Descriptive Study of Occupational Accidents and their Causes among Electricity Distribution Company Workers at an Eight-year Period in Iran

  • Background: Occupational accidents are unplanned events that cause damage. The socio-economic impacts and human costs of accidents are tremendous around the world. Many fatalities happen every year in workplaces such as electricity distribution companies. Some electrical injuries are electrocution, electric shock, and burns. This study was conducted in an electricity distribution company (with rotational 12-hour shift work) in Iran during an 8-year period to survey descriptive factors of injuries. Methods: Variables collected included accident time, age of injured worker, employment type, work experience, injury cause, educational background, and other information about accidents. Results: Results indicated that most of the accidents occurred in summer, and 51.3% were during shift work. Worker negligence (malpractice) was the cause of 75% of deaths. Type of employment had a significant relationship with type of injuries (p < 0.05). Most injuries were electrical burns. Conclusion: High rate of accidents in summer may be due to the warm weather or insufficient professional skills in seasonal workers. Shift workers are at risk of sleep complaints leading to a high rate of work injuries. Acquiring knowledge about safety was related to job experiences. Temporary workers have no chance to work all year like permanent workers, therefore impressive experiences may be less in them. Because the lack of protective equipment and negligence are main causes of accidents, periodical inspections in workshops are necessary.

소아 두부외상의 임상적 분석 (A Clinical Analysis of Pediatric Head Injuries)

  • Objectives : With the advancement of a social life, the pediatric head injuries(PHI) occur greater than ever. Since the PHI differs from adult head injury with regards to mechanism of trauma, prognosis, and mortality, it is important to identify the characteristics of the PHI for its proper treatments and prognosis. Methods : For this study, a series of 365 PHI patients under 15 years of age who were admitted to our hospital, were evaluated from January 1991 to December 1996. The clinical variable studied were age, sex, Glasgow coma score(GCS), causes of trauma, diagnosis, symptoms, associated injuries and Glasgow outcome score (GOS). The characteristics of PHI were evaluated according to presentations of skull fractures, intracranial hemorrhages, associated injuries, GCS at admission and GOS. Results : Mean age of the studied patients was 6.51 years of age. The majority of PHI patients were under the 7 years of age(66.7%). The ratio of male to female was 2.2:1. Seasonally, PHI occurred more frequently during March to August(61.6%). The main causes of the injuries were accidental falls and traffic accidents(47.1% and 46.3%). One hundred ninety seven(54%) patients suffered from skull fractures and 110(30.1%) patients were developed intracranial hemorrhages and acute epidural hematomas(17.8%) which were the most common intracranial hemorrhages. There was statistical significance between skull fractures and intracranial hemorrhage (p=0.032) and between GCS and GOS(p=0.001). However, there was no statistical significance between skull fractures and intracranial hemorrhage(epidural hematomas, subdural hematomas, and intracerebral, intraventricular and subarachnoid hemorrhage)(p=0.061, 0.251 and 0.880). Also there were no significance of prognosis between under the seven and over the 8 years of age(p=0.349). Conclusions : The core management for PHI is prevention from its occurrences. However, when unexpected accident occurs, early diagnosis and treatment for PHI by through examination for associated injuries and other damages even if there is no skull fracture are essential in managing patient's outcome.

대구, 경북지역 학령전기 아동의 사고 발생 현황 (Non-Fatal Injuries among Preschool Children in Daegu and Kyungpook)

  • Objectives : This study was performed to investigate the injury rates and risk factors for preschool children in Daegu city and Kyungpook province. Method : A questionnaire survey about medically attended injuries during the preschool period was performed in nine primary schools located in Daegu city, Pohang city and Goryung County. The overall injury rate was estimated using person-year. The causes and patterns of the injuries, and their risk factors were examined. Result : A total of 469 medically attended injuries were reported in 330 of the 959 study subjects during the preschool period. The overall annual injury rate was 7.5 per 100 children. The injury rate increased sharply during the period from infant (2.4) to 1 year of age (7.5), and the peak injury rate (9.2) was reported for 5 year olds. The most common causes of injuries were falling (36.0%), followed by being struck by an object (23.7%), and traffic accidents (14.1%). Among the traffic accidents, 72.8% occurred while playing on the road, riding a bicycle or roller-skating. A proportional hazard model showed that males (hazard ratio=1.49, p<0.001 compared with female) and the mother's higher education level (hazard ratio of college or higher= 1.51, p=0.013; high school=1.32, p=0.085 compared with those of middle school or lower) were significant risk factors of childhood injury. Conclusion : The results of this study suggested that efforts for children's safety should be made, especially from the toddler stage, and in male children. To develop a more specific childhood injury prevention program, a surveillance system for injuries should be established. Further study of the relationship between mother's occupation and injury rates is also needed.

