Five Cases of Viral Warts Treated by Korean Medicine (사마귀 한방치험 5례 증례보고)

  • Objectives : The purpose of this study is to report the effect of Korean medical treatment for viral warts. Methods : All patients were treated by Bojungikki-tang gami. One of them was treated with only herbal medicine. The others were treated with herbal medicine, acupuncture, sweet bee venom acupuncture and moxibustion. To evaluate the results of these treatments, we observed condition of the patients and took the photos of lesions. Results : After Korean medical treatment, the viral warts of all patients remarkably disappeared and there have been no recurrence at all. Conclusions : We suggest that viral warts can be treated by Korean medical treatment.

The Clinical Study on Acupuncture Sensation in CC, CF and BV Herbal Acupuncture -The Basic Study on Placebo Herbal Acupuncture- (CC, CF, BV 약침의 침감에 대한 임상적 연구 -placebo 약침을 위한 기초연구-)

  • Objective : This study was designed to find out whether NS(normal saline) is able to be constituted as an appropriate control group for CC(Cervi Cornu Parvum herbal acupuncture), CF(Carthami-Flos herbal acupuncture) and BV(bee venom herbal acupuncture). Method : NS and three herbal acupuncture were inserted into Quchi(LI 11) of the subjects. After 5 minutes the subjects completed a questionnaire rating the intensity of 21 kinds of acupuncture sensation; hurting, penetrating, sharp, aching, intense, spreading, radiating, tingling, pricking, stinging, pulling, heavy, dull, numb, electric, shocking, hot, burning, cool, pulsing, and throbbing. We compared subjective evaluations of acupuncture sensation between or among the groups. Results : As for CC half items of the acupuncture sensation were significantly different from NS. As for CF all items were not significantly different from NS. As for CC all items were significantly different from NS except one item. In general the score of CF acupuncture sensation was lower than the others and the score of BV acupuncture sensation was higher than the others from comparison of sensation among herbal acupunctures(CC, CF, BV). Conclusion : We found that NS is able to be an appropriate placebo herbal acupuncture for CF. Further study is needed for new placebo herbal acupuncture for CC and BV.

The Clinical Study on Effects of Thoracolumbar Junction's Hyeopcheok Points(夾脊穴) for Low Back Pain (요둔통 환자에 흉요추이행부 협척혈 병행 치료 효과의 임상적 연구)

  • Objectives : This study was performed to evaluate the effects of thoracolumbar junction's Hyeopcheok points(夾脊穴) for low-back pain in the thoracolumbar junction syndrome. Method : 1. Measurement : We investigated 26 patients with low-back pain. The unpleasantness of pain was measured by visual analogue scale(VAS), and the Oswestry low-back pain disability index(ODI). We compared the VAS and ODI scores of the two groups statistically. 2. Intervention : Use Acupuncture and bee venom injection in thoracolumbar junction's Hyeopcheok points that display main tender points. Also, herb medication and physical therapy was used. Results and Conclusion : As a result of treatment, thoracolumbar junction's Hyeopcheok points is very effective for low-back pain. However, more cases are required to oriental medicine in order to prove the availability.

A Clinical Study on Local Reactions of BV and BVP Skin Test (BV와 BVP의 Skin Test 국소반응에 대한 연구)

  • Objective : The purpose of the study was to observe local reactions of BV and BVP skin test, and to compare those between BV and BVP. Methods : We performed BV or BVP Skin test to 42 patients, and then we measured wheal and erytherma on skin test area by digital caliper after 15 minutes. Results : 1. In BV Skin test, the mean size of wheals was $l0.l0{\pm}0.78mm$ and the mean size of erythermas was $29.85{\pm}6.23mm$. 2. In BVP Skin test, the mean size of wheals was $7.05{\pm}3.20mm$ and the mean size of erythermas was $19.58{\pm}7.62mm$. 3. The local reactions of BVP Skin test decreased significantly(P<0.01) than those of BV skin test.

