Effects of Self-breast Pumping in Primiparous Women after Cesarean Delivery (제왕절개술 초산모의 자가 착유 행위 효과)

  • Purpose: This study was done to identify the effects of self-breast pumping on breastfeeding rates and, the degree of breast milk fullness among primiparous women giving birth by cesarean section. Methods: The study design was a non-synchronized posttest control group experiment with repeated measures. The participants were 60 women, 31 in the experimental group who used a manual pump 5 times a day after exclusive breastfeeding and 29 in the control group who breastfed exclusively with no other interventions. Results: While self-breast pumping did not improve breastfeeding rates or the degree of breast milk fullness at any of the time points studied, breastfeeding rates continued to remain high till 12 weeks postpartum in both groups. Conclusion: Although some modifications in research methods will be required to identify the effects of breast pumping and exclusive breastfeeding, both these approaches can be used as interventions to improve breastfeeding rates and breast milk quantity.

A Survey on Understanding and Actual Condition of Breast-feeding of Woman at Oriental Medicine Hospital Postpartum Care Center (한방병원 산후조리센터 산모를 대상으로 한 모유수유 실태 및 인식조사)

  • at Jeonju Wonkwang University Oriental Medicine hospital postpartum care center. Results The majority of women(79.2%) had planned breast-feeding. However, only 32.1% of women currently have fed breast-feeding. 58% of mothers who did partial breast-feeding and breast-feeding currently have planned to breast-feed more than 7 months. The main reason of bottle-fed is concern of insufficient amount of breast milk(72.2%). Also the main reason of breast-feeding is good for baby's health(90%). The average score on the test of the understanding about the methods and advantages of breast-feeding was $13.98{\pm}1.92$ out of total 18.49.1% of women thought that taking herbal medicine would affect baby during breast-feeding. Also 71.7% of women thought that herbal treatments (acupuncture, herbal medicine) would be helpful when the amount of breast milk is insufficient. Conclusions This survey has showed that in spite of full breast-feeding plan, the percentage of actual breast-feeding rate is low. Timely education is needed to increase breast-feeding rate. It is worth to consider herbal treatment to increase breast milk amount. In addition to that it is necessary to create a hospital environment in which mothers can easily begin breast-feeding as soon as possible after delivery. Moreover, beyond the medical field, political and social supports for breast-feeding are needed.

A scientific understanding of Mammary gland and physiology of lactation in Ayurveda.

  • Ayurveda scholars have well described about the physio-anatomical aspect of mammary gland (Stana), physiology of lactation, importance of breast milk (Stanya) in growth and development of baby, various factors affecting the lactation and causing changes in property of milk, Galactogouge (Stanyajanana), and drugs for purification of mother milk (Stanya Shodhana). The recent studies provide evidence for above descriptions of Ayurveda. Breast milk (Stanya) is the nearly complete sole source of nourishment for infants. It has been considered as subsidiary tissue (Upadhatu) of blood plasma (Rasa Dhatu) as it is formed out of Rasa Dhatu (Plasma) and its quality and quantity gets affected by quality of nutrient fraction of food formed after complete digestion (Aahar Rasa). It provides health (Aarogya), strength and immunity (Bala) to the feeding child and gives innumerable beneficial effects like protection against not only acute infections like URTI, diarrhoea but also on chronic illnesses like CVD, metabolic disorders too. The Ayurveda description related to Mammary gland and physiology of lactation still need a better understanding for its implementation on promotion of health. Thus an attempt has been made to compile and analyze the view of Ayurveda scholars on Breast (Stana), Breast milk (Stanya) and physiological aspect of lactation as well as to draw a possible scientific understanding for the relevance.

The Effects of Breast Milk Olfactory Stimulation on Physiological Responses, Oral Feeding Progression and Body Weight in Preterm Infants (모유후각자극이 미숙아의 생리적 반응, 구강수유 진행 및 체중에 미치는 영향)

  • Purpose: This study was conducted to evaluate the effect of breast milk olfactory stimulation on physiological responses, oral feeding progression, and body weight in preterm infants. Methods: A repeated measures design with nonequivalent control group was used. The participants were healthy, preterm infants born at a gestational age of 28~32 weeks; 12 in the experimental group and 16 in the control group. Data were collected prospectively in the experimental group, and retrospectively in the control group, by the same methods. Breast milk olfactory stimulation was provided 12 times over 15 days. The data were analyzed using the chi-square test, Mann-Whitney U test, Wilcoxon signed rank test and linear mixed models using SPSS 19. Results: The gastric residual volume (GRV) of the experimental group was significantly less than that of the control group. The heart rate, oxygen saturation, respiration rate, transition time to oral feeding, and body weight were not significantly different between the two groups. Conclusion: These findings indicate that breast milk olfactory stimulation reduces GRV and improves digestive function in preterm infants without inducing distress.