수원지역 초등학교 학생의 구강악안면 외상에 관한 실태 조사 (Incidence of Oral and Maxillofacial Injuries of a Elementary School Children in Suwon City)

  • The purpose of this study was to analyze the prevalence of Oral and maxillofacial injuries of elementary school children in Suwon, Korea. The sample consisted of 850 boys and girls in a elementary school children in the city. This study conducted a survey by sending questionnaires to school-aged boys and girls, to study a condition of traumatic injuries of teeth. 1. The students who had a traumatic injuries in a oral & maxillofacial area are 125 persons(14.7%); boys 93 persons(21%), girls 32 persons (7.9%). 2. Incidence of traumatic injuries was 1 time 102 persons(81.6%), 2 times 14 persons(11.2%), others 9 persons(7.2%). 3. The causes of traumatic injuries were fall down 65.6%, blow 10.4%, sports 9.6%, traffic accidents 7.2%, tripping 3.2%, others 4%. 4. The distributions of sports related traumatic injuries were roller blade 7 persons, bicycle 2 persons, football 1 person, baseball 1 person, basketball 1 person. 5. Types of traumatic injuries were soft tissue injury(40.8%), tooth fracture(36.8%), extrusion(8.0%), jaw fracture(3.2%), others(11.2%).

전치부 외상에 대한 임상적 고찰 (A CLINICAL STUDY OF TRAUMATlC INJURIES OF ANTERIOR TEETH)

  • The more civilized, the more incidence had happened in our daily life, particularly in industrial factories, by traffic accident, and in sports, etc. in the dental field. The accidental trauma is usually involved the teeth and surronuding tissues. And the author intended to study of the traumatic injuries of anterior teeth by individual tooth, age, sex, cause of trauma, and how they were treated. The following results were obtained: 1. The patients with traumatic injuries of the anterior teeth were 1.8% among the total dental out-patients. 2. By the age group, the twenties was most frequent in both sexes, and the teenagers next. 3. The predominant causes of traumatic injuries in the dental field were traffic accident (23.4%), fighting (19.2%), falling (18.5%), and sports (7.3%). 4. In the classification of injuries of the teeth, uncomplicated crown fractures (31.2%), subluxations (14.7%), and the complicated crown fractures (13.2%) were presented. 5. By the individual tooth in the traumatic cases, the upper cental incisors were most frequently injuried (49.8%). 6. In the treatments fillings and endodontic treatments (33.2%), splints (13.5%), extractions (11.7%) were main after care method.

특정사인제거정도가 부분노동력여명에 미치는 연장효과에 관한 연구 (The Study on Potential Gains in Working Life Expectancy according to the Degree of Reduction of Specific Causes of Death)

  • This study was carried out to calculate working life expectancy and its potential gains according to the degree of reduction in the specific causes of death. It sought to ascertain what potential gains in labor force longevity might be reasonably achieved through efforts to reduce mortality from injuries and poisoning, diseases of circulatory system, neoplasms and the other causes of death. The data were drawn from the three sources such as "The 1982 Causes of Death Statistics," "Abridged Life Table for Korea 1978~'79" and "The 1982 annual Report of Economically Active Population" issued by Economic Planning Board. Analytical tools used in this study were the cause-deleted life table and the Wolfbein Wool's working life table method. Partial life expectancy was adopted as an index of this study, This application will be widely used as a good demographic tool for analyzing the dynamics of labor force and causes of death. Some of the findings are summerized as follows. 1. Partial life expectancies from initial age 15, 25, and 45 respectively to terminal age 65 are 44.86, 35.59 and 17.41 year in life expectancy itself, 37.08, 32.83 and 15.21 year in working life expectancy, 7.78, 2.76 and 2.20 years in inactive life years. 2. Potential gains in working expectancy from initial age 15, 25 and 45 to terminal age 65 by the complete elimination of the specific causes of death are 1.36, 0.94 and 0.27 years in injuries and poisoning, 0.88, 0.83 and 0.54 years in diseases of circulatory system, 0.56, 0.54 and 0.37 years in neoplasms, 1.13, 1.02 and 0.58 years in the other causes of death. The relationship between degree of reduction in causes of death and potential gains in working life years is in direct proportion. The prime orders of reduction effectiveness in age groups by the causes of death are injuries and poisoning in age 15-24, the other causes of death in age 25-49 and diseases of circulatory system in age 50 and over. 3. If it were possible to reduce 25%, 50% and 75% in mortality condition 1978~'79, the average length of working life would be 38.08, 39.13 and 40.17 years in age 15, and 33.68, 34.57 and 35.44 years in age 25, and 15.67, 16.14 and 16.63 years in age 45.

