EFFEECTS OF NON-NEWTONIAN FLUID MODEL ON HEMODYNAMICS IN CEREBRAL SACCULAR ANEURYSMS (낭상 뇌동맥류 혈류유동에서 비뉴우토니안 유체 모델의 영향)

  • The importance of shear thinning non-Newtonian blood rheology on the hemodynamic characteristics of idealized cerebral saccular aneurysms were investigated by carrying out CFD simulations assuming two different non-Newtonian rheology models (Carreau and Ballyk models). To explore effects of vessel curvature, a straight and a curved vessel geometry were considered. The wall shear stress(WSS), relative residence time(RRT) and velocity distribution were compared at the different phases of cardiac cycle. As expected, blood entered the aneurysm at the distal neck and created large vortex in both aneurysms, but with higher momentum on the curved vessel. Hemodynamic characteristics such as WSS, and RRT exhibited only minor effects by choice of different rheological models although Ballyk model produced relatively higher effects. We conclude that the assumption of Newtonian fluid is reasonable for studies aimed at quantifying the hemodynamic characteristics, in particular, WSS-based parameters, considering the current accuracy level of medical image of cerebral aneurysm.

Effect of Chuna on circulatory system - Based on Experimental Chuna Science - (추나의 순환계에 미치는 영향에 대한 고찰 - 실험추나학을 중심으로-)

  • Objectives : Many studies are being done in the method of experimental chuna science in China. The aim of this review is to understand and study the methods of experimental chuna science, focusing on the effects of chuna on circulatory system. Methods : We reviewed "實驗推拿學" and Chinese journals were searched using China National Knowledge Infrastructure(CNKI). Korean journals were searched using 3 Korean web databases(OASIS, NDSL, RISS). Results : By experimental chuna science, effect of chuna on blood pressure, cardiovascular system, hemorheologic system, blood flow velocity, circulation and lymphatic system were proved. Conclusions : Experimental chuna science is necessary for improvement of chuna to a higher level. In Korea, not many experimental studies are being done, especially in chuna and it has very narrow target diseases. For this reason we need to learn and adapt the methods of experimental chuna science of China.

Influence of Wall Motion and Impedance Phase Angle on the Wall Shear Stress in an Elastic Blood Vessel Under Oscillatory Flow Conditions (맥동유동하에 있는 탄성혈관에서 벽면운동과 임피던스 페이즈앵글이 벽면전단응력에 미치는 영향)

  • The present study investigated flow dynamics of a straight elastic blood vessel under sinusoidal flow conditions in order to understand influence of wall motion and impedance phase angle(time delay between pressure and flow waveforms) on wall shear stress distribution using computational fluid dynamics. For the straight elastic tube model considered in the our method of computation. The results showed that wall motion induced additional terms in the axial velocity profile and the pressure gradient. These additional terms due to wall motion reduced the amplitude of wall shear stress and also changed the mean wall shear stress. Te trend of the changes was very different depending on the impedance phase angle. As the wall shear stress increased. As the phase angle was reduced from 0$^{\circ}$to -90$^{\circ}$for ${\pm}$4% wall motion case, the mean wall shear stress decreased by 10.5% and the amplitude of wasll shear stress increased by 17.5%. Therefore, for hypertensive patients vulnerable state to atherosclerosis according to low and oscillatory shear stress theory.

Effects of Voluntary Intrathoracic Pressure Adjustments on Prefrontal Brain Function and Cerebrovascular Dynamics (자발적인 흉강 내압 조정이 전전두엽 뇌기능 변화 및 뇌혈관 역학에 미치는 영향)

  • This study aimed to investigate the effects of voluntary intrathoracic pressure adjustment during the Valsalva maneuver (VM) on changes in prefrontal brain function and cerebral blood flow dynamics using diagnostic ultrasound and near-infrared spectroscopy (NIRS). Sixteen healthy adults performed VM by adjusting their expiratory pressure. Their regional oxygen saturation (rSO2) and oxidized hemoglobin (HbO) levels were measured to confirm changes in prefrontal lobe function. To confirm hemodynamic changes in cerebral blood vessels, this study measured peak systolic velocity (PSV), heart rate (HR), vascular stiffness (STIFF), and pulse wave velocity (PWV) in the common carotid artery before and after the VM. Results showed significant cerebrovascular physiological changes after 30mmHg VM. In particular, PSV increased significantly following VM, whereas PWV and STIFF significantly decreased. A similar trend was observed in 40mmHg VM to 30mmHg, but no significant change was observed except for HR, which showed a significant decrease. Furthermore, rSO2 tended to increase in the prefrontal region after preforming 30 and 40mmHg VM, but it did not show a significant difference. In contrast, HbO significantly decreased after performing 30 and 40mmHg VM. This trend did not show any difference depending on intrathoracic pressure. In conclusion, VM performance at both intrathoracic pressure levels has the same effect on brain function, but induces difference changes in cerebrovascular vessels' physiological function. Thus, at 40mmHg VM, it interferes with effective vascular relaxation due to high intrathoracic pressure. However, 30mmHg VM has an effective effect on cerebrovascular function by causing a significant increase in the elasticity of arterial blood vessels. Such VM performance can effectively improve cerebrovascular function.

