A Study on Effect of Individual Education about Chemical Therapy for Preventing Cancer on Quality of Life of Patient Who Suffers From Cancer (항암화학요법에 관한 개별교육이 암 환자의 삶의 질에 미치는 효과)

  • The disease occurrence rate of the cancer is rapidly increased and It is becoming the main factor for the death The chemical therapy for preventing the cancer is recently used for many patients and thus extended the life of the patients. However, the side effect caused by the medical substances when performing the chemical therapy for preventing the cancer and the consequent mental and social problem are incurred to deteriorate the quality of the life. Therefore, it is needed to help the patient In order to reduce the above problems, and so this study was executed in order to examine the effect of the individual education on the quality of the life of the patients who are treated by the chemical therapy for preventing the cancer. The study was performed from Jan. 19. 2004 to Apr. 18. 2004 for the patients who were firstly treated by the chemical therapy for preventing the cancer in a university hospital located in the downtown in Seoul. The individual education on the chemical therapy for preventing the cancer was provided to 40 patients of the experimental group. The study plan is the experimental plan before and after the sole group, and it is the beginning experimental plan. The title of the booklet on the chemical therapy for preventing the cancer is "Cancer, The more you know, the more you can be cured", and the book was composed up of the side effect of the chemical therapy for preventing the cancer, treatment way for the side effect, and guide of the daily life. The survey with the questionnaire sheet was distributed to the experi- mental group before the education, the survey sheet was made out when they were hospitalized for 3-4 weeks after the education, and the data were examined by using SPSS statistical program with making our the survey questionnaires and the change of the quality of the life before and after the education of the patients who are treated by the chemical therapy for preventing the cancer was analyzed by using the parred t-test. The research result was verified that it has the meaningful that the quality of the life for the physical field, mental field, social field, and spiritual field after the individual education. The suggestion concluded by the above research result is as follows. First, there is the limitation to interpret the result since it was the beginning experimental plan for the sole group. Thus, it is suggested that the similar experimental plan should be executed with the expansion of the research subject and also with the contrast group. Second, it is suggested that the study on the change of the quality of the lifeaccording to the support of the individual education and family of the patients who are treated by the anti-cancer therapy.

Health Risk Behaviors Affecting the Process of Pregnancy (임신경과에 영향을 미치는 건강위험행위)

  • Comprehensive review of the literature was conducted to determine 1) selected risk factors and its impact that affect pregnancy outcome such as smoking, alcohol consumption, and substance abuse 2) these factors can facilitate future strategies for health promotion and prevention for both pregnant women and fetus. Review of literature were extracted from searching MEDLINE(1966 - Oct. 2000). CINAHL (1982 - Oct. 2000) and the domestic literature. The following factors were identified: 1. The effects of risk behaviors on pregnancy. ${\cdot}$Maternal smoking was associated with the occurrence of premature or LBW delivery, fetal growth retardation, extremities defects, heart defects and sudden infant death syndrome. ${\cdot}$Maternal alcohol consumption was associated with spontaneous abortion, premature or LBW delivery, morphologic/neurologic problems, especially fetal alcohol syndrome. ${\cdot}$Heroin was associated with withdrawal after birth in which were born to heroine addicts for gestational age and lung maturation in animal studies. ${\cdot}$Cocaine was associated with spontaneous abortion, abruptio placenta and a poor response to environmental stimuli. ${\cdot}$So far, the effects of caffeine on pregnancy was controversial, but severe caffeine consumption was associated with premature or LBW delivery, spontaneous abortion, still birth and dystocia. 2. Intervention methods and its effects identified were as follows ${\cdot}$Conducted intervention for smoking, alcohol and drug consumption were single or combined. ${\cdot}$Intervention methods were counseling, phone contact, mailing, use of educational videotape, booklet, support person and alternatives such as nicotine patch. ${\cdot}$The interventions increased the rates of smoking cessation during pregnancy and awareness of the risk of drug consumption, and decreased amount of alcohol consumption. ${\cdot}$The intervention outcome found positive effect on birth weight and length. 3. Our recommendations were as follows ${\cdot}$The personal and social cognition should be enhanced through education and the mass media. ${\cdot}$It's necessary to educate and give information of preconceptional care, planned pregnancy and early prenatal care for optimal pregnancy outcome. ${\cdot}$It's necessary to develop comprehensive assessment tool which is reliable and valid on smoking, alcohol consumption and substance abuse to identify supportive or interventional program.

