Philips ARGUS 감마카메라와 바늘구멍조준기를 이용한 소동물 SPECT 시스템의 개발 및 성능 평가 (Development and Performance Evaluation of an Animal SPECT System Using Philips ARGUS Gamma Camera and Pinhole Collimator)

  • 목적 : 임상용 Philips ARGUS 감마카메라와 특별 제작한 작은 구경의 바늘구멍조준기를 이용하여 animal SPECT를 개발하였다. 본 연구에서는 이 시스템의 물리적인 성능을 평가하고 소동물 실험에 적합한지를 평가하였다. 대상 및 방법: 스텝 모터와 이를 제어할 수 있는 소프트웨어를 이용한 피사체 회전장치를 개발하였다. 작은 입구(0.5, 1.0, 2.0 mm)의 바늘구멍조준기를 제작하였고 평면 공간해상도, 민감도, 단층촬영해상도 등을 포함한 물리적 성능을 모든 입구 크기에 대해 실험하였다. 조준기 입구로부터의 거리에 따른 사용 가능한 시야를 측정하기 위하여, 같은 간격만큼 떨어진 여러 선선원의 영상을 얻고 영상의 시야 내에서 보이는 선 영상의 개수를 이용하여 사용 가능한 시야를 측정하였다. 시야의 정중앙에 놓인 내경 0.5 mm, 길이 12 mm의 Tc-99m 선선원을 이용하여 거리에 따른 평면 공간 해상도를 측정하였다. 전체 반값두께를 'mm'단위로 얻기 위한 환산인자를 평면 영상에서의 두개의 서로 떨어진 선선원으로부터 계산하였다. 시스템 민감도를 측정하기 위하여 내경 1.0 mm의 Tc-99m 점선원을 사용하였다. 또한 냉소반점 모형과 열소반점 모형, 그리고 [I-123] FP-CIT를 정맥내 주사한 흰쥐의 뇌 영상의 SPECT 영상을 얻었고 여과후역투사 방법으로 재구성하였다. 결과: 사용 가능한 시야의 크기는 조준기의 초점으로부터의 거리에 비례하였고 이들의 관계는 선형 함수로 근사되었다(y=1.4x+0.5). 3 cm에서 1.0 mm 조준기로 측정한 민감도와 평면해상도는 각각 71 cps/MBq과 1.24 mm이었다. 1.0 mm 바늘구멍조준기에 대하여 [I-123] FP-CIT를 이용한 흰쥐의 뇌 SPECT 영상에서 각 반구의 줄무늬체 도파민 전달체 분포가 잘 구분되어 보였다. 결론: Philips ARGUS 스캐너와 작은 구경의 바늘구멍조준기로 개발한 새로운 소동물 SPECT 시스템이 소동물 영상을 얻는데 충분한 성능을 가짐을 입증하였다.

뇌 PET과 MR 영상의 자동화된 3차원적 합성기법 개발 (Development of an Automatic 3D Coregistration Technique of Brain PET and MR Images)

