A Study on the Reflectivity according to the Material of Biopsy Gun Needle Used in Ultrasound Biopsy (초음파 조직검사에 사용되는 Biopsy Gun Needle의 재질에 따른 반사율 연구)

  • The examination needle used in ultrasound biopsy is a medical device used to determine whether there is an abnormality in the tissue. Typically, stainless steel is the standard material used for such needles; however, this study wanted to identify a material that could better enhance sound compared to traditional stainless steel. In this study, six types of needle materials available with the biopsy gun were inserted into pork and ultrasound images according to the curved probe and linear probe were evaluated using ultrasound equipment. The findings revealed significant improvements in ultrasound acoustic enhancement with alternative materials compared to stainless steel (p<0.05). The results regarding the depth of each ultrasound image using the curved probe showed that tungsten and brass had high sound enhancement(p<0.05), while with the linear probe, sound enhancement was high in brass, pla, aluminum, and copper(p<0.05). Due to these results, the previously used stainless needle showed lower ultrasound acoustic enhancement than the five types of materials being compared. Consequently, the outcomes of this study provide valuable insights for the development of new needle technologies aimed at minimizing patient risks and improving diagnostic accuracy.

A Case of Orbital Meningioma Diagnosed by Fine Needle Aspiration Biopsy (세침흡인 생검으로 진단된 안와 수막종 - 1예 보고 -)

  • Orbital meningioma is a rare neoplasm that, even when suspected by CT or echographic examination, requires careful histologic study for precise identification. Fine needle aspiration(FNA) biopsy has become the diagnostic technique of choice in recent years for investigating orbital masses. There have been a few previous reports on FNA biopsy of orbital menigioma. We experienced a case of orbital meningioma in a 11-yr-old boy, diagnosed by FNA biopsy. The cytohistologic features of aspirated material (intranuclear inclusions, psammoma bodies, and cells arranged in whorls) made it easy to diagnose a meningloma.

A Comparison of Thoracoscopic and Open Lung Biopsy for the Diffuse Infiltrative Lung Disease (미만성 침윤성 폐질환에 대한 비디오 흉강경 폐생검과 개흉 폐생검의 비교)

  • Background: The diffuse infiltrative lung disease often requires biopsy for its final diagnosis. Unlike the limited exposure that can be achieved through small thoracotomy incisions in open lung biopsy technique, the thoracoscopic approach allows visualization and biopsy of nearly entire surface of the lung without morbidity of large standard thoracotomy. The purpose of this study was to compare the diagnostic efficacy and operative safety of thoracoscopic lung biopsy(TLB) with open lung biopsy(OLB) in the diagnosis of diffuse infiltrative lung disease. Material and Method: From March 1993 to August 1997, 81 patients were referred for diagnostic lung biopsy. 51 of them underwent standard open lung biopsy and the remaining 30 patients underwent thoracoscopic lung biopsy. Result: Mean operative time was 63 minutes for TLB and 79 minutes for OLB (p=0.04). The volume of biopsy specimen was not different between two groups(TLB 7.8 cm3, OLB 6.9 cm3 : p=0.72) and the diagnostic accuracy of each methods was comparable (TLB 100%, OLB 96%). The duration of hospital stay was significantly less in TLB (TLB 13days, OLB 22days : p=0.01). The duration of parenteral narcotics administration was also less for TLB(TLB 2.5days, OLB 5.2days, p=0.05). Meanwhile, the duration of chest tube drainage, the frequency of parenteral narcotic injection were not significantly different between two groups. Complications occurred in 2 among the TLB patients (6.67%) and 4 among the OLB patients (7.84%). There was no operative mortality in both groups. Conclusion: We concluded that TLB is a good alternative procedure to OLB in the diagnosis of diffuse infiltrative lung disease with lower morbidity and comparable diagnostic accuracy.

Surgical Lung Biopsy for Diffuse Infiltrative Lung Disease (미만성 침윤성 폐질환의 외과적 폐생검)

  • Background: The diffuse infiltrative lung disease requires surgical lung biopsy for its final diagnosis. We evaluated the effect of surgical lung biopsy for final diagnosis of duffuse interstitial lung disease and compared video assisted thoracoscopic lung biopsy(TLB) with open lung biopsy(OLB). Material and Method: We evaluated the patients who underwent surgical lung biopsy from March 2000 from December 2005, retrospectively, We divide to two groups(OLB and TLB group) and compared them. Result: There were 36 patients and cough was the most common pre- operative symptom. Surgery time, anesthetic time, hospital stay, duration of chest tube indwelling, specimen volume and the rate of post-operative complication were not significantly different between two groups. Histologic diagnosis was confirmed in all cases. There was one post-operative death who had suffered from respiratory failure since pre-operative period. Conclusion: Surgical lung biopsy is effective method in final diagnosis for diffuse infiltrative lung disease. Video assisted thoracoscopic lung biopsy is lesser invasive method than open lung biopsy and provide similar results, so it is basic diagnostic method of surgical lung biopsy.

