두개골 성형술에서 석고와 신속조형모델을 이용한 간단한 Methylmethacrylate 삽입물 제작법 (Easy Molding Technique of Methylmethacrylate Implant Using Plaster and Rapid Prototyping Model in Cranioplasty)

  • Purpose: Methylmethacrylate is the most commonly used alloplastic material in cranioplasty. However during the polymerization of methylmethacrylate, a significant exothermic reaction takes place. This reaction may result in thermal injury to the brain tissue and other soft tissues. Also it is difficult to make three-dimensional methylmethacrylate implant that is perfectly matched to the defect during the operation time. We report on the molding technique of methylmethacrylate implant using plaster mold and the rapid prototyping model in cranioplasty. Methods: A 44-year-old male was referred to the department for severe frontal hollowness. He was involved in an automobile accident resulting in large frontal bone defect with irregular margin. The preformed patient-specific methylmethacrylate implant was made using plaster mold and the rapid prototyping model before the operative day. The methylmethacrylate implant was placed in the frontal defect and rigidly fixed with miniplates and screws on the operative day. Results: The operation was performed in an hour. In the 6 months follow-up period, there were no complications. Patient was satisfied with the results of cranioplasty. Conclusion: Safe cranioplasty was performed with the preformed patient-specific methylmethacrylate implant using plaster mold and the rapid prototyping model. The result of this method was satisfactory, aesthetically and functionally.

골아세포가 배양된 치과 임플란트용 Ti-Ta합금의 전기화학적 특성 (Electrochemical Characteristics of Osteoblast Cultured Ti-Ta Alloy for Dental Implant)

  • Electrochemical behaviors of surface modified and MC3T3-E1 cell cultured Ti-30Ta alloys have been investigated using various electrochemical methods. The Ti alloys containing Ta were melted by using a vacuum furnace and then homogenized for 6 hrs at $1000^{\circ}C$. MC3T3-E1 cell culture was performed with MC3T3-E1 mouse osteoblasts for 2 days. The microstructures and corrosion resistance were measured using FE-SEM, XRD, EIS and potentiodynamic test in artificial saliva solution at $36.5{\pm}1^{\circ}C$. Ti-Ta alloy showed the martensite structure of ${\alpha}+{\beta}$ phase and micro-structure was changed from lamellar structure to needle-like structure as Ta content increased. Corrosion resistance increased as Ta content increased. Corrosion resistance of cell cultured Ti-Ta alloy increased predominantly in compared with non cell cultured Ti- Ta alloy due to inhibition of the dissolution of metal ion by covered cell. $R_p$ value of MC3T3-E1 cell cultured Ti-40 Ta alloy showed $1.60{\times}10^6{\Omega}cm^2$ which was higher than those of other Ti alloy. Polarization resistance of cell-cultured Ti-Ta alloy increased in compared with non-cell cultured Ti alloy.

ACR 팬텀을 이용한 치아 임플란트 자기공명영상 인공물 분석 (Analysis of the Dental Implants MRI Artifacts by Using the ACR Phantom)

  • 자기공명영상 장치의 정도관리를 위한 ACR 팬텀은 팬텀내의 여러 구조물을 통하여 자기공명영상 화질을 평가할 수 있다. 본 연구는 3.0T 장비에서 Head coil에서 ACR 팬텀을 이용하여 임플란트와 치아 교정용 철사를 부착하여 영상의 고스트 신호 백분율과 절편 두께 정확도를 분석할 수 있었다. T1강조영상 첫 번째 절편과 열한 번째 절편의 임플란트 보철에서 절편위치 정확도는 수신대역폭이 230에서 좋게 나타났으며, 교정용 철사가 부착했을 때는 수신대역폭이 130일 때가 좋았다. 고스트 신호 백분율은 SE T1강조영상 일곱번째 절편에서 임플란트 보철에 추가된 교정용 철사의 경우에는 수신대역폭 230이 좋게 나타났다. 자기공명영상 검사에서 임플란트 보철 환자의 경우에 적절한 수신대역폭을 선택하여 영상의 왜곡과 신소 소실이 감소된 영상을 획득할 수 있을 것으로 사료된다.

