성견 하악골에 식립한 다양한 임플란트 표면에서의 골 치유 양상에 관한 연구 (The Study of Bone Healing Response According to the Various Surfaces of the Implant in the Mandible of the Mature Dog)

  • 성공적인 임플란트 시술을 위해 최근에는 다양하게 표면 처리된 임플란트가 소개되고 있으며, 이러한 표면 처리가 임플란트 표면에서 세포 반응에 영향을 준다는 많은 보고가 있다. 본 연구는 성견 하악골에 현재 사용 중이거나 개발 중인 다양하게 표면 처리된 8종류의 임플란트를 식립하여 조직학적 및 조직계측학적 분석을 함으로써 임플란트 표면 처리 방법이 골 치유에 어떤 영향을 주는지 비교 평가하고 임플란트 식립 후 시기에 따른 골내 치유 양상을 알아보고자 하였다. 8가지의 서로 다르게 표면 처리된 임플란트를 이용하여 총 72개의 임플란트를 9마리의 성견 하악골에 식립하였고, 2주, 4주, 8주에 각각 3마리씩 희생하였다. 골편을 절단 및 처리하여 시편을 제작한 후 조직학적 분석을 하였으며, 조직계측학적 분석으로 bone to implant contact(BIC)를 측정 비교하였다. 조직학적 분석 결과 전반적으로 골 형성이 좋은 상태로 2주의 실험군에서도 임플란트의 인접부위에서 상당량의 골 형성을 볼 수 있었다. 4주의 실험군에서는 골 형성이 전반적으로 관찰되었으나, 신생골은 기저골과 구분되었고, 8주 실험군에서는 신생골이 성숙되어 기저골과 잘 연결되어 있었다. 8가지 서로 다른 임플란트에서 표면에 따른 차이는 보이지 않았다. 조직계측학적 분석 결과 2주군에서 4주군과 8주군에 비해 BIC가 유의할만하게 낮게 나타났으며 4주군과 8주군 사이에는 유의성 있는 차이를 보이지 않았다, 또한 표면 처리에 따른 차이도 보이지 않았다.

  • 급성 견관절 탈구에 동반된 골성 반카르트 병변은 골편과 상완인대의 정확한 해부학적 정복과 안정된 고정이 치료에 필수적이며, 이의 실패는 견관절 재발성 불안정성과 외상 후 관절염을 유발할 수 있다. 반카르트 병변에 큰 뼈 조각 또는 분쇄된 골절 파편이 있는 경우, 관절경 수술 시 제한된 시야와 조작의 어려움으로 큰 골편을 골절 부위에 정확하고 견고하게 고정하는 것이 어렵다. 개방형 시술 시는 골편에 도달하고 고정하기 위하여 광범위한 연부 조직 유리 및 절제가 필요하여 이는 신경과 혈관 합병증, 견갑하건의 약화 및 견관절 강직 등, 외과적 접근법과 관련된 병적 상태를 유발할 수 있다. 저자들은 관절경적 수술이 용이하지 않은 크기의 관절와 골절이 동반한 견관절 전방 탈구 환자에서 골절면에 봉합나사 고정 후 골 터널을 만든 골편에 봉합사를 통과한 후 골편 및 상완인대를 함께 매듭 하여, 해부학적 위치로 안정된 정복을 얻는 수술방법을 보고하고자 한다.

Wire or Hook Traction for Reducing Zygomatic Fracture

  • Background: Variable methods have been introduced for reduction of the zygomatic fractures. The Dingman elevator is used widely to reduce these fractures but is inappropriate in certain types of fractures which require atypical traction vectors. We introduce and examine an alternate method of reducing zygomatic fractures using wire and hook traction. Methods: A retrospective study was performed for all zygomatic fracture patients admitted between 2008 and 2014. Medially rotated fractures were reduced by using a wire looped through an intermaxillary screw secured on the medial side of the zygoma. Laterally rotated fractures were reduced using a hook introduced through an infrazygomatic skin incision. Results: No accidental bleeding or incomplete reduction was observed in any of the cases. Postoperative imaging demonstrated proper reduction immediately after the operation. Follow-up computed tomography study at 1 month after operation also demonstrated proper reduction and healthy union across the previous site of fracture. Conclusion: The hook and wire method allowed precise application of traction forces across zygomatic fractures. The fractured bone fragment could be pulled in the direction precisely opposite to the vector of impact at the time of trauma. Soft tissue damage due to dissection was minimized. In particular, this method was effective in reducing rotated bone fragments and can be an alternative option to using the zygoma elevator.