  • Oral cancers are one of the most common cancers worldwide today. They are usually neglected by the common population when compared to systemic cancers such as the lung cancer, colon cancer etc. However, they also may be extremely fatal if left untreated even at a very initial stage of the lesion. Early detection and treatment gives the best chance for its cure. The five-year survival rate of oral cancer still remains low and delayed diagnosis is suggested to be one of the major reasons. The detection and diagnosis are currently based on clinical examination, histopathological evaluation of the biopsy material and molecular methods. Several diagnostic aids have been developed over the years for early detection of oral cancer. The purpose of this article is to review the advanced available diagnostic adjuncts for the detection of oral cancer.

Sex Determination of Boving Embryos by Polymerase Chain Reaction (PCR 방법을 이용한 소 수정란의 성판별)

  • In sexing early mammalian embryos, viability of biopsied embryos and accuracy of sexing are both important. We have been previously developed efficient methods for biopsy of mouse embryos and sex identification from a single blastomere by PCR. In this study, squeeze method used for biopsy of mouse embryos was applied to bovine embryos. Compact bovine morulae were obtained by flushing uteri on Day 6 after the onset of standing estrus. A small number of blastomeres could be isolated from bovine morulae by the biopsy method. All 13 biopsied morulae were survived and 10 embryos developed to normal blastocyst after 24 h of culture. Subsequently, sex of the bovien embryos was identified from a few blastomeres by PCR amplifying a Y-specific bovine DNA sequence. Among 13 embryos analyzed, 7 embryos were determined as males and 6 embryos as females. Thus, bovine embryos at morular stage could be successfully biopsied by the squeeze method and sex of the bovine embryos determined from biopsied material by PCR.

  • Hematopoietic stem-cell transplantation (HSCT) is a treatment for immune deficiency, autoimmune diseases, and hematopoietic malignancies. The main complication of allogenic HSCT is graft-versus-host disease (GVHD). Oral mucosal biopsy is needed for a definitive diagnosis and treatment planning of GVHD, but this procedure causes bleeding and bacteremia in a poor general condition. We evaluated the efficacy of laser-assisted biopsy as a minimally invasive treatment. Three cases were described in this article. All patients' medical records, clinical photographs, and histopathologic findings were reviewed. All patients felt comfortable and no severe complications occurred. The quality of the obtained biopsy material was adequate for a definitive diagnosis of GVHD. Laser-assisted, minimally invasive biopsy of the oral mucosa does not cause bleeding, and it reduces the chances of infection, bacteremia, and postoperative scarring compared to the usual histopathologic biopsy procedure. It would thus be advantageous to use this procedure to biopsy GVHD patients.

Cytologic Findings of Fine Needle Aspiration Biopsy of Ancient Schwannoma (퇴행성 신경초종의 세침흡인 생검에 대한 세포학적 소견)

  • Ancient (degenerated) schwannomas are benign tumors that display pronounced degenerative changes including cyst formation, calcification, hemorrhage, and hyalinization. The tumors are usually infiltrated by large numbers of inflammatory cells such as neutrophils, lymphocytes, siderophages, and histiocytes. They are located in deep locations such as the retroperitoneum and must be differentiated from malignant soft tissue tumors because spindle cells with nuclear atypia may be present. In the fine needle aspiration biopsy. The cytologic findings of two cases of ancient schwannoma occurring in posterior mediastinum and retroperitoneum, respectively, are described. Computerized tomographic findings of both cases showed changes of cystic degeneration or necrosis. A case occurred in retroperitoneum revealed features of destruction of vertebral body to suggest a malignant soft tussue tumor radiologically. The cytologic findings of aspiration biopsy of both tumors revealed that the tumor cells were spindle in shape with elongated nuclei. Some of tumor cells were arranged in a palisading fashion. Cell structures that resemble the Verocay bodies were observed. Some of the tumor cells showed pleomorphic bizarre nuclei, but no mitotic activity or chromatin clumping was seen. Inflammatory cells, siderophages and histiocytes were scattered in fibrillar material. These cytologic findings are important in the diagnosis of ancient schwannoma and in the differentiation of this tumor from the malignant spindle cell tumor.

A Case of Wegener's Granulomatosis (Wegener씨 육아종증 1례)

  • Wegener's granulomatosis is characterized by 3 criteria: 1. Necrotizing granulomas with vasculitis of upper and lower respiratory tracts 2. A systemic vasculitis 3. Focal necrotizing glomerulitis. This disease is one of the nonhealing disease in the otolaryngologic and ophthalmologic fields. A 48years old Korean male patient was seen with the complaints of nasal discharge, foul odor and frequent nasal bleeding. The patient was admitted after biopsy of the nasal cavity which diagnosed tuberculous granuloma, for biopsy of the maxillary sinus. After biopsy by Caldwell-Luc's approach this patient was complained with severe headache, visual impairment and cough. And so this patient was readmitted for further evaluation. Generally, the diagnosis was made after autopsy sometimes several years later after reevaluation of the case. Tuberculous granuloma was the pathological diagnosis on the basis of resected material in various cases. The correct diagnosis was made at autopsy occasionally. It is our intention to present this case with. literature review.