  • The Fluorescence-Activated Cell Sorter (FACS) is a well-established instrument used for identifying, enumerating, classifying and sorting cells by their physical and optical characteristics. For a miniaturized FACS device, a disposable plastic microchip has been developed which has a hydrodynamic focusing chamber using soft lithography. As the characteristics of the spatially confined sample stream have an effect on sample throughput, detection efficiency, and the accuracy of cell sorting, systematic fluid dynamic studies are required. Flow visualization is conducted with a laser scanning confocal microscopy (LSCM), and three-dimensional flow structure of the focused sample stream is reconstructed from 2D slices acquired at $1\mutextrm{m}$ intervals in depth. It was observed that the flow structure in the focusing chamber is skewed by unsymmetrical velocity profile arising from trapezoidal cross section of the microchannel. For a quantitative analysis of a microscopic flow structure, Confocal Micro-PIV system has been developed to evaluate the accelerated flow field in the focusing chamber. This study proposes a method which defines the depth of the measurement volume using a detection pinhole. The trajectories of red blood cells (RBCs) and their interactions with surrounding flow field in the squeezed sample stream are evaluated to find optimal shape of the focusing chamber and fluid manipulation scheme for stable cell transporting, efficient detection, and sorting

Correlation between the morning hypertension on ambulatory blood pressure monitoring and the left ventricular mass in children

  • Purpose: Although high morning blood pressure (BP) is known to be associated with the onset of cardiovascular events in adults, data on its effects in children with hypertension are limited. Our retrospective study aimed to define the clinical characteristics of children with morning hypertension (MH) and to determine its associated factors. Methods: We reviewed 31 consecutive patients with hypertension, confirmed by the ambulatory blood pressure monitoring (ABPM). We divided these patients into 2 groups: the MH group (n=21, 67.7%), morning BP above the 95th percentile for age and height (2 hours on average after waking up) and the normal morning BP group (n=10, 32.3%). We compared the clinical manifestations, laboratory results, and echocardiographic findings including left ventricular hypertrophy (LVH) between the groups. Results: The early/atrial (E/A) mitral flow velocity ratio in the MH group was significantly lower than that in the normal morning BP group. In addition, LV mass was higher in the MH group than in the normal morning BP group, although the difference was not statistically significant. The age at the time of hypertension diagnosis was significantly higher in the MH group than in the normal morning BP group (P =0.003). The incidence of hyperuricemia was significantly higher in the MH group than in the normal morning BP group. Conclusion: Older patients and those with hyperuricemia are at higher risk for MH. The rise in BP in the morning is an important factor influencing the development of abnormal relaxation, as assessed by echocardiography. Clinical trials with longer follow-up periods and larger sample sizes are needed to clarify the clinical significance of MH.

  • 본 논문에서는 심전도(ECG)와 SPO2를 이용하여 혈류속도를 측정하였다. 심전도(ECG)와 손가락 검지손가락 끝의 SPO2농도로 얻는 맥파를 동시에 측정하여 두 파형의 피크값의 시간차를 분석하여 혈류속도를 계산하는 방법을 연구하였다. 심전도(ECG)와 SPO2를 동시 측정 분석하는 시스템으로서 정상인 10명과 임상참가자 3명에 대한 심전도(ECG)와 손가락 검지 손가락 끝의 SPO2농도로 얻는 맥파를 동시에 측정하여 비교 분석한 결과 자료에서 보는 것과 같이, 심혈관계 환자들이 일반적으로 혈류 속도가 빠르다는 것일 볼 수 있다. 또한 정상인들 중에서도 남자와 여자의 속도차이가 있음을 알 수 있다.

