Role of vitamin D for orthodontic tooth movement, external apical root resorption, and bone biomarker expression and remodeling: A systematic review

  • Objective: This systematic review aimed to evaluate the correlation between vitamin D levels and the rate of tooth movement, external apical root resorption, bone biomarker expression, and bone remodeling. Methods: Three databases (PubMed, Scopus, and Web of Science) were systematically searched from inception until 14th March 2023 to identify studies investigating the correlation between orthodontic tooth movement and vitamin D in animals and humans. The quality assessment was made in accordance with the Joanna Briggs Institute Critical Appraisal Checklist. Results: Overall, 519 records were identified, and 19 were selected for the qualitative synthesis. Eleven studies investigated the effect of local administration (injections in the periodontal ligament, to the gingiva distal to the teeth, or submucosae palatal area) and systemic administration (oral supplementation) of vitamin D on tooth movement, external apical root movement, pro-inflammatory cytokines, and bone remodeling factors. The remaining eight studies investigated the correlation between serum vitamin D levels and salivary vitamin D levels on bone turnover markers and tooth movement. Conclusions: The findings of this systematic review support that vitamin D3 local injections might increase the rate of tooth movement via the receptor activator of the nuclear factor-kB/osteoprotegerin axis. However, the non-uniform study designs and the different protocols and outcome methods make it challenging to draw reliable conclusions.

Relationship among Bone Mineral Density, Body Composition, and Metabolic Syndrome Risk Factors in Females

  • Osteoporosis is a disease that increases the fracture rates and a major cause of increased mortality and morbidity in the elderly people. This study is to determine which components of body composition and metabolic syndrome risk factors are important to bone health, we analysed the relationship among bone mineral density (BMD), body composition and metabolic syndrome risk factors in females. Totally 630 females participated in a medical check-up program (mean age 47 years) were selected for this study. Body composition analysis was performed by segmental bioelectrical impedance method, muscle mass, and percent body fat were measured. We also measured metabolic syndrome risk factors including abdominal obesity, HDL-cholesterol, triglyceride, blood pressure and fasting glucose level. Metabolic syndrome was defined by NCEP-ATP III criteria. The lumbar spine and femoral neck BMD were measured using the dual energy X-ray absorptiometry. Osteopenia and osteoporosis were observed in 180 and 51 persons, respectively. Muscle mass and HDL-cholesterol decreased in osteopenia and osteoporosis groups compared to the control group, and the grade was shown progressively by the symptoms. Significant positive correlation between BMD and muscle mass was observed. Multi variable regression analyses showed that % body fat and muscle mass were independent predictors of BMD after adjustment of age, height and weight. In conclusion, the BMD showed negative correlation with the metabolic and body composition was associated with BMD.

좁은 치조골에 변형된 치조능 분할술/확장술을 이용한 임플란트 동시 식립 : 3 가지 증례보고 (Simultaneous Implant Placement with Modified Ridge Splitting/Expansion Technique in the Narrow Edentulous Alveolar Ridge : 3 Cases Report)

  • The modified ridge splitting/expansion technique combined with guided bone regeneration (GBR) for implant surgery is used to expand the narrow and atrophied edentulous alveolar ridge. Also, the simultaneous implant placement after ridge splitting/expansion technique can reduce the treatment and healing time. This case report includes three patients with a narrow edentulous alveolar ridge of the 2 to 4mm. All three patients underwent a fracture of thin buccal cortical bone plate, and these defects were corrected by the use of the guided bone regeneration (GBR). After 7 to 18 months, all surgical area was stable, and all implant showed a good healing state on the clinical and radiographic examination. In conclusion, though this surgical method is technique sensitive, the modified ridge splitting/expansion technique combined with GBR for implant surgery is recommended for a horizontal augmentation in the narrow edentulous alveolar ridge.

골수공여자들의 불편감 (Discomfort of Donors associated with Bone Marrow Donation)

  • The study was designed to provide the fundamental information for understanding discomfort of bone marrow donors and for promoting an individual comfort by comparing the difference on discomfort between unrelated donors and related donors. The subject of the study was fifty related donors and thirty unrelated donors who was in the C University Hospital. This survey had been carried out and collected from October, 1998 to March, 1999. The scale of discomfort of donors associated with bone marrow donation were assessed by the questionnaire deviced by Kim Sang Dol and amended by the researcher. Data was analyzed by $x^2-test$, t-test, two-way ANOVA, and Pearson Correlation Coefficients. The results were as follows. 1. Considering the general characteristics of bone marrow donors according to gender, male was consisted of $60\%$ and female was consisted of $40\%$. Of those related donors are consisted for $62.5\%$ and accounted for $37.5\%$ of those unrelated donors. Considering the classification to the job, employee of company were major donors which was $35\%$, and next order was student, individual businessman, and housewife. Considering the education level. college students were $48.7\%$ and students who have less than high school level were $42.5\%$. 2. According to the above the results regarding discomfort of bone marrow donation, it is especially shown that the major cause for discomfort of bone marrow donors is on physical factor. The concrete examples for physical factor are pain in the region of bone marrow harvest and pain in the injection part by fluid therapy and blood-sampling, an immovability of the body after bone marrow harvest, and difficulties on walking. Considering physiological factor, there are an uneasiness about leading to injure their health, vague fear about the hospital. and a tedium at hospital. Environmental factors for discomfort of bone marrow donors are insufficient explanation for needle gauge and procedure of bone marrow donation and difficulty on following medical schedule. Therefore. it is necessary to establish more effective and systematically organized program for nursing intervention based on the research results. An effective program is only useful in getting rid of discomfort of bone marrow donors.

