지역사회 코호트 여성의 요골과 경골에서의 골다공증 유병률과 관련 요인 분석 (Osteoporosis Prevalence of Radius and Tibia and Related Factors Using Multiple Bone Sites Quantitative Ultrasound Measurement of the Korean Health and Genome Study Cohort Women)

  • This study was conducted to investigate osteopenia and osteoporosis prevalence of radius and tibia using Quantitative Ultrasound (QUS) and to identify affecting factors of osteoporosis. A total of 4,340 women aged 40 - 69 years, living in Ansung (rural) and Ansan (mid-sized) area, and free of illnesses affecting bone metabolism participated in the community-based cohort study. Among them 4,059 subjects measured radius bone density and 4,089 measured tibia. The T-score threshold, defined as < -1.0 and $\le$-2.5, was used to identify subjects with osteopenia and osteoporosis by WHO criteria. The crude prevalence of osteoporosis in radius and tibia was $8.4\%$ and $23.3\%$ respectively; after adjustment for age, it changed $6.3\%$ and $18.8\%$. In simple logistic regression analysis, the prevalence of osteoporosis increased by aging, non-marital status, low education, low income. Otherwise, high intakes of Ca/P, thiamin, riboflavin, vitamin B6, and vitamin E were decreased osteoporosis prevalence. Compared to the normal BMI (body mass index) group 08.5 $\le$ BMI < 23), the odds ratio (ORs) of the low BMI group (BMI < 18.5), and high BMI groups (BMl25-30, BMI $\ge$ 30) were significantly increased. The OR of osteoporosis decreased across increasing quartiles of intakes of Ca, P and Ca/P. Therefore, maintaining normal BMI and increasing Ca intake and Ca/P ratio may have a beneficial effect on bone health of Korean women.

난소절제쥐에서 Arginine 첨가 식이가 골밀도 및 골대사에 미치는 영향 (Effects of Arginine Supplementation on Bone Mineral Density and Bone Markers in OVX Rats)

  • As far as we know, there were no studies of the effect of L-arginine on bone metabolism in post-menopausal women or ovariectomized rats. The primary objective of the current study was to determine whether arginine supplementation was associated with alterations in femoral and spinal bone mineral density (BMD) and bone markers in ovariectomized (Ovx) rats. Forty female Sprague-Dawley rats were divided into two groups, Ovx and sham groups, which were each randomly divided into two subgroups that were fed control and arginine supplemented diet. All rats were fed on experimental diet and deionized water ad libitum for 9 weeks. Bone formation was measured by serum osteocalcin and alkaline phosphatase (ALP) concentrations. Bone resorption was measured by deoxypyridinoline (DPD) crosslinks immunoassay and corrected for creatinine. Serum osteocalcin, growth hormone, insulin-like growth factor-1 (IGF-1), parathyroid hormone (PTH) and calcitonin were analyzed using radioimmunoassay kits. Bone mineral density (BMD) and bone mineral content (BMC) were measured using PIXImus (GE Lunar Co, Wisconsin, USA) in spine and femur. The serum and urine concentrations of Ca and P were determined. The plasma was analyzed for arginine. Diet did not affect weight gain, mean food intake, and plasma arginine concentration. Urinary Ca excretion was decreased by arginine supplementation in Ovx rats, but statistically not significant. The Ovx rats fed arginine-supplemented diet were not significantly different in ALP, osteocalcin, crosslinks value, PTH, calcitonin and IGF-1 compared to those fed control diet. The arginine-supplemented group had significantly higher serum Ca and growth hormone than control group. Spine and femur BMD were significantly increased by arginine supplementation on 5th and 9th weeks after feeding. Our findings indicate that dietary L-arginine supplementation decreased bone mineral density loss in Ovx rats. Therefore, dietary arginine supplementation may represent a potentially useful strategy for the management of osteoporosis.

식이 단백질의 종류와 함황아미노산 함량이 성장기 쥐의 골밀도에 미치는 영향 (The Effect of Dietary Protein Source and Sulfur Amino acid Content on bone Metabolism in Growing Rats)

