치명적으로 만발한 신경베체트병 (A Fatal Case of Full-Blown Neuro-Behcet Disease)

  • We reported a 48-year-old man with Behcet disease, who presented with right hemiparesis. His first brain MRI showed multiple enhanced lesions. During the recovery, he had an episode of left 6th nerve palsy without new lesions in a follow-up MRI. Third episode was cervical myelitis, resulting in respiratory difficulty and quadriplegia without any reflexes. The myelitis was not responsive to immunotherapy. He died of respiratory failure complicated with pneumonia. This is a rare case of full-blown neuro-Behcet disease.

  • Pulmonary and/or bronchial aneurysm with systemic thrombophlebitis can be seen in the case of both Behcet's disease and Hughes-Stovin syndrome. These two diseases may be indistinguishable since the clinical, radiological, and histopathological findings are similar in both cases. In particular, from the perspective of pulmonary involvement, hemoptysis is the leading cause of death in both conditions and is followed by aneurysmal rupture. Here, we report a case in which surgical resection was performed for a patient presenting recurrent hemoptysis as an atypical presentation of Behcet's disease.

  • Muscular involvement in Behcet's disease is rare manifestation in spite of basic characteristic that is vasculitis can invade multi-organ of the entire body. A few cases has been announced involving the lower extremities, the arm and generalized weakness. Like our case, myositis involving the masticatory muscles with clinically diagnosed Behcet's disease was presented with the magnetic resonance imaging (MRI) and the clinical findings, which is, not yet documented in the literature.

  • Behcet씨 병의 경우 중추 신경계 침범여부는 조기에 진단 치료되어야 할 정도로 불량한 예후를 나타내는 지표로써 아프타성 구내염, 생식기 궤양, 표피성 혈관염, 포도막염, 활막염 등 전신적 증상에 주의를 기울임으로써 조기 진단 및 치료를 하여야 예후의 호전에 기여할 것으로 생각된다. 저자들은 51세 여자 환장서 중추 신경계 증상을 동반한 Behcet씨 병 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

Superior Vena Cava Syndrome Without Thrombosis Found in Behcet's Disease

  • Behcet's disease is a rare multisystemic disorder whose main pathological defectis vasculitis, and superior vena cava (SVC) syndrome without thrombosis is a very rare manifestation of the disease. These authors encountered a case of SVC syndrome without thrombosis caused by Behcet's disease. A 33-year-old man visited the hospital for aggravated dyspnea without any related medical and familial history. He had a threeday history of abrupt swelling of the face, neck, and right arm. He suffered from recurrent oral ulcer, and there were acneiform nodules on his face as well as redness and swelling at the site of the intravenous injection. On the multi-detected computed tomography (CT) chest angiograms (chest angio MDCT), the SVC narrowed without thrombosis. Venogram was carried out, and percutaneous transluminal balloon angioplasty of the SVC stenotic site was performed. The following day, the swelling was found to have subsided. The details of the case are reported herein.

베체트 병의 구강궤양에 대한 국소인자의 영향에 관한 연구 (A Study on the Effects of Local Factors on the Oral Ulcers Observed in Behcet s Disease)

  • Of the many first-occuring major manifestations of Behcet's disease, the oral ulcer is most frequently observed, shows the highest mean recurrence rate, and the longest mean duration period. A study of the relationship between mechanical irritation buy local factors and the recurrence of oral ulcers was carried out to see if there is a correlation. The subjects chosen for the study were 81 patients who were referred to the Department of Oral Diagnosis at Yonsei University Dental Hospital from the Behcet clinic of Severace Hospital after being diagnosed as Behcet's disease patients. 1. By Shimuzu's classification, possible type was the most numerous with 55.55% ; while suspected type was 19.76% ; incomplete type, 17,28% ; and complete type, 7.41% were observed in that order. The male to female ration was 1 to 1.61, and the average age of onset was 17.63 years. 2. Oral ulcers developed most frequently on the tongue (48.28%), and lip(23.15%), buccal mucosa (20.69%), palate\pharynx(4.43%), and gingiva(3.45%) also showed ulceration. 3. 38.27% of the patients were HSV positive : 27.16% were CRP positive ; 12.34% were ASO positive ; 9.87% were RF positive ; and 3.7% were ANA positive. 4. According to the answers to the survey, fatigue(85.18%) was most frequently associated with Behcet's diseage. Trauma by tooth brushing (22.22%), un specified reasons (20.98%), hard food chewing (12.35%), and irritation by dentition and/or prosthesis (13.58%) were observed in association with Behcet's disease. 23.46% had a history of tonsilitis. 5. Oral ulcers on the anterior part of tongue and buccal mucosa were associated with local irritatants. 6. Oral ulcers on the lip and posterior portion of tongue were not associated with local irritants. Local irritation by dentition and oral ulcer on the anterior part of tongue and buccal mucosa was observed to coexit with one another : in other words, lo9cal factors can act to cause of recurrence of an oral ulcer and to delay the healing process in Behcet's disease. Therefore, elimination of local factors along with systemic therapy must be recommended when treating oral ulcers.

  • Although Behcet`s disease, characterized by oro-genital ulceration and ocular abnormalities, is rare and unfamiliar disease to thoracic surgeon, one must remember that it can cause serious complications on the vascular system such as thrombophlebitis,thrombotic obstuction and aneurysm.We report a case of multiple arterial aneurysms with Behcet`s disease. This 31 year-old female had five times surgical interventions during short period[19 months]. 6 aneurysms were developed in this case, 3 were spontaneous aneurysms, 2 were anastomotic aneurysms and 1 was false aneurysm due to arterial puncture.

  • 저자들은 31세 남자환자에서 고열, 흉막성 흉통 및 흉막 유출액의 임상증상을 보이며 검사상 다발성 혈관염 소견을 보였으며, 결핵성 흉막염 및 불명열로 오인되었던 베체트병을 부신피질 호르몬제 및 colchicine, 비스테로이드성 항염증약물을 이용하여 치료한 1예를 경험하였기에 이에 보고하는 바이다.

  • Behcet's disease is a chronic, relapsing multisystem disorder, that may develop into variable neurological manifestations. They include vascular and parenchymal involvement. Vascular involvement is dominated by cerebral venous sinus thrombosis marked by benign intracranial hypertension. Cerebral venous sinus thrombosis can present with all the classical criteria for idiopathic intracranial hypertension, including normal brain CT findings with normal CSF content. But brain MRI is a useful diagnostic method in this situation to confirm the presence of cerebral venous sinus thrombosis. We experienced a case of raised intracranial pressure in a 21-year-old man, caused by cerebral venous sinus thrombosis. We disclosed his symptoms and signs thus fulfilling the diagnostic criteria for Behcet's disease.

  • Objectives : The object of this study is to observe the effect of needle-embedding and acupuncture therapy on shoulder pain in Behcet disease. Methods : Needle-embedding therapy and acupuncture therapy was used to treat shoulder pain. We evaluated the patient through visual analog scale(VAS) and shoulder range of movement(ROM). Results : After weekly four times of needle-embedding therapy and daily acupuncture therapy, patient's VAS was decreased to 4 and ROM of shoulder was also improved. Conclusions : Needle-embedding and acupuncture therapy could be effective to decrease shoulder pain and improve movement of shoulder in Behcet disease patient.
