Effects of a Critical Thinking Course for Korean Nurses in RN-BSN Program

  • Critical thinking is essential to implementing the nursing process requiring timely decision making. The study examined the effects of a critical thinking course for nurses in the RN-BSN program. The study used a pre- and post-test design with a control group. The experimental group was a convenience sample of 24 working nurses attending a health college. The control group was 24 fellow nurses. The nursing process-based critical thinking course was provided to the experimental group two and half hours a week for 8 weeks. Teaching methods included lectures followed by a summary and quiz, brainstorming and action learning, and lecturer feedback. A pamphlet about views of professional occupation was provided to the control group. The data were collected using self-report questionnaires and analyzed to examine differences in the test scores before and after the intervention. The scores of critical thinking disposition, problem-solving process, and clinical competence increased significantly in the experimental group but did not in the control group. The difference between the two groups was also statistically significant in critical thinking disposition, problem-solving process, and clinical competence. It is necessary that a robust educational program or training to enhance critical thinking ability of nurses are provided in clinical settings and that nurses keep going on thinking critically in their practice.

Experience of the Collaborative Practice among Health Professionals in Korea (국내 보건의료 전문가의 협력적 실무 경험)

  • Purpose : This study aimed to understand and describe the experiences of collaborative practice among health professionals in Korea. Methods : Data were collected through individual in-depth interviews conducted from January to March 2021 with 10 health professionals in Korea. All interviews were audiotaped and transcribed verbatim. The transcribed data then underwent qualitative content analysis. Results : Through content analysis, 10 categories and 3 themes were formed. Three themes, "working based on knowledge," "having difficulty in building collaborative relationships," and "playing complementary roles" emerged. Conclusion : The limitation is that the study participants are 10 health professionals in Korea working at healthcare institutions. However, this study is of great significance in that it provided basic data for preparing measures for collaborative practice among health professionals in Korea.

Effect of Ego-Resilience of Nursing Students on Clinical Practice Competency: Focus on the Mediating Effects of Critical Thinking Dispositions (간호대학생의 자아탄력성이 임상수행 능력에 미치는 영향: 비판적 사고성향의 매개효과를 중심으로)

  • Purpose: The purpose of this study was to determine the mediating effect of critical thinking dispositions in the relationship between ego-resilience and clinical practice competency. Method: This study used a descriptive design with a survey method. A total of 193 nursing students were recruited from 4 colleges of nursing in C city, and A and B province in Korea. The data was collected using questionnaires from October 6, 2017 to October 24, 2017. The data was analyzed using descriptive statistics, independent t-tests, one-way ANOVA, Pearson correlation analysis, and linear regression analysis. Result: There were showed significant positive relationships of clinical practice competency with ego-resilience. In addition, critical thinking dispositions had a partial mediating role in the relationship between ego-resilience and clinical practice competency. Conclusion: This study identified the mediating effects of critical thinking dispositions and how to improve clinical practice competency. The results of this study can be used as basic data for establishing an educational approach to enhance clinical practice competency, which is considered a key competency for nursing college students.

ICU Nurses' Compliance with Standards for Critical Care Nursing Practice (중환자실 간호사의 중환자간호 실무표준 이행에 대한 조사)

  • Purpose: To explore nurses' compliance with standards for critical care nursing practice in intensive care units (ICUs) and to provide basic data for high quality of critical care nursing. Method: A total of 616 participants from 61 ICUs which are graded from 1 to 7 throughout the nation were surveyed. Data were collected from February 9 to February 27, 2009 using a questionnaire consisting of 58 questions including 50 nursing activities as indicators. Results: The rate of ICU nurses' compliance with standards for critical care nursing practice was high. As for individual standards, compliance with the standard of assessment was the highest, followed by implementation, diagnosis, and planning in that order. There were differences in compliance according to nurses' ICU experience, work place (unit), and ICU grade. A shortage of manpower was considered as a main cause for noncompliance. Conclusion: The present compliance rate with standards for critical care nursing practice by ICU nurses was identified. Therefore, compliance rate can be used to promote quality of critical care nursing and development of educational programs for ICU nurses.

Nursing Student Academic Achievement depending on College Entrance System-related Factors (대학입학전형 관련 요인에 따른 간호학과 1학년의 학업성취 분석 : 일 대학 간호학과를 대상으로)

  • Purpose: This study aims to understand the influence of college entrance system-related factors and scores of college scholastic ability test on a first year nursing student's academic achievement. Method: Data collected include both a descriptive survey designed to assess college entrance system-related factors with respect to 182 nursing students attending a 4-year nursing college program and their test scores in the first year. The data were analyzed via descriptive statistics, Mann-Whitney U test, Kruskal-Wallis test with Bonferroni's correction, and Spearman's rank correlation coefficient through an SPSS 18.0 program. Results: Student academic achievement with regard to college entrance system-related factors exhibited significant divergence from first year grade point averages (hereafter GPA) and natural science grades. Conclusion: This study confirmed the existence of significant differences, depending on the factors regarding the college entrance system. As a result, conducting assessment to evaluate the level of basic knowledge before college entrance is highly recommended. Also recommended is the development of a support program for students projected to have difficulties with respect to their academic achievement.

