Effects of Oketani Breast Massage on Breast Discomfort, Breast Pumping Time and Breast-milk compositions in Preterm Infants' Mothers (미숙아를 분만한 산모에게 적용한 오케타니 유방 마사지가 유방 불편감, 모유 유축시간과 모유 성분에 미치는 효과)

  • The purpose of this study was to measure breast discomfort, breast pumping time, and breast milk compositions, specially lipid, calorie, and creamatocrit, after Oketani breast massage program. This study is a single group pretest-posttest design. Seventeen mothers who have preterm babies were received Oketani breast massage program. To evaluate the effects of Oketani massage program, data were measured the breast discomfort by the visual analogue scale, breast pumping time, and breast milk lipid, calorie, creamatocrit by a Creamatocrit plus machine, the standard centrifuge with digital calipers. The breast discomfort was significantly reduced after two Oketani breast massage programs(p<.05). In terms of composition of breast milk, lipid, calorie, and crematocrit were significantly increased after second Oketani massage program(p<.05). The Oketani breast massage program applied to mothers who have preterm babies could reduce breast discomfort and increase lipid, calorie, and creamatocrit of breast milk.

Effects of Breast Massage on Breast Pain, Breast-milk Sodium, and Newborn Suckling in Early Postpartum Mothers (유방마사지가 산욕초기 어머니의 유방 통증, 유즙 소디움, 신생아 흡유 횟수에 미치는 효과)

  • Purpose: In this study the effects of breast massage on breast pain, breast-milk sodium, and newborn suckling in early postpartum mothers were investigated. Methods: The design was a non-synchronized nonequivalent control group pretest-post-test design. Sixty postpartum mothers who were admitted to a postpartum care center and had problems with breastfeeding were recruited. Of these mothers, 44 were assigned to the intervention group and received two 30-minute breast massages within 10 days of postpartum period. The others were assigned control group and received only routine care. Breast pain was measured using a numeric pain scale and number of times newborns suckled was observed throughout breastfeeding. Breast milk was self-collected to evaluate breast-milk sodium. Results: Mean age of postpartum mothers was 30 years old. Compared to the control group, women in the intervention group reported significant decreases in breast pain (p<.001), increases in number of times newborns suckled after the first and second massage (p<.001), and a decrease in breast-milk sodium after the first massage (p=.034). Conclusion: Breast massage may have effects on relieving breast pain, decreasing breast-milk sodium, and improving newborn suckling. Breast massage can be used to solve breast problems. Further research is needed to validate our findings.

The Research on the Breast Feeding and its Related Factors of Premature Infant (신생아 중환자실에 입원한 미숙아의 모유수유 실태와 관련요인 분석)

  • Purpose: The purpose of this research is to provide basic informations for the encouragements of premature infants' breast feeding. Method: From August 10 to October 9, 2002, we have carried out a statical research which surveyed 148 mothers of premature infants registered in NICU. The sample had generated cluster-randomly from 25 General Hospitals, all over the Korea peninsula and being surveyed with 74 questionnaires. Result: Mean hospitalized day of premature infants was 27.9 days. The mean total feeding period was 19.1 days and continuous breast feeding period 12.4days. They were interested in breast feeding education-they answered that they would join the breast feeding education if they were given the chance 87.8%. There was significant relation between babies fed only breast milk and the body weight of birth(p<.05). The reasons why mother gave the baby her breast milk include 'for her baby's health' and 'people said breast milk is good for babies' with a portion of 99%. The main reason why mothers could not execute breast feeding was 'the deficit of breast milk volume' 50.0% and other reason were 'because of start to support more nutritions '18.2%, 'difficulty to carry out the breast milk to hospital'13.6%. The reasen why mothers could not try breast feeding at first were 'deficit of breast milk volume'37.0%, 'not to be prepared for breast feeding because of unexpected delivery'32.6%. Conclusion: We need a program to inform importance and excellent of the breast feeding and a plan to increase the premature infants' breast feeding through the importation of fortifier.