Avulsion injuries: an update on radiologic findings

  • Avulsion injuries result from the application of a tensile force to a musculoskeletal unit or ligament. Although injuries tend to occur more commonly in skeletally immature populations due to the weakness of their apophysis, adults may also be subject to avulsion fractures, particularly those with osteoporotic bones. The most common sites of avulsion injuries in adolescents and children are apophyses of the pelvis and knee. In adults, avulsion injuries commonly occur within the tendon due to underlying degeneration or tendinosis. However, any location can be involved in avulsion injuries. Radiography is the first imaging modality to diagnose avulsion injury, although advanced imaging modalities are occasionally required to identify subtle lesions or to fully delineate the extent of the injury. Ultrasonography has a high spatial resolution with a dynamic assessment potential and allows the comparison of a bone avulsion with the opposite side. Computed tomography is more sensitive for depicting a tiny osseous fragment located adjacent to the expected attachment site of a ligament, tendon, or capsule. Moreover, magnetic resonance imaging is the best imaging modality for the evaluation of soft tissue abnormalities, especially the affected muscles, tendons, and ligaments. Acute avulsion injuries usually manifest as avulsed bone fragments. In contrast, chronic injuries can easily mimic other disease processes, such as infections or neoplasms. Therefore, recognizing the vulnerable sites and characteristic imaging features of avulsion fractures would be helpful in ensuring accurate diagnosis and appropriate patient management. To this end, familiarity with musculoskeletal anatomy and mechanism of injury is necessary.

Bragg-curve simulation of carbon-ion beams for particle-therapy applications: A study with the GEANT4 toolkit

  • We used the GEANT4 Monte Carlo MC Toolkit to simulate carbon ion beams incident on water, tissue, and bone, taking into account nuclear fragmentation reactions. Upon increasing the energy of the primary beam, the position of the Bragg-Peak transfers to a location deeper inside the phantom. For different materials, the peak is located at a shallower depth along the beam direction and becomes sharper with increasing electron density NZ. Subsequently, the generated depth dose of the Bragg curve is then benchmarked with experimental data from GSI in Germany. The results exhibit a reasonable correlation with GSI experimental data with an accuracy of between 0.02 and 0.08 cm, thus establishing the basis to adopt MC in heavy-ion treatment planning. The Kolmogorov-Smirnov K-S test further ascertained from a statistical point of view that the simulation data matched the experimentally measured data very well. The two-dimensional isodose contours at the entrance were compared to those around the peak position and in the tail region beyond the peak, showing that bone produces more dose, in comparison to both water and tissue, due to secondary doses. In the water, the results show that the maximum energy deposited per fragment is mainly attributed to secondary carbon ions, followed by secondary boron and beryllium. Furthermore, the number of protons produced is the highest, thus making the maximum contribution to the total dose deposition in the tail region. Finally, the associated spectra of neutrons and photons were analyzed. The mean neutron energy value was found to be 16.29 MeV, and 1.03 MeV for the secondary gamma. However, the neutron dose was found to be negligible as compared to the total dose due to their longer range.

  • The purpose of this study was to evaluate the clinical and computed tomographic features of 7 cases of maxillofacial gunshot injuries in the suicidal patients visited the emergency room, Capital Armed Forces General Hospital. The obtained results were as follows : 1. 백악질 열리의 임상 증례 (Clinical case reports of cemental tear)

  • Purpose: Cemental tear is an uncommon form of root fracture that can lead to rapid localized attachment loss. When it is exposed in the periodontal pocket, it should be removed to prevent accumulation of dental plaque and calculus. Material and Methods: 2 patients were diagnosed as a cemental tear and they were treated with conventional flap operation and subgingival curettage. Additional treatments such as bone graft or guided tissue regeneration were not performed. Result: Symptoms subsided after the treatment. Periodontal pocket has been reduced but no gain of clinical attachment was observed. Remnant of cemental fragment remained after curettage. However, periodontal pocket was stably maintained and there was no recurrence. Conclusion: Periodontal attachment loss associated with cemental tear can be successfully treated with conventional periodontal surgical and nonsurgical procedures.