Effect of Bee Venom Death Receptor Dependent Apoptosis and JAK2/STAT3 Pathway in the Ovarian Cancer (난소암에서 봉독이 세포자멸사와 JAK2/STAT3 Pathway의 억제에 미치는 영향)

  • 목적 : 이 연구는 봉독이 사람의 난소암 세포인 SKOV3와 PA-1에서 death receptor의 발현을 높여 세포자멸사를 촉진함으로써 암세포의 성장을 억제하는지 밝히고자 하였다. 방법 : 난소암의 세포자멸사의 관찰에는 DAPI, TUNEL staining assay를 시행하였으며, 세포자멸사 조절 단백질의 변동 관찰에는 western blot analysis를 시행하였고, 난소암 세포에서 death receptor의 변화를 관찰하기 위해 RT-PCR analysis를 시행하였다. 결과 : 1. DAPI, TUNEL staining assay 결과, 봉독은 투여량에 따라 세포자멸사의 유도를 통해 SKOV3와 PA-1 난소암세포의 증식을 억제하였고, 세포자멸사와 동반하여 DR4와 DR6의 발현이 두 암세포 모두에서 증가하였고, DR3의 출현은 PA-1 세포에서 증가하였다. 2. Death Receptor의 발현 증가에 따라 caspase-3, 8, 9 and Bax를 포함하는 세포자멸사 촉진 단백질의 발현이 동반하여 상승하였고 JAK2, STAT3의 인산화와 Bcl-2의 발현은 억제되었다. 3. siRNA 처리 시 봉독에 의한 DR3, DR4, DR6 발현증가와 STAT3의 활성억제가 역전되었다. 결론 : 이러한 결과는 봉독이 난소암 세포에서 DR3, DR4, DR6의 증가와 JAK2/STAT3 pathway의 억제를 통하여 세포자멸사를 유발한다는 것을 시사하며, 난소암의 예방과 치료에 효과적으로 활용될 수 있을 것으로 기대된다.

A Case Report on Foot Drop Caused by Common Peroneal Nerve Palsy (공통(共通) 비골신경(?骨神經) 마비(麻痺)로 발생한 foot drop 환자 치험 1례)

  • This report present on a case of common peroneal nerve palsy effectively treated with oriental medical therapy. Common peroneal nerve palsy caused by compression of nerve, general illness(diabetes mellitus) etc. It occur foot drop, disorder of ankle dorsi-flexor and eversion, disorder of foot sensory. If nerve palsy is progressed, muscle atropy is occurred. This patient fall foot drop before two month ago without reason. He is diagnosed as peroneal nerve palsy by brain MRI, EMG and doctor's physical examination. This patient effectively treated by oriental medical therapy. Oriental medical therapy is herb medicine(kamisamul-tang), acupuncture(S36, S38, S40, S41, G34, G39, G40, G41, LIV3, LIV4, K3, B60), Electro-acupuncture, bee venom acupuncture, moxibustion, electro physical therapy(EST, SSP). Time of therapy is two months and sequela is nothing.

A Review of Research on the Effects of Acupuncture and Moxibustion Treatment to Complex Regional Pain Syndromes (복합부위통증증후군의 침구치료 효과에 대한 연구동향 고찰)

  • Objectives : This study was performed to review articles concerning acupuncture or moxibustion treatment for Complex Regional Pain Syndromes. Methods : On-line databases including PubMed, Cochrane Library, Ovid-EMBASE, NDSL and OASIS were searched to find articles concerning acupuncture or moxibustion treatment for Complex Regional Pain Syndromes. Several overlapping articles and those not relevant to the topic were excluded, as well as review articles and commentaries. Results : 16 case reports were selected through the procedure, all of which presented successful treatment cases of Complex Regional Pain Syndromes with acupuncture and moxibustion. Most of them presented one single patient case and were published in Korea. However, almost half of them were printed in English, and had been continuously published since 2005. It was remarkable that pharmacopuncture was predominantly used to treat Complex Regional Pain Syndromes. Also, we noted that Chinese scalp acupuncture needs to be studied further as two successful cases were presented from the U.S army. Conclusion : The results show that several case reports have been published regarding Complex Regional Pain Syndromes treated with acupuncture and moxibustion. However, all of them were case reports which are given low status in Evidence-Based Medicine. We expect that a more diverse range of articles, including case control studies, cohort studies and randomized controlled trials will be performed in the near future, and that a unified outcome measure will be developed for Complex Regional Pain Syndromes.

