Prediction of Type 2 Diabetes Remission after Bariatric or Metabolic Surgery

  • Bariatric surgery has evolved from a surgical measure for treating morbid obesity to an epochal remedy for treating metabolic syndrome as a whole, which is represented by type 2 diabetes mellitus. Numerous clinical trials have advocated bariatric or metabolic surgery over nonsurgical interventions because of markedly superior metabolic outcomes in morbidly obese patients who satisfy traditional criteria for bariatric surgery (body mass index [BMI] >$35kg/m^2$) and in less obese or simply overweight patients. Nevertheless, not all diabetes patients achieve the most desirable outcomes; i.e., diabetes remission after metabolic surgery. Thus, candidates for metabolic surgery should be carefully selected based on comprehensive preoperative assessments of the risk-benefit ratio. Predictors for diabetes remission after metabolic surgery may be classified into two groups based on mechanism of action. The first is indices for preserved pancreatic beta-cell function, including younger age, shorter duration of diabetes, and higher C-peptide level. The second is the potential for an insulin resistance reduction, including higher baseline BMI and visceral fat area. Several prediction models for diabetes remission have been suggested by merging these two to guide the joint decision-making process between clinicians and patients. Three such models, DiaRem, ABCD, and individualized metabolic surgery scores, provide an intuitive scoring system and have been validated in an independent external cohort and can be utilized in routine clinical practice. These prediction models need further validation in various ethnicities to ensure universal applicability.

  • To move forward in large steps rather than in small increments, the community would benefit from a systematic and comprehensive database of multi-scale composites and measured properties, driven by comprehensive studies with a full range of types of fiber-reinforced polymers. The multi-scale hierarchy is a promising chemical approach that provides superior performance in synergistically integrated microstructured fibers and nanostructured materials in composite applications. Achieving high-efficiency thermal conductivity and mechanical properties with a simple surface treatment on single-walled carbon nanotubes (SWCNTs) is important for multi-scale composites. The main purpose of the project is to introduce ozone-treated SWCNTs between an epoxy matrix and basalt fibers to improve mechanical properties and thermal conductivity by enhancing dispersion and interfacial adhesion. The obvious advantage of this approach is that it is much more effective than the conventional approach at improving the thermal conductivity and mechanical properties of materials under an equivalent load, and shows particularly significant improvement for high loads. Such an effort could accelerate the conversion of multi-scale composites into high performance materials and provide more rational guidance and fundamental understanding towards realizing the theoretical limits of thermal and mechanical properties.

Probiotics in the Prevention and Treatment of Necrotizing Enterocolitis

  • Necrotizing enterocolitis (NEC) is a disease with high morbidity and mortality that occurs mainly in premature born infants. The pathophysiologic mechanisms indicate that gastrointestinal dysbiosis is a major risk factor. We searched for relevant articles published in PubMed and Google Scholar in the English language up to October 2020. Articles were extracted using subject headings and keywords of interest to the topic. Interesting references in included articles were also considered. Network meta-analysis suggests the preventive efficacy of Bifidobacterium and Lactobacillus spp., but even more for mixtures of Bifidobacterium, Streptococcus, and Bifidobacterium, and Streptococcus spp. However, studies comparing face-to-face different strains are lacking. Moreover, differences in inclusion criteria, dosage strains, and primary outcomes in most trials are major obstacles to providing evidence-based conclusions. Although adverse effects have not been reported in clinical trials, case series of adverse outcomes, mainly septicemia, have been published. Consequently, systematic administration of probiotic bacteria to prevent NEC is still debated in literature. The risk-benefit ratio depends on the incidence of NEC in a neonatal intensive care unit, and evidence has shown that preventive measures excluding probiotic administration can result in a decrease in NEC.

Sacral Acupuncture for Lower Urinary Tract Symptoms: A Systematic Review of Randomized Controlled Trials

  • Lower urinary tract symptoms (LUTS) associated with storage, voiding, and post-micturition reduce quality of life and cause mental health problems. In traditional medicine, Baliao points have been empirically used to treat urinary system diseases. In this review, randomized controlled trials (RCTs) using sacral acupuncture on Baliao points with sham treatment, other remedies, or other acupoints were retrieved from 8 electronic databases up to June 2021. Sixteen RCTs met the inclusion criteria. The quality of the included studies was assessed using a risk-of-bias (ROB) tool. Most of the evaluation indicators used in the included RCTs showed that sacral acupuncture had a significant therapeutic effect compared with the sham control intervention groups, and other remedies. However, all studies using acupoints (other than the Baliao points) as a control intervention had a "high" ROB and only reported secondary processed information, making it difficult to evaluate the efficacy of sacral acupuncture treatment for LUTS. No serious adverse effects were reported for sacral acupuncture, and only a low number of minor side effects were observed. These results suggest that sacral acupuncture could be considered as an alternative to existing treatments, with the added benefit of low cost. Large-scale, long-term RCTs are required in the future.