Effectiveness of Education Interventions for the Management of Cancer Pain: A Systematic Review

  • Background: Many cancer patients experience poor pain control due to various factors, including misconceptions regarding the use of opioid analgesics. For management of cancer pain, interventions involving education of both patients and physicians have been attempted. Objectives: This review aimed to assess the current evidence of the benefits of education for the management of cancer pain. Methods: We searched the Medline, EMBASE, Cochrane library, and major Korean databases to identify relevant studies. We included most study designs, but excluded case series. The primary outcomes were pain intensity and quality of life (QoL). Two reviewers assessed the risk of bias using the Cochrane's tool for RCT and Risk of Bias Assessment tool for Non-randomized Studies (RoBANS) for non-randomized studies, independently. Results: After extensive searches, 3,324 publications were screened, and 32 studies were selected. The education interventions used in the included studies included a wide variety of education methods, but the most common method was a booklet produced for patients. Regardless of the education method used, the results of the meta-analysis were as follows. The SMDs of the most severe, average, and current pain in the RCTs were significant. The SMD of worst, average, and current pain were -0.34 (-0.55, -0.13), -0.40 (-0.64, -0.15), and -0.79 (-1.35, -0.23). In the non-randomized studies, the effects on average pain were significant, but those on worst and current pain were not. Conclusions: Education intervention reduced the pain of cancer patients. Therefore, patient education could be considered to be an effective method of cancer pain management. However, our data should be interpreted with caution, and studies using standardized protocols are needed to confirm these observations.

말기암환자 가족에 대한 호스피스 케어의 정보적 지지 제공 효과

  • Cancer is the second leading cause of the death in Korea. Family caregivers of dying patients manifest many psychological and physical symptoms of stress, and they often seek for informational support from health care providers. Unfortunately, however, few systematic studies identify the actual effect of such support on family caregivers. This study, thus, intends to evaluate the effect of informational support for hospice care. One group pretest-posttest design was used, employing the stress-coping model by Cohen and Wills as a conceptual framework. This research was conducted from July 1 to November 15, 1998, initially with 32 subjects sampled from hemato-oncology department of two general hospitals in Seoul, but reduced to 18 at the end due to the untimely death of patients or caregiver's refusal during the course of study. Informational supports were programmed to provide the family caregivers with 8 times of education and counseling as well as 24-hour hot-line for 4 weeks. A booklet that explains the various problems of hospice care was also prepared and distributed to all subjects. Data were collected by using self-report questionnaires and reviewing medical records. The tools used in this study were based on the Weinert's PRQ-II(scale of perceived social support), Spielberger's state anxiety inventory, and CES-D. Also included in the data collection were the general characteristics of family caregivers and patients, and the pain intensity and the performance status of patients. The data were analyzed with descriptive statistics, Wilcoxon sign rank test and paired t-test using SPSSWIN program. The results of the study were as follows: 1.The perceived social support of family caregivers was not significantly increased with informational support for hospice care(t=1.64, one tailed p=.060). 2.The anxiety of family caregivers was significantly reduced following informational support for hospice care(t=3.48, one tailed p=.002). 3.The depression of family caregivers was significantly reduced following informational support for hospice care(t=-2.18, one tailed=.022). 4.The pain intensity of patients with terminal cancer was significantly reduced following informational support for hospice care(t=-2.41, two tailed p=.027). The results suggest that the informational support provided to family caregivers of patients with terminal cancer reduced not only their anxiety and depression but also the pain intensity of patients. Further study is necessary to consolidate the conceptual framework of this study with expanded number of subjects. Nevertheless, it was certain that the informational support program for hospice care was very helpful to both caregivers and patients. Thus, the informational support program is strongly recommended for the hospitals which have no hospice unit yet.

