Between 1997 May and 2007 May, One hundred and seventeen patients were treated. There were 129 cases of sinus elevation using a lateral window opening procedure and 258 implants placed simultaneously or delayed. The cumulative survival rate of the implants calculated. The implants were evaluated according to surgical site, quality and quantity of bone, graft material, membrane used, the length and diameter of the implant and complications. 1. The 10-year cumulative survival rate of the implants by sinus augmentation using lateral window approach was 96.90%. 2. There was no difference in the survival rate between the implant placed simultaneously with sinus elevation (one stage) and the procedure performed in the delayed procedure (two stage). 3. There was no difference in the survival rate according to the type and amount of graft materials. 4. There was no difference in the survival rate according to the implant site, bone quality and quantity. 5. There was no difference in the survival rate when the $CollaTape^{(R)}$ or Gore-Tex was placed in the window of the lateral wall. 6. There was no difference in the survival rate of the implant length and diameter. 7. The survival rate was as low as 75.00% when there were more than two complications. Implant placement with sinus augmentation using the lateral window approach is a predictable treatment method. Although the vertical height of residual ridge is insufficient and the quality of bone is poor, the normal survival rate of the implants would be expected if an appropriate graft material and membrane is used with greater effort to prevent complications.
The interest in male osteoporosis is increasing as the incidence of osteoporotic fractures has increased not only in female but also in male due to the increase of old age population thanks to the development of medical science and science in general. Therefore, this study is to find factors related to bone mineral density of male older than 40 years old, to investigate the incidence of the male osteoporosis and to provide a basic result for prevention and medical treatment for the male osteoporosis. The incidence of the osteopenia and the osteoporosis at L-spine was 45% and 12.9% respectively and the incidence of them at femur was 51.9% and 7.63% respectively, among male older than 40 years old who took a medical examination. It was higher than the existing study results conducted to male older than 50 years old in USA and Europe. The incidence of them at both of L-spine and femur showed a significant difference depending the age groups. As the age increases, the average bone mineral density decreases at both of L-spine and femur. And as the weight increases and the body mass index is higher, the incidence of the osteoporosis decreases. There was no significant relation with the incidence of the osteoporosis depending on the exercise, the smoking and the drinking, but the number of exercise, smoking and drinking changes the quantity of bone and are factors influencing the bone mineral density of male person.
Decalcification is routinely performed to obtain a pathological diagnosis using bone marrow biopsy. During the decalcification process using a conventional acidic solution, such as HCl, the antigenicity of tissue is damaged. Especially DNA and RNA in the bone marrow are impaired. Hence, there is the need for a standardized decalcification protocol that preserves the antigenicity of tissue. To this end, we compared the effects of two commonly used decalcifiers: Commercial decalcifier (Calcl-Clear Rapid, HCl) and the EDTA (12.5%, pH 7.0). Bone marrow biopsies sampled from 71 patients were decalcified in accordance with the protocols of respective groups-HCI versus EDTA. The differences of decalcification protocols were analyzed with respect to Hematoxylin & Eosin staining, Gomori'sreticulum staining, and immunohistochemical staining and molecular analysis. Immunohistochemical staining used Ki-67, CD20 and CD138 as primary antibodies and molecular analysis was conducted through the DNA concentration analysis, in situ hybridization (ISH) and immunoglobulin heavy chain (IGH) gene rearrangement. On the routine histopathology analysis, there was no difference between HCl and EDTA. Moreover, in case of immunohistochemical staining, the cytoplasmic membrane or cytoplasmic CD markers was well preserved. However, nuclear proteins, such as Ki-67, were stained with low quality. Conversely, according to the molecular analysis, the EDTA protocol preserved the DNA and RNA compared with the HCI. The differences of DNA quantity and quality were statistically significant between protocols of HCl and EDTA. We used 38 cases in HCl and 12 cases in EDTA. Consequently, the EDTA protocol maintains the antigenicity of the protein on tissue and is acceptable for examination with molecular base analysis. Decalcification of bone marrow biopsy by EDTA is highly recommended for the examination of immunohistochemical staining and molecular analysis.
