Case Report of Acute Bronchitis Treated with Dojukgangki-tang (도적강기탕(導赤降氣湯)을 투여한 급성기관지염 치험 3례)

  • Objectives: Acute bronchitis is a common inflammatory disease with symptoms related to temporary inflammation of the airways. It is caused by a viral infections in over 95% of cases. A small minority of cases are caused by bacteria; however, there is little evidence that antibiotics are effective in the treatment of the condition. This report is a case of administering Dojukgangki-tang to three patients with acute bronchitis. Methods: Patients diagnosed with acute bronchitis were treated with Dojukgangki-tang, and the effects were studied using the bronchitis severity scale (BSS) and acute bronchitis severity scale (ABBS). Results: After treatment with Dojukgangki-tang for 2 weeks, average BSS score changed from $11.33{\pm}2.08$ to $1.67{\pm}1.15$. Average ABSS score decreased from $9.00{\pm}3.00$ to $2.00{\pm}1.00$. Conclusions: This study suggests that Dojukgangki-tang is effective in relieving coughs and sputum related to acute bronchitis.

Analysis of Korean Medical status of Acute Bronchitis, Chronic Bronchitis and Allergic Rhinitis patients (급·만성 기관지염과 알레르기성 비염의 국내 의료 이용 현황 분석)

  • Objectives: Patients with respiratory diseases are increasing as air pollution due to fine dust gets worse. Diseases that occupy a large proportion of respiratory diseases in medical institutions are acute bronchitis, chronic bronchitis and Allergic rhinitis. The number of patients with all three diseases is gradually increasing. This study was to suggest assignment of medicine policy for improving accessibility to Korean medical treatment of respiratory diseases analyzing the current treatment status of acute bronchitis, Chronic bronchitis and Allergic rhinitis patients in Western medical and Korean medical institutes in this situation. Methods: This study used 2017 National patient sample data from the Korean Health Insurance Review and Assessment Service for research. Acute bronchitis, Chronic bronchitis, Allergic rhinitis was according to KCD code. This research contains Socio-demographic analysis classified by sex and age, the number of three disease's patients. Also the number of medical treatment, the expense of recuperation cost, medical practices were analyzed compare with Western and Korean medicine. Results: The incidence of three diseases is higher among female than male. There are many patients under the age of 10 in the case of acute bronchitis and allergic rhinitis, while there are many patients over 50 years of age in chronic bronchitis. Western medical treatment take up a larger proportion than Korean medical treatment in part of the number of three disease's patients, the number of medical treatment, the expense of recuperation cost. Conclusions: Much more patients of these diseases visit in Western medical clinic and hospital than Korean medical clinic and hospital for treatment. There are many parts of Korean medical treatment that are not covered by Health insurance benefits like herbal decoction, pharmacopuncture, etc. Korean medicine need to do the efforts for expanding medical field in variety. Also it is demanded for institutional support for reduction of the cost burden and improving on accessibility of Korean medical treatment in order to treat with the increase in respiratory diseases due to fine dust.

  • From 1962 to 1968, serum samples were collected from fowls of six provinces in Korea. The investigator tested for serum neutralizing antibody against the Beaudette strain of intecfious bronchitis virus. Twenty(40 percent)serum samples out of fifty revealed a neutralization index of more than 30. Neuralizing antibodies of infectious bronchitis virus are widely spread among chickens in Korea. Intensive poultry farming zones, adult chickens have been neutralizing antibody of infectious bronchitis virus.

  • Plastic bronchitis는 길고, 분지된 기관지원주(bronchial cast)의 형성과 기관지원주의 현미경 검사상, 염증세포를 포함하고 있는 농후한 점액의 층상 구조를 특징으로 하는 질환으로, 과거에는 fibrinous bronchitis, pseudomembranous bronchitis 그리고 Hoffman's bronchitis로 불려졌으며, 소아와 성인에서 발생하는 드문 질환으로 알려져 있다. 본 교실에서는 갑작스러운 발열, 기침 및 호흡 곤란을 동반하고, 흉부 단순 X-선 검사상 좌측 흉부에 무기폐 소견을 보인 19개월 남아에서, 기관지경 검사를 시행하여 제거한 조직의 육안 및 광학현미경 검사상 plastic bronchitis를 의심할 수 있는 1례를 경험하였기에 보고하는 바이다.

  • Observation of transmissive antibody was made on the rabbit inoculated in the virus of avian infection bronchitis. Throughout the studies, the following experimental results were obtained and summarized here. 1. The antibody of rabbit inoculated with the virus of avian infectious bronchitis were persisted for 2-years. 2. The antibody of adult rabbit against the avian infectious bronchitis virus were transmitted to the suckling rabbit and persisted for 15-weeks.

