Respiratory Arrest during Cervical Epidural Block -A case report- (경부 경막외 차단중 발생한 호흡정지 -2예 보고-)

  • Cervical epidural block can be useful in the management of acute and chronic pain of the head, neck, shoulder, and arm, for selected patients. In spite of the widespread use of cervical epidural blocks for pain, there is limited published data on the specific technique and complications regarding the procedure. High levels of epidural block do not appear to be associated with clinically significant circulatory or ventilatory changes unless the concentrations of local anesthetics used are great enough to produce paralysis of intercostal and phrenic nerves. However, high level of epidural block is associated with sympathetic block which may affect responses of circulatory and ventilatory systems. Accordingly, the possibility of major complications of cervical epidural block must be borne in mind. We experienced two cases of respiratory arrest during cervical epidural block with bupivacaine. This is a report regarding complications of cervical epidural block.

Evaluation of the Block Effects in Response Surface Designs with Random Block Effects over Cuboidal Regions

  • In may experimental situations, whenever a block design is used, the block effect is usually considered to be fixed. There are, however, experimental situations in which it should be treated as random. The choice of a blocking arrangement for a response surface design can have a considerable effect on estimating the mean response and on the size of he prediction variance even if the experimental runs re the same. Therefore, care should be exercised in the selection of blocks. In this paper, in the presence of a random block effect, we propose a graphical method or evaluating the effect of blocking in response surface designs using cuboidal regions. This graphical method can be used to investigate how the blocking has influence on the prediction variance throughout all experimental regions of interest when this region is cuboidal, and compare the block effects in the cases of the orthogonal and non-orthogonal block designs, respectively.

  • In this article it is considered that how the slope-rotatability property of a second order design for response surface model is affected by block effects and how the design points are assigned into the blocks so that the blocked design may have the property of slope-rotatability. If an unblocked design is blocked properly, it could be a slope-rotatable design with block effects and this property is named as block slope-rotatability. We approach this problem from the moment matrix of the blocked design, which plays an important role to get the variances of the estimates, and suggest conditions of block slope-rotatability.

  • Developed is a new environment-friendly concrete-block retaining wall system. The conventional analysis methods are not directly applicable because the proposed concrete-block wall system is made of by interlocking the blocks with shear keys. Therefore, the shear analysis as well as stability analysis have been conducted to secure the safety of block-wall system. Overall slope stability analysis was also performed. An appropriate strengthening method was developed to ensure the safety when the block-wall system is relatively high. The method of analysis for strengthening the concrete-block wall system was also proposed. The proposed environment-friendly concrete block retaining wall system shows reasonable safety and can be a good construction method for retaining walls and river bank walls.

  • This paper proposes a modified block matching algorithm which reduces an amount of matching computation by using only pixels contributing greatly to block matching. In consideration of algorithm implementation and additional information, the proposed algorithm divides a matching block into sub-blocks, selects some sub-blocks using their complexities, and execute the block mating with them. Simulation results show that the proposed algorithm performs a valid block matching with some sub-blocks.

  • Splanchnic nerve block is effective in relief of upper abdominal cancer pain. But we often get confused at the unexpected complaints of pain accompanying a metastatic of referred origin in spite of the precise block under image intensifier. We could found aut a thermographic change from the diffuse hyperemission state of abdomen before the block to the localized state after the block. It may be helpful in differentiating objectively the accuracy of the block at the unexpected complaints of pain by using thermography before and after the block.

  • Limit states of a tension member are the yielding of gross section, fracture of net section, and block shear failure. Block shear failure is very complicated than other limit state because of interaction of tension and shear failure. Block shear failure is studied continuously since the 1970s. However, failure model to estimate the strength of block shear failure provided in current design specifications is not reflective of the failure mode observed in the various experimental studies. Comparisons between the experimental results and design rules in various specifications about the block shear failure were conducted in this study. Also, the need for further studies of block shear failure were proposed.

  • Fractal coding is image compression techniques using one of image characteristics self-transformability. In fractal image coding, the encoding process is to select the domain block similar to a range block. The reconstructed image quality of fractal image coding depends on similitude between a range block and the selected domain block. Domain block similar to a range blocks. In fact, the error of the reconstructed image adds up the generated error in encoding process and the generated error in decoding process. But current domain block estimating function considered only the encoding error. We propose a domain block estimating function to consider not only the encoding error but also the decoding error. By computer simulation, it was verified to obtain the high quality reconstructed image.

  • Block copolymers that two or more polymer chains are covalently linked have drawn much attention due to self-assembly into nanometer-sized morphology such as lamellae, cylinders, spheres, and gyroids. In this article, we first summarize the phase behavior of block copolymers in bulk and thin films and some applications for new functional nanomaterials. Then, future perspectives on block copolymers are described.