  • This study was performed to evaluate the effect of dietary protein source and sulfur amino acid content on bone metabolism in ra. Thirty male rats (body weight 145$\pm$2g) were divided into three groups. The rats in the first group were fed on casein 20% diet as animal protein source and those in the second group were fed on soy 20% diet as plant protein source. Sulfur amino acid ratio of these group was 1.07:1. The rats in the third group were fed on soy 20% diet and the sulfur amino acid were supplemented with the amount contained as much in the soy 20% diet. All rats were fed on experimental diet and deionized water ad libitum for 9 weeks, The total body, spine, femur bone mineral density and bone mineral content were measured using Dual Energy X-ray Absorptiometry Calcium, phosphate, pyridinoline, creatinine in urine and calcium, phosphate, alkaline phosphatase, osteocalcin in serum were measured. During the experimental period, plant protein (soy protein) group had a lower urinary Ca excretion, urine pyridinoline & crosslinks value and had a higher Ca efficiency in total bone and femur bone mineral density than animal protein (casein) group. There were no significant differences in serum calcium, phosphate, alkaline phosphatase and osteocalcin among the three groups of the rats. The findings from this study demonstrated that plant protein (soy protein) is beneficial of bone mineral density because it had a higher Ca efficiency in total bone and femur bone mineral density than animal protein (casein). However, the supplementation of sulfur amino acid on soy results were consistent with prior studies that dietary sulfur amino acid load had a negative effect on calcium balance. The rats fed sulfur amino acid supplementation diet increased urinary calcium excretion and decreased calcium efficiency for total and femur mineral density. 나이가 다른 단계에서 식이단백질 수준이 흰쥐의 Ca 대사에 미치는 영향 (The Effects of Age and Dietary Protein Level on Ca Metabolism in Rats)

  • To study the effects of the age and the dietary protein content on Ca metabolism male rats of 1 month 6 month 12 month of age were fed experimental diets containing 5%, 15% or 50% casein for 4 weeks. Food and ca intake were higher in old rats and in high protein groups. The weight ash and Ca contents of femur and tibia were higher in old rats. The higher dietary protein level resulted in higher skeletal weigh ash and Ca contents. But high protein diet(50% casein) lead to reduced bone mineral density(ash/dry bone weight) and Ca density(Ca/dry bone weight) in 1 month old rats. Low protein diet(5% casein) on the other hand reduced the bone growth even though the bone density was higher in this group. The ill effect of low protein diet was not evident in 12 month old rats. Glomerular filteration rate(GFR) and urinary Ca excretionincreased with age and with dietary protein level especially in 12 month old rats. Serum immunoreactive parathyroid hormone(iPTH) level tended to be higher in aged rats but was not affected by dietary protein level except 1 month old rats where 50% protein group showed significantly higher value. This study showed that the dietary protein level seemed to have different effect on Ca metabo-lism in rats of different age., The low bone density in the high protein group of growing rats may be due to the higher iPTH level and increased urinary Ca. The dietary protein level however had no effects on the bone composition in aged rats even though the higher urinary Ca excretion. In conclusion this study suggests that high protein intake from young may lead to less peak bone mass and to increase the bone loss in later years, which would increase the risk for osteporosis.

Peri-implant bone length changes and survival rates of implants penetrating the sinus membrane at the posterior maxilla in patients with limited vertical bone height

  • Purpose: The aim of this study was to measure the peri-implant bone length surrounding implants that penetrate the sinus membrane at the posterior maxilla and to evaluate the survival rate of these implants. Methods: Treatment records and orthopantomographs of 39 patients were reviewed and analyzed. The patients had partial edentulism at the posterior maxilla and limited vertical bone height below the maxillary sinus. Implants were inserted into the posterior maxilla, penetrating the sinus membrane. Four months after implant insertion, provisional resin restorations were temporarily cemented to the abutments and used for one month. Then, a final impression was taken at the abutment level, and final cement-retained restorations were delivered with mutually protected occlusion. The complications from the implant surgery were examined, the number of failed implants was counted, and the survival rate was calculated. The periimplant bone lengths were measured using radiographs. The changes in initial and final peri-implant bone lengths were statistically analyzed. Results: Nasal bleeding occurred after implant surgery in three patients. No other complications were found. There were no failures of the investigated implants, resulting in a survival rate of 100%. Significantly more bone gain around the implants (estimated difference=-0.6 mm, P=0.025) occurred when the initial residual bone height was less than 5 mm compared to the >5 mm groups. No significant change in peri-implant bone length was detected when the initial residual bone height was 5 mm or larger. Conclusions: This study suggests that implants penetrating the sinus membrane at the posterior maxilla in patients with limited vertical bone height may be safe and functional.