HEN Simulation of a Controlled Fluid Flow-Based Neural Cooling Probe Used for the Treatment of Focal and Spontaneous Epilepsy

  • Brain disorders such as epilepsy is a condition that affects an estimated 2.7 million Americans, 50,000,000 worldwide, approximately 200,000 new cases of epilepsy are diagnosed each year. Of the major chronic medical conditions, epilepsy is among the least understood. Scientists are conducting research to determine appropriate treatments, such as the use of drugs, vagus nerve stimulation, brain stimulation, and Peltier chip-based focal cooling. However, brain stimulation and Peltier chip-based stimulation processes cannot effectively stop seizures. This paper presents simulation of a novel heat enchanger network(HEN) technique designed to stop seizures by using a neural cooling probe to stop focal and spontaneous seizures by cooling the brain. The designed probe was composed of a U-shaped tube through which cold fluid flowed in order to reduce the temperature of the brain. The simulation results demonstrated that the neural probe could cool a 7 $mm^2$ area of the brain when the fluid was flowing atb a velocity of 0.55 m/s. It also showed that the neural cooling probe required 23 % less energy to produce cooling when compared to the Peltier chip-based cooling system.

수종의 골이식재를 이용한 성견의 임플란트 주위 3면 골내낭의 조직학적 관찰 (Histologic findings of three-wall intrabony defects around dental implants using different grafting materials in beagle dogs)

  • 연구목적 : SLA surface dental implant 주위의 3면 골내낭에서 xenogeneic demineralized bone matric putty, porous ${\beta}$-tri-calcium phosphate, 새로이 개발된 non-crystalline calcium phosphate glass를 사용한 치료를 조직학적으로 비교 평가하기 위한 것이다. 연구방법 : 실험동물로는 15개월에서 18개월 사이의 12kg에서 15kg 정도되는 성견을 사용하였다. 20개의 SLA surface implant가 사용되었으며, 성견 하악의 양측에 각각 2개씩 사용되었다. 임플란트 식립전에, 각각의 임플란트 근심면에 straight fissure bur를 이용하여 표준화된 3면 골내앙(근원심 5mm ${\times}$협설 3mm ${\times}$깊이 3mm)을 형성하였다. 형성된 골 결손부에는 demineralized bone matrix putty, porous ${\beta}$-tri-calcium phosphate, non-crystalline calcium phosphate glass를 넣은 것을 각각 실험군으로, 이식재를 넣지 않은 것을 대조군으로 사용하였다. 8주 후에 실험 동물을 희생시키고 조직학적 관찰을 하였다. 결과 : 조직학적 소견상 임플란트 주위에 급성 염증 소견은 보이지 않았으며, non-crystalline calcium phosphate glass은 매우 적은량의 신생골을, ${\beta}$-TCP을 이용한 골내낭에서는 약간의 기저부에서 유래된 신생골이 관찰된다. ${\beta}$-TCP granules 가운데로 상당량의 측면의 골에서 유래된 신생골 형성이 보인다. xenogeneic DBM putty에서는 많은 량의 신생골이 기저부에 형성된 것을 볼수 있으나 대조군과의 차이는 크지 않다. 이식재의 종류와 상관없이 흡수되지 않은 이식재를 임플란트 주위에서 관찰할 수 있었다. 골내낭 안의 이식재들은 모두 connective tissue로 둘러 싸여 있었다. 모든 실험군에서 이식재에서 기인한 신생골 형성과 임플란트 표면에 신생 골유착의 조직학적 증거는 발견되지 않았다.