Purpose: Developments in micro/macrostructures of implants and surgical techniques brought out stable outcomes of implant dentistry. The aim of this study was to evaluate the distributions of implant patients, the types of implanted sites, and the success or survival rates of various implant systems and to analyze the implant placement done at each specificintraoral site and situation. Materials and Methods: The data of dental implantations collected between 1992 and 2006 at the Department of Periodontology in 00000 University Hospital were analyzed. Results: 1. Largest part of the patients were at the age of 40s and 50s in bothgender who lost their teeth mostly by periodontaldiseases and caries at the posterior intraoral sites as major ones. Bone densities of type II(mandible) and III(maxilla) were likely to be seen with quantity of type B. Lengths of the implants between 10 and 15 mm and wide platform took the largest part. 2. Survival rates of $Implantium^{(R)}(98.8%)$, $Xive^{(R)}(100%)$ and ITI $TE^{(R)}(100%)$ were high when $Frialit-2^{(R)}$ showed 82%(poor bone density area) or 87.2%(combined with additional therapy). $IMZ^{(R)}$ had lowest cumulative survival(67.5%) and success rate(49.4%) amongst all. 3. Replacement with 2 wide or 3 regular platforms showed no significant differences in survival rate and marginal bone loss atmandibular posterior area. In single restoration of mandibular second molar, 5-year success rate of machined surface $Br{\aa}nemark^{(R)}(70.37%)$ was lower than that of rough surface $ITI^{(R)}$ SLA(100%). 4. Replacement of single tooth in anterior area showed high survival rate of 94.5%. 5. The success rates of $Br{\aa}nemark$ Ti-Unite and ITI SLA at posterior maxilla with poor bone density both showed stable outcomes. 6. 10-year cumulative survival rate of implants with maxillary sinus augmentation by lateral window approach appeared to be 96.60%. Low survival rate(75%) was shown when there were more than two complications combined. Height of grafted bone remained stable above the implant apex. Conclusions : Rough surfaced implants showed stable outcomes in most of the situation including poor bone density and additional therapy combined.
Purpose: The aim of this study was to radiographically and clinically compare the effect of extracellular matrix (ECM) membranes on dimensional alterations following a ridge preservation procedure. Methods: One of 2 different ECM membranes was applied during a ridge preservation procedure. A widely used ECM membrane (WEM; Bio-Gide, Geistlich Biomaterials, Wolhusen, Switzerland) was applied in the treatment group and a newly developed ECM membrane (NEM; Lyso-Gide, Oscotec Inc., Seongnam, Korea) was applied in the control group. Cone-beam computed tomography (CBCT) scans and alginate impressions were obtained 1 week and 6 months after the ridge preservation procedure. Results were analyzed using the independent t-test and the nonparametric Mann-Whitney U test. Results: There were no significant differences between the ECM membranes in the changes in the dimension, width, and height of the extraction socket or the quantity of bone tissue. Conclusions: The NEM showed comparable clinical and radiographic results to the WEM following the ridge preservation procedure.

Pharmacotherapy was mainly used to treat osteoporosis. However, some researches showed that pharmacotherapy could induce unexpected adverse effects. Some studies showed that whole body vibration affected beneficially osteoporosis. This paper studied the effect of whole body vibration fur osteoporosis compared with the effect of pharmacotherapy. 10 female rats were used and allocated into 4 group, CON, SHAM, DRUG, and WBV. Rats except SHAM group were ovariectomised to induce osteoporosis. Rats in WBV group were stimulated in whole body vibration at magnitude of $1mm_{peak-peak}$ and frequency 45Hz, for 8 weeks (30 min/day, 5 days/week). Rat in DRGU group was orally administered the Actonel (0.58mg/Kg), for 8 weeks (5days/week). The $4^{th}$ lumbar in rats were scanned at a resolution of $35{\mu}m$ at baseline, before stimulation, and 8 weeks after stimulation by In-vivo micro computed tomography. For detecting and tracking changes of biomechanical characteristics (morphological and mechanical characteristics) in lumbar trabecuar bone of rats, structural parameters were measured and calculated from acquiring images and finite element analysis was performed. In the results, loss of quantity and change of structure of trabecular bone in WBV group were smaller than those in both CON and SHAM groups. In addition, mechanical strength in WBV group was stronger than that in both CON and SHAM groups. In contrast, biomechanical characteristics in WBV group were similar with those in DRUG group. These results showed that reasonable whole body vibration was likely to treat osteoporosis and be substituted partly for drug treatment.

Ochnaflavone is a medicinal herbal product isolated from Lonicera japonica that inhibits cyclooxygenase-2 (COX-2) dependent phases of prostaglandin $D_2(PGD_2)$ generation in bone marrow-derived mast cells (BMMC) in a concentration-dependent manner with $IC_{50}$ values of $0.6{\mu}M$. Western blotting probed with specific anti-COX-2 antibodies showed that the decrease in quantity of the $PGD_2$ product was accompanied by a decrease in the COX-2 protein level. In addition, this compound consistently inhibited the production of leukotriene $C_4 (LTC_4)$ in a dose dependent manner, with an $IC_{50}$ value of $6.56 {\mu}M$. These results demonstrate that ochnaflavone has a dual cyclooxygenase-2/5-lipoxygenase inhibitory activity. Furthermore, this compound strongly inhibited degranulation reaction in a dose dependent manner, with an $IC_{50}$ value of $3.01{\mu}M$. Therefore, this compound might provide a basis for novel anti-inflammatory drugs.