Nutritional Quality and Variation of Meat and Bone Meal

  • Meat and bone meal is a valuable protein and mineral source in diets of production animals and contributes to the protein, energy and mineral component of diets. The aim of the present study was to more accurately characterise the apparent ileal amino acid digestibility of meat and bone meals produced in New Zealand and evaluate routine in vitro assays used in practise to measure meat and bone meal quality. A total of 94 commercial meat and bone meals from 25 New Zealand rendering plants over a two and a half year period were analysed for proximates, gross energy, gross amino acid content (incl. hydroxyproline, hydroxylysine and lanthionine), apparent ileal amino acid digestibility, pepsin nitrogen digestibility, protein solubility and bone content. The mean crude protein content of the 94 meat and bone meal samples was 56.8% with a range of >35% units and a coefficient of variation of 9.8%. The mean crude fat and ash content were 10.0 and 28.4% respectively. These latter components showed a large range (16 and 43%, respectively) with coefficients of variation above 22%. Amino acid digestibility between samples was highly variable with lysine and sulphur amino acids digestibility ranging between 45.8-89.0 and 38.2-85.5%, respectively. Pearson correlation coefficients are presented between crude protein content and individual gross amino acids, crude protein content and individual digestible amino acid content, and pepsin N digestibility and individual digestible amino acid content. There was a significant relationship between the digestible amino acid nitrogen content and the crude protein content while pepsin nitrogen digestibility was not correlated to ileal amino acid nitrogen digestibility (r=-0.06). Meat meals with a high protein content had relatively low hydroxyproline and hydroxylysine levels something that was attributed to the levels of collagen from bone. The data indicated that lanthionine (formed upon heat treatment of cysteine with a hydroprotein) is not a good indicator of the heat treatment employed to meat and bone meals. Step-wise multiple regression equations to predict the apparent digestible content of amino acids from rapid in vitro assays are presented. The most selected variables included ash and crude fat content. In general the equations derived for the essential amino acids had a higher degrees of fit (R2) compared to the non-essential amino acids. The R2 for the essential amino acids ranged from 0.43 for histidine and 0.68 for leucine. These equations provide a means of more rapidly estimating the apparent ileal digestible amino acid content (protein quality) of meat and bone meal using standard analyses.

일부대학생의 생활습관과 골밀도의 관계 (The relation between a practical life and a bone mineral density for college students)

  • 청소년기에 형성된 골밀도는 25세에서 35세 전후로 최고치가 되며, 형성된 골밀도는 중년기와 노년기에 감소된다. 골밀도 감소는 골다공증의 발생과 골절 위험성 증가 등을 유발하여 사회적인 건강문제로 대두되고 있다. 골다공증은 사전 예방이 매우 중요하며, 골밀도가 감소되는 중년기 이후보다 골밀도가 형성되고 최고로 발달되는 시기인 청년기에 적극적인 관리와 예방이 중요하다. 따라서 본 연구는 대학생의 골밀도 상태와 생활습관 및 신체활동과의 관계를 파악하여 골밀도 향상을 위한 자료 개발에 도움이 되는 시사점을 도출하고자 일부 대학생 119명을 대상으로 골밀도 측정과 생활습관에 대한 설문을 실시하여 다음과 같은 결론을 도출하였다. 첫째, 대상자의 일반적 골밀도 상태는 정상 88명(73.9%), 골감소증 29명(24.4%), 골다공증 2명(1.7%)으로 정상이 가장 높게 나타났으나 골감소증도 다소 높게 나타났다. 둘째, 성별에 따른 생활습관과의 관계에서는 음주 경험, 흡연경험, 운동 경험이 통계적으로 유의하게 나타났다. 음주경험, 흡연경험, 운동경험이 있는 경우는 남학생 군이 모두 높게 나타났다. 셋째, 일반적 특성에 따른 골다공증 상태와의 관계에서는 성별과 흡연경험이 집단 내에서 통계적으로 유의한 차이를 나타냈다. 성별에서는 남학생에게서는 골감소증이 가장 높게 나타났고, 여학생은 정상이 높게 나타났으며 이는 통계적으로 유의하였다. 흡연경험에 따른 골다공증 상태에서는 흡연경험이 없는 군에서는 정상이 가장 높게 나타났고, 흡연경험이 있는 군에서는 골감소증이 가장 높게 나타났으며 통계적으로 유의하였다.

