Sequencing of panfacial fracture surgery: a literature review and personal preference

  • Background: Treating panfacial fractures (PFFs) can be extremely difficult even for experienced surgeons. Although several authors have attempted to systemize the surgical approach, performing surgery by applying a unidirectional sequence is much more difficult in practice. The purpose of this study was to review the literature on PFF surgery sequence and to understand how different surgical specialists-plastic reconstructive surgery (PRS) and oral maxillofacial surgery (OMS)-chose sequence and review PFFs fixation sequence in clinical cases. Methods: The PubMed and Google Scholar databases were scoured for publications published up until May 2020. Data extracted from the studies using standard templates included fracture part, fixation sequence, originating specialist, and the countries. Bibliographic details like author and year of publication were also extracted. Also, we reviewed the data for PFFs patients in the Trauma Registry System of Dankook University Hospital from 2011 to 2021. Results: In total, 240 articles were identified. This study comprised 22 studies after screening and full-text analysis. Sixteen studies (12 OMS specialists and 4 PRS specialists) used a "bottom-top" approach, whereas three studies (1 OMS specialist and 2 PRS specialists) used a "top-bottom" method. However, three studies (only OMS specialists) reported on both sequences. In our hospital, there were a total of 124 patients with PFF who were treated during 2011 to 2021; 64 (51.6%) were in upper-middle parts, 52 (41.9%) were in mid-lower parts, and eight (6.5%) were in three parts. Conclusion: Bottom-top sequencing was mainly used in OMS specialists, and top-bottom sequencing was used at a similar rate by two specialists in literature review. In our experience, however, it was hard to consistently implement unidirectional sequence suggested by a literature review. We realigned the reliable and stable buttresses first with tailoring individually for each patient, rather than proceeding in the unidirectional sequence like bottom-top or top-bottom.

Comparison of Methanotrophic Activity at Top and Bottom Layers in Up-flow Biofilters (상향식 바이오필터에서 상.하층의 메탄 산화 특성 비교)

  • The methane oxidation characteristics at the top and bottom layers in up-flow biofilters were investigated. Two biofilters were packed with perlite and tobermolite (biofilter A: respectively top and bottom; biofilter B: respectively bottom and top) and then compared. The methane oxidation rate was analyzed with the packed bed of the biofilter layers. The bacterial population in the biofilter was characterized using quantitative real-time PCR. For the methane oxidation rate of the biofilter A column, the perlite top part ($845.16{\pm}64.78{\mu}mol{\cdot}VS^{-1}{\cdot}h^{-1}$) gave a relatively higher value than the tobermolite bottom part ($381.85{\pm}42.00{\mu}mol{\cdot}VS^{-1}{\cdot}h^{-1}$). For the methane oxidation rate of the biofilter B column, the tobermolite top part ($601.25{\pm}37.78{\mu}mol{\cdot}VS^{-1}{\cdot}h^{-1}$) provided a relatively higher value than the perlite bottom part ($411.07{\pm}53.02{\mu}mol{\cdot}VS^{-1}{\cdot}h^{-1}$). The pmoA gene copy numbers, responsible for methanotrophs, in the top layer of biofilter A (1.27E+13 pmoA gene copy number/mg-VSS) was higher than in the bottom layer (3.33E+13 pmoA gene copy number/mg-VSS). However, the population of methanotrophs in biofilter B was not significantly different between the top and bottom layers. These results suggest that although the methane oxidation rates of perlite and tobermolite in the top parts of biofilter A and B were high, methanotroph populations were higher in the bottom parts of both biofilters, with a rapid decline in methane concentrations within the biofilters.

Mechanical Characteristics of Municipal Waste Incineration Bottom Ashes (생활폐기물 소각 바닥재의 역학적 특성)

  • Due to the population growth and development of industry, waste from household and industries has increased. As the advanced countries experienced these problems, they have already started research on recycling methods of waste incineration ashes. Domestic recycling rate of incineration ash became up to 80 percent as high as the level of developed countries, but the recycling was limited to fly ash for admixture in concrete. In case of bottom ash, most of bottom ash was reclaimed in the landfills. Therefore, basic physical property and mechanical experiments for bottom ash were conducted in this study to evaluate the possibility of incineration bottom ash as an alternative construction materials. Bottom ashes from three different landfills with two different incineration methods were tested. Incineration methods are Stoker type Incinerator and Pyrolysis-Melting Treatment. Bottom ash can be used as an alternative granular material for construction based on the basic physical property and mechanical characteristics similar to those of sandy materials. However, the incineration method should be considered since it can affect the material and mechanical characteristics of the incineration bottom ash.