Review of Randomized Controlled Trials on Pharmacopuneture Treatment for Musculoskeletal Diseases (무작위대조군연구(RCT)를 중심으로 한 근.골격계 질환의 약침 치료에 관한 고찰)

  • Objectives : The purpose of this study is to review RCTs on pharmacopuncture treatment for musculoskeletal diseases and to establish standards of pharmacopuncture treatment model. Methods : We searched articles up to date of March 2009 via computerized databases of Pubmed, The Journal of Korean Acupuncture & Moxibustion, Journal of Korean institute of Herbal Acupuncture, Journal of Oriental Rehabilitation and Journal of Korean Oriental Medicine. Only Randomized Controlled Trials (RCT) concerning the effects of pharmacopuncture on musculoskeletal diseases. The pharmacopuncture treatment methods were assessed based on STRICTA and items considering the feature of pharmacopuncture. And the methodological quality of the trials was assessed by FEAS and modified Jadad score. Results : Eighteen trials of pharmacopuncture on musculoskeletal diseases were analyzed. Except for 4 trials comparing the effect of SBV and BV, positive outcome was reported in ten trials. Among eighteen trials; most of the trials were about Bee Venom acupuncture, and most of the trials used about five acupuncture points, mainly local acupuncture points. But, the amount of injection to each point and total injection were various. And most of trials were lack in the information about method of stimulation. The adjusted FEAS score ranged from 0 to 12, and modified Jadad scoreranged from 1 to 5. Conclusions: To standardize pharmacopuncture treatment, we need more well-designed, high quality clinical trials. And methodological assessment tools designed for pharmacopuncture treatment are also needed.

  • Objectives : The purpose of this study was to evaluate Oriental Medicine Therapy on the Lumbosacral plexitis patient. Methods : The patient was treated with dry needle acupuncture, bee venom pharmacupuncture, herb medicine, indirect moxibustion and physical therapy. We estimated S kinds of Symptoms by Visual Analog Scale, lower limb motor grade test, gait condition test and sensory examination to evaluate the effect of Oriental conservative treatment. Results : After treatment, patient's visual analogue scale score were generally decreased 10 to 4 and lower limb muscle power and gait condition were improved. Conclusions : Oriental Medicine Therapy might be an effective method to treat the Lumbosacral plexitis patient who was diagnosed as Wei Symptom. It is necessary to have more observation and many cases of patients with Lumbosacral plexitis.

The Effect of Bee Venom Acupuncture on Protease Activity and Free Radical Damage in Synovial Fluid from Collagen-induced Arthritis in Rats (쥐의 Collagen 유발 관절염의 활액에서 단백분해효소의 활성 및 유리기 손상에 미치는 봉독약침의 억제효과)

  • 봉독은 거풍습(祛風濕), 지통(止痛), 해경평천(解痙平喘), 소종강압(消腫降壓)의 효능으로 오랫동안 통증과 염증성 질환을 치료하는데 이용되어져 왔는데 최근에는 면역관련질환치료에 응용하여 좋은 결과가 보고되고 있다. 본 연구는 Rheumatoid arthritis와 유사한 형태의 대표적 실험모델로 알려진 실험용 쥐의 Type II collagen 유발 관절염(Type II collagen induced arthritis : CIA)의 활액에서 봉독약침이 단백분해효소와 유리기 손상에 미치는 면역억제효과를 알아보기 위해 실행되었다. 본 실험에서는 CIA가 유발된 실험용 쥐에 봉독약침($5{\mu}l/kg$)을 처리한 실험군과 대조군으로서 CIA 유발 쥐에 생리식염수를 처리한 군(CIA군), 정상적인 쥐에 생리식염수로 처리한 군(정상군)으로 구분하여 각 군들의 일련의 표본에서 세포질, 리소좀, 간질성 단백분해효소의 활성과 유리기로 인한 단백질 손상정도를 (carbonyl 유도체를 측정하여)서로 비교하였다. 그 결과 각 군의 활액표본에서는 많은 종 류의 단백분해효소가 정산군보다 CIA군에서 유의하게 활성이 높았으며, 봉독약침($5{\mu}l/kg$)을 처리한 군에서 효소들의 활성이 유의하게 감소하였다. 그러나 각 군들의 혈장표본에서는 이 효소들의 활성은 서로 유의한 차이가 없었다. 이는 혈장속의 면역반응과 연관되리라고 추측되는 단백분해효소들의 활성변화는 병인적 측면에서 RA와 같은 염증성관절 질환과는 큰 상관성이 없다는 것을 의미한다. Carbonyl 유도체 측정으로 평가한 유리기 손상은 활액과 현장표본에서 모두 봉독약침($5{\mu}l/kg$)을 처리한 군에서 유의성 있게 감소하였다. 이상의 결과로 볼 때 단백분해효소와 유리기의 활성은 RA의 병인학적 측면에서 모두 잠재적인 중요성을 가지고 있으므로 향후 새로운 RA치료법은 이들 단백분해효소의 활성저해와 유리기의 소거능을 포함해야 한다고 사료되며 봉독약침은 이러한 2가지 효능을 포함한 효과적인 치료라고 평가된다.