Vitamin B-6 Nutritional Status of Breast-fed and Formula-fed Preterm Infants (모체의 비타민 B-6 섭취상태가 조산아의 비타민 B-6 영양상태에 미치는 영향)

  • Concentrations of total vitamin B-6 in human milk as well as individual, B-6 vitamers have important implications for the nutritional management of breast-fed(BF) infants. Vitamin B-6 status was assessed in 3 groups of infants : two groups preterm (PT) BF infants whose mothers were supplemented with 2 or 27mg pyridoxine(PN)-HCI ; a sub group of formula-fed (FF) PT infants. Mothers and infants were assessed weekly during the 28-day post feeding. Throughout the neonatal period, levels of total vitamin B-6 and percentages of pyridoxal(PL) in breast milk were lower in PT than T mothers, even in mothers supplemented with 27mg PN-HCI. Total vitamin B-6 levels in PT milk paralleled maternal supplementation but percentage distributions of B-6 vitamers did not change. Vitamin B-6 intakes of BF preterm infants paralleled their mothers' level of infants in the 2mg group was suggested by vitamin status parameters. Vitamin B-6 inadequacy of infants correlated with their plasma pyridoxal-5-phosphate(PLP) levels and erythrocyte alanine aminotransferase(E-ALAT) activity; all parameters such as plasma PLP, PL/PLP ratio and stimulation % of E-ALAT were highest for FF PT infants. The positive correlation of vitamin B-6 levels in breast milk gestational age may contraindicate its adequacy for some PT infants.

The effects of human milk proteins on the proliferation of normal, cancer and cancer stem like cells

  • Human breast milk (HBM) provides neonates with indispensable nutrition. The present study evaluated the anti-cancer activity of diluted and pasteurized early HBM (< 6 weeks' lactation) on human breast cancer cell lines. The cell lines MCF7 and MDA-MB231 were exposed to 1 % HBM from the 1st, 3rd, and 6th weeks of lactation and exhibited reduced proliferation rates. As controls, breast cell lines (293T and MCF-10A), breast cancer cell lines (MCF-7 and MDA-MB-231), and $CD133^{hi}CXCR4^{hi}ALDH1^{hi}$ patient-derived human cancer stem-like cells (KU-CSLCs) were treated with prominent milk proteins ${\beta}$-casein, ${\kappa}$-casein, and lactoferrin at varying doses (10, 50, and $100{\mu}g$) for 24 or 48 hrs. The impact of these proteins on cell proliferation was investigated. Breast cancer cell lines treated with ${\kappa}$-casein and lactoferrin exhibited significantly reduced viability, in both a dose- and time-dependent manner. Interestingly, ${\kappa}$-casein selectively impacted only cancer (but not normal breast) cell lines, particularly the more malignant cell line. However, ${\beta}$-casein-exposed human breast cancer cell lines exhibited a significantly higher proliferation rate. Thus, ${\kappa}$-casein and lactoferrin appear to exert selective anti-cancer activities. Further studies are warranted to determine the mechanisms underlying ${\kappa}$-casein- and lactoferrin-mediated cancer cell-selective cytotoxic effects.

Taurine Level in Human Milk and Estimated Intake of Taurine by Breast -Fed Infants during the Early Period of Lactation (수유 초기 모유 중 타우린 함량과 영아의 섭취량 추정)

  • Taurine is only supplied to the infants from the breast-fed or formula milks because the enzyme activities of taurine biosynthesis are limited in early stages of infants . The objectives of present study were to quantitate the contents of tarurine in human milk and to estimate the intake of taurine by breast-fed infants during early period of lactation. Thirty -three lactating women, volunteered and delivered in R hospital in Serou. were recruited. Milk samples were collected every day at B1-B5 day(from 1 to 5 day dafter start of milk secretion), 15th and 30th day postpartum. Taurine contents were determined by HPLC equipped with RF-detector. The intake of taurine by infants was estimated by multiplization with the infant milk intake reported in our laboratory. The content of taurine was 406$\pm$174nmol/ml at B1-B5 day, and then gradually decreased to 359$\pm$125nmol/ml at 15th day and to 304$\pm$94nmol/ml at 30th day postpartum. The estimated intake of taurine was almost same as 24-25mg/day at B1-B5 day, and 15th , 30th days postpartum . This results was due to the increase of the intake of milk by infants.