Alveolar restoration following rapid maxillary expansion with and without corticotomy: A microcomputed tomography study in sheep

  • Objective: This study examined bone microstructure restoration after rapid maxillary expansion (RME) with and without corticotomy over multiple retention periods. Methods: Eighteen male Dorper sheep were randomly distributed into three groups (n = 6 each group): group 1, RME with corticotomy on the buccal and palatal sides; group 2, conventional RME treatment; and group 3, no treatment. Post-RME, trabecular bone microstructure and new bone formation were evaluated by using microcomputed tomography (microCT) and histomorphometry after a 4- or 12-week retention period. Intergroup differences in bone quality and bone remodeling were analyzed by using two-way analysis of variance with Bonferroni post-hoc test. Results: The bone volume fraction (bone volume [BV]/total volume [TV]) values relative to the control in groups 1 and 2 were 54.40% to 69.88% after the 4-week retention period and returned to approximately 80% after the 12-week retention period. The pooled BV/TV values of the banded teeth in groups 1 and 2 were significantly lower than those of the control after the 4-week retention period (p < 0.05). However, after the 12-week retention period, the pooled BV/TV values in group 2 were significantly lower than those in groups 1 and 3 (p < 0.05). Histomorphological analysis showed that the new bone formation area in group 1 was approximately two to three times of those in group 2 and control. Conclusions: Corticotomy significantly enhanced the restoration of bone quality after the retention periods for banded teeth. This benefit might result from the increased new bone formation after corticotomy.

방사성골괴사 극복을 위한 피브린지지체의 효용성 평가 (Evaluation the Effectiveness of Fibrinogen to Overcome Bone Radiation Damage)

  • 방사선 치료는 방사선 부작용을 수반한다. 특히 혈관계의 장해를 수반하게 된다. 따라서 방사선의 조사가 이루어진 뼈의 재생부위에는 산소와 영양분 결핍이 발생된다. 결국에는 뼈를 재생할 수 없는 방사성골괴사 (osteoradionecrosis)가 세포 환경적으로 만들어 진다. 전례연구에 따르면 방사성골괴사 상태를 극복하기 위해 골형성 단백질-2 (Bone Morphogenetic Protein-2)를 사용한다. 이번 연구에서는 쥐의 두정부에 방사선 조사 후 뼈의 재생에 가장 많이 사용되는 생체재료인 피브린 지지체에 골형성 단백질-2 를 처리하여 방사성 골괴사 부위에 이식한 후 뼈의 재생능력을 알아보고자 하였다. 또한 몇 주부터 뼈 재생 효과가 발생되는지를 검증하고자 하였다. 실험결과에 따르면 방사선이 조사된 쥐의 두개골 결손모델에서는 4 주초기 뼈 형성 기간 보다는 후반 뼈 형성시기인 8 주가 지나야 뼈 형성의 효과가 발생하는 결과를 얻을 수 있었다. 더군다나 쥐의 두 개부 결함 모델에서 피브린 지지체의 재생 뼈 형성은 결손 조직의 내부에서부터 형성되는 결과를 얻었다.

까마귀쪽나무 열매 추출물의 면역증가 및 뼈 건강 효과 (Immunostimulatory and Bone Health-Promoting Activities of Litsea japonica Fruit Extract)

  • 본 연구는 까마귀쪽나무 열매 추출물이 대식세포인 RAW264.7세포에서의 면역증가 효능과 조골세포인 MC3T3-E1 세포에서 뼈건강 효능에 미치는 효과를 확인하고자 하였다. 면역증가 효능을 알아보기 위해서 까마귀쪽나무열매 열수추출물(LJF-W)과 70% 에탄올추출물(LJF-70E)을 저농도(10 ㎍/mL), 중농도(100 ㎍/mL) 및 고농도(1,000 ㎍/mL)로 각각 사용하였고, 양성대조군으로는 홍삼 진세노사이드(Rg1+Rb1+Rg3 5.5 mg/g)를 이용하여 실험을 진행하였다. 실험 결과 열수추출물(LJF-W)과 70%에탄올추출물(LJF-70E) 처리군에서 NO 생성량이 무처리구 그룹에 비하여 통계적으로 유의하게 증가하였다. 염증성 cytokine인 TNF-α, IL-6, IL-1β의 생성량은 열수추출물(LJF-W)에서 무처리구에 비해 통계적으로 유의하게 증가하였으나, 70% 에탄올추출물(LJF-70E)에서는 차이가 없음을 관찰할 수 있었다. 또한 뼈건강 효능을 확인하기 위해서 까마귀쪽나무열매 추출물에 의한 조골세포인 MC3T3-E1세포의 세포증식능을 확인한 결과, 열수추출물과 70%에탄올추출물 처리군에서 조골세포 증식능이 무처리구에 비하여 통계적으로 유의하게 증가하였다. 이 결과를 통해 까마귀쪽나무열매 열수추출물(LJF-W)이 면역증가와 뼈건강에 대한 효능성분으로 가능성이 있음을 확인하였다.