  • The main purpose of this study is to differentiate Sasang Constitutions of men and women by measuring 5 parts of the body. By the definitions about Four Chos(四焦) in ${\ulcorner}$The Discourse on Viscera and Bowels(臟腑論)${\lrcorner}$, the body is divided into four parts and sets up the five borderlines. They are Top-line, Bosom-line, Stomach-line, Navel-line and Bottom-line. Top-line(第一線) is the horizontal distance between Rt. & Lt. Coracoid processes passing through the acupuncture point CV-22(天突穴) or the horizontal distance between Rt. & Lt. beginning points of Axillary lines. Bosom-line(第二線) is the horizontal distance passing through Rt.& Lt. Nipples. Stomach-line(第三線) is the horizontal distance passing through Rt.& Lt. acupuncture point ST-19(不容穴). Navel-line(第四線) is the horizontal distance passing through the navel(CV-8; 神闕穴) and the acupuncture point ST-25(天樞穴). Bottom-line(第五線) is the horizontal distance between Rt. & Lt. Anterior Superior Iliac Spine(ASIS). The measured numerical values of Five Lines are the most important factors to classify the Four Constitutions(Taeyangin, Taeumin, Soyangin, Soeumin). Among the five lines, Stomach-line(Middle-line) is the base line to decide Yin and Yang. As a result, I could differentiate the four Constitutions as follows. 1. Taeyangin's Top-line is more than 36cm long. Top-line is the longest. Bottom-line is the shortest. The length-difference between Top-line and Bottom-line is more or less 10cm long. 2. In case of Taeumin, Navel-line is the longest. The numerical values of Middle and Bottom lines are almost the same or Bottom-line is shorter than Middle-line by 1 or 2 cm long. In some special case, the five lines are all the same and the body just looks like a log. 3. In case of Soyangin, just like Taeyangin, Top-line is the longest. Bottom-line is the shortest. The length of Top-line is ranged between 30cm to 35cm long. From Top-line to Bottom-line, the length gradually diminishes each by 2cm: for example, 34-32-30-28-26. 4. In case of Soeumin, the longest one is Bottom-line. Specially, Navel-line is shorter than Middle-line. Bottom-line is longer than the Middle.

A Note on the Modified Mild-Slope Equation (修正 緩傾斜方程式에 대한 小考)

  • Recently the modified mild-slope equation has been developed by several researchers using different approaches, which, compared to the Berkhoff's mild-slope equation, includes additional terms proportional to the square of bottom slope and to the bottom curvature. By examining this equation, it is shown that both terms are equally important in intermediate-depth water, but in shallow water the influence of the bottom curvature term diminishes while that of the bottom slope square term remains significant. In order to examine the importance of these terms in more detail, the modified mild-slope equation and the Berkhoff's mild-slope equation are tested for the problems of wave reflection from a plane slope, a non-plane slope, and periodic ripples. It is shown that, when only the bottom slope is concerned, the mild-slope equation can give accurate results up to a slope of 1 in 1 rather than 1 in 3, which, until now, has been known as the limiting bottom slope for its proper application. It is also shown that the bottom curvature term plays an important role in modeling wave propagation over a bottom topography with relatively mild variation, but, where the bottom slope is not small, the bottom slope square term should also be included for more accurate results.

  • The flexural and horizontal shear behavior of overlaid concrete slabs is investigated in the present study. An experimental program was set up and several series of overlaid concrete slabs have been tested to study the effect of different surface preparation ; and dowels between old slab and overlay under service load. The present study indicates that the overlaid concrete slabs behave integrally with existing bottom slabs up to yield range for rough and doweled joints.

  • This study empirically examined the effects of data warehousing methodology on the success of data warehousing. Research model and two hypotheses were set up to identify the relationships among two types of data warehousing methodology(top-down/bottom-up, dw configuration) and success based on the investigations of such theories as data warehousing, data warehousing methodology, IS success and so forth. And the survey instrument(questionnaire) was developed to collect data. Ultimately 183 questionnaires from 61 korean firms were collected. Findings showed that two types of data warehousing methodology affect significant effect on the success of data warehousing.