손상 및 외상 사망 환자의 재원일수 특성에 관한 융합 연구 -퇴원손상심층조사자료를 중심으로 (A Convergence Study on the Characteristics of Length of Hospital Stays of Injured and Traumatic Death Patients - Based on the Korea National Hospital Discharge Injury Survey Data)

  • 이 연구는 퇴원손상심층조사 자료를 이용해 손상 및 외상 사망환자의 재원일수 특성을 분석하여 손상 및 외상으로 인한 사망 예방을 위한 기초 자료 제공을 목적으로 시행되었다. 조사대상은 퇴원손상심층조사 자료에서 2014년 1월 1일부터 12월 31일까지 퇴원한 환자 중, 치료결과가 '사망'이며, 주진단이 손상 및 사고의 외인(S00-T98)인 환자 233명이었다. 연구결과, 여자의 재원일수가 남자보다 길었다. 주진단은 기타 내부 인공삽입장치, 삽입물 및 이식편의 합병증(T85)에서 재원일수가 가장 길었다. 이상의 연구결과로 볼 때 손상으로 인한 사망 재원일수 영향 요인을 파악하여 이들을 집중적으로 관리하기 위한 정책개발이 필요한 것으로 판단된다.

외상성 횡경막 손상 13례 보고 (Traumatic diaphragmatic injuries: report of 13 cases)

  • Thirteen cases of traumatic diaphragmatic injuries treated at the Dept. of Thoracic & Cardiovascular Surgery in Kyung-Hee University Hospital from Jan. 1973 to Dec. 1982, were reviewed in this study. 1. Of 13 cues, 11 were male and 2 were female, a ratio of 5.5:1. This ratio revealed high incidence in male patients. The age distribution was ranged from 2 to 59 years. 2. The causes of traumatic diaphragmatic injuries; 6 were traffic accidents, 4 were stab wounds, 1 was falling down, 1 was gun shot wound and 1 was kick. 3. Operation were performed in 11 patients. No operation was done in 2 patients. 4. There were 100% of other associated injuries, the most frequent was having hemothorax. 5. 2 cases of death occurred in not operated patients. One was intracranial hematoma, and the other was hypertensive encephalopathy.

Judet's strut를 사용한 늑골 고정술의 임상적 고찰 (Clinical Ewperiences of Rib Fixation Using Judet's Strut)

  • For the purpose of evaluation of clinical characteristics in multiple rib fracture due to accident , 24 cases treated by surgical rib fixation using Judet`s strut for multiple rib fracture and flail chestduring the period from June 1993 to October 1994 were reviewed. There were 17 males and 7 females.They ranged in age from 19 years old to 56 years old. The causes of rib fracture were traffic accident in 18 cases, fall down in 3 cases, compression in 2 cases stab wound in 1 case. The number of rib fracture were five in 7 cases, six in 5 cases, four in 5 cases, three in 3 cases. Associated intrathoracic injuries were hemopneumothorax in 12 cases, hemothorax in 10 cases, lung laceration or hemorrhagic contusion in 7 cases. Associated extrathoracic injuries were abdominal injuries in 21 cases, orthopedic problem in 7 cases, head trauma in 4 cases. The most common fractured site was posterolateral portion of the ribs. The causes for operation were flail chest, severe rib displacement and pain, hemothorax or hemopneumothorax with continuous air leakage and stab wound. There were 6 postoperative complications ; one with hydrothorax, two with fibrothorax, two with wound infection and one case of death due to multiful organ failure. Postoperatively, all patients became comfortable and complained less painful. Twenty patients restored spontaneous breathing without ventilator support, three patients were ventilated during a day and one patient expired after 2 days. There were no morbidity and mortality related to operation.

소아청소년의 치과손상 발생에 대한 평가 (Assessment on Development of Dental Injuries in Child and Adolescent)

  • Backgrounds: In order to prevent dental injuries that often occur in child and adolescent, it is intended to investigate and assess actual state of the injury development, present epidemiological background, and consider and discuss for preparing preventive means against the injury development. Purpose: It was attempted to understand major features of dental injuries developing in child and adolescent and indentify high risk factors of dental injuries in child and adolescent. Methods: In this study, 523 cases of computerized data collected as disease entities of dental injuries among 1-18 years old patient visiting S university hospital located in Seoul in 2009 were analyzed and following results were obtained. Results: It was found that the ratio of dental injuries by genders in child and adolescent was 66.14% of male and 33.86% of female. It was shown also that causes of dental injuries by ages were more in order of falling, bumping, chewing, traffic accident, sports, violence, and crash. In addition places where dental injuries occur by ages were home in less than 5 year old group, park, playground, and play yard in 6-11 year old group, park, playground, and play yard also in 12-14 year old group, and stairs, road, and outdoor places such as mountain climbing, beach, and camping in 15-18 year old group. It was found that time rages when dental injuries in child and adolescent often develop were 15-19 o'clock for falling, 15-19 o'clock for crash, 15-19 o'clock for bumping, 19-03 o'clock for violence, 15-19 o'clock for traffic accident, 15-19 o'clock for sports activity, and 15-19 o'clock for chewing. Conclusion: Background of dental injury inducing factors are very complicated and diversified, so deep study and analysis are required for its prediction. Therefore, it seems necessary to identify risk factors by phases such as before, at, and after accident, establish strategies to reduce injury development, and develop and utilize necessary programs.
