근관치료를 받은 전치부에서 수복방법과 치조골높이에 따른 응력분석에 관한 연구 (STRESS ANALYSIS OF ENDODONTICALLY TREATED ANTERIOR TEETH BY ALVEOLAR BONE HEIGHT AND RESTORATION METHOD)

  • To study the mechanical behavior depended on the restoration method and alveolar bone height at endodontically treated teeth. a finite element model was made which was applied by four types of restoration methods and alveolar bone height on upper central incisor and then 1 Kg force was applied on each model as follows; 1) $45^{\circ}$ diagonal load on incisal edge. 2) $26^{\circ}$ diagonal load on lingual surface. and 3) horizontal load on labial surface. The author analyzed the displacement and stress of teeth and their supporting tissue by finite element method according to three type of loading conditions. The results were as follows : 1. The displacement by restoration method and the stress in dentin was found greater in restoration without a post than in that with a post. 2. The displacement and stress was found about the same when compared : A) in Resin model and PFM model applied by restoration method without a post and B) in PRC model and CPC model applied by restoration method with a post. 3. The lower alveolar bone height was. the greater was the displacement and stress. 4. The lower alveolar bone height was. the greater slightly was the stress of restoration without a post than in that with a post. 5. The stress in loading condition was the greatest in P1 in dentin and post. and was greatest in P3 in alveolar hone. 6. In the restoration method without a post. stress concentration in labial dentin was distributed to a figure of long belt in adjacent part to periodontal ligament. while in restoration method with a post. it was distributed in adjacent part to post side. And in all types of restoration method stress concentration in alveolar bone was distributed along the compact bone of labial and lingual surface.

Investigation of a pre-clinical mandibular bone notch defect model in miniature pigs: clinical computed tomography, micro-computed tomography, and histological evaluation

  • Objectives: To validate a critical-size mandibular bone defect model in miniature pigs. Materials and Methods: Bilateral notch defects were produced in the mandible of dentally mature miniature pigs. The right mandibular defect remained untreated while the left defect received an autograft. Bone healing was evaluated by computed tomography (CT) at 4 and 16 weeks, and by micro-CT and non-decalcified histology at 16 weeks. Results: In both the untreated and autograft treated groups, mineralized tissue volume was reduced significantly at 4 weeks post-surgery, but was comparable to the pre-surgery levels after 16 weeks. After 16 weeks, CT analysis indicated that significantly greater bone was regenerated in the autograft treated defect than in the untreated defect (P=0.013). Regardless of the treatment, the cortical bone was superior to the defect remodeled over 16 weeks to compensate for the notch defect. Conclusion: The presence of considerable bone healing in both treated and untreated groups suggests that this model is inadequate as a critical-size defect. Despite healing and adaptation, the original bone geometry and quality of the pre-injured mandible was not obtained. On the other hand, this model is justified for evaluating accelerated healing and mitigating the bone remodeling response, which are both important considerations for dental implant restorations.

어성초 추출물 부탄올 분획이 Bone Marrow Stromal Cell 분열능 및 골다공증 랫드에 미치는 영향 (Effects of Houyttnia cordata on Bone Marrow Stromal Cell and Osteoporetic Rat)

  • This study was performed to evaluate the effect of HCB on the bone mass and its related factors in estrogen-deficient animal model. The model rats of osteoporosis showed a significant decrease in bone density, bone ash density, calcium content of femur bone. At the 14th day after ovariectomy-surgery, rats were administered with HCB, extract of Houyttnia cordata, per orally, and continued for 10 weeks. And osteoporosis related parameters were determined to investigate the effect of HCB. Osteoporotic rats showed lower serum estrogen level, higher body weight than normal rats, and showed atrophy of uterine horns.