  • 목적: PET과 MR 영상을 체계적으로 합성i분석하여 각각의 영상기법이 갖는 단점을 보완하고 기능을 향상시킴으로써 보다 정확하고 유용한 임상정보를 얻을 수 있다. 두 영상을 공간적으로 합성하기 위해서 머리 표피 경계점들 간의 거리를 최소화하는 알고리즘을 이용할 경우 경계점 추출의 정확성 및 견실성과 거리 계산 속도가 합성 알고리즘의 성능을 결정하는 중요한 요소가 된다. 본 연구에서는 PET 영상의 경계 추출과 거리 계산 방법을 개선하고 이를 이용하여 PET과 MR 영상을 3차원적으로 합성하였다. 대상 및 방법: 공간적인 합성을 위한 영상처리기법의 핵심인 경계점 추출을 위해 PET영상에서는 방출스캔 sinogram의 경계를 강조한 후 재구성한 횡단면으로부터 2 mm 간격으로 머리 표피 경계점들을 추출하였으며 MR 영상에서는 각 횡단면마다 약 2도 간격으로 경계점들을 추출하였다. 두 영상의 모든 경계점들 간의 평균 유클리디안 거리를 최소화하는 3차원 가상공간 상에서의 위치 이동과 회전 각도를 최소자승법을 이용하여 구한 후 PET영상을 역 전환하여 위치 정합을 하였다. 평균 거리의 계산 속도를 향상시키기 위하여 고정된 대상의 각 경계점을 중심으로 하여 주변 공간 정들에서의 거리를 순차적으로 계산하고 이들의 최소값을 취하는 방법으로 거리지도를 구성하였으며 최소자승법에서 경계점들 간의 위치가 변할 때마다 매번 평균거리를 다시 계산하지 않고 거리지도를 참조하여 평균 거리를 산출하는 방법을 사용하였다. 위치 정합된 두 영상의 동시 표현을 위하여 PET 영상의 화소값에 $0.4{\sim}0.7$부터 1사이의 범위로 정규화된 MR 영상의 화소 값으로 가중치를 주는 가중정규화 방법을 사용하였다. 결과: 방출스캔의 sinogram을 이용함으로써 PET영상의 경계를 견실하게 추출할 수 있었으며, 거리지도를 이용하여 거리 계산을 한 결과 계산 속도를 향상시킬 수 있었다. 정상인의 뇌영상에 대해 위치 정합을 실시한 결과 평균 거리 오차는 2mm 이하였다. 가중정규화 방법을 사용하였을 때 합성된 영상의 정성적인 식별 명확도가 향상하였다. 결론: 견실한 PET 영상 경계점 추출과 거리지도를 이용한 계산 속도의 향상을 통해 뇌 PET과 MR 영상 합성기법의 성능을 개선할 수 있었으며 이를 이용하며 개발한 영상정합 프로그램은 임상 환경에서 유용하게 사용될 수 있을 것이다.

T1영상에서 TR, TE 매개변수에 따른 Gadoteridol의 신호강도 변화 (Gadoteridol's Signal Change according to TR, TE Parameters in T1 Image)

  • 본 논문에서는 조영증강 MRI 검사 시 T1 effect를 만들기 위한 물리적 매개변수 중 대표적인 TR과 TE가 가돌리늄과 결합한 $H_1$ 스핀의 신호강도에 어떤 영향을 미치는지를 정량적으로 평가 분석하고자 하였다. 이를 위해 MR 가돌리늄 조영제인 0.5 mol Gadoteridol로 제작된 MR팬텀을 이용하여 1.5T MRI 장비에서 FSE(Fast Spin Echo)시퀀스로 실험하였다. 이때 FSE 매개변수 중 TR과 TE의 값들을 서로 달리하였다. 이 때 TR 수치는 각각 200, 250, 300, 350, 400, 450, 500, 550, 600 msec, TE는 6.2, 12.4, 18.6, 21.6 msec로 임상적인 T1 effect를 구현하는데 있어 매개변수 값이 물리적으로 벗어나지 않는 범위 내에서 설정하였고, TR, TE가 서로 교차되어 실험되었다. 획득된 영상 데이터를 통해 신호강도 변화를 측정 한 후 이를 분석하였다. 이 때 조영증강 반응시작 지점인 RSP(Reaction Starting Point)는 TR과는 무관하게 TE 6.2 msec에서 100 mmol, TE 12.4 msec에서 50 mmol, TE 18.6 msec에서 40mmol, TE 21.6 msec에서 30 mmol을 나타내었다. 최대 신호강도인 MPSI(Max Peak Signal Intensity)는 TR 200 msec에서는 TE모두 4mmol에서 형성되었고, TR 250에서 600 msec까지는 모두 4, 2, 1, 0.8, 0.6 mmol의 저 농도 영역으로 피크치가 지연되었다. 반응면적인 RA(Reaction Area)는 TR 200-600 msec에서 각각 21183.2, 21536.6, 21875.9, 22114.3, 22419.1, 22895.8, 23208.6, 23189.1, 23210.4 [a.u]로 TE 6.2 msec일 때 가장 수치가 높았다. 본 연구를 통하여 가돌리늄 조영증강 정도는 MR매개변수에 의해 조정 가능하다는 것을 알 수 있었고, 이는 실제 임상에서의 T1 조영증강 검사에 있어서 본 연구의 정량적 데이터를 통하여 진단학적으로 효율적인 TR, TE 매개변수 값으로 활용할 수 있을 것으로 사료된다.