Enhanced compatibility and initial stability of Ti6Al4V alloy orthodontic miniscrews subjected to anodization, cyclic precalcification, and heat treatment

  • Objective: To evaluate the bioactivity, and the biomechanical and bone-regenerative properties of Ti6Al4V miniscrews subjected to anodization, cyclic precalcification, and heat treatment (APH treatment) and their potential clinical use. Methods: The surfaces of Ti6Al4V alloys were modified by APH treatment. Bioactivity was assessed after immersion in simulated body fluid for 3 days. The hydrophilicity and the roughness of APH-treated surfaces were compared with those of untreated (UT) and anodized and heat-treated (AH) samples. For in vivo tests, 32 miniscrews (16 UT and 16 APH) were inserted into 16 Wistar rats, one UT and one APH-treated miniscrew in either tibia. The miniscrews were extracted after 3 and 6 weeks and their osseointegration (n = 8 for each time point and group) was investigated by surface and histological analyses and removal torque measurements. Results: APH treatment formed a dense surface array of nanotubular TiO2 layer covered with a compact apatite-like film. APH-treated samples showed better bioactivity and biocompatibility compared with UT and AH samples. In vivo, APH-treated miniscrews showed higher removal torque and bone-to-implant contact than did UT miniscrews, after both 3 and 6 weeks (p < 0.05). Also, early deposition of densely mineralized bone around APH-treated miniscrews was observed, implying good bonding to the treated surface. Conclusions: APH treatment enhanced the bioactivity, and the biomechanical and bone regenerative properties of the Ti6Al4V alloy miniscrews. The enhanced initial stability afforded should be valuable in orthodontic applications.

rhTGF-${\beta}2$/PLGA 복합체를 electrospray법으로 코팅한 타이타늄 임플란트 골 유착의 microCT 계측: a preliminary rabbit study (Osseointegration of the titanium implant coated with rhTGF-${\beta}2$/PLGA particles by electrospray: a preliminary microCT analyzing rabbit study)

  • 목적: 본 선행 연구는 recombinant human transforming growth factor-${\beta}2$ (rhTGF-${\beta}2$)/ poly lactic-co-glycolic acid (PLGA) 복합체를 타이타늄 임플란트에 처리하였을 때 골 유착에 미치는 영향을 알아보기 위해 시행된 것으로 토끼 모델을 사용하였다. 재료 및 방법: 8개의 임플란트를 300V에서 3분 동안 양극 산화하였다. 그 중 4개는 electrospray법으로 rhTGF-${\beta}2$/PLGA를 코팅하여 실험군으로 설정하였다. 4마리의 New Zealand rabbit의 tibiae에 1개씩의 실험군과 대조군 임플란트를 식립하였으며, 3주와 6주에 2마리씩 희생하여 micro-computed tomography(microCT) 촬영 후 분석하였다. 결과: Scanning electron microscope (SEM) 사진에서 rhTGF-${\beta}2$/PLGA 입자가 임플란트 표면에 균일하게 분산되어 있음을 확인하였다. MicroCT 분석 결과 통계적으로 유의하지는 않지만 rhTGF-${\beta}2$/PLGA를 처리한 임플란트가 bone volume/total volume (BV/TV)와 trabecular thickness (Tb.Th) 값이 더 높은 경향성을 보였으며, cross sectional view에서 더 많은 골이 형성되었음을 확인하였다. 결론: rhTGF-${\beta}2$/PLGA 표면처리된 임플란트가 주변 골의 양적 성장을 촉진시킬 수 있으며 임플란트 초기 골 유착을 증진시킬 수 있는 가능성을 보였다.