The influence of screw type and osseointegration ratio on stress distribution in two different endosseous implants

  • The purpose of this study is to examine the effect of partial osseointegration situation on bone loading patterns around two different free-standing screw shaped implants (Nobel Biocare, Gothenburg, Sweden and Degussa-Huls, Hanau, German). Two dimensional axisymmetric Finite element models of two implants(10mm length and 4mm diameter) were created according to different bone quantity, quality and osseointegration ratio in maxilla and mandible bone. At the same time uni-cortical and hi-cortical fixation were analyzed. Generally, full bond case showed less stress than partial bond case in overall area and mandibular model showed less amount of stress than that of maxilla model. Maximum stress of the Branemark implant is higher than that of ANKYLOS regardless of bonding ratio at crestal and apex region. However, more stress concentration was noted in ANKYLOS implant at screw body area especially in mandible. The effect of bicortical fixation on crestal bone stress reduction is dramatical in mandible however, there was no significant effect in maxillary case. The effect of partial bond on stress distribution was more significant at screw body and apex region than in crestal region. Partial bond cases demonstrated greater stress accumulation in trabecular bone than cortical bone. It is concluded that the more accurate model of implant and bone which affects stress and strain distribution is needed to mimic in vivo behavior of implants.

  • A finite element analysis has been utilized to analyze stress and strain fields and design a new configuration in orthopedics and implant dentistry. Load transfer and stress analysis at implant bone interface are important factors from treatment planning to long term success. Bone configuration and quality are different according te anatomy of expecting implantation site. The purpose of this study was to compare the stress distribution in maxilla and mandible accord-ing to implant length and bone engagement types. A three dimensional axi-symmetric implant model(Nobel Biocare, Gothenburg, Sweden) with surrounding cortical and cancellous bone were designed to analyze the effects of bone engagement and implant length on stress distribution. ANSYS 5.5 finite element program was utilized as an interpreting toot. Three cases of unicortical anchorage model with 7, 10, 13 mm length and four cases of bicortical anchorage model with 5, 7, 10 and 13 mm length were compared both maxillary and mandibular single implant situation. Within the limits of study, following conclusions were drawn. 1. There is a difference in stress distribution according to cortical and cancellous bone thickness and shape. 2. Maximum stress was shown at the top of cortical bone area regardless of bone engagement types. 3. Bicortical engagement showed less stress accumulation when compared to unicortical case overall. 4. Longer the implant future length, less the stress on cortical bone area, however there is no difference in mandibular bicortical engagement case.

내측연결 시스템에서 임플란트 고정체의 경부 형태에 따른 지지골에서의 유한요소 응력분석 (Finite Element Stress Analysis in Supporting Bone according to Crest Module Shape of Fixture in Internal Connection System)

  • The external contour of an implant can have significant effects on the load transfer characteristics and may result in different bone failure rates for different implant system. The purpose of this study was to investigate the effects of crest module shape and occlusal load direction on bone failure modes of five commercially available dental implant systems. Five different implant systems with internal connection; ITI (Model 1), Astra (Model 2), Bicon (Model 3), Friadent (Model 4), and Paragon (Model 5), comparable in size, but different in thread profile and cest module shapes, were compared using the finite element method. Conclusively, in the internal connection system of the implant-abutment connection methods, the stress-induced pattern at the supporting bone according to the abutment connection form had differenence among them, and implants with narrowing crestal module cross-sections at the top of the cortical bone created more favorable load transfer characteristics in this region. But it is considered that the future study is necessary about how this difference in the magnitude of the stress have an effect on the practical clinic.

임플랜트-지대주의 연결방법에 따른 임플랜트 보철의 유한요소 응력분석 (FINITE ELEMENT STRESS ANALYSIS OF IMPLANT PROSTHESIS ACCORDING TO CONNECTION TYPES OF IMPLANT-ABUTMENT)