Effect of pectoralis major myofascial release massage for breastfeeding mothers on breast pain, engorgement, and newborns' breast milk intake and sleeping patterns in Korea: a randomized controlled trial

  • Purpose: Supportive interventions to improve breastfeeding practice are needed in nursing. This study investigated the effects of pectoralis major myofascial release massage (MRM) on breast pain and engorgement among breastfeeding mothers and on breast milk intake and sleep patterns among newborns. Methods: Breastfeeding mothers who had delivered between 37 and 43 weeks and had 7-to 14-day-old newborns were recruited from a postpartum care center in Gunpo, Korea. Participants were randomized to the MRM or control group. The outcome variables were breast pain and breast engorgement among breastfeeding mothers and breast milk intake and sleep time among newborns. The experimental treatment involved applying MRM to separate the pectoralis major muscle and the underlying breast tissue in the chest. After delivery, the first MRM session (MRM I) was provided by a breast specialist nurse, and the second (MRM II) was administered 48 hours after MRM I. Results: Following MRM, breast pain (MRM I: t=-5.38, p<.001; MRM II: t=-10.05, p<.001), breast engorgement (MRM I: right, t=-1.68, p =.100; left, t=-2.13, p=.037 and MRM II: right, t=-4.50, p<.001; left, t=-3.74, p<.001), and newborn breast milk intake (MRM I: t=3.10, p=.003; MRM II: t=3.09, p=.003) differed significantly between the groups. Conclusion: MRM effectively reduced breast engorgement and breast pain in breastfeeding mothers, reducing the need for formula supplementation, and increasing newborns' breast milk intake. Therefore, MRM can be utilized as an effective nursing intervention to alleviate discomfort during breastfeeding and to improve the rate of breastfeeding practice (clinical trial number: KCT0002436).

Study on Vitamin h Intake of Breast-fed Infants during the First S Months of Lactation (수유 첫 5개월간 모유 영양아의 비타민 A 섭취량에 관한 연구)

  • This study was performed to investigate the vitamin A intake of exclusively breast-fed infants and to compare the value with the Recommended Dietary Allollwances(BDA) for Korean infants. We examined retinol and ${\beta}$-carotene contents of breast milk in 32 lactating women by HPLC analysis and also the consumed volume of the milk of their infants by the test-weighing method during the first 5 months of lactation. Total vitamin A contents of the milk shelved 67.3, 56.5, 51.2, 45.2, 39.0 and 34.lR.E./100m1 at 0.5, 1, 2, 3, 4 and 5 months of lactation respectively. Vitamin A intake of breast-fed infants were 320, 371, 368, 325, 292 and 251R.E/day during the five-month nursing period. Vitamin A intake per body weight of breast-fed infants was measured at 86.4, 79.8, 63.6, 49.1, 40.0 and 31.9R.E/kg/day. Body weight gain of infants was not affected by the vitamin A intake from the milk during the lactation. We conclude that breast-fed infants most likely receives vitamin A from the milk adequately compared with the RDA for Korean infants. (Korean J Nutrition 31(9) : 1433-1439, 1998)

  • Background: Breast feeding is considered to be mutually beneficial for both mothers and infants, though the effect of lactation problems on development of breast lesions (whether benign or malignant) is not clear. Objectives: This study was conducted to identify possible relations between lactation problems and benign and malignant breast disease. Materials and Methods: 308 patients referred to two referral breast clinics in Tehran, the capital city of IR Iran, between January 2008 and January 2011, were recruited. They were interviewed by a standard questionnaire regarding breast feeding problems. The study population was classified in 3 major groups; breast feeding without any problem, unwillingness to breast feed according to whether mothers' preference not to feed or some breast problems like mastitis, and finally insufficient milk that caused the mothers to feed their babies with formula. Results: Recruiting binary logistic regression method, mother's unwillingness to feed her child by breast milk, and also breast problems such as mastitis and abscess during lactation period showed significant relation with both benign and malignant breast diseases (p value<0.01). Surprisingly, inadequate milk was not associated with any of these conditions. Conclusions: We concluded that lactation problems which involve normal milk drainage from the breast may play an important role in whether the mother wll subsequently develope both benign and malignant pathologies. In contrast in the situation that the production of the milk is not sufficient and there are no intentional or unintentional problems in drainage of the produced milk, future problems would not be more common.

