge, attitude and practice changes of the three group nurses were as follows. 1. In the pre education state the mean point of all nurses' attitude for rehabilition was 59, but in the post education state that was 90. The difference between pre and post attitude is very significant(t=-14.1. p<0.0001l). 2. In the pre education state the mean point of all nurses' knowledge for rehabilition was 45, but in the post education state that was 78. The difference between pre and post knowledge is very significant(t=-12.7, p<0.000l). 3. In the pre education state the mean point of all nurses' practice for rehabilition care was 37, but in the post education state that was 62. The difference between pre and post practice is very significant(t=-7.3, p<0.000l). 4. In practice point, the two group nurses who have been educated continuously were superior to the other(t=-3.9. p<0.00l). 5. All points between the urban and rural nurses were no significant differences(p>0.l). 6. All changes of the attitude, knowledge and practice did not affected by age(F=0.58, p>0.l). professional career(F=O.61, p>0.l), educational background(F=0.97, p>0.l).