  • Purpose : This study was to assess the loading distributing characteristics of implant systems with internal connection or external connection under vertical and inclined loading using finite element analysis. Materials and methods : Two finite element models were designed according to type of internal connection or external connection The crown for mandibular first molar was made using cemented abutment. Each three-dimensional finite element model was created with the physical properties of the implant and surrounding bone This study simulated loads of 200N at the central fossa in a vertical direction (loading condition A), 200N at the centric cusp tip in a 15$^{\circ}$ inward inclined direction (loading condition B), or 200N at the centric cusp tip in a 30$^{\circ}$ outward inclined direction (loading condition C) respectively. Von Mises stresses were recorded and compared in the supporting bone, fixture, abutment and abutment screw. Results : 1. In comparison with the whole stress or the model 1 and model 2, the stress pattern was shown through th contact of the abutment and the implant fixture in the model 1, while the stress pattern was shown through the abutment screw mainly in the model 2. 2. Without regard to the loading condition, greater stress was taken at the cortical bone, and lower stress was taken at the cancellous bone. The stress taken at the cortical bone was greater at the model 1 than at the model 2, but the stress taken at the cortical bone was much less than the stress taken at the abutment, the implant fixture, and the abutment screw in case of both model 1 and model 2. 3. Without regard to the loading condition, the stress pattern of the abutment was greater at the model 1 than at the model 2. 4. In comparison with the stress distribution of model 1 and model 2, the maximum stress was taken at the abutment in the model 1. while the maximum stress was taken at the abutment screw in the model 2. 5. The magnitude of the maximum stress taken at the supporting bone, the implant fixture, the abutment, and the abutment screw was greater in the order of loading condition A, B and C. Conclusion : The stress distribution pattern of the internal connection system was mostly distributed widely to the lower part along the inner surface of the implant fixture contacting the abutment core through its contact portion because of the intimate contact of the abutment and the implant fixture and so the less stress was taken at the abutment screw, while the abutment screw can be the weakest portion clinically because the greater stress was taken at the abutment screw in case of the external connection system, and therefore the further clinical study about this problem is needed.

Effect of deep transfer learning with a different kind of lesion on classification performance of pre-trained model: Verification with radiolucent lesions on panoramic radiographs

  • Purpose: The aim of this study was to clarify the influence of training with a different kind of lesion on the performance of a target model. Materials and Methods: A total of 310 patients(211 men, 99 women; average age, 47.9±16.1 years) were selected and their panoramic images were used in this study. We created a source model using panoramic radiographs including mandibular radiolucent cyst-like lesions (radicular cyst, dentigerous cyst, odontogenic keratocyst, and ameloblastoma). The model was simulatively transferred and trained on images of Stafne's bone cavity. A learning model was created using a customized DetectNet built in the Digits version 5.0 (NVIDIA, Santa Clara, CA). Two machines(Machines A and B) with identical specifications were used to simulate transfer learning. A source model was created from the data consisting of ameloblastoma, odontogenic keratocyst, dentigerous cyst, and radicular cyst in Machine A. Thereafter, it was transferred to Machine B and trained on additional data of Stafne's bone cavity to create target models. To investigate the effect of the number of cases, we created several target models with different numbers of Stafne's bone cavity cases. Results: When the Stafne's bone cavity data were added to the training, both the detection and classification performances for this pathology improved. Even for lesions other than Stafne's bone cavity, the detection sensitivities tended to increase with the increase in the number of Stafne's bone cavities. Conclusion: This study showed that using different lesions for transfer learning improves the performance of the model.

해면질골에서 다공율, 주파수 및 전파각에 대한 초음파 위상속도의 의존성 연구 (Study on the Dependence of Ultrasonic Phase Velocity on Porosity, Frequency and Propagation Angle in Cancellous Bone)

  • 본 논문에서는 유체에 포화된 다공성 매질에 적용되는 Biot 모델 및 modified Biot-Attenborough (MBA) 모델을 이용하여 해면질골에서 다공율 및 주파수에 대한 초음파 위상속도의 의존성을 예측하였다. 또한 선행 연구자들에 의하여 사람 및 소의 해면질골에서 측정된 값과 비교하였다. 해면질골에서 위상속도는 다공율 및 주파수가 증가함에 따라 감소하는 것을 알 수 있었다. 또한 전파각 효과를 포함하도록 수정된 Biot 모델 및 MBA 모델뿐만 아니라 Schoenberg 모델을 이용하여 해면질골에서 전파각에 대한 위상속도의 의존성을 예측하였다. 본 논문에서 이용된 이론적인 모델들은 초음파와 해면질골의 상호작용에 대한 물리학적인 이해를 증진시키는데 기여할 뿐만 아니라 골다공증을 진단하기 위하여 유용하게 이용될 수 있을 것으로 기대된다.