A Case Report of Partial Remission of End-stage Ovarian Cancer Patient with Lung Metastasis Treated with Carboplatin/Paclitaxel and Traditional Korean Medicine (Carboplatin/Paclitaxel과 한방치료를 병행하며 부분 관해 된 폐전이 동반 말기 난소암 1례)

  • Objectives: The purpose of this study is to report the effect of combination treatment of Traditional Korean medicine (TKM) and chemotherapy on a ovarian cancer with lung metastasis patient. Methods: One ovarian cancer with lung metastasis patient was treated by TKM in conjunction with Carboplatin/paclitaxel since Feb. 2020. Repeat cycle every 3 weeks for 6 times. The patient has been treated with TKM at the same time. To evaluate the patient, symptoms were measured by Numeric Rating Scale (NRS), Eastern Cooperative Oncology Group (ECOG) and tumor size was measured by scanning with Computed Tomography (CT). Blood tests including cancer biomarker were conducted during treatment. Adverse events were evaluated by the National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), version 5.0. Results: After treatment with Carboplatin/paclitaxel and TKM during 2 months, the size of the ovarian cancer was decreased(Partial Response, PR), size and malignant pleural effusion at right lung disappeared. And no evidence of newly developed metastatic lesions. After 2 months, the tumor response was stable disease while improving the performance and other symptoms. Conclusions: This case provides us conjunctive treatment with Conventional and Eastern medicine may have substantial benefit for patients with end-stage ovarian cancer.

Study on Economic Analysis of Offshore and Ground-mounted Solar Photovoltaics (해상과 지상 태양광 발전 경제성 비교에 관한 연구)

  • The rapid expansion of industrialization and population growth worldwide has led to a significant surge in energy demand, perpetuating heavy reliance on finite fossil fuel reserves. Although prevailing policies primarily target ground-mounted solar photovoltaics, there is a noticeable increase in the adoption of floating solar power generation systems on water surfaces. Nonetheless, adequate studies and legislative reviews on offshore solar photovoltaics in Korea are lacking. The absence of well-defined criteria for the economic analysis of floating solar photovoltaics presents hurdles to their economic feasibility. This study conducted a comprehensive cost-benefit analysis of offshore photovoltaics to evaluate their economic viability and compared four types of solar photovoltaics based on the operating area and technology: ground-mounted, floating on inland water, pontoon-based offshore, and flexible system offshore. Perspectives from both central and local government entities, emphasizing social aspects, as well as inputs from private companies with a financial focus were considered. The findings revealed variations in economic viability depending on the operating area and technology employed. This study aims to contribute to the advancement of market maturity and technology within the realm of offshore solar photovoltaics.

Review of Randomized controlled Trials on Korean Herbal Medicine Treatment for Premature Ovarian Insufficiency (조기난소부전의 한약 치료에 대한 무작위 대조군 임상 연구 고찰)

  • Objectives: This study was performed to analyze randomized controlled trial, which studied the effect of korean herbal medicine on premature ovarian insufficiency. Methods: Researchers searched for randomized controlled trial of based on korean herbal medicine and premature ovarian insufficiency. The paper search was conducted through 7 online databases on April 20, 2024. Results: 12 studies were included after selection and exclusion criteria. 4 studies compared herbal medicine with hormone replacement therapy or placebo. 8 studies compared combined treatment of herbal medicine and hormone replacement therapy, with hormone replacement therapy alone. All studies reported significant improvement on hormone, antral follicle count (AFC), total effective rate and associated symptoms. There were no serious adverse effects. Conclusions: This study suggests that herbal medicine has benefit for treating symptoms and ovarian function of premature ovarian insufficiency. For reliable evidence, further research on pregnancy or long-term follow-up is needed.