D대학 체육관 이용 20대 성인의 흡연에 따른 구강건강관련 특성 및 골밀도 비교연구 (A comparative study on oral health-related characteristics and BMD of 20s adults according to smoking status)

  • Objective : This study was to survey the differences of bone mineral density and oral health-related characteristics according to smoking status. Methods : The data for this study were collected by Oral health-related questionnaire and BMD measurements of 20s adults(205) who were using gym in D college areas from July 5th to 9th, 2010. Results : 1. There were a significant difference of 65.4 % of male in smoker group, 59.8% of women in non smoker group(p=0.000). 2. There were a significant difference in subjective oral symptoms among yellow teeth(p=0.000). 3. There were no difference in the T-score of DMB according to smoking. Conclusion : According to the study on smoking, only subjective oral symptoms were shown while the differences at bone mineral density were insignificant. Above findings suggest that further study about the expanding the number and ages of the subjects, smoking, BMD, and oral health related studies is necessary.

Study on the Health-related Quality of Life in Patients after Surgery for Malignant Bone Tumors

  • Aim: We conducted a study in China to assess the health-related quality of life (HRQoL) in patients treated on for malignant bone tumors after surgery, and investigate the possible determinants. Methods: The subjects were 120 patients surgically treated by amputation and limb-salvage for bone tumors during the period of June 2008 to June 2010. The Medical Outcomes Study Short Form 36 (SF-36) was employed to measure the HRQoL of all the patients before and after surgery. Results: With regard to the results of the general quality of life tool (SF-36), we observed a significant improvement of all the indexes of HRQoL after 6 months (p<0.05). PF, RP and BP scores showed significant increase between surgery after 6 and 12 months (p<0.05). The means of the HRQoL of bone tumor patients in our study were still much lower than those of general population in every domain, even 12 months after surgery. Logistic regression showed that female patients were found to have lower scores in physical component summary (PCS) than males (OR=0.64, 95% CI=0.35-0.89). Patients older than 15 years had lower scores in mental component summary (MCS) (OR=0.60, 95% CI=0.32-0.86). Ablative surgery was related to both lower MCS and PCS scores (For MCS, OR=0.54, 95% CI=0.31-0.83; for PCS, OR=0.43, 95% CI=0.25-0.73). Conclusion: Our study showed the treatment for bone tumor could greatly alter the HRQoL of patients. Age, sex and type of surgery were associated with physical or mental HRQoL after surgery.

Coincidence of calcified carotid atheromatous plaque, osteoporosis, and periodontal bone loss in dental panoramic radiographs

  • Purpose: This study was performed to assess the correlation of calcified carotid atheromatous plaque (CCAP), the mandibular cortical index, and periodontal bone loss in panoramic radiographs. Materials and Methods: One hundred eighty-five panoramic radiographs with CCAP and 234 without this finding were evaluated by 3 observers for the presence of osseous changes related to osteoporosis and periodontal bone loss. Chi-squared and Mann-Whitney U tests were used to compare the two groups for an association of CCAP with the mandibular cortical index and periodontal bone loss, respectively. Results: There was a statistically significant coincidence of CCAP and osseous changes related to osteopenia/osteoporosis, with a p-value <0.001. There was no statistically significant coincidence of CCAP and periodontal bone loss. When comparing the 2 groups, "With CCAP" and "Without CCAP", there was a statistically significant association with the mean body mass index (BMI), number of remaining teeth, positive history of diabetes mellitus, and vascular accidents. There was no statistically significant association with gender or a history of smoking. Conclusion: This study identified a possible concurrence of CCAP and mandibular cortical changes secondary to osteopenia/osteoporosis in panoramic radiographs. This could demonstrate the important role of dental professionals in screening for these systemic conditions, leading to timely and appropriate referrals resulting in early interventions and thus improving overall health.

한국인 채식 남성에서 비타민 D 수용체 유전자의 TaqI 다형성의 분포 및 종골의 골밀도와의 관련성 (The Distribution of TaqI RFLP in the Vitamin D Receptor Gene in Korean Vegetarian Men and its Association with Calcaneal Bone Mineral Density)

  • Numerous studies reported the significant association between genetic polymorphisms in the vitamin D receptor (VDR) gene and various bone phenotypes such as bone mineral density (BMD) and bone quality, although conflicting results were produced. The objective of this study was to investigate the association between a TaqI RFLP in the VDR gene and calcaneal BMD in Korean vegetarian men, and its interaction with nutrition status as an environmental factor. BUA (broadband ultrasound attenuation), SOS (sound of speed) and stiffness index of the calcaneus were measured using an ultrasound bone densitometer in 266 Korean men (age: $mean{\pm}SD;\;50.9{\pm}12.0$ year), and a TaqI RFLP in the VDR gene analysed by PCR-RFLP method. In total subjects, the distribution of TT, Tt and tt genotypes occurred with frequencies of 90.8%, 8.8% and 0.4%, respectively. There were no significant associations between this polymorphism and osteopenia-osteoporosis or several bone phenotypes in our subjects irrespectively of nutrition status (P>0.05). Therefore, our results suggest that a TaqI RFLP in the vitamin D receptor gene does not contribute to the susceptibility to the calcaneal BMD in Korean men.
