Microtensile bond strength of CAD/CAM-fabricated polymer-ceramics to different adhesive resin cements

  • Objectives: This study evaluated the microtensile bond strength (${\mu}TBS$) of polymer-ceramic and indirect composite resin with 3 classes of resin cements. Materials and Methods: Two computer-aided design/computer-aided manufacturing (CAD/CAM)-fabricated polymer-ceramics (Enamic [ENA; Vita] and Lava Ultimate [LAV; 3M ESPE]) and a laboratory indirect composite resin (Gradia [GRA; GC Corp.]) were equally divided into 6 groups (n = 18) with 3 classes of resin cements: Variolink N (VAR; Vivadent), RelyX U200 (RXU; 3M ESPE), and Panavia F2 (PAN; Kuraray). The ${\mu}TBS$ values were compared between groups by 2-way analysis of variance and the post hoc Tamhane test (${\alpha}=0.05$). Results: Restorative materials and resin cements significantly influenced ${\mu}TBS$ (p < 0.05). In the GRA group, the highest ${\mu}TBS$ was found with RXU ($27.40{\pm}5.39N$) and the lowest with VAR ($13.54{\pm}6.04N$) (p < 0.05). Similar trends were observed in the ENA group. In the LAV group, the highest ${\mu}TBS$ was observed with VAR ($27.45{\pm}5.84N$) and the lowest with PAN ($10.67{\pm}4.37N$) (p < 0.05). PAN had comparable results to those of ENA and GRA, whereas the ${\mu}TBS$ values were significantly lower with LAV (p = 0.001). The highest bond strength of RXU was found with GRA ($27.40{\pm}5.39N$, p = 0.001). PAN showed the lowest ${\mu}TBS$ with LAV ($10.67{\pm}4.37N$; p < 0.001). Conclusions: When applied according to the manufacturers' recommendations, the ${\mu}TBS$ of polymer-ceramic CAD/CAM materials and indirect composites is influenced by the luting cements.

CAD/CAM Zirconia All Ceramic Restoration and Red-White Esthetics

  • For the prosthodontic results which are compatible with esthetics to be fulfilled, the first prerequisite would be periodontal intervention and stability, the second one may be functional competency and the ultimate goal should be ascribed to esthetic considerations. Other words, esthetic dentistry is a whole entity which encompasses the biological stability of the periodontium, physical accuracy of the prosthodontic structure and finally, the beauty which can be found in natural dentition. It also implies the harmonized lip line which reveals the well-balanced tooth morphology and health gum profiles (Red-White Esthetics). Largely, there lie some differences in the input system of the 3-dimentional data from the prepared abutments between respective computer-assisted systems available now. But the manufacturing systems (CAM) are very similar between them, to say, comprise numeric control systems with whole 3-dimensional milling units according to the restorations to be made. Now the author is going to present CAD/CAM Zirconia All Ceramic Restoration on the topics for the Red-White Esthetics, periodontal control and maintenance, treatment for the discolored teeth, post & core build-up works for the devitalized teeth, characteristics of the Zirconium oxide All Ceramics, fabrication procedures, clinical considerations and its application to diverse clinical situations.

치과용 캐드캠 시스템으로 제작된 고정성 보철물의 품질 분석 (Analysis of quality for fixed prostheses fabricated by dental CAD-CAM system)

  • Purpose: The purpose of this study was analysis of quality of fixed prostheses fabricated by dental CAD/CAM system. Methods: The ten same cases of stone models were manufactured by dental scannable model, and stone models were scanned using the dental scanner for changing digital model. Ten digital models were completed. The design of zirconia core for zirconia based all-ceramic crown was conducted by the dental CAD program. The samples were fabricated using the pre-sintered zirconia block by dental milling machine. Marginal gaps were analyzed using by silicone replica technique at the eight parts for quality analysis of samples. One-way ANOVA was used for statistical analysis(${\alpha}=0.05$). Results: The mean for marginal gaps were $93.2{\sim}99.1{\mu}m$ at the eight parts. One-was ANOVA didn't show significant differences(p=0.089). Conclusion: The fixed dental prostheses fabricated by dental CAD/CAM were within clinically acceptable range.

CAD/CAM System을 활용하여 제작된 Coping Crown의 제작방식 및 재료에 따른 변연 적합도에 관한 연구 (The Study on Marginal discrepancy of Manufactured Coping Crown according to Production method and materials by using CAD/CAM System)