Purpose. The aim of the study was to compare the lingualized implant placement creating a buccal cantilever with prosthetic-driven implant placement exhibiting excessive crown-to-implant ratio. Materials and Methods. Based on patient's CT scan data, two finite element models were created. Both models were composed of the severely resorbed posterior mandible with first premolar and second molar and missing second premolar and first molar, a two-unit prosthesis supported by two implants. The differences were in implants position and crown-to-implant ratio; lingualized implants creating lingually overcontoured prosthesis (Model CP2) and prosthetic-driven implants creating an excessive crown-to-implant ratio (Model PD2). A screw preload of 466.4 N and a buccal occlusal load of 262 N were applied. The contacts between the implant components were set to a frictional contact with a friction coefficient of 0.3. The maximum von Mises stress and strain and maximum equivalent plastic strain were analyzed and compared, as well as volumes of the materials under specified stress and strain ranges. Results. The results revealed that the highest maximum von Mises stress in each model was 1091 MPa for CP2 and 1085 MPa for PD2. In the cortical bone, CP2 showed a lower peak stress and a similar peak strain. Besides, volume calculation confirmed that CP2 presented lower volumes undergoing stress and strain. The stresses in implant components were slightly lower in value in PD2. However, CP2 exhibited a noticeably higher plastic strain. CONCLUSION. Prosthetic-driven implant placement might biomechanically be more advantageous than bone quantity-based implant placement that creates a buccal cantilever.
In this study, we investigated the preventive and therapeutic effects of antler-extract for osteoporosis. Rats were ovariectomized bilaterally and were fed up with Ca- and P-free diet in order to induce osteoporosis. Body weight, organ weight, the weight of femur and bone ash quantity were examined for 5 weeks. We also performed histological and electronical microscopic examinations. 1. After adminstration of female and male antler extract to osteoporosis-induced rats at the doses of 625 and 1250 mg/kg, respectively, the body weights were significantly increased compared with those of normal control group's which was 230.2:t2.3-281.0:t2.5g (p<0.05). 2. The weights of both right and left femur of osteoporosis-induced rats, administered with female or male antler-extract, little decreased compared with those of normal control group. 3. The bone ash quanties of femur of osteoporosis-induced rats, administered with female or male antler-extract, little decreased compared with those of normal control group. 4. The weights of liver, spleen, and kidney of osteoporosis-induced rats, administered with female or male antler-extract, decreased compared with those of normal control group. 5. Histological and electronic microscopical findings were (1) that in normal control rats the connectional of lacunae appeared well and were without loss of bone mineral, (2) that in ovariectomized rats the connections of lacunae were mostly broken and were with loss of bone mineral compared with those of normal control rats, (3) that in osteoporosis-induced rats, administrated with female or male antler-extract, the shape of lacunae and the connections of them were similar to those of normal control rats. These findings suggest a possible protective and therapeutic effects of female or male antler extract against bone loss in ovariectomized rats.

Purpose: To clarify the results of simple bone cyst (SBC) treatment in children by percutaneous autologous bone marrow grafting and xenografting. Materials and Methods: We studied seven cases (4 males, 3 females) of SBC, which were treated by percutaneous autologous marrow and heterograft bone grafting from January 1996 to February 1999. Their mean age at surgery was 10 years (6 to 15), and the mean follow-up period was 35.6 months (20 to 52). Three cases were located in the proximal and middle humerus; three cases were in the proximal femur; and one case occurred in the ilium. Mean volume was 14.7 $cm^2$ (10 to 23). Six cases were active, and one was inactive. Five patients had a history of receiving a mean of 3.2 steroid injections. The mean quantity of bone marrow used in treatment was 14.3 ml (10 to 20), and the mean amount of $Lubboc^{(R)}$ heterograft bone (Transphyto S.A. Clermont Ferrand, France) used was 6.4 blocks (5 to 10). Results were analyzed using the modified Neer classification. Results: Five cases completely healed with obliteration of the cyst cavity (Grade IV). Two cases demonstrated sclerosis around a partially visible cyst (Grade III). All treatment results were satisfactory and without intraoperative or postoperative complications. Conclusione: Percutaneous autologous marrow and heterograft bone grafting is recommended as an effective treatment method for simple bone cyst. It offers ease of operative technique, a high rate of healing, a low recurrence rate, low morbidity, a low incidence of postoperative complications, and free from bone graft donor site problems.
