정상 성인의 가로막 이동거리 및 폐활량에 대한 고유수용성신경근촉진법 Bilateral pattern with Spiral pattern의 즉각적 효과 (Immediate Effects of Bilateral Pattern with Spiral pattern of PNF on Diaphragm Excursion and Vital Capacity in Normal Adults)

  • Purpose: This study aimed to examine the immediate effects of different breathing training techniques on diaphragm excursion and vital capacity in healthy adults. Specifically, the study focused on comparing respiratory exercise without PNF, bilateral pattern respiratory exercise, and bilateral pattern with spiral pattern respiratory exercise. Methods: Twenty-seven healthy adults in their 20s participated in the study. Diaphragm excursion and vital capacity were evaluated under three different conditions. A one-way repeated ANOVA was used to analyze the differences in diaphragm excursion and vital capacity among the interventions. Results: Statistically significant differences were observed in diaphragm excursion among the interventions, comparing respiratory exercise without PNF, bilateral pattern respiratory exercise, and bilateral pattern with spiral pattern respiratory exercise. Similarly, statistically significant differences were found in vital capacity among the interventions without PNF respiratory exercise, bilateral pattern respiratory exercise, and bilateral pattern with spiral pattern respiratory exercise. Conclusion: The study demonstrated that incorporating the spiral technique in respiratory exercise led to increased diaphragm excursion and lung capacity compared to other interventions. These findings suggest that PNF respiratory exercise combined with the spiral pattern may have clinical implications for the treatment of respiratory diseases. Further research is warranted to explore the long-term effects and clinical application of these approaches.

Bilateral Cortical Blindness Caused by Tentorial Herniation due to Brain Tumor

  • Two patients, one with glioblastoma multiforme [GM] in the right thalamus and the other with meningioma at the right frontal convexity, had suffered bilateral cortical blindness after transtentorial herniation. On one of those patients, bilateral cortical blindness had occurred due to acute obstructive hydrocephalus caused by GM and on the other patient, cortical blindness had developed after acute hemorrhage from meningioma. Bilateral occipital lobes of those patients showed signal change on the brain magnetic resonance image [MRI]. There were no ophthalmologic abnormalities on fundoscopy and ophthalmologic examination. After recovery of consciousness, cortical blindness was detected in both patients, and during gradual recovery period, visual function was slowly recovered. The pattern of visual evoked potential [VEP] at 7 weeks and 12 weeks after herniation was normalized gradually. Cortical blindness due to herniation was reversible, even though the high signals of bilateral visual cortex still existed on MRI 16 month later in case 2.

최대 근수축시 외측운동과 양측운동이 근력과 근활동에 미치는 영향 (Effects of Unilateral and Bilateral Movement on Muscle Strength and Activity During Maximum Contraction)

  • In the present study, we aimed to elucidate how muscle strength and activity are affected by movement pattern(bilateral [BLM] & unilateral movement [ULM]) and movement velocity($0^{\circ}$/s, $60^{\circ}$/s, $120^{\circ}$/s) at maximum effort, and to elucidate the relationship between a left/right asymmetry and bilateral deficit. A total of 18 healthy males participated in the study. Each participant performed maximum knee extension bilaterally and unilaterally while the EMG and moment were recorded, and then the relationships between the asymmetry and bilateral deficit were analyzed. The peak moments for the isokinetic motion at $60^{\circ}$/s and $120^{\circ}$/s and overall muscle activities of lower extremity were significantly reduced for the BLM in comparison to the ULM. And though the asymmetry in ULM were maintained during BLM at all velocities, the bilateral deficits at the velocity of $0^{\circ}\acute{y}$/s and $120^{\circ}\acute{y}$/s were significantly correlated with increased asymmetries of muscle strength in ULM. In conclusion, the reduction in the muscle strength exhibited in bilateral knee extension was shown to arise partially from a reduction in muscle activity, and left/right asymmetry was found to be associated with mechanical reduction in bilateral movement. These findings suggest that training aimed at increasing muscle strength must involve methods and strategies intended to reduce left/right asymmetry.

A case of canine bilateral ovary granulosa cell tumor and mammary complex carcinoma

  • An 11-year-old poodle bitch was presented for investigation of multicentric mammary masses. Abdominal sonography and radiography demonstrated abnormal enlargement of uterus and ovaries. Blood analysis revealed high progesterone concentration. The ovariohysterectomy and mastectomy were performed. Histopathologically, the mammary masses revealed complex carcinoma-tubulopapillary carcinoma with papillary pattern and tubule pattern. In the uterus, cystic endometrial hyperplasia was observed. Scattered inflammatory cells were observed in the endometrial stroma and mucinous material was protruded from endometrial surface. Also, in the ovaries, bilateral ovary granulosa cell tumor was detected. The bitch made a complete recovery following the ovariohysterectomy and mastectomy. This case was a very rare multiple tumor occurrence with bilateral ovary granulosa cell tumor and mammary complex carcinoma. High progesterone concentration was characterized clinically in the bitch.