The first discovery of larval Gnathostoma hispidum (Nematoda: Gnathostomidae) from a snake host, Agkistrodon breuicaudn (한국산 살모사에서 최초로 발견한 돼지악구충의 제3기 유충)

  • The present study was performed to observe the infection status of several kind of animals with indigenous Gnnthostomn in Korea, and morphological characteristics of gnathostome larvae detected from pit-viper, Askistronon breuiccudus, for the species bullfrogs. several kinds of snakes, j.e., 55 Elnphe rufodorsata, 2 Dinodon rufozonatum rufzonatum. 62 Rhnbdophis tisrinus tigrinus and 87 Askistronon app., and 438 cats were examined. A total of 21 larval gnathostomes was detected from 12 pit-vipers, A. breuicnudus. They were 2.233 x 0.343 mm in average size and covered with about 210 transverse rows of minute cuticular spines. Their characteristic head bulbs were provided with 4 rows of hooklets of which average numbers in each row were 36.8, 39.0, 41.7 and 44.3. posteriorly. In the cross sections of midgut level, the intestinal wall consisted of a single layer of 19-25 elongate epithelial cells with a single nucleus. SEM observation of the larvae revealed unique features of head bulb , cuticular spines on transverse striations and a cervical papilla. On the basis of above morphological characteristics, they were identified as the advanced third-stage larvae of Gnothostomo hispinum. It was first confirmed that the pit-viper. ABkistronon breuicaunus is the snake intermediate host of G. hispidun.

The Effects of Exercise Therapy Applied in an Efficacy Expectation Promoting Program on Self-Efficacy and Metabolism.- in NIDDM(Non-Insulin Dependent Diabetes Mellitus) Patients - (효능기대증진프로그램을 적용한 운동요법이 자기효능과 대사에 미치는 영향-인슐린 비의존성 당뇨병 환자를 중심으로-)

  • This study was conducted to investigate whether exercise therapy applied in an efficacy expectation promoting program based on the self-efficacy theory of Bandura(1977) would increase self-efficacy and metabolism in NIDDM patients. The study design was a nonequivalent control group pre-test post-test quasi-experimental desist The exercise therapy applied in the efficacy expectation promoting program was composed of a staged exercise program, a small booklet relating personal experience with diabetes mellitus and a telephone coaching program on performance accomplishment. vicarious experience and verbal persuasion, which are all induction modes of efficacy expectation. The subjects of the study were twenty eight NIDDM patients who received follow-up care regularly through the out-patient department of endocrine medicine in one general hospital which had a diabetic clinic. Fourteen were assigned to the experimental group and fourteen to the control group. The experimental group participated in the exercise therapy applied in the efficacy expectation promoting program from three to five times per week for four weeks and the control group did not have the program. The collected data were analyzed using the X²-test, t-test, paired t-test, and Cronbach's Alpha using SPSS /PC/sup +/. The results are summarized as follows 1. Experimental group had higher efficacy score than control group(t=5.98, p=.00). And. There was a significant different in the efficacy score before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(t=-6.42, p=.00). 2. Experimental group did not have lower level of glucose metabolism than control group(FBS : t=.32, p=.75, HbAlC : t=.60, p=.55, pc 2hrs. glucose : t=-.29, p=.78). But, There was a significant different in the aunt of glucose metabolism before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(FBS : t=3.63, p=.003, HbAlC t=4.20, p=.00, pc 2hrs. glucose : t=1.93, p=.001). 3. Levels of lipid metabolism were partly a significant different between Experimental group and control group(triglyceride t=-1.87, p=.07, HDL cholesterol : t=-.29, p=.77. body weight : t=1.78, p=.09, Total cholesterol : t=-2.17, p=.04). And, There was partly a significant different in the amount of lipid metabolism before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(triglyceride : t=2.50, p=.03, HDL cholesterol : t=-.43, p=.67, body weight : t=5.34. p=.00, Total cholesterol : t=2.26, p=.04). In conclusion, it was found that exercise therapy applied in an efficacy expectation promoting program was an effective nursing intervention for increasing self-efficacy and metabolism.