Changes on Breast Milk Intake and Weight of Breast-fed Infants during the Lactation (모유 영양아의 모유 섭취량과 체중 변화)

  • In order to investigate the body weight change and human milk intake of breast-fed infants, we examined thirty-three infants with test-weing method in Sokcho area during the first 5 months of lactation. The average birth weight of infants was 3300g. They grew 3300, 3805, 4676, 5878, 6786, 7403, and 8111g when they became 0.5, 1, 2, 3, 4, and 5 months old. The is human milk intake was 515, 650, 718, 731, 746 and 796g/day during the lactatin. Human milk intake of boys was significantly higher than that of girls at 1(p<0.05), 2(p<0.01), and 3(p<0.01) months of lactation. During lactatin, the number of feedings per day decrerased. The human milk intakes per feeding had increased from 54.4g at 0.5 months to 98.9g at 5 months of lactation The human milk intake on infants was not affected by gestational period and birth weight. From this survey, we found a difference between boys and girls in human milk intake.

Study on Vitamin I Intake of Exclusively Breast-fed Infants (모유 영양아의 비타민 E 섭취에 관한 연구)

  • In order to investigate the vitamin I intake of exclusively breast-fed infants, we examined 33 lactating women and their infants at 0.5, 1, 2, 3, 4 and 5 months of lactation. Vitamin E contents of human milk were determined by HPLC analysis. Vitamin E contents of the milk showed 539, 520, 464, 422, 409 and 351$\mu\textrm{g}$/100m1 during the lactation respectively. Vitamin E contents of the human milk were not affected by energy, lipid and protein consumption of lactating women. Vitamin E intake of breast-fed infants averaged 3375 and 269$\mu\textrm{g}$/day in boys and girls during the first 5 months of lactation. Vitamin E intake per body weight of breast-fed infants appeared 725, 752, 600, 461, 420 and 334$\mu\textrm{g}$/kg/day respectively. We conclude that breast-fed infants most likely receives adequate vitamin E from the human milk compared with recommended dietary allowances for Korean infants.   • Objectives : This study was performed to provide the basic data for oral health of infants, through the comparison of Streptococcus mutans activities(the representative bacteria of initial dental caries), in main nutritional foods for infants: breast milk, milk powder, and soya milk. Methods : The study targeted 30 infants and the saliva was sampled after the oral examination. The pH of sampled saliva was measured; and the sample of 'the saliva', 'the saliva and breast milk', and 'the saliva and formula milk' by researcher were produced; and the measurement was repeated 7 times. Results : As a result of examining the change of colony number of Streptococcus mutans by hour, the bacterial activity took in the source of nutrition took place in the first place; and in the group with dental caries, breast-feeding at night occurreds most frequently. Due to the combined activities, such as the increase in the contact time, the existence rate of dental caries appeared high. However, as the improvement of breast-feeding is closely related to the nutrition supply to babies, rather than the improvement of breast-feeding, the 12 hours after the culture in which Streptococcus mutans activities started was considered. Conclusions : In case of breast-feeding at night, it is necessary to remove the dental plaque in the mouth, before 12 hours has passed. Especially, using toothbrush and toothpaste to decrease the occurrence of dental caries, after the eruption of teeth, is recommended for oral administration.

  • Persistent organic pollutants (POPs) have spread throughout the global environment to threaten human health and damage ecosystems. with evidence of POPs contamination in wildlife, human blood. and breast milk documented worldwide. Breast milk is an ideal medium for assessing exposures to POPs. POPs enter humans largely as contaminants of dietary animal products, where they sequester in adipose tissue, serum, and breast milk and equilibrate at similar levels on a fat weight basis.
