Repair of the Traumatic Oronasal Communication in a Dog with a Palatal Prosthesis : a Case Report (개에서 창상성 구비강 개통의 아크릴 부목을 이용한 치유 증례)

  • A 5-year-old intact female mixed dog was admitted to the Veterinary Teaching Hospital for reconstructive surgery of traumatic oronasal communication in the palatal region after suffering a facial gunshot injury. The surgical procedure involved making buccal mucosal flaps as well as a transposition flap of the hard palate mucoperiosteum from the tissue adjacent to the defect, followed by applying a prosthetic appliance to protect the surgical flaps. A satisfactory outcome was obtained using a palatal prosthesis.

  • This study was conducted to compare the chip removal capability of root canal enlarging instruments. Fifty five buccal or mesial canals from upper and lower molar teeth were randomly selected and divided into three groups; Reamer, K-type file and K-flex file. Fifteen canals in each group were individually enlarged with one of three tested instruments and irrigated with 5cc of normal saline solution. After instrumentation, each canal was dried with paper point and the canal was splited longitudinally and the cleanness of root canal surface was evaluated under stereoscope by three observers. The results were as follows; 1. Most of the canals experimented showed varying degree of the presence of debris. 2. The canals prepared with reamers showed the most clean canal surface. 3. There was no significant difference in debridement effect between k-type file and k-flexfile.

  • Local anesthesia is administered to control pain, but it may induce fear and anxiety. Root planing is a non-surgical periodontal therapy; however, when it is performed in an extensive manner, some tissue removal is inevitable. Notably, this removal may be so painful that local anesthesia is required to be administered to the area scheduled for the treatment. Although patients tend to accept root planing easily, they frequently express a fear of local anesthesia. Intraosseous anesthesia (IA) is an intraosseous injection technique, whereby local anesthetic is injected into the cancellous bone supporting the teeth. A computer-controlled IA system (CIAS) exhibits multiple benefits, such as less painful anesthesia, reduced soft tissue numbness, and the provision of palatal or lingual, as well as buccal, anesthesia via single needle penetration. In this report, we present two cases of root planing that were performed under local anesthesia, using a CIAS.

  • The patient,20 year-old female, complained of anterior open bite. She had tongue thrusting habit and suffered from maxillary sinusitis since 12-years old. Cephalometric analysis revealed slightly forward relationship in maxilla, but normal in mandible. The anterior open bite was corrected through retraction and extrusion of anterior segment and mesial movement of posterior segment under multibanded system after extraction of 4-first premolars. After 2 years and 3 months, she gained ideal overbite, overjet and good interdigitation of buccal segment.

  • Adenomatoid odontogenic tumor (AOT) is a tumor of odontogenic epithelium with varying degrees of inductive changes in the connective tissue. The common radiographic appearance of AOT is a unilocular radiolucency associated with an unerupted tooth. Detectable radiopacities have been reported in many cases. We present a case of AOT in a 9-year old-female patient. Cystic lesion with numerous, punctuate radiopaque foci was observed on the anterior region of the mandible. These radiopacities were situated mostly on the buccal side of impacted tooth on the multi-planar images of cone beam computed tomograph. Characteristic duct like structures and amyloid like material were observed on histopathologic finding. (Korean J Oral Maxillofac Radiol2009; 39 : 99-102)

A Case Report of the Allopuinol-Associated Angiokeratoma in the Oral Mucosa (Allopurinol과 연관된 구강내 혈관각화종(Angiokeratoma)의 증례보고)

  • Angiokeratoma is a cutaneous vascular disorder that occurs at any sites as trunk,extremities, fingers and toes etc. Although solitary or multiple cutaneous lesions have been reported, oral lesions have been very care. A 72-year-old man who had an exophtic, essile mass with dark red, black colored which located on buccal mucosa, was treated with excisional biopsy. He had no specific systemic history except for the medication of allopurinol, for treatment of gout since 10 years. Final diagnosis was determined as angiokeratoma by evaluation of clinical and histopathological finding, and the lesion has not been recurred for two months by decrease of allopurinol. It has been emphasized that the relationship between oral mucosal disease and the complication of allopurinol. Allopurinol is widely used for gout treatment, which we will report a case on the allopurinol-associated angiokeratoma in the oral mucosa.

  • The authers observed a multiple miliary osteoma which was found in the 70-year-old male patient at routine dental examination and obtained the following features; 1. The osteomas are situated in the buccal cheeks of both side, but overlying skin shows normal. 2. The small osteomas range from 2.0㎜ to 2.0㎜ in diameter. 3. The osteomas appeared in proximity to the surface at a special radiograph which made with the tube directed parallel to the surface of the face when the cheek is being blown outwardly. 4. Each osteoma appears as a small doughnut shaped shadow with a radiolucent center.

  • The diagnostic value of a intra oral film is related to projecting technic and interpretation. The intra-oral film is a single plane representation of a three dimensional object, therefore superimposition is inevitablly present. The purpose of this article is to show how foreign objects in the jaw may be localized. The author used double exposure technics, that are changed angulation of vertical or horizontal to one film. The obtained results are as fallow: 1. In the upper anterior region, the moving distance of the labially impacted reference object was greater than that of the palatally impacted one. 2. In the upper molar region, the moving distance of the mesiobuccal root apex was the greatest and that of palatal root apex was the shortest. 3. In the lower molar region, the change of the alveolar bone level in the buccal side was greater than that of lingual side.

Familial gigantiform cementoma (가족성 거대 백악종)

  • Familial gigantiform cementoma is a rare fibre-cemento-osseous disease of the jaws which appears to be transmitted as an autosomal dominant trait with variable expressivity of the phenotype. A 7-year-old girl visited DKUDH complaining of the painless facial deformity. Clinically, significant facio-lingual expansion was observed at the left maxilla, left mandibular body and symphysis portion. Malposition of lower anterior teeth was found. Panoramic radiograph and CT scan showed the extensive expansile mixed lesion at maxilla and mandible. Bone scan revealed hot spot at the maxilla and left side of mandible. Histologic examination revealed moderately dense fibrous connective tissue with scattered masses resembling cementum. The patient's mother had a history of the mandibular resection due to benign tumor. Her younger brother had buccal expansion of right mandible. We report our finding of a family that has exhibited clinical, radiographic and histologic findings consistent with the familial gigantiform cementoma.

  • The salivary duct cyst is a rare disease and usually found in the parotid gland. A 55-years-old man presented swelling in the left buccal area and pathological diagnosis was a salivary duct cyst. Though its recurrence has been reported rare, the presented case showed recurrent swelling after enucleation. The recurrent lesion was successfully treated by incision and drain insertion.