  • This paper is to propose new features and models for process innovation after classifying in three categories ; conventional six sigma, lean six sigma and 3rd generation six sigma. First considering two project types which are bottom-up and tod-down, DMAIC process is linked up with QC story 15 steps. Secondly, I present Koreanized lean six sigma model using Japanese production technology and principles. Lastly, this paper also depicts a new 3rd generation six sigma model utilizing MBNQA management quality system.

Remediation of Water Quality Using Up-Flow Circulation Apparatus(UFCA) in a Reservoir (용승순환장치를 이용한 호소의 수질개선에 관한 연구)

  • This study was conducted to analyse the effects of the UFCA for treating polluted water in a reservoir. The UFCA mixes water by circulation of surface and bottom water layers. The circulation supplies oxygen to bottom of the reservoir, resulting in water quality improvement. With a UFCA in use, we surveyed the changes of temperature, pH, transparency, depth, conductivity, DO, COD, BOD, T-N, T-P and Chlorophyll-a for 7 months from Feb. to Aug. in 2004 in our experimental reservoir. There was little difference in the surface and bottom temperatures of the reservoir because of water mixing by the UFCA. However, pH was changed from 7 to 9. The transparency of water was about 80 cm through the all periods. Conductivity was $150\;{\mu}S/cm$ in early Feb., but increased to $270\;{\mu}S/cm$ in early March. Little change was seen in DO with depth, but it was maintained above 6 mg/l in June and July. BOD increased from 2.1 to 12.2 mg/l. The study reservoir did not undergo any eutrophication during the period of our experiment, but the comparison reservoir had an algae-bloom. The COD in the experimental reservoir increased from 5.4 to 14.5 mg/l. The COD concentration of the experimental reservoir was higher than comparison reservoir at the beginning of the study but in August this situation was reversed. SS concentration increased from 13.5 to 23.5 mg/l in Feb., but it fell from between 8.5 to 11.2 mg/l in July. T-N was increased from 1.3 to 4.9 mg/l. It increased up to 3 times in the rainy season as compared to other components. However the comparison reservoir increased up to 40 times higher than the experimental reservoir in the same period. T-P increased from 0.04 to 0.17 mg/l. The ratio of T-N to T-P increased from 20:1 to 40:1 which means that T-P was a growth limiting factor for algae and aquatic plants. Chlorophyll-a increased from 20 to 120 mg/l, and its concentration was correlated with T-P, such that Chlorophyl-a concentration increased with increased of T-P concentration. The concentrations of COD, T-N, T-P and other parameters were higher in the experimental reservoir than in the comparison reservoir but this situation was reversed in July, when the most severe eutophication occurred. The results show that overall the experimental reservoir was greatly remedied by UFCA. The UFCA accelerated the degradation of aquatic organic materials through effective supply of air with up-flow and circulation of water. We conclude that the UFCA can be very effective in aspect of the remediation of water quality incontaminated reservoirs and lakes.

The Movement Characteristics of Suspended Solid(SS) in Harbor Construction(I) - In a Riprap Work - (항만공사시 부유토사의 거동 특성에 관한 연구(I) -사석공사시-)

  • This paper intends to analyze the movement characteristics of the suspended solid(SS) generated in a riprap work which is the most widespread process in harbor construction. The generation and movement behaviors of SS are investigated when 1) basic ripraps (0.001~0.03㎥) are cast for breakwater construction, 2) ripraps are loaded on carrier for riprap casting in the sea-wall construction of dredged material pocket, and 3) ripraps for inner filling are cast. The result of the present study shows that the SS generation is considerable when basic ripraps are cast for breakwater construction and the SS diffusion reaches up to 500m in high tide period. When ripraps are loaded on carrier, the SS is generated by the ripraps running down into sea from carrier, thereby showing a high concentration of SS in the vicinity of carrier. In the surface layer, SS distribution is confined in the vicinity of the carrier due to the prompt sinking of the SS. However the SS in the bottom layer is diffused approximately up to loom and then the SS concentration reaches to the background one. In the riprap (less than 0.03㎥) casting for inner filling using pork crane, SS is diffused approximately up to 300m in the surface layer and more than 300m in the bottom layer. It is quite interesting that the movement of the SS generated in harbor construction is mainly controlled by wind drift in the surface layer and tidal current in the bottom layer, and also the diffusion range of SS in the bottom layer is much larger than that in the surface layer.