A study on the patient's awareness of oral health in public health center (보건소 내원 환자들의 구강보건 인식도에 관한 조사)

  • There are two purposes in this study. The first one is to collect some oral health information through the investigations upon the oral health care of Korean adults. Referring to the result of the investigation. I wanted to furnish fundamental data with oral health education for each age and with a publicity booklet editing. That is the second purpose of this research. Among the people who visited Seo-gu Public Health Center in Kwang-ju, 207 people answered the questionnaires. There were 86 men and 121 women from the 20-aged to the 59- aged. The questionnaires covers knowledge aquisition path for dental caries prevention, the cognition degrees for the causes of dental caries and its prevention, the cognition degrees for the causes of periodontal disease and its prevention, the degrees for oral health methods, and the importance of oral health. The conclusions are as follows; 1. The cognition degree for dental caries prevention: The cognition for pit and fissure sealant was appeared most highly in thirties by 85.3%. but 62.1 % in fifties was answered, "Never heard". The cognition degree for fluoride application was appeared most highly in thirties by 73.5%. and the cognition degree for water fluoridation was most highly in forties by 54.2%. 2. The knowledge for pit and fissure sealant was acquired mostly through dental hospital in every age by 54.2%. 3. The knowledge for fluoride application was learned mostly through dental hospitals in twenties and thirties by 32.7%. and mostly through TV or radio health programs in forties and fiftieseach by 35.7% and 50.0%. 4. The knowledge for water fluoridation was acquired through TV or radio programs in all ages such as twenties. thirties. forties and fifties. Its rate was 57.8%. 5. The cognition degree of the cause of dental caries: 53.1 % of all ages think that dental caries can most frequently be caused by being lack of toothbrushing. 6. The cognition degree of the cause of periodontal disease: 58.5% people of all ages think that both dental plaque and calculus might be the main cause of periodontal disease. 7. The cognition degree of dental caries prevention: 72.8% people think that dental caries can be prevented by right tooth brushing method. and 8.7% people think that they can be prevented by scaling. However, 10.7% people of them were not interested in dental caries prevention. 8. The importance of oral health: 35.3% people think that teeth health is the most important, and 63.8% people think oral health is one of the most important health problems. Forties answered that tooth health was the most important thing, and fifties. thirties and twenties followed in the order. 9. The cognition for oral health maintenance: all ages(twenties. thirties. forties. and fifties) answered that right toothbrushing method was the best way to keep oral health by 69.1%.

The Short Term Effects of an Early Intervention Program for Alcoholism Prevention in Adults: A Pilot Study (성인대상 알코올중독 예방 조기개입의 단기효과: 예비 연구)

  • The aim of this study was to investigate the short term effects of early intervention on alcohol knowledge, alcohol expectancies, drinking refusal self-efficacy for alcoholism prevention in adults and to examine its applicability in the field. This study was a non-equivalent control group pre-post quasi-experimental design. The participants were currently working, and were 12-25 points on result of the Alcohol Use Disorders Identification Test(AUDIT). The participants were a total of 25 men: 13 in the intervention group and 12 in the control group. The intervention group participated in early intervention program once a week for 4 weeks and the control group received only a booklet of alcohol related information. A chi-square test, t-test, Mann-Whitney U test were analyzed by using IBM SPSS Statistics 20.0. The intervention group had significantly higher the alcohol knowledge(t=1.258, p= .002), the negative alcohol expectancies(t=-1.346, p= .042), and the drinking refusal self-efficacy(t=2.817, p= .010) than the control group. In the future, it is need to apply the criteria of problem drinkers considering cultural characteristics, recruitment of participants, and motivational enhancement intervention using motivational interviewing. Especially, web-based alcohol related information providing program will help raise awareness of problem drinking. Early intervention program needs to be applied in community settings where problem drinkers are easily accessible.