Human Milk Microbiota: A Review (모유 미생물총에 대한 고찰)

  • A common belief is that human milk is sterile. However, the development of culture-independent molecular methods, especially Next Generation Sequencing, has revealed that human milk harbors diverse and rich bacterial communities. Although studies aimed at characterizing the microbiota of human milk have produced different findings, Staphylococcus and Streptococcus are presumed to be normal members of the microbiota. Factors that influence variation in the microbiota are unclear; however, the postpartum time, route of delivery, maternal obesity, and health status may be influential. The origin of the microbiota is a hotly debated topic. Human milk bacteria are thought to be introduced through bacterial exposure of the mammary duct during breast feeding and/or the entero-mammary pathway from the maternal gastrointestinal tract. Although the exact mechanism related to the entero-mammary pathway is unknown, it is presumed that bacteria penetrate the intestinal epithelium and then migrate to the mammary gland, dendritic cells, and macrophages. In this review, various relevant studies are introduced.

A Study on Factors Related to the Practice of Breast-feeding (모유수유 실천과 관련 요인)

  • Purpose: The purpose of this study is to examine and analyse factors related to the actual status and practice of breast-feeding during an infant period to grasp elements detrimental to breast-feeding and, therefore, provide basic information useful for its effective practice and encouragement. Method: From April 1 to June 30, 2001, this study was carried out with the mothers of 337 who were 6 months old, as of the surveyed date, of infants born in 2002 and registered in Seongju-gun Public Health Center, Gyeongsangbug-do Province. The method for its survey included both of the visiting and telephone interviews, and questions were mainly about the mothers' general characteristics(3 questions), infants' general characteristics(3 questions), environmental characteristics of infant delivery( 4 questions), support to breast-feeding(4 questions), understanding of breast-feeding(5 questions), and feeding type during the 1 to 6-month period after birth. Result: 1. The feeding type during the I-month period after birth showed that the breast-feeding accounted for 42.4%, which was higher than dry milk-feeding(30%) or mixed milk-feeding(26.8%). However, it began to be lower than the dry milk-feeding from the 2-month period after birth. During the 6-month period, the breast-feeding accounted for 28.6% which was lower than 56.5% of the dry milk-feeding. 2. The mothers who were encouraged by their delivery clinic to and were educated to breast-feed infants accounted for 55.4% and 41.4%, respectively, which were relatively low. The understanding of breast-feeding indicated that the responses were positive from the view point of mother & infant health, but negative from mother's physical form. 3. It was shown that the lower the educational background of mother(p〈0.05) and the higher the unemployment of mother(p〈0.001), the higher the positive understanding of breast-feeding, and that the higher the entire support to breast-feeding, the more positive their understanding. 4. It was also shown that the lower the educational background of mother(p〈0.05), the higher the unemployment of mother(p〈0.001), the more the experience in breast-feeding at a delivery clinic(p〈0.01) and the faster the initial feeding(p〈0.001), the higher the rate of breast-feeding. 5. The factor having an effect on breast-feeding included a delivery clinic's encouragement to breast-feed(p〈0.001), understanding of breast-feeding(p〈0.01), father's comment on feeding method(p〈0.05) and mother's employment(p〈0.05). Discussion: In order to encourage the breast-feeding, as shown above, it is required, fist of all, to offer pregnant women an education about importance and excellence of breast-feeding and its appropriate method before delivery in advance to result in a positive comprehension of the breast-feeding. To do that, both the publicity activities and program development designed to encourage the breast-feeding must be performed in advance at the government level. In addition to that, the mother-infant space as ‘rooming-in’ available for breast-feeding immediately after delivery must be prepared on the basis of legal and administrative support. Finally, female employees' leave after childbirth must be performed for the purpose of productive welfare and circumstances also be prepared for breast-feeding, such as a children's home at work.

Research on Biological and Sociobehavioral Factors Affecting Determinants of Breast Feeding Duration (모유영양의 실시 시간에 영향을 주는 생리적.사회행동적 요인 분석)

  • The process of breast feeding was investigated from the 36th week of pregnancy to 12 weeks postnatal, in order to analyze the factors affecting breast feeding duration and provide the basic data on an education program for successful breast feeding. The general characteristics of study group had no marked effects on the breastfeeding duration. The young and highly condition during pregnancy and the degree of morning sickness affected the practice of breastfeeding. In the study of prenatal sociobehavioral factors, intended duration affected the breastfeeding duration, and the maternal perception that breastfeeding is good for weight loss after delivery as well as infant formula milk is not as good as breast milk, led to successful breast feeding. Among the postnatal biological factors, the duration of gestation affected the breast feeding. Among the postnatal biological factors, the duration of gestation affected the breastiffeding outcome. In the case of early parturition, breastfeeding rate was low. Among the postnatal biocultural factors, time of first feed, milk volume and maternal perception of infant's sucking ability affected the bresatfeeding duration. From these result, it is suggested that an education program such as participation of nursing class to the pregnant women should be provided for successful breastfeeding.