  • 본 연구는 CAD/CAM 시스템을 활용하여 제작방식 및 재료에 따른 Coping crown의 변연 적합도를 비교하여 최적의 제작방식 및 재료를 비교하고자 하였다. 제작방식 및 재료에 따른 코핑 크라운을 제작하였고, 리플리카 테크닉을 사용하여 변연적합도 측정을 실시하였다. 제작법과 재료에 따른 비교분석을 위해 이원배치분산 분석을 실시하였으며 서로 다른 평균값들의 비교 분석을 위해서 일원배치분산분석을 실시하였다. 연구결과 합금을 Milling법으로 제작한 MM군은 114.6, MS군은 111.4 값을 나타냈고, Milling/Sintering 법으로 제작한 MSC군은 67.2, MSS군은 50.9값을 나타냈고, Printing 법으로 제작한 SLME군은 100.5, SLMR군은 113.2값을 나타냈고, 세라믹의 Milling/Sintering 법으로 제작한 MSZ군은 35.6, MSC군은 36.3값을 나타냈고, 제작방식과 재료에 따른 이원배치분산분석 결과는 제작방식에에 따른 유의한 차이는 보이지 않았으나, 재료에 따른 유의한 차이는 나타내었다. 가장 높은 변연적합도를 보인 군은 세라믹 재료의 Milling/sintering 법이 가장 낮은 값을 보이며 최적의 방법으로 검증이 되었다. 그러나 CAD/CAM 시스템을 활용하여 제작된 Coping crown의 변연 적합도는 임상에서 허용이 가능한 수치값에 해당하므로 임상에 적용이 가능한 것으로 나타났다.

  • The purpose of this case report was to present an example of an esthetic and functional rehabilitation of anterior teeth with tetracycline discoloration and minor morphological abnormality of a 39-year old female. A chairside computer-aided design/computer-aided manufacturing (CAD/CAM) system with CEREC AC was applied for the prosthetic procedure and all ceramic crowns made with lithum disilicate (IPS e.max CAD) restored the esthetic and functional features of sixteen anterior teeth successfully.

In vivo wear determination of novel CAD/CAM ceramic crowns by using 3D alignment

  • PURPOSE. To determine wear amount of single molar crowns, made from four different restoratives, and opposing natural teeth through computerized fabrication techniques using 3D image alignment. MATERIALS AND METHODS. A total of 24 single crowns (N = 24 patients, age range: 18 - 50) were made from lithium disilicate (IPS E-max CAD), lithium silicate and zirconia based (Vita Suprinity CAD), resin matrix ceramic material (Cerasmart, GC), and dual matrix (Vita Enamic CAD) blocks. After digital impressions (Cerec 3D Bluecam, DentsplySirona), the crowns were designed and manufactured (Cerec 3, DentsplySirona). A dualcuring resin cement was used for cementation (Variolink Esthetic DC, Ivoclar). Then, measurement and recording of crowns and the opposing enamel surfaces with the intraoral scanner were made as well as at the third and sixth month follow-ups. All measurements were superimposed with a software (David-Laserscanner, V3.10.4). Volume loss due to wear was calculated from baseline to follow-up periods with Siemens Unigraphics NX 10 software. Statistical analysis was accomplished by Repeated Measures for ANOVA (SPSS 21) at = .05 significance level. RESULTS. After 6 months, insignificant differences of the glass matrix and resin matrix materials for restoration/enamel wear were observed (P>.05). While there were no significant differences between the glass matrix groups (P>.05), significant differences between the resin matrix group materials (P<.05) were obtained. Although Cerasmart and Enamic were both resin matrix based, they exhibited different wear characteristics. CONCLUSION. Glass matrix materials showed less wear both on their own and opposing enamel surfaces than resin matrix ceramic materials.

수종 지르코니아 세라믹의 굴곡강도에 관한 연구 (THE STUDY OF FLEXURAL STRENGTH OF VARIOUS ZIRCONIA CERAMICS)

  • Statement of problem: Increasing demand of esthetic restorations made lots of kinds of ceramic materials. Among them, zirconia has been being focused by many dentists. But, mechanical properties of zirconia were still unclear. Purpose : The purposes of this study were to analyze the flexural strength of various zirconia ceramics which had been currently used for clinic i.e., In-Ceram Zirconia(Vita Zahnfabrik, Bad $S\"{a}ckingen$, Germany), Celay Zirconia(Vita Zahnfabrik, Bad $S\"{a}ckingen$, Germany) and CAD/CAM Zirconia (Adens Zi-Ceram. Seoul , Korea). Material and methods: The four point bending test(ASTM Cl161) was used to measure the flexural strength of a specimen before and after circular heat treatment and fatigue loading. Results : 1. The average value of flexural strengths of CAD/CAM Zirconia, Celay Zirconia, In-Ceram Zirconia in dry condition were 806.5 MPa, 669.9 MPa, 605.6 MPa, respectively. There was a statistically significant difference in strength among the types (P<0.05). 2. After thermocycling, the average flexural strengths of CAD/CAM Zirconia, Celay Zirconia, In-Ceram Zirconia were 791.2 MPa, 604.2 MPa, 605.4 MPa, respectively. CAD/CAM Zirconia showed statistically significant higher strength(P<0.05). The others showed no significant difference after thermocycling(P>0.05). 3. After fatigue loading in wet condition. the average flexural strengths of CAD/CAM Zirconia, Celay Zirconia, In-Ceram Zirconia were 806.0 MPa, 674.9 MPa, 601.7 MPa, respectively. There was a significant difference in strength among the types(P<0.05). 4. There was no statistically significant difference in strength of the specimens according to experimental methods except for before and after thermocycling in Celay Zirconia(P>0.05). Conclusion: Besides high esthetic quality, zirconia had sufficiently high mechanical strength.