골내 결손부에서 법랑기질 유도체의 임상적 효과 (Clinical effect of enamel matrix derivative(EMD) in the treatment of periodontal intrabony defects)

  • tachment level was changed from $8.67{\pm}1.72mm$ to $7.00{\pm}1.60mm$ (control); from $8.93{\pm}2.23mm$ to $6.00{\pm}1.92mm$ (test); and bone probing depth was decreased from $10.20{\pm}1.90mm$ to $9.07{\pm}1.95mm$ (control); from $10.14{\pm}2.14mm$ to $7.43{\pm}2.06mm$ (test). This study indicates that treatment of periodontal intrabony defects with EMD is clinically superior to treatment without EMD (OFD alone) in every parameter evaluated. Within the limits of this study, the application of EMD in intrabony defects resulted in clinically significant gain of clinical attachment level and decrease of bone probing depth. And further controlled clinical studies are required to confirm the effectiveness of the EMD in the treatment of various osseous defects.

calcium carbonate와 fibrin-fibronectin sealant system 혼합이식이 치주골내낭 치유에 미치는 영향 (Effects of mixture of fibrin-fibronectin sealant system and calcuim carbonate in periodontal intrabony defects)

  • Calcium carbonate(CC) is biocompatible and gradually absorb to be replaced by bone when implanted into bone tissue. Fibrin-fibronectin sealant system (FFSS) is a product of human-derived plasma. The effect is hemostasis, tissue fixation and adhesion, We expect synergic effects of this two materials in periodontal regeneration. When FFSS was grafted with bone graft in intrabony defects, could be eliminated exofolication of bone graft materials. This study evaluated above materials for periodontal regeneration of 6mm intrabony defects in 36 patients. lap surgery was carried in 14 defects of control group. experimental group 1 was 11 defects grafted with calcium carbonate, experimental group 2 was 11 defects which were grafted with calcium carbonate with FFSS. The clinical parameters evaluated included changes in attachment level, probing depth, gingival recession at 6 months. Postsurgery probing depth reduction was 3.1 ${\pm}$ 0.9mm in control, 3.8 ${\pm}$ 1.6mm in experimental group 1, 4.1 ${\pm}$ 1.1mm in experimental group 2. The result clinically and statistically improved compared to baseline(P<0.01), but the difference found among the groups were not statistically significant. Postsurgery clinical attachment level was 1.6 ${\pm}$ 1.2mm in control, 3.5 ${\pm}$ 2.0mm in experimental group 1, 3.3 ${\pm}$ 1.2mm in experimental group 2. All of the control and experimental groups resulted in a statistically significant reduction from baseline(P<0.01). The reduction of the experimental groups were statistically significant from control(P<0.05). But the change between experimental group 1 and experimental group 2 was not statistically significant. We conclude that mixture of CC and FFSS is effective to periodontal regeneration in intrabony defect.

조골세포에서 Aggregatibacter actinomycetemcomitans 생균의 파골세포분화유도 cytokine 발현 유도능 및 침투능 (Induction of osteoclastogenesis-inducing cytokines and invasion by alive Aggregatibacter actinomycetemcomitans in osteoblasts)

  • Osteoblasts regulate osteoclastogenesis by production of various cytokines. Aggregatibacter(A) ac-tinomycetemcomitans is one of periodontopathogens which invades gingival tissue. Therefore, clarifying the effect of alive A. actinomycetemcomitans on osteoblasts is important to understand the mechanism of alveolar bone resorption in periodontitis. We investigated induction of osteoclastogenesis-inducing cytokines, adherence, and invasion by A. actinomycetemcomitans in osteoblasts. Osteoblasts were isolated from mouse calvaria and expression of cytokines was determined by RT-PCR. When the ratio of the number of A. actinomycetemcomtians to the number of osteoblasts was 10:1, 50:1 and 100:1, RANKL mRNA expression was increased. A. actinomycetemcomitans also increased expression of macrophage inflammatory protein (MIP) -1${\alpha}$, interleukin (IL)-1${\beta}$, and tumor necrosis factor (TNF)-${\alpha}$. A. actinomycetemcomitans attached to and invaded osteoblasts at ratio of 1000:1. These results suggest that A. actinomycetemcomitans increases osteoclastogenesis-inducing ability of osteoblasts by stimulating the expression of RANKL, MIP-1${\alpha}$,IL-1${\beta}$, and TNF-${\alpha}$ and that invasion of A. actinomycetemcomitans provides a means by which the bacteria escape from immune system and antibiotic therapy.