COMPARATIVE ELECTROMYOGRAPHIC ANALYSIS OF MASTICATORY MUSCLES BETWEEN BILATERAL AND UNILATERAL MASTICATORS

  • There are several variations in normal mastication. In them, unilateral mastication is chewing, predominantly on a preferred side of the dentition and hardly on e non-preferred side. Continual unilateral mastication may alter the coordination of masticatory muscles. Although they studied about these EMG of masticatory muscles, there were no information about characteristics of masticatory muscle activity in unilateral mastication. Therefore, In this study, we investigated the activity of the masseter and anterior temporal muscles during rest, clenching in maximum intercuspation and gum chewing in habitually unilateral mastication group compared with normal group and tried to know effects of continual unilateral mastication on activity of masticatory muscles. The results of this study were as follows 1. In electromyographic activity during rest, in bilateral mastication group pattern of muscle activity of right and left side was symmetrical. But, in unilateral mastication group, records of anterior part of temporal muscle was higher than that of bilateral mastication group (p<.01) and patterns of muscle activity of right and left side in both muscle were asymmetrical.(p<.05) 2. In electromyographic activity during clenching in maximum intercuspation, records of superficial part of masseter muscle were higher than anterior part of temporal muscle in both group. Muscle activity of temporal muscle in unilateral mastication group was a little higher han bilateral mastication group and asymmetry of activity pattern in temporal and masseter muscle was shown but these differences were not statistically significant. (p<.05) 3. In electromyographic activity during gum chewing, temporal muscle was activated earlier than masseter muscle and maximum bite force is derived from masseter muscle in both group. In unilateral mastication group, electromyographic activity of masseter and temporal muscle of preferred chewing side, regardless of right or left side chewing, was higher than that of bilateral mastication group and especially, difference in masseter muscle was statistically significant. (p<.01) Based on the above results, our study suggested that recording of masticatory muscle activity will be helpful in the effective diagnosis and treatment of some types of the parafunctional habits.

3차원 가상모형을 이용한 하악 제2소구치 교두 변이 대칭성에 대한 융복합적 분석 - 전북지역 일부 대학생을 중심으로 (Convergence analysis of cusp variation symmetry of the mandibular second premolars using 3-dimensional virtual models - Focusing on college students in Jeollabuk-do)

  • 본 연구는 3차원 가상모형을 이용하여 하악 제2소구치 교두 변이 양상을 정성적으로 분석하고, 교두 변이에 따른 치아 표면적의 정량적 분석과 함께 좌·우측 대칭성을 분석하고자 하였다. 127개의 하악 제2소구치 가상치아를 준비한 후, RapidForm2004(INUS technology INC, Seoul, Korea)을 이용하여 각 교두의 절대적/상대적 단면적, 전체 치관 단면적, 교합면의 중심구 형태 등을 분석하였다. 통계방법은 Independent t-test, Kruskal-Wallis test 및 교차분석을 시행하였다. 연구 결과, 중심구의 형태는 양측간 높은 대칭성을 보였다. 좌측에서 U 형태이면 우측도 100.0% U 형태를 나타냈으며, 나머지 H 형태와 Y 형태는 각각 73.7%, 78.9% 비율로 양측간 동일한 중심구 형태를 나타냈다(p<.001). 본 연구에서 얻은 결과는 CAM/CAM을 이용한 보철물 제작 시 참고자료로 활용이 가능할 것이며, 향후 맹출 직후의 하악 제2소구치를 포함한 더 많은 샘플을 대상으로 한 후속연구가 이루어지기를 기대한다.

중앙형 추간판탈출증의 진단에서 체열촬영의 의의 (A Study on the Infrared Thermographic Imaging in Diagnosis of the Central Type of Herniated Disc)

  • Infrared thermographic imaging visualizes noninvasively various abnormal condition by detecting the skin temperature. As the imaging represents the objective condition by the changes in blood flow under the control of autonomic nervous system, it is used to diagnosis and monitor the lumboscral radiculopathy. And asymmetry is important in the diagnosis of disc herniation. The most common type of disc herniation occurs psoterolaterally. This frequently causes nerve root compression leading to a radiculopathy in the distribution of the involved nerve root, most of which also provoke the asymmetric changes in thermography. Central disc herniation, which accounts for 5% to 35% of disc herniation, is typically associated with low back pain. But radiculopathy is usually abscent unless central disc heriniaton is large enough to cause compression of the cauda equina. To evalute the diagnostic value of the thermographic imaging in the diagnosis of central disc herniation, the imaging of 15 normal subjects and 48 patients with central disc herniation documented by CT scan were analyzed. The patients had either bilateral radiculophathy or no radiculopathy. The imaging of patient group with non rediculopathy did not show any significant thermal difference to control. While bilateral radiculopathy group reveled hypothermic pattern compared twith control. Thermal difference between left and right side did not present any significance in non radiculopathy group but hypothermia in bilateral radiculopathy group. Large herniation group demonstrated hyperthemic pattern while the others showed no significant change. Cranial caudal thermal difference did not show any difference between experiment groups. These results shows that infrared thermographic imaging can be used central disc herniation with bilateral radiculopathy, while it seems to little useful on the diagnosis of non radiculopathic disc herniation.