비가역 방사성추적자 파라메터 영상을 위한 다중선형분석법 (Multiple Linear Analysis for Generating Parametric Images of Irreversible Radiotracer)

  • 동적 PET 데이터는 구획모델과 Nonlinear Least Squares(NLS)방법을 사용하여 분석함으로서 각종 생화학적 물질의 생체 대사율 등을 정량화 할 수 있다. 하지만 NLS방법은 변수의 초기 값이 적절하지 않을 경우 지역적인 최소점에 빠지거나 계산시간이 길어 동역학 변수들을 각 화소마다 구해야 하는 파라메터 영상(parametric image) 구성에는 효과적이지 않다. Patlak 도표분석법(PGA)은 비선형 방정식을 선형화하여 값을 추정함으로서 간단하면서 적은 계산량으로 인해 파라메터 영상을 구성하는데 많이 사용되고 있으나 잡음성분과 선형구간 선정에 따라 값이 영향을 받는 단점이 있다. 따라서 이 연구에서는 3구획 비가역 모델에 적합한 다중선형분석법(MLAIR)을 고안하였으며 3구획 비가역 모델의 대표적 예인 $[^{18}F]Fluoride$ PET을 이용하여 미니돼지에서의 뼈 섭취률을 계산하여 PGA방법과 비교 분석해 보았다. 대상 및 방법: 3마리의 미니돼지를 대상으로 100MBq의 $[^{18}F]Fluoride$를 대퇴부 정맥에 주사하면서 ECAT EXAET 47 PET 스캐너를 이용하여 60분간 PET 영상을 얻었다. 케타민과 자일라진을 이용하여 30분 간격으로 마취하였으며 실험동물을 진공 쿠션을 이용하여 반드시 누운 자세로 위치하도록 고정 시켰다. 입력함수인 혈장 내 농도곡선은 대퇴동맥으로부터 스캔 시작과 함께 혈액 채취를 통해 얻었다. ROI분석을 위해 대퇴골두, 척추 뼈, 근육에 ROI를 그려 조직 내 시간-방사능 곡선을 얻었다. $k_4$가 0인 3구획 비가역 모델로부터 MLAIR와 PGA방법을 사용하여 관심영역에서의 뼈 섭취률 $K_i$와 파라메터 영상을 구성하였다. PGA방법은 선형구간의 시작점인 $t^*$선택에 따른 영향을 보기 위해 분석구간을 변화시켜가며 분석하였다. 결과: ROI 분석결과 추정된 $K_i$값은 NLS방법에 비하여 MLAIR방법과 PGA방법 모두에서 과대 추정되었으나 두 분석방법 끼리는 비슷한결과를 보였다. Patlak 기울기는 $t^*$선택에 따라 값이 변하였으며 Patlak 상수는 Fluoride 섭취가 높은 대퇴골두나 척추뼈에서 30분이 지나서야 일정한 값으로 나타났고 섭취가 낮은 근육에서는 10분만에 일정해졌다. 파라메터 영상에서는 제안한 MLAIR방법이 PGA방법에 비해 영상의 질을 향상시킴을 알 수 있었다. 또한 PGA방법을 이용하여 구성한 파라메터 영상은 $t^*$값이 커질수록 급격히 영상의 질이 저하됨을 볼 수 있다. 특히 Fluoride 섭취가 높은 영역에서 Patlak 상수가 일정해지는 시간인 $t^*$값이 30분일 때 파라메터 영상은 MLAIR와 크게 차이가 났다. 결론 결론적으로 제안한 MLAIR방법은 선형구간을 정할 필요 없이 모든 데이터를 사용하는 이점이 있으며 선형적인 방법을 통해 $K_i$값을 얻을 수 있어 계산시간을 단축 시켜 줄 뿐 아니라 잡음성분에 강해 파라메터 영상의 질을 크게 향상 시켜 줌으로 비가역 3구획모델에서의 PGA방법을 대체할 새로운 파라메터 영상구성방법으로 적합할 것이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