  • 3차원 위상대조도 기법의 단점인 긴 검사시간으로 인해 임상적 유용성이 떨어진다. 본 논문은 시간매개변수인 병렬영상기법(Grappa) 변경에 따른 혈류정보의 정량적 평가를 통해 차이점을 알아보고 임상적 활용정도를 평가하였다. 자체 제작한 혈류팬텀을 통해 실험하였으며, Grappa을 5번 변경하여 실험하였다. 연구결과, Grappa를 적용하지 않았을 경우 혈류속도는 평균 9.42로 총 4단계를 적용한 혈류속도 차이율은 각각(1.4, 1.5, 0.4, 1.5%)이었으며 flow와 WSS도 각각(1.4, 1.6, 0.8, 25%), (2.1, 1.4, 1.1, 0.8%)이었다. 통계적으로도 유의하지 않아 Grappa를 적용하지 않은 검사법과 혈류정보가 동일하였다.(P>0.05) 긴 검사 시간으로 인해 임상적 활용이 떨어지는 짧은 검사시간을 요하는 소아환자나 중등도 환자 또는 폐쇄공포증 환자에게 시간을 단축한 3차원 위상대조도 기법을 사용하여 단점을 보완한다면 임상적 활용가치가 높아질 것이라 사료된다.

Assessment of Afterload by Doppler Aortic Flow Velocity Measurement (대동맥 혈류속도 측정에 의한 후부하 상태의 평가)

  • 배경: 심근 수축력과 더불어 전신저항은 개심술 후 적절한 심박출량을 유지하기 위한 매우 중요한 요인이다. 후부하의 변화를 비관혈적으로 평가할 수 있는지 알아 보고자 대동맥 혈류파형의 변화를 관찰하였다. 대상 및 방법: 개흉한 8마리의 잡견에서 연속파형 Doppler 심초음파를 이용하여 nitroprusside 단독 또는 epinephrine과의 병합투여 잔후로 상행 대동맥 혈류를 측정하였다. 결과: Nitroprusside의 투여로 동맥압과 전신저항은 약물의 투여량에 비례하여 감소함으로써(p<0.05 vs 기준치), 대동맥 혈류파형의 최고속도, 평균가속 및 분거리(속도시간적분$\times$심박동수)는 증가하였고, 가속시간은 짧아졌으며(p<0.05 vs 기준치), 가속시간/박출시간의 비는 변화가 없었다. Nitroprusside 투여로 인한 전신저항의 변화는 속도시간적분이나 Doppler 시간간격 지표보다는 최고속도(r=0.60, p=0.001)와 평균가속(r=0.52, p=0.003)과 상관관계가 있었다. Epinephrine과의 병합투여로 전신저항이 증가하지 않는 한 nitroprusside 단독투여에 비해서 최고속도, 평균가속 및 속도시간적분이 더 크게 증가했으나, 전신저항이 증가되면 이 Doppler 지표들은 감소하는 경향을 보였다. 결론: Doppler 심초음파를 이용하여 상행 대동맥 혈류파형의 최고속도와 평균가속을 연속적으로 측정함으로써 후부하의 변화를 비관혈적으로 평가할 수 있으리라 생각된다.

The fruit of Acanthopanax senticosus Harms improves arterial stiffness and blood pressure: a randomized, placebo-controlled trial

  • BACKGROUND/OBJECTIVES: Arterial stiffness and endothelial dysfunction are 2 of the independent predictors for cardiovascular disease, while Acanthopanax senticosus Harms (ASH) is a traditional medicinal plant that can improve cardiovascular health. This study aimed to investigate the efficacy of the fruit of ASH on vascular function in apparently healthy subjects. SUBJECTS/METHODS: A 12-week, randomized, double-blind, placebo-controlled design, consisting of healthy adults with at least 2 of the following 3 conditions: borderline high blood pressure (BP; 120 mmHg ≤ systolic BP ≤ 160 mmHg or 80 mmHg ≤ diastolic BP ≤ 100 mmHg), smoking (≥10 cigarettes/day), and borderline blood lipid levels (220 ≤ total cholesterol ≤ 240, 130 ≤ low density lipoprotein cholesterol ≤ 165, or 150 ≤ triglyceride ≤ 220 mg/dL). Randomly assigned 76 subjects who received a placebo or 2 doses of ASH fruit (low, 500 mg/day; high, 1,000 mg/day) completed the intervention. Brachial-ankle pulse wave velocity (baPWV), flow-mediated dilation, carotid intima-media thickness, and BP were measured both at baseline and following the 12-week intervention. Endothelial nitric oxide synthase (eNOS) phosphorylation was assessed by western blotting. RESULTS: Compared with the placebo group, the low-dose group showed more significant changes after the 12-week intervention period in terms of systolic BP (0.1 vs. -7.7 mmHg; P = 0.044), baPWV (31.3 vs. -98.7 cm/s; P = 0.007), and the ratio of phospho-eNOS/eNOS (0.8 vs. 1.22; P = 0.037). CONCLUSIONS: These results suggest that ASH fruit extract at 500 mg/day has the potential to improve BP and arterial stiffness via endothelial eNOS activation in healthy adults with smoking and the tendency of having elevated BP or blood lipid parameters.