Cytomegalovirus Pneumonia: High-Resolution CT Findings in Ten Non-AIDS Immunocompromised Patients

  • Objective: To describe the HRCT findings of cytomegalovirus (CMV) pneumonia in non-AIDS immunocompromised patients Materials and Methods: This retrospective study involved the ten all non-AIDS immunocompromised patients with biopsy-proven CMV pneumonia and without other pulmonary infection encountered at our Medical Center between January 1997 and May 1999. HRCT scans were retrospectively analysed by two chest radiologists and decisions regarding the findings were reached by consensus. Results: The most frequent CT pattern was ground-glass opacity, seen in all patients, with bilateral patchy (n = 8) and diffuse (n = 2) distribution. Other findings included poorly-defined small nodules (n = 9) and consolidation (n = 7). There was no zonal predominance. The small nodules, bilateral in eight cases and unilateral in one, were all located in the centrilobular region. Consolidation (n = 7), with patchy distribution, was bilateral in five of seven patients (71%). Pleural effusion and bilateral areas of thickened interlobular septa were seen in six patients (60%). Conclusion: CMV pneumonia in non-AIDS immunocompromised patients appears on HRCT scans as bilateral mixed areas of ground-glass opacity, poorly-defined centrilobular small nodules, and consolidation. Interlobular septal thickening and pleural effusion are frequently associated.

등속성 무릎 굴곡과 신전 시 외측 및 양측운동의 역학적 특성 (Mechanical Properties of Unilateral & Bilateral Movement in Isokinetic Knee Extension and Flexion)

  • 본 연구는 외측운동과 비교하여 양측운동의 운동형태와 속도에 따른 역학적 특성을 규명하는데 그 목적이 있다. 이를 위해 13명의 20-30대 남성을 대상으로 좌우 외발과 양발의 등속성 무릎 굴곡과 신전을 3가지 속도(120, 240, $450^{\circ}$/s)에서 실시하였다. 분석 결과, 3가지 속도의 무릎의 굴곡과 신전에서 대체적으로 외측운동의 합보다 양측운동에서의 역학적 출력(토크와 파워)이 감소하는 양측운동손실이 나타났다. 그러나 $450^{\circ}$/s의 신전에서는 양측운동에서 오히려 토크의 증가하는 양측운동강화가 나타났다. 특히 동일 근육에서 발생하는 각 개인의 양측운동지수는 운동속도가 변하더라도 일정 부분 유의한 상관을 보였다(120신전-240신전, 120굴곡-240굴곡, 240굴곡-450굴곡). 반면 굴곡과 신전과 같이 주동근이 다른 운동에서 집단 전체의 양측운동지수는 속도에 따라 유사한 패턴의 경향을 보였으나 각 개인의 양측운동지수는 유의한 상관이 나타나지 않았다. 이를 통해 각 개인의 근신경적 특성이 양측운동의 결과를 좌우하는 주요한 요인이 될 수 있음을 추측할 수 있다.

기능성 소화불량증에서 위운동성 장애 진단을 위한 양도락 지표 연구 (Study of Ryodoraku Parameters for Diagnosing Gastric Dysmotility in Functional Dyspepsia)

  • Objectives : The aim of this study was to investigate the characteristics of Ryodoraku and association of Ryodoraku with gastric dysmotility in functional dyspepsia(FD). Methods : Subjects were 154 patients with FD and 18 patients with asthma. We calculated the average Ryodoraku score(RS, ${\mu}A$) and each variation from physiologic range of 12 Ryodoraku points, and investigated the incidence when left and right points were simultaneously below(bilateral deficiency) or above(bilateral excess) physiologic range. Postprandial regularity of normal slow waves, power ratio, and postprandial % of bowel sound were obtained by electrogastrography and enterotachography, and were used as gastric dysmotility index. Results : 1. Bilateral excess of H4, H5, F1, and F4 and bilateral deficiency of H4, H5, and H6 were characteristic in FD compared with asthma patients. 2. Incidence of gastric dysmotility in bilateral deficiency of H4, H5, and H6 was 100%, and was higher than in total FD patients(88.3%). 3. There was a positive correlation between the variation of H6 and % postprandial bowel sound. 4. Deficient tendency of H4, H5, and H6 was more evident when RS was above $40{\mu}A$ in FD. Conclusions : These findings suggest that gastric dysmotility in FD can be diagnosed when a pattern of H4, H5, H6 bilateral deficiency and F1, F4 bilateral excess is shown at the same time. We think this phenomenon is related to low activity of the vagus nerve rather than meridian pathway with result based on positive correlation between variation of H6 and postprandial % of bowel sound.