Development of Economic Analysis Indicators and Case Scenario Analysis for Decision-making support for Off-Site Construction Utilization of Apartment Houses (OSC 활용 의사결정 지원을 위한 경제성 분석 지표 개발 및 사례 시나리오 분석 - 공동주택 PC공법을 중심으로 -)

  • Recently, the Ministry of Land, Infrastructure and Transport presented the '6th Construction Technology Promotion Basic Plan' and 'Smart Construction Revitalization Plan (2022.7.20)'. Off-Site Construction (OSC), which involves construction and production of PC (Precast Concrete) and Modular, etc., has advantages in shortening the construction period, reducing costs, improving quality, reducing construction waste, and reducing safety accidents. However, the construction cost is high compared to the traditional RC construction method, which has hindered its utilization and spread. In this study, OSC utilization was improved. An economic analysis indicator and methodology that can support decision-making in the planning and design stages for multi-unit housing were proposed. The factors used in the economic analysis of OSC (based on the PC method) of apartment houses were reviewed. As for the indicators used in the cost and benefit section, 'Construction Period', 'Disaster Occurrence', 'Waste Generation', and 'Greenhouse gas Emission', which reflect the technical advantages of OSC, were derived. In addition, a scenario analysis was conducted based on actual apartment housing case data for the presented economic analysis indicators and benefit calculation standards. The level of benefit that offsets the difference between the existing RC construction method and the construction cost was reviewed. In future studies, it will be necessary to conduct additional case studies to apply the measurement criteria for detailed indicators and supplement the benefit indicators.

Evaluation Criteria and Preferred Image of Jeans Products based on Benefit Segmentation (진 제품 구매자의 추구혜택에 따른 평가기준 및 선호 이미지)

  • The purpose of this study was to find differences in evaluation criteria and to find differences in preferred images based on benefits segmented groups of jeans products consumers. Male and female Korean university students participated in the study. Quota sampling method was used to collect the data based on gender and a residential area of the respondents. Data from 492 questionnaires were used in the analysis. Factor analysis, Cronbach's alpha coefficient, cluster analysis, one-way ANOVA, and post-hoc test were conducted. As a result, respondents who seek multi-benefits considered aesthetic criteria(e.g., color, style, design, fit) and quality performance criteria(e.g., durability, ease of care, contractibility, flexibility) more importantly when evaluating and purchasing jeans products. Respondents who seek brand name considered extrinsic criteria(e.g., brand reputation, status symbol, country of origin, fashionability) more importantly than respondents who seek economic efciency. Respondents who seek multi-benefits such as attractiveness, fashion, individuality, and utility tend to prefer all the images: individual image, active image, sexual image, sophisticated image, and simple image when wearing jeans products. Respondents who seek fashion are likely to prefer individual image, and respondents who seek brand name more prefer both individual image and polished image. Mean while, respondents who seek economical efficiency less prefer sexual image and polished image.

Development of Analytic Hierarchy Process or Solving Dependence Relation between Multicriteria (다기준 평가항목간 중복도를 반영한 AHP 기법 개발)

  • Transportation project appraisal should be precise in order to increase the social welfare and efficiency, and it has been evaluated by only a single criterion analysis such as benefit/cost analysis. However, this method cannot assess some qualitative items, and cannot get a proper solution for the clash of interests among various groups. Therefore, the multi-criteria analysis, which can control these problems, is needed, and then Saaty has developed one of these methods, AHP(Analytic Hierarchy Process) method. In AHP, the project is evaluated through weighted score of the criteria and the alternatives, which is surveyed by a questionnaire of specialists. It is based on some strict suppositions such as reciprocal comparison, homogeneity, expectation, independence relationship between multi-criteria, but supposing that each criterion has independence relation with others is too difficult in two reasons. First, in real situation, there cannot be perfect independence relationship between standards. Second, individuals, even though they are specialists of that area, do not feel the degree of independence relation as same as others. This paper develops a modified AHP method for solving this dependence relationship between multi-criteria. First of all. in this method, the degree of dependence relationship between multi-criteria that the specialist feels is surveyed and included to the weighted score of multi-criteria This study supposes three methods to implement this idea. The first model products the degree of dependence relationship in the first step for calculating the weighted score, and the others adjust the result of weighted score from the basic AHP method to the dependence relationship. One of the second methods distributes the cross weighted score to each standard by constant ratio, and the other splits them using Fuzzy measure such as Bel and Pl. Finally, in order to validate these methods, this paper applies them to evaluate the alternatives which can control public resentments against Korean rail path in a city area.