Using Service Design Tools in Community Nutrition Research: A Case Study in Developing Dietary Guidelines for Young Adults (서비스 디자인 도구의 지역사회영양학 분야 활용: 청년 식생활 가이드 개발 사례)

  • Objectives: Recent epidemiological data reported that young adults in their 20 ~ 30s are a vulnerable population with unhealthy dietary practices and a few signs of deteriorated health indicators. However, there are no dietary guidelines that are specifically developed for the young adult population. This study introduces some data collection tools that are mostly used in the service design field, and demonstrates how these tools can be used in nutrition research for developing dietary guidelines for specific target groups. Methods: To understand the context of food choices among young people, 39 people were enrolled to complete a probes booklet. Thematic analysis and word cloud were performed to capture the main themes from the probes and a persona was developed based on the findings. Results: Data from the probes enabled us to grasp the various contextual meanings of eating practices among young people. Most participants understand what a healthy diet is and often have a willingness to practice it. However, there were very few participants who were following the practices. We created four types of persona for developing dietary guidelines: healthy eating, emotional eating, convenient eating, and trendy eating. Conclusions: Probes and persona were used in order to understand the lives of young adults and develop targeted messages. We hope that this introduction will be helpful to researchers who are looking for new ways of understanding their target population in the field of community nutrition.

A Study on the Hyun-Mu Sutra(玄武經) of Jeungsan (증산계 『현무경』 연구)

  • In this study, source criticism (an establishment of authentic text) of the Hyun-Mu Sutra(玄武經) among different editions is studied and an attempt of a new interpretation appropriate to that is attempted. The Hyun-Mu Sutra, a scripture written in 1909, began to communicate with the world through the religions of Jeungsanism. In particular, it was remarkable that The Hyun-Mu Sutra was absorbed as canon textbooks Jeonkyung(典經), the Scriptures of Daesoonjinrihoe, The Fellowship of Daesoon Truth(大巡眞理) from a loner and secret pull-out of heritage traditions. However, this scripture though written in 1909 and more than 100 years has passed, remained in a state unestablished authentic text. The Hyun-Mu Sutra is the scripture consisted of 25 pages by the religions of Jeungsanism[Gang Il-sun 姜一淳(1871~1909)]. 33 page type of Hyun-Mu Sutra has been distributed in the world until now the authentic text of The Hyun-Mu Sutra. However, as a result of the examination, diagnostic scripture(病勢文) was found to have been added by descendants. After a review of authentic text of The Hyun-Mu Sutra, it concluded that there is no diagnostic scripture in primary The Hyun-Mu Sutra. Though The Hyun-Mu Sutra is a booklet of a small amount, the notation and expression is so unique, it has been in secrecy to read its contents. Interpretation way of The Hyun-Mu Sutra up to now can be summarized in two as follows. 1) approaches by I-ching 2) approaches by ten celestrial stemps and twelve earthly branches(10干12支). Approaches by I-ching among this sometimes was supplemented with Buddhist classification methods. Nevertheless, these studies can be evaluated limited because it fails to secure authentic text of The Hyun-Mu Sutra. In this study, the contents of The Hyun-Mu Sutra was examined itemized by focusing on the following four points. 1) The icon of The Hyun-Mu Sutra(玄武經符) is similar as normal talisman(符籍) but it has other features. 2) 'Reverse Fonts'(反書體)[the opposite view of the standard fonts(正書體), reflected in the mirror fonts] and size or location used in text is not in uniform. 3) letters in scripture were pointed and points were stamped in the left and upper and lower characters. 4) "Spiritual poem" (詠歌, the Korean traditional music with a view of elegance as an origin of eco), and the music with the Five-Sounds[宮Gung, 商Sang, 角Gak, 徵Chi, 羽Wu) were related. As a result, content analysis of The Hyun-Mu Sutra is carried out in the next four points. 1) The icon of The Hyun-Mu Sutra (玄武經符) has been primarily developed by Jeungsan. 2) 'Reverse Fonts'(反書體)[the opposite view of the standard fonts(正書體), reflected in the mirror fonts] and reverse location such as '宙宇' [the reverse of '宇宙'] represents based on a new world based on a forward and reverse I-ching(正易). 3) Dot and neighbor points is a symbolic map that guides the position of lateral new world(後天) and era(人尊) 4) Spiritual poem is the entrance to achieve the Realization of Do(道通). The above can be considered as the results of this study.