콩단백질이 성장기 수컷흰쥐에서 골함량과 골밀도에 미치는 영향 (Effects of Soy Protein on Bone Mineral Content and Bone Mineral Density in Growing Male Rats)

  • 성장기 수컷 흰쥐를 대상으로 콩단백질과 콩의 isoflavones의 함량이 골밀도 및 골함량에 미치는 효과를 알아본 결과는 아래와 같다. 1) 식이 효율은 대조군인 casein군에 비하여 콩단백질군이 낮았다. 2) 단백질의 종류나 isoflavones의 유무에 따른 혈 중이나 뇨 중 Ca과 P의 함량에는 유의적인 차이가 없었다. 3) 체중 당 총골밀도는 두 군의 콩단백질군이 대조군 보다 높았고, 콩단백질 내에서는 isoflavones를 함유한 군이 더 높았다. 4) 체중 당 spine의 골밀도나 골함량은 두 군의 콩단백질군이 대조군 보다 높았고, 콩단백질 내에서는 골밀도는 isoflavones를 함유한 군이 유의적으로 더 높았다. 5) 체중 당 femur의 골밀도는 콩단백질군이 대조군 보다 높았고, 콩단백질 내에서는 isoflavones를 함유한 군이 더 높았으나 통계적 유의성은 없었다. 결론적으로 콩단백질은 체중당 총골함량이나 칼슘함량에 있어서 동물성 단백질인 casein 보다 유익하였고 특히 isoflavones를 함유한 콩단백질은 체중당 총골밀도와 spine 골밀도에서 isoflavones 함량이 낮은 콩단백질에 비하여 더 유익하여 콩단백질과 isoflavones은 성장기 수컷 흰쥐에서 골밀도 향상에 유익함을 확인하였다. 따라서 성장기에 콩의 섭취는 casein 단백질에 비하여 골밀도나 골함량을 높이는데 유익하며 특히 isoflavones를 함유한 콩의 섭취는 골밀도를 높이는데 더욱 유익하다고 사료된다.

Rutin Improves Bone Histomorphometric Values by Reduction of Osteoclastic Activity in Osteoporosis Mouse Model Induced by Bilateral Ovariectomy

  • Objective : Osteoporosis is a disease of unbalanced bone metabolism that results in low bone mineral density with increased bone fragility and propensity for fractures. The increased rate of bone fracture due to osteoporosis places a significant burden on public health care expenditures. Therefore, numerous studies have been designed and performed to identify the drugs or health foods that can improve the bone quality or quantity. This study was designed to evaluate and analyze the therapeutic effects of rutin on histomorphometric values of the spine and femur in an osteoporotic mouse model induced by bilateral ovariectomy. Methods : Thirty female ICR mice (8 weeks old) underwent either a sham operation (only abdominal incision, sham group, n=10) or bilateral ovariectomy (n=20). The ovariectomized (OVX) animals were randomly divided into two groups : untreated OVX group (OVX-C, n=10), or rutin-administered group (OVX-R, n=10). The OVX-C group received weight-adjusted doses of saline vehicle and the OVX-R group received 50 mg/kg of rutin intraperitoneally, starting 1 day after surgery. At 4 and 8 weeks after surgery, serum estrogen, osteocalcin, alkaline phosphatase (ALP), and the telopeptide fragment of type I collagen C-terminus (CTX-1) were analyzed. Interleukin (IL)-1β, IL-6, IL-10, and tumor necrosis factor (TNF)-α were also analyzed. Bone histomorphometric parameters of the 4th lumbar vertebra and femur were determined by micro-computed tomography. Results : In OVX-C group, ALP, osteocalcin, CTX-1, IL-1β, IL-6, and TNF-α levels were significantly increased at 4 and 8 weeks compared to sham operation group. Rutin administration after OVX statistically significantly reduced ALP, CTX-1, IL-1β, IL-6, and TNF-α levels at 4 and 8 weeks. Rutin administration also improves bone histomorphometric parameters including trabecular bone volume fraction, trabecular thickness, and trabecular number. Trabecular separation was also decreased in OVX-R group compared to OVX-C group. Conclusion : The present study demonstrated that rutin has therapeutic effects on improving bone histomorphometric values in an OVX mouse model. The improvement in histomorphometric values may be associated with the reduction of osteoclastic activity via inhibition of IL-1β, IL-6, and TNF-α. In future studies, the mechanism for the effect of rutin on osteoporosis should be demonstrated more clearly to use rutin in human osteoporosis.