Treatment of a naso-orbito-ethmoid fracture using open reduction and suspension sutures: a case report

  • Naso-orbito-ethmoidal (NOE) fractures are complicated fractures of the mid-face. The treatment of NOE fractures is challenging and a comprehensive treatment strategy is required. We introduce a case of NOE fracture treated with open reduction and suspension sutures. A 28-year-old woman presented with a unilateral NOE fracture. To reduce the frontal process of the maxilla, a suspension suture was made by pulling the fragment using a double arm suture via a transcaruncular incision. The suture thread was placed in the horizontal plane. Another suspension suture on the inferior orbital rim assisted reduction procedure, and they passed through the overlying skin. The reduction alignment could be finely adjusted by tightening the transcutaneous suture threads while checking the degree of bone alignment through the subciliary incision. The two suture threads were suspended using a thermoplastic nasal splint. An additional skin incision on the medial canthal area, which would have resulted in a scar, could be avoided. Four months postoperatively, computed tomography showed an accurate and stable reduction. The patient was satisfied with her aesthetic appearance, and functional deficits were not present.

해부학적 잠김 압박 금속판을 이용한 원위 비골 골절의 치료 (Operative Treatment with Anatomically Preshaped Locking Compression Plate in Distal Fibular Fracture)

  • Purpose: Preshaped Locking compression plate(LCP) has holes with fixed angle between screw and plate and have advantage firm fixation because it has stability of angular and axial deformity. We evaluated usefulness of LCP after open reduction and internal fixation in distal fibular fracture. Materials and Methods: Between December 2011 and May 2012, 23 patients with fracture of distal fibula were followed up at least 12 months underwent open reduction and internal fixation with LCP. There were 15 males and 8 females with a mean age 39.8(20~69) years. According to Danis-Weber classification, there were 20 cases of type B and 3 cases of type C. There were 13 cases of isolated lateral malleolus fractures, 1 case of bimalleolar fracture, 6 cases of trimalleolar fractures and 3 cases of distal tibia fractures with proximal fibula fracture. Intraoperatively, we assessed whether preshaped LCP fit lateral margin of distal fibula or not and evaluated quality of reduction and postoperative complications. The cases were analyzed by radiological bone union time and clinical results according to the criteria of Meyer Results: Of all cases, complete bone union was achieved and average radiological bone union time was 7.3(6~12) weeks. The clinical results were excellent in 18 cases(78%), good in 5 cases(22%). There were 5 cases of plate with 3 holes, 13 cases of plate with 4 holes, 2 cases of plate with 5 holes, 1 case of plate with 6 holes and 2 cases of plate with 7 holes. The average number of screws at proximal fragement was 2.5 and at distal fragment was 3.5. In 14 cases (60.8 %), we needed re-bending of plate because the distance between plate and lateral cortical margin of distal fibula was more than 5 mm at anteroposterior X-ray after reduction. All cases have anatomical reduction and there were no complications of wound infections. There were no complaint about hardware irritation. Conclusion: At fractures of distal fibula,preshaped LCP had a excellent stability although far cortex was not fixed with screw and bending of plate. And there are less complications of hardware irritation and wound problems. But, Some complement would be needed because there were no complete fitting between precontour of LCP and lateral cortical margin of distal fibula.

Association of polymorphisms in bone morphogenetic protein receptor-1B gene exon-9 with litter size in Dorset, Mongolian, and Small Tail Han ewes

  • Objective: The present study was to investigate the association of polymorphisms in exon-9 of the bone morphogenetic protein receptor-1B (BMPR-1B) gene (C864T) with litter size in 240 Dorset, 232 Mongolian, and 124 Small Tail Han ewes. Methods: Blood samples were collected from 596 ewes and genomic DNA was extracted using the phenol: chloroform extraction method. The 304-bp amplified polymerase chain reaction product was analyzed for polymorphism by single-strand conformation polymorphism method. The genotypic frequency and allele frequency of BMPR-1B gene exon-9 were computed after sequence alignment. The ${\chi}^2$ independence test was used to analyze the association of genotypic frequency and litter size traits with in each ewe breed, where the phenotype was directly treated as category. Results: The results indicated two different banding patterns AA and AB for this fragment, with the most frequent genotype and allele of AA and A. Calculated Chi-square test for BMPR-1B gene exon-9 was found to be more than that of p value at the 5% level of significance, indicating that the population under study was in Hardy-Weinberg equilibrium for all ewes. The ${\chi}^2$ independence test analyses indicated litter size differences between genotypes was not the same for each breed. The 304-bp nucleotide sequence was subjected to BLAST analysis, and the C864T mutation significantly affected litter size in singletons, twins and multiples. The heterozygosity in exon-9 of BMPR-1B gene could increase litter size for all the studied ewes. Conclusion: Consequently, it appears that the polymorphism BMPR-1B gene exon-9 detected in this study may have potential use in marker assisted selection for litter size in Dorset, Mongolian, and Small Tail Han ewes.