Job Descriptions of Hospital Based Home Care Nurse Practitioners in Korea by DACUM Technique (의료기관 가정전문간호사의 직무분석)

  • Purpose: The aim of this study was to develop and to analyze the task of hospital based home care nurse practitioners in Korea. Method: The definition of home care nurse practitioners and job description was developed based on developing a curriculum(DACUM) by 7 panels who have experienced in home care nursing. One hundred fifty four nurses who were working at hospital based on home care were participated. Result: Fourteen kinds of duties were identified : the selection of home care patients; basic home care nursing; advanced home care nursing; patient/family education and counseling; medical decision making and coordination of patient service; management of home care supplies and drugs for patients; management of medical records; management of home care the agency; management of home care personnel; management of the home care supplies for agency; home care public relations; improvement of home care quality; management of long-term care service; and self-improvement. Ninety-six tasks were classified. Conclusion: The abilities for quality improvement and the advanced nursing practice of home care nurses should be empowered.

Development of the Patient-Centered Nursing Culture Scale for Hospitals (병원 조직의 환자중심간호문화 측정도구 개발)

  • Purpose: This study aimed to develop a scale measuring the Patient-Centered Nursing Culture (PCNC) and provide a basic tool to improve PCNC in Korea. Methods: A conceptual framework and construct factors were extracted through extensive literature review and in-depth interviews with nursing professionals. In total, 59 items were derived based on the pilot survey. Data were collected from 357 nurses working at general hospitals and analyzed for verifying the reliability and validity of the scale. Results: Nine factors containing 54 items were extracted from the exploratory factor analysis to verify the construct validity. The nine factors were top management leadership, policy and procedure, education and training, middle management leadership, supportive teamwork, nursing workplace environment, professional competence, patient-centered nursing activity, and nurses' values. These items were verified by convergent, discriminant, and concurrent validity testing. The internal consistency reliability was acceptable (Cronbach's ${\alpha}=.96$). Conclusion: The developed PCNC scale is expected to be used as the tool for the development of theory and improvement of PCNC, the empirical testing for cause and effect of PCNC, the development of interventions, education and training programs for improving PCNC, and indicators for evaluation or accreditation of hospital service quality.

Mothers' Parenting Experience of Premature Infants: Q Methodological Approach (미숙아 어머니의 양육경험: Q 방법론적 접근)

  • Purpose: This study was done to identify the parenting experience of mothers of premature infants in order to provide basic data for educational solutions and desirable directions. Methods: Q-methodology was used as it provides a method of analyzing the subjectivity of each item. The participants were 33 mothers of premature infants who sorted 34 selected Q-statements which were then classified into the shape of a normal distribution using a 9-point scale. Subjectivity on parenting experience among the mothers was analyzed using the pc-QUANAL program. Results: Four types of parenting experience were identified. Type I was named 'struggling', type II, 'self blame', type III, 'information collecting', and type IV, 'self-introspection'. Conclusion: The results of this study indicate that different approaches to educational programs are needed for mothers of premature infants based on the four types of parenting experience.

Nursing students관 Knowledge on Pain Management (간호학생의 통증관리에 관한 지식)

  • Pain management of postoperative patients or patients suffering from terminal cancer became one of the most important task of nurses in recent years. This study analyzed basic knowledge of 175 student nurses in Taegu on the pain assessment and management. The questionnaires included five areas : Assessment, pharmacological actions, medication, classification of opioids, and psychological dependence of analgesics. The results indicated that nursing students, in general, lacked knowledge on pain management : Correct responses on pain assessment varied between 9.7 and 100% , classification of opioids was in ranges of 61.7 to 95.4% ; Pharmacological actions was 16.6 to 80% : and medication was 14.9 to 85.7%. Less than 12.6% of students correctly identified the frequency of psychological dependence. These results suggested that the need of reinforcement on the pain management in formal nursing education.

A Study on Taegyo Types of the Pregnant Woman- Q-methodological Approach - (임산부의 태교에 대한 태도유형 분석-Q 방법론적 접근-)

  • The purpose of this study was to identify structural patterns in the subjectivity of Taegyo in pregnant woman and to provide a basis for nursing interventions. Thirty pregrant women were the subjects of this study. They were in the gestation period of pregnancy between 12 weeks to five days post partum and they lived in the city of Taejon. The research method employed Q-methodology which is based on self-psychology and abductive logics. The results revealed three different types of the Taegyo ; 1) Peaceful mind type, listening to music and appreciation of art for quietness of mind and also the support and concern of husband. 2) Self control type, recognizes herself as a pregrant woman and trys to do her works spontaneously and also adapt herself to changing of circumstances. 3) Dependent oriented type, needs absolute help from the surroundings under the specific condition of pregnancy. This results show that investigating the specific experience, characteristic and structure of the Taegyo of a pregnant woman can be used as a part of the nursing assessment and also provides useful basic data of nursing interventions during pregnancy.