Effects of Nature Herb Mixture Containing Laminaria japonica Areschoung on Fine Dust-induced Bronchitis in Mice (미세먼지로 유도된 기관지염증에 다시마가 함유된 복합물의 효과)

  • The purpose of this study was to investigate the effects of natural herb mixture containing Laminaria japonica Areschoung (LAM) on fine dust-induced bronchitis in mice. Laminaria japonica Areschoung is main content of LAM, which is including fucoidan. Fucoidan extracted from phaophyta is known to prevent bronchitis and to defend against bacteria and virus infection. In this study, we experienced the effect of LAM on bronchitis and investigated gene expressions (e.g ; IL-8, COX-2, MCP-1) and bio-markers (e.g ; IL-8, PGE2, MUC5AC) associated with bronchitis by using A549 cells. Also, we investigated whether LAM can suppress the bronchitis in fine dust-induced animal models. We injected fine dust (50 ㎕) twice as INT (Intra-Nasal-Trachea) method. Then LAM (200 mg/kg and 400 mg/kg) were oral administered for 14 days. We analyzed the number of immune cells, immunoglobulin E, bio-markers level associated with bronchitis. LAM significantly decreased bio-marker (IL-1β, IL-6, IL-8, TNF-α, Histamine, PGE2), immune cells (white blood cell, neutrophil, lymphocyte, monocyte), and immunoglobulin E, that are increased by fine dust. Taken together, this study suggest that LAM can be used as effective herbal extract for bronchitis.

  • Plastic bronchitis is a rare disease characterized by marked airway obstruction, via the formation of large gelatinous or rigid airway cast. In Korea, there were a few case reports with plastic bronchitis not in adults, but in children. So we report a case of an adult who was diagnosed as plastic bronchitis with eosinophilic casts, with no history of atopic and cardiac disease.

A Systemic Review of Clinical Trials Using Medication for Acute Bronchitis: A Pre-study on the Development of Traditional Korean Medicine Clinical Practice Guideline (급성기관지염의 약물중재 임상시험에 대한 체계적 문헌 고찰: 임상시험 가이드라인 개발을 위한 예비연구)

  • Objectives: The aim of this study is to make evidence-based data for developing a traditional Korean medicine clinical practice guideline for acute bronchitis. Methods: We searched 3 international databases(PubMed, EMBASE, CENTRAL) and 7 domestic databases (KoreaMed, Kmbase, NDSL, KISS, KISTI, OASIS, KoreaTK) to identify randomized controlled trials (RCTs) of acute bronchitis using medicine in recent 10 years. The chosen trials were analyzed by their study design, age range, intervention group, control group, primary and second outcome measure, inclusion and exclusion of participants and adverse events. Results: 15 RCTs are finally included in this study and most of their medications are herbal medicine. For diagnosis and outcome measure of acute bronchitis, Bronchitis Severity Score(BSS) was mostly used. Other measurements eligible are coughing fits, quality of life scale, sputum viscosity, change of individual symptoms and patient's satisfaction. Test duration was for average 7days and safety assessment was held by recording adverse events. Except for anti-inflammatory and antibiotic trials, all medications are found to be effective and well-tolerated. General risk of bias of chosen trials is evaluated low. Conclusions: A well designed clinical trials for traditional Korean medicine of acute bronchitis is needed and this study is expected to make it available.

Fatal plastic bronchitis with eosinophilic casts in a previously healthy child (건강하였던 소아에서 발생한 치명적인 호산구성 증식성 기관지염)

  • Plastic bronchitis is a rare disease characterized by the recurrent formation of branching mucoid bronchial casts that are large and more cohesive than those that occur in ordinary mucus plugging. Casts may vary in size and can be spontaneously expectorated, but some require bronchoscopy for removal. Plastic bronchitis can therefore present as an acute life-threatening emergency if obstruction of the major airways occurs. Three of 22 reported patients with eosinophilic casts were fatal, with death due to central airway obstruction. Here, we report a child with no history of atopy, allergy, or congenital heart disease who was diagnosed with plastic bronchitis with eosinophilic casts. Although he was administered intravenous (iv) antibiotics; iv corticosteroids; and a vigorous pulmonary toilet regimen, including chest physiotherapy and routine bronchoscopic removal of casts, he had brain death secondary to hypoxic brain damage. Plastic bronchitis can be fatal when casts obstruct the major airways, as in the present case. Clinicians should intervene early if a patient exhibits signs and symptoms consistent with plastic bronchitis.

Inhibition of Experimental Lung Inflammation and Bronchitis by Phytoformula Containing Broussonetia papyrifera and Lonicera japonica

  • Broussonetia papyrifera and Lonicera japonica have long been used in the treatment of inflammatory disorders, especially respiratory inflammation, in Chinese medicine. Previously, phytoformula (BL) containing B. papyrifera and L. japonica was found to exert strong anti-inflammatory activity in vitro and in vivo. In this study, the effects of BL on lung inflammation including bronchitis were examined in vitro and in vivo. BL (10-100 ${\mu}g$/ml) inhibited nitric oxide (NO) production of lipopolysaccharide (LPS)-treated alveolar macrophages, MH-S cells, primarily by down-regulating inducible NO synthase. BL also inhibited production of the proinflammatory cytokines, TNF-${\alpha}$ and IL-6. Against an animal model of pleural cavity inflammation, BL (200-400 mg/kg) significantly inhibited 5 h and 24 h carrageenan-induced pleurisy in rats when administered orally. Additionally, BL inhibited experimental bronchitis induced by intratracheal instillation of LPS to rats. Taken together, these results indicate that BL may be effective for the treatment of human lung inflammation as well as bronchitis.
